Perception and Use of Complementary and Alternative Medicine in Diabetic Patients in Busan Area (보완대체요법에 대한 당뇨병환자의 인식 및 이용현황)

  • The purpose of this study was to investigate the perception and utilization of Complementary and Alternative Medicine (CAM) for diabetic patients in Busan. The subjects were 227 patients at two general hospitals (over 400 bed). This study was performed through the interviewing process using questionnaires conducted from January to April, 2010. Of a total of subjects, 109 patients (48.0%) had taken CAM at least one time. The patients used CAM recognized that they were 'auxiliary medicines' (39.4%), 'supplementary health foods' (32.1%) and 'medicines' (19.3%), but inexperienced patients perceived them as 'supplementary health foods' (29.6%), 'unscientific treatment' (22.6%) and 'medicines' (20.2%), It was significantly different between two groups of patients (p < 0.001). The number of CAM types used for the patients was 51. The CAM types were plant foods (64.3%), dietary supplement (23.6%) and animal diets (12.1%). The patients used an average of 5.9 different kinds of CAM and an average of 3.8 years them and paid \93,345 per month. The patients with diabetic complications and for longer morbidity periods used CAM for significantly (p < 0.05) longer periods. The mean effectiveness scores of the patients used CAM were 3.31/5.00 for efficacy satisfaction, 3.58/5.00 for fewer side effects compared to those of oral drugs, 3.60/5.00 for psychological stability, 3.81/5.00 for easiness to use, and 3.06/5.00 for economic satisfaction. Of the patients that used CAM, 55.9% did not consult with doctors about CAM. More than two-thirds of the patients (77.1%) did not feel the need to consult with doctors.

The Effect of Psychobiographical Intensive Interview on Parent's Insight and Parent-Child Relationship Characteristic (심리전기적 심층면담(PII)이 부모의 통찰수준 및 부모-자녀 관계특성에 미치는 효과)

  • The purpose of this study was to investigate the effect of psychobiographical intensive interview(PII) on the parent's insight, parent-child relationship characteristics and counseling session. For this, the PII was conducted to 10 mothers with school age children. PII was semi-structured form that included the intensive interview for exploring psychobiographical information and interpretation counseling of it. The results showed that the level of insight into parent's child problem and relation to parent's own problem was significantly increased. On the other hand, parent-child relationship characteristics (parental efficacy, parenting attitude, and parenting stress) were found to be significant only in some of the parenting stresses. We investigated the possibility as a counseling tool for PII through counseling session impact assessments. As a result of that, we confirmed that there were positive results in task impacts, relationship impacts, and helpful impacts.

Educating restaurant owners and cooks to lower their own sodium intake is a potential strategy for reducing the sodium contents of restaurant foods: a small-scale pilot study in South Korea

  • BACKGROUND/OBJECTIVES: This study was conducted to evaluate the feasibility of a sodium reduction program at local restaurants through nutrition education and examination of the health of restaurant owners and cooks.SUBJECTS/METHODS: The study was a single-arm pilot intervention using a pre-post design in one business district with densely populated restaurants in Seoul, South Korea. The intervention focused on improving nutrition behaviors and psychosocial factors through education, health examination, and counseling of restaurant personnel. Forty-eight restaurant owners and cooks completed the baseline survey and participated in the intervention. Forty participants completed the post-intervention survey. RESULTS: The overweight and obesity prevalences were 25.6% and 39.5%, respectively, and 74.4% of participants had elevated blood pressure. After health examination, counseling, and nutrition education, several nutrition behaviors related to sodium intake showed improvement. In addition, those who consumed less salt in their baseline diet (measured with urine dipsticks) were more likely to agree that providing healthy foods to their customers is necessary. This study demonstrated the potential to reduce the sodium contents of restaurant foods by improving restaurant owners' and cooks' psychological factors and their own health behaviors. CONCLUSIONS: This small pilot study demonstrated that working with restaurant owners and cooks to improve their own health and sodium intake may have an effect on participation in restaurant-based sodium reduction initiatives. Future intervention studies with a larger sample size and comparison group can focus on improving the health and perceptions of restaurant personnel in order to increase the feasibility and efficacy of restaurant-based sodium reduction programs and policies.

Comparison of Ramosetron Plus Dexamethasone with Ramosetron Alone on Postoperative Nausea, Vomiting, Shivering and Pain after Thyroid Surgery

  • Background: Postoperative nausea and vomiting (PONV), postanesthetic shivering and pain are common postoperative patient complaints that can result in adverse physical and psychological outcomes. Some antiemetics are reported to be effective in the management of postoperative pain and shivering, as well as PONV. We evaluated the efficacy of dexamethasone added to ramosetron on PONV, shivering and pain after thyroid surgery. Methods: One hundred and eight patients scheduled for thyroid surgery were randomly allocated to three different groups: the control group (group C, n = 36), the ramosetron group (group R, n = 36), or the ramosetron plus dexamethasone group (group RD, n = 36). The patients were treated intravenously with 1 and 2 ml of 0.9% NaCl (group C); or 2 ml of 0.15 mg/ml ramosetron plus 1 ml of 0.9% NaCl (group R); or 2 ml of 0.15 mg/ml ramosetron plus 1 ml of 5 mg/ml dexamethasone (group RD) immediately after anesthesia. Results: Incidence of nausea and the need for rescue antiemetics, verbal rating scale (VRS) 1 hour pain value, ketorolac consumption, and incidence of shivering were significantly lower in group R and group RD, than in group C (P < 0.05). Moreover, these parameters were significantly lower in group RD than in group R (P < 0.05). Conclusions: Combination of ramosetron and dexamethasone significantly reduced not only the incidence of nausea and need for rescue antiemetics, but also the VRS 1 hour pain value, ketorolac consumption, and the incidence of shivering compared to ramosetron alone in patients undergoing thyroid surgery.

A Systematic Review of Interventions with Low-Income School-Age Children and Adolescents (저소득층 학령기 아동·청소년 대상 건강관련 중재에 관한 체계적 문헌고찰)

  • Purpose: The purpose of this study was to review health-related interventions in nursing studies for low-income school-age children and adolescents. Methods: Searches among CINAHL, MEDLINE, PubMed, EMBASE, DBpia, and RISS identified 27 intervention studies published from January 2000 to April 2018. Results: Twenty-seven intervention studies were identified: 12 included psychosocial adaptations and 15 included the healthy lifestyle promotion and disease prevention. The settings were mainly schools and community welfare centers. Many studies were based on social cognitive theory and interventions were provided in a group format. Depression, self-esteem, resilience, self-efficacy for vegetable and fruit consumption, physical activity, and health-related knowledge improved significantly after the health-related interventions. However, the findings were inconsistent with regard to anxiety, peer relationships, body mass index (BMI), blood pressure, cholesterol, and glucose. Conclusion: There is potential for enhancing outcomes for psychosocial, physical health, and health-related knowledge among low-income school-age children and adolescents. Integrated interventions addressing the physical as well as psychological health of low-income children and adolescents should be conducted. It would be prudent to consider the ethnicity and family background of the child or adolescent. However, rigorous study designs and scientific validation are needed for further evidence.

Inhibitory effects of xylamine on the arterial contraction in rats (흰쥐 대동맥 수축에 대한 xylamine의 억제효과)

  • The therapeutic efficacy of xylamine in the field of psychological medicine has been recognized for years and the drug is used to treat depression and some other conditions, but little is known about its mechanism of action on vascular system. Therefore, the present study was designed to investigate the influence of xylamine on the contractile responses of isolated rat thoracic arteries to phenylephrine(PE) and potassium chloride(KCl). Xylamine produced a concentration-dependent relaxation in PE-precontracted endothelium intact(+E) rat aortic rings, but not in a KCl-precontracted aortic rings. Also, xylamine inhibited the PE-induced contraction in concentration-dependent manner, but not in the high KCl-induced contraction in +E rings. This concentration-dependent inhibition was suppressed by the removal of the endothelium (-E). The inhibitory effects of xylamine($0.3{\mu}M$) on the PE-induced contractions were suppressed by N(G)-nitro-L-arginine(L-NNA), N(omega)-nitro-L-arginine methyl ester(L-NAME), aminoguanidine, dexamethasone, methylene blue, 1H-[1,2,4]oxadiazolo [4,3-a]quinoxalin-1-one(ODQ), indomethacin, ryanodine, tetrabutylammonium(TBA), lidocaine, procaine and 0 mM extracellular $Na^+$, but not by 2-nitro-4-carboxyphenyl-n,n-diphenylcarbamate(NCDC), lithium, nifedipine, verapamil, 0 mM extracellular $Ca^{2+}$, glibenclamide and clotrimazole. These findings suggest that xylamine could act as a vasorelaxant and direct inhibitor of arterial contraction. This vasorelaxation involves an endothelial nitric oxide (NO)/cGMP (guanosine 3',5'-cyclic monophosphate) pathway or cyclooxygenase system, and an interference with $Ca^{2+}$ release, TBA-sensitive $Ca^{2+}$-activated $K^+$ channels and $Na^+$$ channels.

  • This research was a review and analysis of published articles and theses in Korea on Intervention Programs for patients with Diabetes Mellitus. A comprehensive search of databases was undertaken(Korean studies, Riss4u). using research terms such as"diabetes", "diabetes and education" "diabetes and exercise", "diabetes and intervention", "diabetes and education or exercise." 53 studies were analyzed focusing on type, application method, dependant variable and effect using descriptive statistics. 8 intervention type and 113 dependant variables were used. The most frequently used applied education and counseling and glycometabolism, self-care, self-efficacy the most frequently used dependant variable. The effects of dependant variables no effect or were different effects. Further reserarch in the digital convergence should requires the consideration of Structured content and exercise and the effect of the measurement variables, including the psychological variable effect.

Multidimensional Factors Influencing Health of Low-Income Women: The Moderating Effect of Social Support Resource (저소득층 여성의 건강에 영향을 미치는 다차원적 요인 : 사회적 지지 자원의 조절효과)

  • This study aims to figure out factors influencing health and moderating effect of social support resource between those factors and health of low income women in diverse spheres. The data of low income women were 239 cases and were utilized SPSS ver. 21 program. The main results of this study are as follows: firstly, health of low income women is identified medium level. Secondly, education level, self-efficacy and social support resource are related to the self-rated health of low income women. Especially, social support resource functions as moderators on between education and self-rated level of the health of low income women. Consequently, the findings suggest the need to expand the social intervention range to promote health of low income women from cost benefit to psychological support such as capacity building for self-management and establishment of safety-net to extend social support. And subsequently, the time attribute of the factors related to low-income women's health and the diverse characteristics of the subjects should be considered.

과학고등학교 학생들의 수학불안감소와 수학성취도 향상을 위한 인지/행동 훈련의 효과

  • 'I'his study investigated students' attitude toward mathematics. and how behavior/cognitive training affects level of math anxietv and level of math achievement. Subjects were all the freshmen attending Taejon Science High School, and they were given Mathematics Attitudes Scale and Attributional Style Questionnaire prior to and post training sessions. Twenty out of 84 freshmen voluntarily participated in nine sessions of training program. Participants were asked to do self-evaluation. Math achievement was measured prior to and post training. and was compared between two groups. Training program utilized behavior/cognitive approach. such as understanding one's feeling through muscle relaxation, breathing and meditation; modifying negative attributional style; imitating effective cognitive strategies for math problem solving, and so on. 'I'he result shows that students' math confidence in general was relatively low out of expectation, a nd they perceived teachers not supporting their math abilities :IS much as expected. On the other hand, students in general had strong math achievelment needs, and considered math utility very high. Sex difference was seen in the attitude toward female math abilities, to result that female students had more positive perception than male students. Female students of 'I'aejon Science High School seem free from conventional idea about female abilities including theirs. Participants' ~attitude change was compared with non-participants. and participants showed statistically significant change in their math confidence, and also in their math achievement. Participants had much higher math confidence and ~achievement than non-participants. And, they showed increased level of perceiving teachers' expectation. more realistic in needs, and more involvement in math. Math achievement was found positively related to math confidence, and participants' math achievement change was explained by their belief in math utility. Not only training program effect hut also participants' voluntary involvement and teacher\ulcorner' support of the program and participation seem to increase their math achievement. Based upon the result of study it was suggested that behavior-/cognitive training program be provided along with academic curricula for gifted students of Korea to help their emotional and psychological development enhance the efficacy of their cognitive learning.

Study on Diseases Scope of Prescriptions Related with the Palpitation in "Shanghanlun" ("상한론"에 나타난 계(悸)와 관련된 처방들의 현대 질환 범위 고찰)

  • This article is a study on palpitation of which disease cause, disease mechanism and formulas were analyzed with reference to annotations on "Shanghanlun" and "Jinkuiyaolue". And the scope of modern diseases related with palpitation was drawn by research on clinical papers. The source books are "Zhujieshanghanlun" and "Jinkuiyaoluefanglun" and the clinical papers are searched in China Academic Journals(CAJ) of China National Knowledge Infrastructure(CNKI). 13 clauses in "Shanghanlun" and 9 clauses in "Jinkuiyaolue" and 12 formulas are related with palpitation. Disease mechanisms of palpitation were classified as yang deficiency, yin deficiency, qi deficiency, blood deficiency, retained fluid, cold, etc and these days, qi stagnation, phlegm turbidity, blood stasis and fire heat are also considered as disease mechanisms. Modern diseases related with palpitation are arrhythmia(extrasystole, atrial fibrillation, bradycardia, tachycardia, sick sinus syndrome, atrioventricular block), vascular diseases(arterial occlusion, phlebothrombosis, Buerger's disease, coronary artery disease, vasculitis), blood pressure disorder(hypertension, hypotension) and heart diseases such as heart failure, angina pectoris, myocardial infarction, myocarditis, cardiomyopathy, pericardial effusion. And diseases related with psychological change(cardiac neurosis, anxiety neurosis, neurosis, depression, hyperthyroidism, hypothyroidism), pyrexia, anemia, drug intoxication, etc are also related with palpitation. Zhen Wu Tang showing an efficacy in dilating blood vessels and strengthening cardiac function, Wuling Powder with diuretic effect and Fried Glycyrrhizae Decoction acting on the ${\beta}$ receptor are applied to heart failure in different ways. Fried Glycyrrhizae Decoction(308 cases), Zhen Wu Tang(154), Wuling Powder(54), Xiao Chaihu Tang(34), Sini San(20) are reported to have been clinically applied to cardiovascular diseases and Zhen Wu Tang and Wuling Powder mainly applied to heart failure, Fried Glycyrrhizae Decoction, Lizhong Wan, Sini San and Zhen Wu Tang chiefly applied to arrhythmia related diseases. This study focuses on the general research and consideration on clinical applications and is a preliminary study to understand relations between Korean Medicine's symptoms and categories of modern diseases.
