Diagnostic Performance of Diffusion Weighted Imaging of Malignant and Benign Pulmonary Nodules and Masses: Comparison with Positron Emission Tomography

  • Background: Diffusion-weighted imaging (DWI) makes it possible to detect malignant tumors based on the diffusion of water molecules. However, it is uncertain whether DWI has advantages over FDG-PET for distinguishing malignant from benign pulmonary nodules and masses. Materials and Methods: One hundred-forty-three lung cancers, 17 metastatic lung tumors, and 29 benign pulmonary nodules and masses were assessed in this study. DWI and FDG-PET were performed. Results: The apparent diffusion coefficient (ADC) value ($1.27{\pm}0.35{\times}10^{-3}mm^2/sec$) of malignant pulmonary nodules and masses was significantly lower than that ($1.66{\pm}0.58{\times}10^{-3}mm^2/sec$) of benign pulmonary nodules and masses. The maximum standardized uptake value (SUVmax: $7.47{\pm}6.10$) of malignant pulmonary nodules and masses were also significantly higher than that ($3.89{\pm}4.04$) of benign nodules and masses. By using optimal cutoff values for ADC ($1.44{\times}10^{-3}mm^2/sec$) and for SUVmax (3.43), which were determined with receiver operating characteristics curves (ROC curves), the sensitivity (80.0%) of DWI was significantly higher than that (70.0%) of FDG-PET. The specificity (65.5%) of DWI was equal to that (65.5%) of FDG-PET. The accuracy (77.8%) of DWI was not significantly higher than that (69.3%) of FDG-PET for pulmonary nodules and masses. As the percentage of bronchioloalveolar carcinoma (BAC) component in adenocarcinoma increased, the sensitivity of FDG-PET decreased. DWI could not help in the diagnosis of mucinous adenocarcinomas as malignant, and FDG-PET could help in the correct diagnosis of 5 out of 6 mucinous adenocarcinomas as malignant. Conclusions: DWI has higher potential than PET in assessing pulmonary nodules and masses. Both diagnostic approaches have their specific strengths and weaknesses which are determined by the underlying pathology of pulmonary nodules and masses.

  • Myopericytoma is a benign tumor that occurs in soft tissues. Myopericytoma in the lungs is very rare. We report the case of a 63-year-old woman presenting with cavitary masses in the left lung and multiple tiny nodules in both lungs. She underwent surgery, and a histological examination revealed primary pulmonary myopericytoma.

Vegetative endocarditis associated with Arcanobacterium haemolyticum in a Holstein cow: a case report

  • A 4-year-old Holstein cow with progressive atrophy and ataxia was submitted for diagnosis. The postmortem examination revealed multifocal yellowish nodules in the pulmonary parenchyma and vegetative masses in the mitral and tricuspid valve of the heart. Both kidneys were severely enlarged, with multiple yellow nodules on the parenchyma. Histopathologically, pulmonary abscesses, vegetative endocarditis, suppurative glomerulonephritis, and fibrino-purulent arthritis were observed. The tiny β-hemolytic bacterial colonies were isolated from the lesions and identified as Arcanobacterium haemolyticum by the VITEK 2 system (bioMérieux, USA). This is the first documented report of an A. haemolyticum infection in a Holstein cow in Korea.

  • Alveolar soft-part sarcoma is a rare soft tissue sarcoma of young adults with unknown histogenesis, and the organ most frequently involved in metastasis is the lung. We report the CT findings of three patients of pulmonary metastases of alveolar soft-part sarcoma, which manifested as clearly enhanced pulmonary nodules or masses. On enhanced scans, some of the masses were seen to contain dilated and tortuous intratumoral vessels.

개에서 국소형과 범발성형 조직구육종 2예 (Localized and disseminated histiocytic sarcoma in two dogs)

  • A male mixed breed dog with unknown age (case 1) and 5-year-old female golden retriever (case 2) were admitted to the Department of Veterinary Pathology at College of Veterinary Medicine, Seoul National University after sudden death. At necropsy, stifle region of case 1 was replaced by coalescing firm tan nodules approximately 1-3 cm in diameter. Neoplastic nodules were also presented in the lung. In case 2, the spleen was enlarged 3-5 times its normal size and numerous firm tan nodules were noted in the kidney, pericardium, diaphragm and lymph nodes. Histopathologically, the neoplastic masses of case 1 consisted of plump spindle cells having abundant cytoplasm and indistinct cell borders that formed interlacing bundle. In case 2, the masses were consisted of round to polygonal cells with abundant eosinophilic cytoplasm and eccentric nuclei. Numerous multinucleated giant cells containing nuclei of various sizes were noted. Immunohistochemically, neoplastic cells of all two cases stained intensely with vimentin and lysozyme. Based on the gross and light microscopic findings together with immunohistochemistry, both cases were diagnosed as localized histiocytic sarcoma with pulmonary metastasis and disseminated histiocytic sarcoma, respectively.

  • Kaposiform hemangioendothelioma (KHE) is a rare, locally aggressive vascular tumor of intermediate malignancy with resemblance to Kaposi sarcoma. It occurs predominantly in pediatric age groups as a cutaneous lesion with focal infiltration into the adjacent soft tissue and bone. Although visceral involvement is very uncommon, several cases with bone, retroperitoneal, or mediastinal involvement have been described. KHE has been reported to occasionally occur in unusual sites such as the thymus, tonsils, larynx, paranasal sinuses, deltoid muscle, spleen, uterine cervix, thoracic spine, and even the breast. Multifocal KHE is an extremely rare entity with few reports available in the literature, none of which describes pulmonary involvement. Herein, we report a unique case of multifocal KHE in a 13-year-old boy presenting with a huge soft tissue mass in the upper extremity complicated by bilateral pulmonary nodules that developed into large, necrotic tumor masses.

미니어쳐 핀셔에서 신경증상을 동반한 폐션암 (Pulmonary adenocarcinoma with brain metastasis in a Miniature Pincher)

  • A 13-year-old female Miniature Pincher was euthanized after suffering from respiratory insufficiency and seizure. At necropsy, firm tan masses approsimately 4 to 5 cm in diameter were noted at the right caudal love and left cranial lobe of the lung. On cross sections of the cerebellum and the verebrum, several compressive firm round masses, 0.5 to 1 cm in diameter, were noted. similar looking nodules were also present in the mediastinal lymph nodes. Histopathologically, the neoplastic masses consisted of cuboidal shape epithelial cells that form glandular structure. The neoplastic cells have hyperchromatic nuclei with prominent nucleoli and moderate amount of cytoplasm. The degree of mitosis was high (4-6/400X). Multiple areas of necrosis, hemorrhage, and tumor emboli were also noted. Metastasis to the mediastinal lymph nodes, pancreas, and brain was confirmed. Based on the gross and histopathologic examinations, a diagnosis of primary metastatic pulmonary adenocarcinoma was made. Clinicaly recognized neurological signs were therefore due to brain metastasis.

폐의 좁쌀 결절로 발현된 림프종 사례: 증례 보고 (Diffuse Large B-Cell Lymphoma Manifesting as Miliary Nodules in the Lung: A Case Report)

  • 악성 림프종은 흉부 CT에서 결절, 종양, 폐 경화, 간유리음영 등 다양한 폐질환으로 나타날 수 있으며 이러한 폐 병변은 다른 질환의 양상과 유사하여 진단에 어려움을 줄 수 있다. 본 증례는 호흡곤란과 발열을 주 증상으로 하는 67세 남성 환자로 해당 환자의 흉부 CT상 크기가 작은 다발성 결절이 양측에 미만성으로 나타나 혈행성 전이, 좁쌀 결핵 또는 진균 감염을 의심해 볼 수 있었다. 그러나 추가적으로 시행한 혈액 검사, 영상 검사 및 조직 생검 결과 폐를 침범한 미만성 큰 B세포 림프종이 진단되었다. 저자들은 폐의 좁쌀 결절로 발현한 매우 드문 림프종을 보고한다. 환자의 적절한 진단을 위해서는 환자의 병력, 신체 진찰, 혈액 검사 및 영상 소견의 포괄적인 평가가 필요하다.

경화 양상으로 나타난 폐의 Mucormycosis (Pulmonary Mucormycosis with an Appearance of Consolidation)

  • 폐모균증은 치명적인 결과를 초래하는 기회감염으로 조기진단 및 치료가 요망된다. 하지만 본 증례와 같이 폐모균증의 방사선학적 소견은 비특이적이므로 조기 진단이 용이하지 않다. 이에 저자들은 폐모균종의 발생 위험인자를 내포한 환자가 항생제에 반응을 보이지 않으면서 방사선 소견상 폐의 경화, 종괴, 결절, 공동 등의 소견을 보이는 경우 폐모균증올 고려함이 필요하다고 생각되어 문헌고찰과 함께 본 증례를 보고하는 바이다.

폐에 발생한 양성 전이성 근종-1례 보고- (Benign Metastasizing Leiomyoma of Lung -A case report-)

  • We performed thoracoscopic resection for diagnosis in 41 year-old-female presenting with multiple pulmonary nodules in both lung fields, which was detected incidentally on routine chest x-ray and followed by additional exmaminations including chest CT scan and percutaneous needle aspiration biopsy under the presumptive diagnosis of metastatic cancer. During thoracoscopy, the result of the frozen section analysis of multiple masses revealed strong evidence of leiomyoma. In her past medical history, she had undergone myomectomy, and hysterectomy, 7 year ago and 10 year ago, respectively. Based on permanent, special staining of specimen, estrogen receptor assay and review of past specimen of uterine myoma the final diagnosis was benign metastasizing leiomyomata from uterine myoma, the report was very uncommon in Korean and English literatures. The patient has been followed up for 2 year without special therapy, such as hormonal therapy.
