  • Objective : Stroke is a serious disease despite of the recent improvement in medical treatment. Hence, identification of modifiable risk factors for stroke is important. This case-control study was done to demonstrate that relation between risk factors(familial history of stroke, hypertension, diabetes mellitus and heart disease), lifestyle factors(alcohol drinking, psychologic stress, exercise, liking salty and hot foods) and the incidence of stroke. Methods : We interviewed 214 stroke patients(116 in men, 98 in women) as case group and 205 non-stroke patients(118 in men, 87 in women) as control group from January 1, 2001 to July 31, 2001 in oriental hospital of Kyung Hee medical center. We investigated risk factors, lifestyle factors of stroke in all patients and stroke-subtype of case group. Stroke-subtype consists of cerebral infarction and cerebral hemorrhage. Results : The percentage of familial history in the case group was significantly higher than that of control group, especially in the patients of cerebral infarction. The most ordinary preceding disease was hypertension. And the preceding disease of hypertension, diabetes mellitus was associated positively with the risk of stroke. The percentage of exercise was significantly higher than that of control group. The percentage of alcohol drinking, psychologic stress within 1 week and liking salty and hot foods was higher in the case group. However, there was no significant difference between case and control group. Conclusion : According to the above results, we found that familial history of stroke, preceding disease(hypertension, diabetes mellitus) affected to the incidence of stroke and exercise might be predisposing risk factors for stroke.

  • The objective of this study was to determine the effect of a psycho-educational group intervention in reducing psychologic distress and enhancing coping in Korean breast cancer patients. The patient selection criteria were age younger than 70 years, having any postoperative adjuvant therapy, and surgery undergone within the previous 12 months as of the start of the study and there were 70 patients eligible for this criteria. They were randomized into three groups; experimental group(24 patients), wait-list control group(24 patients) and wait-list group(36 patients). We conducted a 8-week, structured, psychosocial group intervention, which used psycho-educational strategies combining education and psychological support. Subjects were assessed for psychological distress and coping by administering the Beck Depression Inventory(BDI) and the Ways of Coping Checklist-Revised(WCCL-R) at the baseline and after 8 weeks. Forty eight patients were participated and thirty five patients completed the study. The experimental group had significantly lower scores than the controls for depression on the BDI(p=.012) after 8-week intervention. However, coping did not show a significant difference on WCCL-R after the intervention. Despite of some limitations, the results of this study suggest that a short term psychosocial group intervention produces a significant improvement in the quality of life of patients with primary breast carcinoma in Korea in terms of managing depression.

  • Orthognathic surgery, ie, the surgical correction of congenital and acquaired anormalities of the jaws, is currently a field of vigorous activity among oral surgeons. Surgical correction of maxillofacial deformities benefits the patient both psychologically and functionally. Facial mutilation may precipitate adverse psychologic effects. Function is frequently impaired by malrelation of the jaws. In addition, speech, specifically articulation, is often handicapped in patient with severe jaw deformities. Therefore, in dealing with these patient, above mentioned problem should be considered deliberately. In this regard, the authors have treated 2 cases of mandible defect caused by gunshot and explosives with iliac bone graft. In both cases, the patient had Tantalum plate on defect area. But it became lose and gave functional disturbances. So, the authors have removed Tantalum plate and performed bone transplantation with autogenous soild iliac bone graft on that area with good results.

  • Purpose: The purpose of this study was to determine whether a relaxing music was effective in reducing stress response of patients admitted to a coronary care unit with the diagnosis of acute myocardial infarction. Method: The research design was the chosen convenience sample of a randomized control group pre-posttest quasi-experimental design. Forty patients, 20 for an experimental group and another 20 for control group. The study was to provide the patients three different kinds of relaxing music. The experimental group listened to relaxing music for a 20-minute one time a day for 3days. The control group was just provided with a 20-minute period of rest. Test for hypothesis was done by repeated measured ANOV A. Result: The experimental group which received relaxing music showed a significantly lower level of electrodermal response, myocardial oxygen consumption and respiration rate, and a higher level of peripheral skin temperature than the control group. The experimental group which received relaxing music showed a significant reduction in state anxiety than the control group. Conclusion: The results showed that relaxing music is an effective nursing intervention for reducing physiologic and psychologic stress response of the patients with acute myocardial infarction in a critical intensive care unit.

  • Insomnia is a disorder of initiation and maintenance of sleep that is derived from multiple factors such as psychologic, physiological and environmental problems. A number of stroke patients suffer from insomnia classified as one of the sleep disorders associated with physical illness and on the contrary insomnia may have profound deleterious effects on the natural course of stroke. Sedative-hypnotics including benzodiazepine and non-benzodiazepine have widely been used in chronic insomniacs. However, most hypnotics cause dependence, tolerance, impaired daytime function and rebound insomnia. Therefore, we are looking forward to proposing an effective oriental treatment for insomnia. We report two cases of insomniacs, treated with Changpoulgeumtang. After the treatment, insomnia and other accompanied symptoms were improved.

  • We experienced a case of occupational lead poisoning employed in a secondary lead smelting plant for 12 years. The patient was 39-year-old male and had been felt dizziness, recent memory impairment and intermittent severe abdominal pain for 2 years. On admission, blood lead level was $92.9{\mu}g/dl$, urinary lead level was $19.9{\mu}g/l$ and zinc protoporphyrin level was $226.0{\mu}g/dl$. On the blood test, hemoglobin was 10.6g/dl and showed normocytic normochromic anemia. There were no abnormal findings in the biochemical and hormonal tests. Decrease of I.Q. and use of words in speaking were found in the psychiatric and psychologic examinations. We observed the finding of motor polyneuropathy in the nerve conduction velocity test. Computed tomographic finding showed calcification lesions in the basal ganglia, dentate nuclei, caudate nuclei, and especially characteristic multiple calcifications were located in the subcortical white matter.
