Diagnosis of Abusive Head Trauma : Neurosurgical Perspective

  • Abusive head trauma (AHT) is the most severe form of physical abuse in children. Such injury involves traumatic damage to the head and/or spine of infants and young children. The term AHT was introduced to include a wider range of injury mechanisms, such as intentional direct blow, throw, and even penetrating trauma by perpetuator(s). Currently, it is recommended to replace the former term, shaken baby syndrome, which implicates shaking as the only mechanism, with AHT to include diverse clinical and radiological manifestations. The consequences of AHT cause devastating medical, social and financial burdens on families, communities, and victims. The potential harm of AHT to the developing brain and spinal cord of the victims is tremendous. Many studies have reported that the adverse effects of AHT are various and serious, such as blindness, mental retardation, physical limitation of daily activities and even psychological problems. Therefore, appropriate vigilance for the early recognition and diagnosis of AHT is highly recommended to stop and prevent further injuries. The aim of this review is to summarize the relevant evidence concerning the early recognition and diagnosis of AHT. To recognize this severe type of child abuse early, all health care providers maintain a high index of suspicion and vigilance. Such suspicion can be initiated with careful and thorough history taking and physical examinations. Previously developed clinical prediction rules can be helpful for decision-making regarding starting an investigation when considering meaningful findings. Even the combination of biochemical markers may be useful to predict AHT. For a more confirmative evaluation, neuroradiological imaging is required to find AHT-specific findings. Moreover, timely consultation with ophthalmologists is needed to find a very specific finding, retinal hemorrhage.

Differentiation of Malignant from Benign Soft-Tissue Solid Tumors: Clinical and MR Finding Complex (연조직고형종양의 악성과 양성 감별: 임상과 자기공명영상 복합소견)

  • Purpose: The recent development of MR has made to possible radiological diagnosis in various soft tssue tumors. But multifarious components within soft tissue tumors and their periodic change have made to difficult even differentiation of malignant from benign soft tissue tumors solely on the MR. So authors retry to differentiate malignant from benign soft tissue tumors with clinical and MR finding complex. Materials and methods: We were analysed 82 pathologically confirmed soft tissue solid tumors (37 cases as malignancy including intermediate tumors and 45 cases as benign including inflammatory masses) which are correlated with clinical findings such as age, size, and location, MR findings such as tumor border, texture on T2 and contrast-T1 images, and enhancement area retrospectively. Many typical lipoma and cysts including of ganglion and abscess are rejected in the benign soft tissue tumor group because not difficult to diagnose on MR. Results: Malignant soft tissue tumors were more frequent in 21~40 and 61~80 years old of the age, above 3.0 cm of the size, trunk-pelvis-lower extremities of the location, and MR findings with irregular border and above 50% of the enhancement area than those of benign soft tissue tumors. Conclusion: The clinical finding that divided to two locations as trunk-pelvis-lower extremities and upper extremities-shoulder-spine was statistically significant to differentiate malignant from benign soft tissue solid tumors. However, the others would provide some useful informations to differentiate them never specific.

Recent updated diagnostic methods for esophageal motility disorders (식도의 운동장애에 관한 최신지견)

  • Classification of esophageal motility disorders not yet finalized and is still ongoing as the new disorders are reported, and the existing classification is changed or removed. In terms of radiology, the primary peristalsis does not exist, and the lower end of the esophagus show the smooth, tapered, beak-like appearance. The esophageal motility disorder, which mostly occurs in the smooth muscle area, show the symptoms of reduction or loss (hypomotility) or abnormal increase (hypermotility) of peristalsis of the esophagus. It is important to understand the anatomy and physiology of the esophagus for the appropriate radiological method and diagnosis. Furthermore, the symptom of the patient and the manometry finding must be closely referred for the radiological diagnosis. The lower esophageal sphincter can be normally functioning and open completely as the food moves lower. Sperandio M et al. argues that the name diffuse esophageal spasm must be changed to distal esophageal spasm (DES) as most of the spasm occurs in the distal esophagus, composed of the smooth muscle. According to Ott et al., usefulness of barium method for diagnosing the esophageal motility disorder is Achalasia 95%, DES 71% and NEMD 46%, with the overall sensitivity of 56%. However, excluding the nutcracker esophagus or nonspecific disorder which cannot be diagnosed with the radiological methods, the sensitivity increases to 89%. Using videofluoroscopy and 5 time swallows, the average sensitivity was over 90%. In conclusion, the barium method is a simple primary testing method for esophageal motility test. Using not only the image but also the videofluoroscopy with good knowledge of the anatomy and physiology, it is believed that the method will yield the accurate diagnosis.

A Study of the Changes of Breast Uptake in Menstrual Cycle on 18F-FDG PET/CT (월경 주기에 따른 18F-FDG PET/CT에서 유방 섭취 변화에 관한 고찰)

  • $^{18}F-FDG$ PET/CT has been known a useful modality to diagnose high-glucose-using cells such as cancer cells by glucose metabolism of FDG. Mainly, FDG takes on cancer and inflammatory cells; however, there have been FDG uptakes on normal tissues by individual physiological characteristics, occasionally. Especially, in fertile females, unusual FDG uptake of breast changes as the menstrual cycle, and disturb diagnosis. Therefore, the study aimed to evaluate the change of breast FDG uptake in menstrual cycle on $^{18}F-FDG$ PET/CT. 160 females ($34{\pm}3.5$ years old) who do not undergo a gynecologic anamnesis and have regular menstrual cycle over the previous 6 months were examined. They were divided 4 groups (each 40 patients) as flow phase, proliferative phase, ovulatory phase and secretory phase using Pregnancy Calculator 0.14. and history taking. Discovery Ste (GE Healthcare, Milwaukee, Mi, USA) was used a s PET/CT. We analyzed SUVs on accumulated region on breast, and 3 nuclear medicine specialists did the Blind test. SUVs on the Breast were flow phase ($1.64{\pm}0.25$), proliferative phase ($0.93{\pm}0.28$), ovulatory phase ($1.66{\pm}0.26$) and secretory phase ($1.77{\pm}0.28$). It showed high uptake value in secretory, flow phase and ovulatory phase (p<0.05). In gross analysis, the accumulation of breast was divided into 3 grades as comparing with lung and liver. The breast's uptake was equal to lung (Grade I); between lung and liver (Grade II); equal to or greater than liver (Grade III). The results showed high uptake value in secretory, flow phase and ovulatory phase (p<0.05). In fertile females, FDG uptake of breast changed as menstrual cycle, and it available to diagnose breast disease. Therefore, we consider reducing false-negative finding of breast disease, by doing examination on appropriate period through history taking about individual menstrual cycle.

Correlation Analysis between Radiological Result and Radiating Pain in Neck Pain (경항통 환자에 있어서 방사통과 영상의학검사 소견상의 연관성 분석)

  • Objectives : To compare the differences between the symptoms and the findings of MRI(magnetic resonance imaging) and x-ray, we studied the patients with neck pain or radiating pain, which has been diagnosed as cervical herniated disc recently. Methods : We randomly selected among the 143 patients with x-ray and cervical spine(C-spine) MRI films who have visited Ja-seng hospital with neck pain and neck and radiating pain from April 1 of 2010 to May 1. We used SPSS 13.0 for windows in analyzing statistical data of study results and the level of significance was below 0.05. Results : 1. There were no significant differences between the presence of radiating pain and the amount of cervical herniation(p>0.05). 2. If the finding of a x-ray showed narrowing, based on MRI findings, the amount of herniation was more severe(p>0.05). 3. There were no significant differences between the presence of radiating pain and the findings of x-ray(p>0.05). 4. Among the 143 cases, which showed findings beside HIVD(herniation of intervertebral disc) were 13 cases. 88 cases of straightening(61.5%). 78 cases of uncovertebral joint arthrosis(54.5%). 25 cases of stenosis(17.5%), 13 cases of retrolisthesis(9.1%), 8 cases of osteophyte(6.6%), 4 cases of spondylolisthesis(2.8%), 2 cases of hemangioma(1.4%), 3 cases of OPLL(ossification of posterior longitudinal ligament)(2.1%), 2 cases of block vertebrae(1.4%), 2 cases of spondylitis(1.4%), 1 case of kyphosis(0.1) and 1 case of ligamentum flavum hypertrophy(0.1%). Conclusions : The findings from this study suggest that there was no relation between radiating pain and radiological result. On the other hand, diagnosis of x-ray and MRI showed significant relevance. The narrower disc space there were, the severer the state of herniation there existed.

Mixed Noise Reduction Filters for CR Images (CR X선 영상의 복합잡음 감소에 관한 연구)

  • This study is to decrease compound noise in x-ray films. This study compared Signal to noise ratio(SNR), Peak signal to noise ratio(PSNR), Mean square error(MSE) to surface of the earth. In addition, we evaluated noise elimination effect according to the kernel size of Median filter. This experiments show that some filters are useful by finding image that is near in circle image comparing circle picture with each processed picture. In noise power value, when cutoff frequency was compared with other filters of cutoff frequency. Cutoff frequency of $2/3\pi{\sim}3/4\pi$ is good and it shows good SNR and PSNR. Therefore, it can display high filter effect. As Median Filter's Kernel size grows SNR value gets bigger, which shows better filter effect. Most pictures are distorted after filter application in medical treatment image. It is important to keep spatial resolution in most medical images. Visual estimation as well as quantitative indicators should be necessary for a better image.

  • A sarcoidosis is a multisystemic granulomatous disorder that has a predilection for pulmonary involvement, and the common radiological findings for the disease are bilateral nodular or reticulonodular patterns. Pseudoalveolar sarcoidosis is a rare presentation of sarcoidosis. The radiological finding is an alveolar pattern that involves or compresses the alveoli by clustered interstitial granuloma. A 58-year-old man was admitted due to incidental findings of a unilateral consolidative lesion as seen on chest radiography. A chest computed tomography (CT) examination showed multiple bronchoalveolar consolidations that were suspicious of a malignancy. However, a percutaneous needle biopsy revealed non-caseating granuloma with an asteroid body that was compatible with sarcoidosis. After one month, the consolidative lesions improved without any treatment.

IN-VIVO DOSE RECONSTRUCT10N USING A TRANSMISION FACTOR AND AN EFFECTIVE FIELD CONCEPT (팬텀투과계수와 유효조사면 개념을 이용한 종양선량 확인에 관한 연구)

  • The aim of this study Is to develop a simple and fast method which computes in-vivo doses from transmission doses measured doting patient treatment using an ionization chamber. Energy fluence and the dose that reach the chamber positioned behind the patient is modified by three factors: patient attenuation, inverse square attenuation. and scattering. We adopted a straightforward empirical approach using a phantom transmission factor (PTF) which accounts for the contribution from all three factors. It was done as follows. First of all, the phantom transmission factor was measured as a simple ratio of the chamber reading measured with and without a homogeneous phantom in the radiation beam according to various field sizes($r_p$), phantom to chamber distance($d_g$) and phantom thickness($T_p$). Secondly, we used the concept of effective field to the cases with inhomogeneous phantom (patients) and irregular fields. The effective field size is calculated by finding the field size that produces the same value of PTF to that for the irregular field and/or inhomogeneous phantom. The hypothesis is that the presence of inhomogeneity and irregular field can be accommodated to a certain extent by altering the field size. Thirdly, the center dose at the prescription depth can be computed using the new TMR($r_{p,eff}$) and Sp($r_{p,eff}$) from the effective field size. After that, when TMR(d, $r_{p,eff}$) and SP($r_{p,eff}$) are acquired. the tumor dose is as follows. $$D_{center}=D_t/PTF(d_g,\;T_p){\times}(\frac{SCD}{SAD})^2{\times}BSF(r_o){\times}S_p(r_{p,eff}){\times}TMR(d,\;r_{p,eff})$$ To make certain the accuracy of this method, we checked the accuracy for the following four cases; in cases of regular or irregular field size, inhomogeneous material included, any errors made and clinical situation. The errors were within 2.3% for regular field size, 3.0% irregular field size, 2.4% when inhomogeneous material was included in the phantom, 3.8% for 6 MV when the error was made purposely, 4.7% for 10 MV and 1.8% for the measurement of a patient in clinic. It is considered that this methode can make the quality control for dose at the time of radiation therapy because it is non-invasive that makes possible to measure the doses whenever a patient is given a therapy as well as eliminates the problem for entrance or exit dose measurement.

Influence of Regularization Parameter on Algebraic Reconstruction Technique (대수적 재구성 기법에서 정규화 인자의 영향)

  • Computed tomography has widely been used to diagnose patient disease, and patient dose also increase rapidly. To reduce the patient dose by CT, various techniques have been applied. The iterative reconstruction is used in view of image reconstruction. Image quality of the reconstructed section image through algebraic reconstruction technique, one of iterative reconstruction methods, was examined by the normalized root mean square error. The computer program was written with the Visual C++ under the parallel beam geometry, Shepp-Logan head phantom of $512{\times}512$ size, projections of 360, and detector-pixels of 1,024. The forward and backward projection was realized by Joseph method. The minimum NRMS of 0.108 was obtained after 10 iterations in the regularization parameter of 0.09-0.12, and the optimum image was obtained after 8 and 6 iterations for 0.1% and 0.2% noise. Variation of optimum value of the regularization parameter was observed according to the phantom used. If the ART was used in the reconstruction, the optimal value of the regularization parameter should be found in the case-by-case. By finding the optimal regularization parameter in the algebraic reconstruction technique, the reconstruction time can be reduced.

Anterior Cruciate Ligament Reconstruction Using Bone-Patellar Tendon-Bone Autograft - The comparison of mid term & long term follow-up results - (골-슬개 건-골을 이용한 전방 십자 인대 재건술 - 중기 및 장기 추시 결과의 비교 -)

  • Purpose : To compare the mid term with long term follow up results of the anterior cruciate ligament(ACL)> reconstruction using patellar tendon. Materials and Methods : Seventy-six cases, which could be followed up more than 2 years were included in this study. They were classified into two groups, mid-term group(41 cases) with mean follow-up periods of 2 years and 10 months and long-term group(35 cases) off years and 6 months. Clinical and radiological results and peripatellar complications were evaluated & compared. Results . There was no statistically significant difference in radiological finding and anterior instability between two groups. However, the incidence of anterior knee pain, paresthesia at incision site, kneeling pain, crepitus and quadriceps weakness was lower in long-term group than in mid-term group. Conclusion : ACL reconstruction using patellar tendon graft could successfully restone the stability of knee joint, but there are some complications in donor site and patellofemoral joint. However this complications were gradually decreased with long term follow-up.
