Unexpected Lymph Node Pathology in Neck Dissection for Head and Neck Cancer (두경부 종양환자에서 경부 림프절의 예기치 않은 병리적 소견)

  • Neck dissection has become an integral part of the staging and management of head and neck tumors. This paper reports a series of head and neck patients who had pathological findings in their neck dissection specimens, which were unrelated to their primary tumors. In 7 cases, there was unexpected pathology in the cervical lymph nodes which was not related to the primary tumor. Four cases were squamous cell carcinomas and 3 were thyroid carcinomas. In 3 cases of squamous cell carcinomas, there were no evidence of metastatic squamous cell carcinoma in neck dissection specimen; however, the lymph nodes were found to be involved by metastatic papillary carcinoma in one larynx cancer, metastatic adenocarcinoma in the other larynx cancer, tuberculosis in one nasopharynx cancer. In three of neck dissection specimen of carcinoma(two thyroid carcinomas, one laryngeal carcinoma), dual nodal pathology was found: Each of these specimen contained carcinoma with tuberculosis of the lymph nodes in three cases. In one thyroid carcinoma, there was no evidence of metastasis; however, the lymph nodes were found to be involved by tuberculosis. Preoperative assessment did not reveal any findings to alert us to the possibility of a synchronous pathological process in the cervical nodes of this group of 7 patients. In particular, there was no evidence of active pulmonary tuberculosis in the 5 patients with active lymph node disease.

Mammary Squamous Cell Carcinoma in a Collard Peccary (Tayassy Tajacu) (페커리 (Collard peccary)에서 발생한 유선 편평 세포 선암종)

  • A 10-year-old female collared peccary (Tayassu tajacu) was referred due to a large palpable mass in caudal abdomen. Physical examination revealed a firm and freely moveable mass (15 ${\times}$ 9 cm) in the right side of caudal abdomen. Thoracic radiographic findings revealed a few well circumscribed nodules. This case was diagnosed as mammary gland squamous cell carcinoma with pulmonary metastasis based on histopathological and immunohistochemical findings. This type of mammary gland tumors is uncommon in wild animals. The patient survived for one month after the diagnosis. This is the first case report of mammary squamous cell carcinoma in a collard peccary and the second case report of metastatic carcinoma to lung in a collared peccary.

Synchronous Monomelic Juxta-articular Multicentric Chondrosarcoma -A Case Report- (동시성 단지성 관절 근접 다발성 연골 육종 -증례 보고-)

  • Chondrosarcoma is one of the most common types of primary bone sarcoma. With the exception of the mesenchymal subtype, chondrosarcomas are usually low-grade lesions and rarely show multicentricity or distant metastasis. Only rare cases of multicentric chondrosarcomas have been reported in association with Ollier's disease and Maffucci's syndrome. To our knowledge, no report has been issued of a synchronous multicentric chondrosarcoma occurrence across a joint. We experienced a 30-year-old man with a synchronous monomelic juxta-articular multicentric chondrosarcoma across a shoulder joint in the absence of pulmonary and visceral metastases. He was treated by curettage and cement filling with allograft in the acromion and wedge resection with cement filling in the proximal humerus. At the 18-month follow-up, there was no evidence of recurrence, and the patient had full range of motion without pain.

Synchronous Double Primary Malignant Neoplasm Consisted of Myxofibrosarcoma of the Forearm and Adenocarcinoma of Rectum (전완부의 점액섬유육종과 직장의 선암이 동반된 동시성 다발성 원발성 종양)

  • Multiple primary malignant neoplasm of the combination of the musculoskeletal system and the gastrointestinal system were very rare. A case of synchronous double primary malignant neoplasm consisted of myxofibrosarcoma of forearm and adenocarcinoma of rectum in a 52 year-old man was found. The patient had pain and swelling on forearm for 1 year. Histologically, the lesion on forearm showed myxofibrosarcoma. In systemic evaluation, the adenocarcinoma of rectum was found by the sigmoidoscopy, and metastasis on lung and intracardiac mass were found by the CT scan. We performed surgical excision and pre and postoperative chemotherapy after pathologic confirmation. He died of pulmonary thromboembolism after postoperative 2 months. We report this case of exceedingly rare combination of the musculoskeletal system and the gastrointestinal system.

  • Primary germ cell tumors of the mediastinum are rare, accounting 1-5 % among all germ cell tumors and 10% of all neoplasms in this area. Approximately 85 % of these tumors occur in men with a mean age 29 years. 'These tumors are mainly found in the anterior mediastinum and appear grossly as large lobulated masses. They are frequently invasive at the time of diagnosis and almost 90% of patients are symptomatic. Primary nonseminomatous germ cell tumor arising in the posterior mediastinum is very rare. We report a case of 37-year old male arising from the posterior mediastinum. Serum tumors markers including alpha-fetoprotein and $\beta$-hCG which are usually elevated in germ cell tumor were not elevated. He was found to have a primary mediastinal embryonal carcinoma with pulmonary metastasis at open exploration. He was treated with debulking surgery and cisplatin-based chemotherapy, died of sepsis after 15 months postoperatively.

  • Sarcoidosis is a somewhat common pulmonary disease, but the concurrence of lung cancer and sarcoidosis in the same patient is very rare. Because sarcoidosis usually presents as mediastinal lymphadenopathies, this concurrence in a lung cancer patient detected radiologically is apt to be misunderstood to be mediastinal metastases, and it is thus considered to be an unresectable disease. We report a case of lung cancer associated with sarcoidosis that developed in a 65-year-old woman who underwent surgery. Radiological studies revealed a $1.9{\times}1.7$ cm mass in the left upper lobe with multiple enlarged bilateral mediastinal lymph nodes (2R, 3a, 4R, 4L, 5, 6, 7, 8R). Pathologic findings showed that the mass was a well-differentiated adenocarcinoma and all of the enlarged mediastinal lymph nodes were granulomas without cancer metastasis. We report this case with a review of the literature.

A Case of Patient with Recurring Non-Small Cell Lung Carcinoma Treated with Samchilchoongcho-Jung in Conjunction with Afatinib (재발성 비소세포성 폐암환자의 Afatinib과 삼칠충초정 병용투여 경과관찰 1례)

  • Objective: The purpose of this study is to report the case of a patient with recurring non-small cell lung carcinoma (NSCLC) taking Samchilchoongcho-Jung with Afatinib. Methods: An NSCLC patient diagnosed with multiple bone and pulmonary metastasis was taking Afatinib (20 mg/day) and suffering from stomatitis caused by the Afatinib. The patient was treated with Samchilchoongcho-Jung (1,500 mg/ day) for 3 months. The tumor size was measured with computed tomography, and laboratory findings, including tumor markers (CEA, Cyfra 21-1), were also followed up. Stomatitis was measured by a numeric rating scale, and adverse events were evaluated using the National Cancer Institute Common Terminology Criteria for Adverse Events (NCI-CTCAE), version 4.0. Results: After combined treatment, stable disease was shown on computed tomography. The tumor marker levels of CEA and Cyfra 21-1 were decreased, and the stomatitis significantly improved. NCI-CTCAE 4.0 showed no adverse events. Conclusion: This case study suggests that Samchilchoongcho-Jung may have a synergic effect, in conjunction with Afatinib, on the treatment of patients with recurring NSCLC.

Surgical Treatment of Myasthenia Gravis (중증 근무력증의 외과적 치료)

  • Myasthenia gravis is a functional neuromuscular disorder with characteristic voluntary muscle weakness. The role of thymus in pathogenesis of this disorder has become apparent that thymectomy in treatment has gained acceptance. Between January 1976 and June 1987, twenty patients underwent thymectomy for myasthenia gravis at the Department of Thoracic and Cardiovascular Surgery, College of Medicine, Korea University. A clinical study that is focused on the factors affecting the operative results was analyzed and the following results were obtained. Among the 20 patients, male to female ratio was 8:12 and the age of onset was ranged from 3 years to 67 years. The chief complaints in order of frequency were as follows; ocular symptoms such as ptosis and diplopia[7 cases], general weakness[4 cases], swallowing difficulty[3 cases], dyspnea[3 cases], dysphasia[1 case], headache[1 case] and dizziness[1 case]. The severity of disease was classified by modified Osserman`s method that Group IIa was 8 cases, Group IIb; 7 cases, Group IIc; 3 cases and Group I; 2 cases. In histopathology of thymus, the most frequent finding was hyperplasia[11 cases] followed by thymoma[4 cases], normal tissue[3 cases] and malignant` thymoma[2 cases]. There were two cases of postoperative complications; one case was wound infection and the other was mediastinitis. One case of malignant thymoma died due to respiratory failure with pulmonary metastasis. There was 16 cases[80%] of improvement after thymectomy as follows; complete remission was 4 cases[20%], marked improvement was 9 cases[45%] and subjective improvement only was 3 cases[15%]. The effect of severity and duration of disease on operative result has statistically significant. The effect of thymus histopathology on operative result was not statistically significant. But there were comparable results between thymoma cases and non-thymoma cases.

Chest Wall Implantation of Lung Cancer after Percutaneous Transthoracic Fine Needle Biopsy -A report of one case- (경피적 폐생검술에 의해 흉벽에 전이된 폐암 -1례 보고-)

  • Percutaneous transthoracic fine needle biopsy is a popular technique in the diagnosis ot'pulmonary and pleural diseases and its complication rate is very low. One of the rarest but potential complications is that of implantation of malignant cells along the needle tract and subsequent development of a chest wall mass . We experienced a case of chest wall implantation of lung cancer after percutaneous transthoracic fine needle biopsy. The patient was a 59 year old female who had undergone right upper lobectomy for squamous cell carcinoma of the lung (T,N,Mo). 6 months after operation, a loculated mass was palpated at the right posterior .chest wall where the percutaneous transthoracic fine needle biopsy had been performed before operation. We carried out wide excision of this mass and confirmed squamous cell carcinoma histopathologically.

Study on Antitumor Activity and Antimetastatic effect of Kamigumguesingihwan(KGSH) (가미금궤신기환(加味金櫃腎氣丸)의 항암(抗癌) 및 항전이(抗轉移) 효과(效果)에 관(關)한 연구(硏究))

  • To evaluate the antitumor activity and antimetastatic effects of Kamigumgusingihwan(KGSH) studies have ken done. The results were obtained as follows: 1. KGSH extracts exhibited a weak cytotoxicity against A549, SK-OV-3, B16-F10, and SK-MEL-2 cell lines. But exhibited potent cytotoxicity against P388 cell line in a dose-dependent manner. 2. The concentration inhibiting adhesion of A549, to complex extracellular matrix up to below 30% of control was recognized at $10^{-3}g/ml$ of KGSH 3. KGSH extracts showed a weak inhibitoty effect on DNA topo-isomerase I from calf thymus. 4. The T/C% was 137% in KGSH treated group in S-180 bearing ICR mice. 5. In pulmonary colonization assay, a number of colonies in the lungs were decreased significantly in KGSH treated group as compared with control group. 6. In hematological changes in B16-BL6 injected C57BL/6, numbers of WBC were decreased insignificantly in KGSH treated groups, and also those of platelet were increased insignificantly in KGSH treated groups as compared with control. 7. In CAM assay, KGSH extracts inhibited angiogenesis at $15{\mu}g/egg $concentration significantly as compared with control. Taken together these results, it is strongly demonstrated that KGSH significantly suppressed tumor metastasis by blocking cell adhesion to extracellular matrix. Therefore, KGSH is expected to be clinically a potent antimetastatic drug for the prevention and treatment of cancer.
