  • The current study aims to propose a reorganization plan for the national emergency management system to improve the current organizational structure for responding to national disasters and emergency situations. As a theoretical framework, the current study identified four key elements of successful disaster response systems: responsiveness, controllability, expertise, and devotedness. On the basis of the four key elements of disaster response systems, this study critically reviewed the current state of the organizational structure of the Korean national emergency response system by discussing the issues inherent in the current structure and by doing a comparative analysis of two high-profile national disaster cases-the Sewol ferry disaster in 2014 and the Gwangsan Rescue of buried people in 2013. Then, this study proposed the reorganization plan for the national disaster response system in which the NEMA is under direct control of the Prime Minister of Korea. It coordinates and controls the related government departments, such as the police, maritime police, and military during the national disaster and emergency situation. This study also proposed a reorganizational plan for the regional disaster response system in Korea. Finally, it was suggested that the status of firefighters should be elevated to the national public servant level in order to achieve organizational efficiency and solve existing problems that come from the current separated systems.

  • Objectives: No studies have examined the association between preterm birth rates and socioeconomic factors in Japan using nationwide statistical data. We analyzed the association between preterm birth rates and household occupation using Vital Statistics data. Methods: Aggregated Vital Statistics data from Japan from 2007 to 2019 were obtained from the Ministry of Health, Labour and Welfare. From the data, the number of births according to year, age group, gestational period, number of pregnancies, and household occupation were used in this study. Crude preterm birth rates and preterm birth rates adjusted by maternal age according to household occupation were calculated for each year. Poisson regression analysis was conducted to evaluate the association between household occupation and preterm births. Results: Unemployed households had the highest preterm birth rate, and households with an occupation classification of "full-time worker 2" (an employee at a large company, civil servant, or board member) had the lowest preterm birth rate throughout each period. Poisson regression analysis revealed that unemployed households were statistically significantly associated with a high preterm birth risk. In contrast, the preterm birth rate adjusted by maternal age remained stable throughout each period regardless of household occupation, and preterm birth rates were found not to have increased in recent years in Japan. Conclusions: Unemployed households had higher preterm birth rates than other household occupations. Further studies investigating the characteristics of unemployed households are needed to identify the reasons for this disparity.

Rural areas have twice longer length of vacancy period per health center. Kangwon-do has the longest average length of vacancy period since 1980(2.79 months/year), and Daejeon has the shortest length of vacancy period(0.21 months/year). Chung-buk has no physician directors. The civil servant's rank for the directorship has promoted from the fifth level to the fourth level since 1990. A comparison between the physician director & non-physician director was made as follows : first, the proportion of physician directors had maintained rather high before 1980 s; 62.5% in 1963, 78.3% in 1970, 70.4% in 1980. It decreased to 44.1% in 1990 and 47.6% in 1997. Instead, non-physician directors has abruptly increased since 1980s (12.4% in 1980, 55.4% in 1990 and 50.8% in 1997). Second, physician directors mainly locate in the urban area(58.0% in 1997), but non-physician directors mainly in the rural area(67.2% in 1997). Third, since 1980, the average length of service for physician directors and for non-physician directors has become similar. Fourth, the mean age of physician directors is 45.1 years, and that of non-physician directors 55.7 years. The latter is 10 years older than the former.
