The Effect of Sa-Am Lung Tonifying Acupuncture on Radial Pulse in Healthy Human Subjects (폐정격(肺正格) 자침이 촌관척(寸關尺) 맥파 변화에 미치는 영향)

  • Objectives : The purpose of this study is to find the effects of Sa-Am lung tonifying acupuncture on radial pulse through the parameters difference. Methods : Forty healthy subjects participated in this study, acupuncture group and control group were divided. Compared various parameters of radial pulse between two groups, and the significantly changed 16 parameters ware measured using 3 dimensional pulse imaging system(DMP-3000) before, right after, 30 minutes after, and 60 minutes after acupuncture at Chon, Gwan, Cheok in acupucture group. Results : After lung tonifying acupuncture, radial pulse 16 parameters were showed changes significantly according to the time at each measuring location. 1. Heart rate, T2/T, T5, T5/T and W/T significantly decreased, W significantly increased. 2. Modulus of elasticity significantly decreased in left Cheok. 3. Amplitude of H4 significantly increased in left Gwan and right Cheok. 4. AIx, AIxHR significantly decreased in left Gwan and significantly increased in right Chon. 5. Area of pulse, area of W significantly increased in right Cheok. 6. Pulse energy, energy/min, EIx significantly decreased in left Cheok. 7. Maximum variation time of each parameter is showed different according to the parameter. Conclusions : Lung tonifying acupuncture exerts an influence on radial purse parameters. Further radial pulse change study on various acupuncture treatment is required.

Properties of Pulse Waveforms by Posture Changes : Standing, Sitting, Supine Posture (측정 자세의 변화에 따른 맥의 변화 특성 : 선 자세, 앉은 자세, 누운 자세)

  • Objectives : Informations on pulse diagnosis in literature are based on diagnosing pulse waveforms on supine posture. However, today's pulse waveforms are measured on various postures for the convenience of patients or doctors. For objective measurement, the effect of posture on the pulse waveforms should be considered. The objective of this study was to find posture-related changes in the radial pulse waveforms. Methods : We used an instrument, DMP-3000(DAEYOMEDI Co., Ansan, Korea), measuring radial pulse waveforms noninvasively by tonometric method. 25 male subjects participated in the trial. Before measuring radial pulse waveforms subjects had rest for 5 min. The pulse waveforms were measured on the left wrist. Each subject underwent this course on the supine, sitting, and standing posture. We analyzed pulse waveforms with Height-parameters, Time-parameters, Energy, and Elastic rate. Results : Height-parameters(h1~h5) on the supine posture were bigger than those on the sitting and standing posture. In case of Time-parameters, the parameters making up systolic time decrease in order of on standing, sitting, and supine position. However, systolic time and diastolic time didn't have any changes. Energy of pulse was the biggest on supine posture and Elastic rate on standing posture. Conclusions : In this study we found that posture changes affect radial pulse waveforms. For quantification of the changes, more trials should be done. After analyzing much data we might apply parameters of pulse waveforms changed by posture. Also, we might diagnose special disease with properties of pulse waveforms by posture.

The Effects of Sa-am Acupuncture on Radial Pulse in Healthy Human Subjects: A Comparative Study on Tonifications of Kidney and Urinary Bladder (신정격 및 방광정격 자침이 정상 성인의 맥파 변화에 대한 비교 연구)

  • Objectives : The purpose of this study is to observe the effects of Sa-am acupuncture with tonifications of kidney and urinary bladder on radial pulse in healthy subjects. Methods : Sixty healthy subjects participated in this study, and were divided into a kidney tonifying acupuncture group(KI group), urinary bladder tonifying acupuncture group(BL group) and control group. Radial pulse was measured by 3 dimensional pulse imaging system(DMP-3000) before, immediately after, 30 minutes after, and 60 minutes after acupuncture at Cun, Guan, and Chi in each time. Results : 1. BL group exhibited significantly reduced T1 / T, T4 / T, T4 / (T-T4), T5 / T and increased(T-T4) / T compared to KI group. 2. BL group showed increase of radial augmentation index / heart rate(RAI / HR) in left Cun, H5, Ad / Ap in left Chi, H1, As / Ap in right Chi compared to KI group. BL group showed decrease of As / Ap, Aw / Ap in left Chi, H1, pulse depth, Ad / Ap in right Chi compared to KI group. 3. KI group showed increase of Aw / Ap, H1, H2, H4, pulse area in right Guan, pulse depth in right Chi compared to BL group. KI group showed decrease of RAI in left Cun, applied pressure, pulse depth in left Guan, Aw / Ap in right Chi compared to BL group. Conclusions : The effects of Sa-am acupuncture with tonifications of kidney and urinary bladder in healthy humans have been observed on various parameters. The parameters analysed in this study can be used to differentiate the effects of Sa-am acupuncture with tonifications of kidney and urinary bladder on radial pulse.

  • In this paper, through a digital potentiometer and exponentially weighted moving average filter, pulse and PPG waveform measurable device was fabricated in radial artery. If this device is not proper about signal size in analog part, MCU can judge easily by adjusted amplification through digital potentiometer, using exponentially weighted moving average filter is able to filter out more clear value of ADC. I presumed pulse rate as value of measuring time between point of maximum contraction from sensing signal in radial artery of wrist. Therefore, this means can measure stable pulse rate and PPG waveform, finger as well as radial artery, whether signal size of each person is different finger as well as radial artery.

  • Objective: Increased aortic and carotid arterial augmentation index (AI) is associated with the risk of cardiovascular disease. The most widely used approach for determining central arterial AI is by calculating the aortic pressure waveform from radial arterial waveforms using a transfer function. But how the change of waveform by applied pressure and the pattern of the change rely on subject's characteristics has not been recognized. In this study, we use a new method for measuring radial waveform and observe the change of waveform and the deviation of radial AI in the same position by applied pressure. Method: Forty-six non-patient volunteers (31 men and 15 women, age range 21-58 years) were enrolled for this study. Informed consent in a form approved by the institutional review board was obtained in all subjects. Blood pressure was measured on the left upper arm using an oscillometric method, radial pressure waves were recorded with the use of an improved automated tonometry device. DMP-3000(DAEYOMEDI Co., Ltd. Ansan, Korea) has robotics mechanism to scan and trace automatically. For each subject, we performed the procedure 5 times for each applied pressure level. We could thus obtain 5 different radial pulse waveforms for the same person's same position at different applied pressures. All these processes were repeated twice for test reproducibility. Result: Aortic AI, peripheral AI and radial AI were higher in women than in men (P<0.01), radial AI strongly correlated with aortic AI, and radial AI was consistently approximately 39% higher than aortic AI. Relationship between representative radial AI of DMP-3000 and peripheral AI of SphygmoCor had strongly correlation. And there were three patterns in change of pulse waveform. Conclusion: In this study, it is revealed the new device was sufficient to measure how radial AI and radial waveform from the same person at the same time change under applied pressure and it had inverse-proportion to applied pressure.

  • This paper describes a new system that detects radial pulse wave and allows the diagnosis of malfunctions of cardiovascular system by analyzing the waveforms with the newly proposed algorithm. The system consists of a sensor part and a data processing part within which a new detection algorithm is incorporated In acquiring radial pulse signal noninvasively, the sensor used in this system is a new combinational fiber-optic sensor which has a detecting Part and a transmitting Part. Also, In order to analyze the characteristics of pulsation quantitatively, the algorithm proposed in this paper is a method that runs in parallel with both the data of ECG and differential pulse simultaneously. these concepts are based upon the idea that thfee Q points of ECG give obious discrimination of one entire period of pulse in any abnormal cases, and newly defined feature lines at the differential counterpart can be used to recogrlize sDme significant points in one period of pulses.

  • Objectives : The purpose of this study is to find parameters to compare the effects of Sa-Am acupuncture with large intestine tonifying and sedating on radial pulse in healthy subjects. Methods : Sixty healthy subjects participated in this study, divided into large intestine tonifying acupuncture group, large intestine sedating acupuncture group and control group. Radial pulse was measured by 3 dimensional pulse imaging system(DMP-3000) before, right after, 30 minutes after and 60 minutes after acupuncture at Cun, Guan, and Chi in the acupuncture group and in the control group at the same time points. Results : 1. Angle of main peak and magnitude of fourier component significantly changed by integrated analysis of Cun, Guan and Chi. 2. Amplitude of H1, Pulse Power volume / min, elasticity, AIx / HR, Frequency and magnitude of fourier component significantly changed by analysis of Cun, Guan, and Chi. Conclusions : The effect of Sa-Am acupuncture with large intestine tonifying and sedating in healthy human may be observed on time, amplitude, pulse area, augmentation index and fourier components parameters. The parameters analysed in this study may be used to differentiate the effects between Sa-Am acupuncture with large intestine tonifying and sedating on radial pulse. Further studies on the effects of Sa-Am acupuncture using radial pulse are needed.

A Study on Pulse Pattern Using Ultrasonic Device (초음파기기를 이용한 맥상(脈象) 연구)

  • Objectives : Pulse diagnosis is one of the main diagnostic methods of Korean Medicine that understands the patient's condition and illness by reading changes in the patient's pulse, which is described in terms of pulse condition While they are described in detail in medical texts, it is difficult to grasp their true nature, as the written descriptions fail to do justice to the experience of pulse taking it tries to convey. As a way to approach pulse condition the effect of the body's tension on the radial vein was measured using an ultrasonic device, after which the measured changes and how they could be reflected in pulse condition were studied. In other words, changes in the radial vein following induced tension were analyzed. Methods : 1) The thickness of the subject's radial vein was measured using a linear probe of an ultrasonic device[LOGIQ 5 Basic, GE, USA]. 2) Fatigue level was increased through artificial stimulation using the Gripmeter[ks-301, Lavisen, Korea]. 3) Thickness of the radial vein post tension induction was measured. 4) The results were analyzed with the Tukey test or paired t-test as post hoc tests. Results : Thickness of the radial vein of the subject pre- and post- Gripmeter stimulation decreased with significance. Conclusions : Constriction of the radial vein that happened after tension induction could be linked to the Tight Pulse[緊脈] that is related to patterns of contraction and pulling.

A Research of characteristics of left/right pulse wave and blood vessel using Korean medicine pulse diagnosis (맥진기를 이용한 좌우 맥파 및 혈관 특성 연구)

  • Objectives: The pulse diagnosis to identify the symptoms has been considered important in Korean medicine. The position and character of disease would be confirmed by pulse diagnosis of left and right radial artery. This paper is to analyze the characteristics and differences of left and right blood vessels. Methods: In this study, left and right radial artery and dorsalis pedis artery was measured and analyzed by using condenser typed pulse analyzer. Commercially available pulse analyzer was used to measure the radial artery. The pulse wave was measured in 20 laboratory healthy men and women. The blood vessel aging degree and index of augmentation of blood vessel was obtained from the measured pulse wave graph and the characteristics and differences of the left and right blood vessel was analyzed. Results: The significant difference of pulse transit time between the right handed and non-right handed was not found. The correlation of radial artery and dorsalis pedis artery had no significant difference. By obtaining the blood vessel aging index (AGI) and augmentation index (AI) of blood vessel at the left and right radial artery, the significant difference between right handed and non-right handed was not found. Conclusions: The result of this study would help to explain the characteristic of blood vessel with respect to the left and right handed. We suggest that research of pulse wave of the left and right blood vessel using pulse analyzer should be needed in further study.

Development of Clinical Protocol for Acquisition of Change of Radial Pulse Wave Signal in the Cold-Heat Intervention: Explanatory, Randomized, 2×2 cross-over design (냉온 부하에 따른 요골동맥 맥파의 변화 특성을 파악하기 위한 무작위 배정·2×2교차설계 탐색적 임상시험계획서 개발)

  • Objectives The aim of this study is to develop a structured clinical protocol related with acquisition of radial pulse wave in the randomized, $2{\times}2$ cross-over design, and cold-heat intervention trial for a pilot and preliminary study. Methods The protocol was contrived based on wide ranging literature searches for cold-heat intervention experiments and radial pulse diagnoses. Results Sample size of 60 subjects was calculated based on an effect size derived from the previous study designed to detect the pre-post cold-heat differences in the radial pulse. Each subjects will be randomly assigned to the cold (first) to heat (last) group (n=30) or heat (first) to cold (last) group (n=30). All subjects will fill out a case report form and questionnaires related with pattern identification, dietary patterns, sleep quality, and physical activity will be surveyed and used as a secondary outcomes. Safety assessment will be reported at the final stage. Conclusions This protocol will provide an additional reference to future studies related with observation of radial pulse during any interventions and also expect to be used as a guideline for acquisition of reliable radial pulse wave data.
