통합건강관리 프로그램이 지역사회 정신장애인의 신체적, 정신사회적, 영적 건강에 미치는 효과 (The Effects of an Integrated Health Care Program on the Physical, Psychosocial, and Spiritual Health of People with Mental Disorder in Community)

  • Purpose: This study was conducted in order to investigate changes in the physical, psychosocial and spiritual health of people with mental disorder in community participating in the Integrated Health Care Program (IHCP). Methods: This study applied the non-equivalent control group pretest-posttest quasi-experimental design. The participants were 37 chronic psychiatric patients who had been clinically diagnosed with mental disorder and visiting a mental rehabilitation center located in S City (17 in the experimental group, and 20 in the control group). The experimental group participated in the IHCP consisting of 24 sessions for eight weeks. Results: After the intervention, only the participants in the experimental group reported significant improvement in physical (body mass, triglyceride), psychosocial (mental symptoms, depression, self-esteem, ability of problem solving), and spiritual wellbeing when compared with those in the control group. Conclusion: These results indicate that IHCP is effective in improving the physical, psychosocial, and spiritual wellbeing of people with mental disorder. Therefore, IHCP developed in this study is considered a useful nursing intervention for raising the comprehensive health level of people with mental disorder in community.

심리사회적 중재가 간호대학생의 스트레스 지각, 대처방법, 스트레스 반응에 미치는 효과 (Effects of Psychosocial Stress Management on Stress and Coping in Student Nurses)

  • Purpose: The purpose was to examine the effectiveness of a psychosocial stress management program on stress and coping in student nurses. Method: A nonequivalent control group pre-posttest design was used. A convenience sample of 24 students from the Department of Nursing, Wonju College of Medicine, Yonsei University were assigned to experimental (n=12) and control groups(n=12). Stress was measured using the Visual Analog Scale (10 points) and blood cortisol. Coping was measured using a scale modified by So (1995) based on the Ways of Coping Checklist developed by Folkman and Lazarus (1985). Depression was measured using the CES-D by Radloff (1977) and state anxiety, using the State Anxiety Inventory by Spielberger (1976). The psychosocial stress management program included education, relaxation (Korean Dan Jeon-typed Breathing and music), and social support. Result: Results showed for the two groups, 1) a significant difference in stress scores (U= 19.50, p=0.002), 2) no difference in cortisol levels (8am, 4pm), 3) a significant difference in problem-focused coping (U=25.50, =0.007), 4) no difference in emotion-focused coping, and 5) no difference in state anxiety and depression. Conclusion: By utilizing the program, students were able to learn how to control their stress and improve their coping. Therefore with a partial modification, psychosocial stress management can be useful for continual management of stress in student nurses.

작업관련성 근골격계질환과 사회심리적 요인 (Work-related Musculoskeletal Disorders and Psychosocial Factors)

  • Work-related musculoskeletal disorders (WMSDs) have been the most common health problem covered by worker's accident compensation insurance for several years in Korea. Korean government has strengthened related regulations since 2003. People looked forward to decreasing the incidence and prevalence of WMSDs. At first, the expectation could be realized. However, we were bumped against to limit at present. The authors think it is due to the negligence of psychosocial factors for WMSDs. Many researchers reported that the various psychosocial factors were associated with WMSDs or symptoms. Job demand, social support, job satisfaction and decision latitude are the major risk factors in job stress aspect. Work pressure, lack of rest, qualified workload, workload variability and monotonous job are the significant risk factors in work context of work organization. Employment flexibility, downsizing, lean production, contingent work and pay for by the piece are also the risk factors in an organizational context. Furthermore, these risk factors are associated with each other across different dimensions of work organization. Suggestive possible pathways between these risks and WMSDs have been taken note of increasing muscle strain or ergonomic stress and of a cognitive aspect. The authors suggest these risk factors could explain the limitation of the regulation system for WMSDs. In conclusion, the strategy to manage psychosocial factors is the one of the essential approach to prevent WMSDs.

디스트레스 간호중재가 부인암 환자의 디스트레스, 면역 및 삶의 질에 미치는 효과 (Development and Evaluation of the Psychosocial Distress Nursing Intervention for Patients with Gynecological Cancer)

  • Purpose: The purpose of this research was to develop and evaluate a distress nursing intervention for relieving psychosocial distress. Methods: We developed a twelve-week non-pharmacological distress nursing intervention to decrease distress. This intervention includes cognitive behavioral therapy, mindfulness based stress reduction, psychoeducation and supportive therapy. The participants were 27 gynecologic cancer patients who agreed to participate in this study, selected by convenience sampling (experimental group-17 and control group-10). The data collection period was performed from November, 8, 2010 to February, 1, 2011. Data were analyzed by using the SPSS/WIN 18.0 program. Results: There were no difference between the two groups distress score, distress problem and quality of life. The experimental group had significant higher IL-12 and IFN-${\gamma}$ and lower TGF-${\beta}$ between before and after the distress nursing intervention. Conclusion: The findings indicate that the distress nursing intervention was an effective intervention in improving immunologic function of gynecological cancer patients.

아증후군적 양극성 장애 (Subsyndromal Bipolar Disorder)

  • Subsyndromal bipolar symptoms are common during maintenance treatment and appear to be associated with relapse into an episode of the same polarity. This implies subsyndromal symptoms are an important problem in recurrent bipolar disorder and require more additive and infallible therapeutic intervention. Undetected, untreated subsyndromal states lead patients to have poor prognosis and quality of life. The combination of a long undetected illness and significant psychosocial impairment renders early identification and intervention vital for the treatment of bipolar disorders. Methods for early identification includes finding prodromes, using screening tools such as the HCL-32 (Hypomania Checklist-32) and the BSDS (bipolar spectrum diagnostic scale). Various augmentation treatment methods would be needed to reduce subsyndromal symptoms, especially, psychosocial treatment has the potential to help patients address the multiple psychosocial problems associated with this chronic illness. To overcome difficulties of diagnosing subsyndromal disorder and to treat it appropriately, a staging system was suggested by some researchers. It assumes that earlier stages have better prognosis and require simpler therapeutic regimens. Staging may assist in treatment planning and prognosis of bipolar disorder, and emphasize the importance of early intervention. Further research is required in this exciting and novel area.

치료 중인 암환아와 치료 종료 암환아의 사회심리적 적응에 관한 비교 연구 (A Study on the Comparison of Psychosocial Adjustment of Children with Cancer between on and off Treatment)

  • Purpose: The purpose of this study was to compare psychosocial adjustment in children aged 4-11 years with cancer between on and off treatment. Method: The data was collected from parents of children with cancer through interviews with structured questionnaire at the pediatric outpatient department and inpatient ward of one university hospital located in Seoul. The instrument were the Child Behavior Check List(CBCL) developed by Achenbach(1991) and revised to standardize for Korean children by Oh et al(1997). Result: Total behavior problem score for children on treatment was greater than the score off treatment but there was no significant difference. Children on treatment reported higher levels of depression/anxiety, withdrawal, internalizing scores than children off treatment. Social competence score of children off treatment was greater than the score on treatment and the difference of school performance score of children between on and off treatment was not significant. The scores on the withdrawal, somatic complaints, social immaturity, internalizing scales in the cancer group including both on and off treatment was greater than normative findings in the general population. The scores on the school competence of children off treatment were lower than the norms for healthy children. Conclusion: Findings from this study support the importance of nursing interventions to facilitate the adjustment of children with cancer both on and off therapy.

의과대학 학업부적응 학생의 심리사회적 특성과 체계적 지원방안에 대한 탐색 (Exploring the Psychosocial Characteristics and Systematic Support of Academically Maladjusted Students)

  • The purpose of this study was to explore the psychosocial characteristics of academically maladjusted students and the College's current status regarding academic maladjustment based on 4-year medical student cohort data collected from 2018 to 2021 and through interviews with 10 medical education experts using the integrated research method. This study included 223 students who consented to inclusion in the student cohort and participated in the emotional behavior test and college adaptability test of whom 65 students experienced academic maladjustment. Academically maladjusted students had significantly higher scores for social stress, anxiety, sense of inadequacy, attention, hyperactivity, and school maladjustment, and significantly lower scores for relationships with parents, interpersonal relationships, self-esteem, self-confidence, and self-strength. Academic and social integrity, emotional stability, university satisfaction, and university service satisfaction were all significantly lower in the academically maladjusted students than in the non-maladjusted group. The expert interviews indicated that academic maladjustment was mainly recognized as a personal problem, with causes such as lack of motivation and learning methods, vulnerability to stress, lack of social relationships and alienation, lack of support from the family, and insufficient resilience. Systematic support other than counseling for academically maladjusted students is required, and an early diagnosis and preventive intervention are important.

Structural Relationship between Psychosocial Factors Affecting Motivation for Change in Alcoholics

  • The purpose of this study was to investigate the relationship between psychosocial factors, such as self-esteem, stress coping, and social support, and motivation for change in alcohol dependent inpatients. This study included 179 inpatients in the 7clinics for alcoholic in Chonbuk. For the analysis of the study questions, this study utilized the structural equation modeling. The results show that self-esteem was related to motivation for change with mediating role of problem-focused stress coping strategy in alcohol dependent inpatients. The study also reveal that self-esteem and social support was directly associated with the motivation for change. Based on the findings, the importance of programs to promote self-esteem and the involvement of family and acquaintances in the treatment process has been suggested.

산업재해 입원환자를 위한 사례관리실천모형의 효과성에 관한 연구 (A Study on Efficiency of Case Management Practical Model for Industrial Injury Inpatients)

  • This study is to observe the effectiveness of the applied model and to present the improvement plan and directions for development for the case management practical model suitable for the actual condition of Korea Labor Welfare Co. and needs of the industrial injury patients. The concrete purpose of this study is: First, observe the difference of stressor experience and experience degree between the experimental group and the comparative group. Second, observe the difference of stress of the experimental group and the comparative group. Third, find out how the stress affects the support degree and satisfaction degree. Fourth, present the improvement plan of case management model, which can promote the psychosocial rehabilitation of the industrial injury patient based on the research results. The outline of the main research results identified in this study is as follows. The stressors the industrial injury patients perceived are health problems, family matters, the problems concerning hospital recuperation (hospital staff and environmental problems), economical problems, problems of coming back to society, problems with companies, problems with Korea labour Welfare Co. and other problems. And the experience of stressor was prominently lower in experimental group than comparative group in the whole problem, health problem, problems with Korea Labour-Welfare Co. and other problems. The stressor experience degree was conspicuously lower in experimental group in the whole problem experience degree, health problem experience degree, problem with Korea Labour Co. experience degree and other problem experience degree. Besides whether or not the case management is applied is having a prominent affect on the primary factor affecting the stressor experience degree, therefore the patients applied with case management has less stressor than the patient who didn't. The difference of degree of tension experienced by the stressor in both groups, the degree of stress, was not conspicuous in statistics so it shows that the application of case management in this research has not affected the degree of tension. The field which had been the most help was emotional support in help level the experimental group perceived through applying case management about industrial accident patients and recuperation, compensation problem, medical treatment problem, family matters has been helpful in this order. The help level of the whole problem was in higher level than the middle value. The stress factor which affects the case management problem settlement is the whole body of stress. The satisfaction level of help through applying case management was highest in emotional support and family matters, recuperation problem, company problem, compensation problem, and medical treatment problem was the next highest. The satisfaction level of the whole problem was higher than the middle value. The stress factor affecting the satisfaction level of help is the whole body of stress. Therefore to reduce the stress level of industrial accident patients and for them to come back to local societies, we need to reinforce the continuance and responsibility of case management model, increase staff, reinforce the role of counsel and medical treatment, intervene in the patient's plan of leaving the hospital, develop social support system and the need to establish After Care Center.
