정신신경용계 의약품의 치료군 중복처방 평가기준(안) 개발 (Development of Criteria for Therapeutic Duplications of Psycho-Nervous System Drugs)

  • Therapeutic duplication (TD) is frequently reported inappropriate drug use in healthcare settings in Korea. This study was aimed to develop TD criteria for psycho-nervous system drugs (KFDA classification 117 and 119). ATC classification was used to determine therapeutic and/or chemical similarities among the 93 ingredients reviewed. Clinical practice guidelines, textbooks and product labels were referenced for principles and evidences of possible drug combination usage. 16 groups that listed ingredients to be considered as TD were established and the criteria would be helpful to prevent TDs.

  • 본 연구는 3 단계 각성 모드에 따른 자율신경계 반응 모델 구현에 대한 연구이다. 정신생리학(Psycho-Physiology)에서의 각성 시스템 모델은 Boucein(1992, 2006)에 의해 제안되었다. 각성 시스템 1 은 편도체(amygdala)활동과 관계한, 무의식수준에서 주의를 집중시키는 역할을 한다. 각성 시스템 2 는 해마(hippocampus)활동과 관계한, 인지적 각성 단계로서 상황-반응(situation-reaction) 대한 연결을 담당하는 역할을 한다. 각성 시스템 3 은 기저핵(basal ganglia)활동과 관계한, 몸운동(somatomotor)활동과 관계된 뇌 영역의 준비를 활성화시킨다. 각성 시스템은 상황의 변화나 특정 자극이 일어나면, 그에 해당하는 각성 모드에 의해 자율신경계(automatic nervous system)반응이 발생한다. 따라서, 자율신경계반응의 분석을 통하여 각성 시스템의 단계를 평가할 수 있다. 본 연구는 자율신경계반응 중 GSR(galvanic skin Response)과 SKT(skin temperature)분석을 통하여 3단계 각성 모드인 직관적, 인지적, 행동적 모드를 평가하는 연구이다.

Affective Computing Among Individuals in Deep Learning

  • This paper is a study of deep learning among artificial intelligence technology which has been developing many technologies recently. Especially, I am talking about emotional computing that has been mentioned a lot recently during deep learning. Emotional computing, in other words, is a passive concept that is dominated by people who scientifically analyze human sensibilities and reflect them in product development or system design, and a more active concept that studies how devices and systems understand humans and communicate with people in different modes. This emotional signal extraction, sensitivity, and psychology recognition technology is defined as a technology to process, analyze, and recognize psycho-sensitivity based on micro-small, hyper-sensor technology, and sensitive signals and information that can be sensed by the active movement of the autonomic nervous system caused by human emotional changes in everyday life. Chapter 1 talks about overview and Chapter 2 shows related research. Chapter 3 shows the problems and models of real emotional computing and Chapter 4 shows this paper as a conclusion.

감사증진프로그램이 만성 조현병 환자의 심리사회적·신체적 건강에 미치는 효과 (The Effects of Gratitude Enhancement Program on Psycho-social and Physical Health of Chronic Schizophrenia)

  • Purpose: The purpose of this study was to identify the effects of the Gratitude Enhancement Program on self-esteem, interpersonal relationships, gratitude disposition, subjective happiness, stress index in patients with chronic schizophrenia. Methods: The research was designed for a nonequivalent control group before and after the test design. The subjects of this study were 29 patients in the experimental group and 28 patients in the control group. The Gratitude Enhancement Program was conducted three times a week for a total of nine sessions. Data were collected with questionnaires and measured with autonomic nervous system response (stress index) and was analyzed by t-test, chi-square test, Fisher's exact, and repeated measures ANOVA with the SPSS/WIN 23.0 program. Results: There are no significant differences in the homogeneity test of a dependent variable between groups. The experimental group showed significant higher scores of self-esteem, interpersonal relationships, gratitude disposition, and subjective happiness than the control group. There was no significant difference in the stress index between two groups. Conclusion: Based on the results of this study, this Gratitude Enhancement Program can be a good nursing intervention to improve self-esteem, interpersonal relationship, gratitude disposition, and subjective happiness for chronic schizophrenia patients.

국내 건강보험심사청구자료에 근거한 다빈도 치료중복 의약품 약효군 분석 (Analysis of Frequent Therapeutic Duplication Drug Classes Based on National Health Insurance Claimed Data in Korea)

  • Therapeutic duplication of prescriptions is the most frequently reported inappropriate drug use in Korea. To prevent significant problems during drug prescribing and dispensing, prospectively, development of standard including drug lists considered as therapeutic duplications for the prioritized drug classes first would be necessary. This study was aimed to analyze frequent drug classes of therapeutic duplications by healthcare providers in clinical practice settings. National health claims data for drug review and reimbursement (1,426,065 prescriptions dated March 19, 2008) were analyzed. Therapeutic duplication was defined as the prescription including more than 2 ingredients belonging to the same KFDA drug classification numbers that considered to have therapeutic similarities. The following 3 drug classes were mostly frequent therapeutic duplication classes: 114 anti-pyretics, analgesics and anti-inflammatory drugs; 117 drugs for psycho-nervous system; 141 Antihistamines. About 3.5% of overall prescriptions analyzed showed therapeutic duplications. This result might be starting step to develop DUR therapeutic duplication standard.

바이오휘드백을 이용한 점진적 근육이완훈련이 스트레스반응과 면역반응에 미치는 효과 (The Effect of Progressive Muscle Relaxation using Biofeedback on Stress Response and Natural Killer Cell in first Clinical Practice of Nursing Students)

  • Increasingly nursing science is embracing the concepts and methodology derived from psycho-neuroimmunology. It has been previously shown that stress increases and immune function declines in students undergoing examinations. To date, however, no many studies have been reported on stress levels, immune function and interventions in Korean students undergoing their first clinical nursing rotation. It was proposed that nursing students during their first clinical rotation experience increase in stress because of the novelty of the situation and their lack of clinical knowledge. It was also hypothesized that biofeedback and progressive relaxation, methods of self-regulation of involuntary autonomic nervous system responses, would reduce the stress response. The purpose of this study is to test the effectiveness of progressive muscle laxation using biofeedback The effectiveness of the experimental methods was tested by measuring the degree of symptoms of stress (SOS) and the values of ephinephrine, pulse rate, blood pressure and natural killer cells. The subjects of this study were thirty nursing students divided into two groups: experimental group was progressive muscle relaxation group using biofeedback and control group. This study was conducted for 8 weeks of clinical practice. Biofeedback training was done by software developed by J&J company (1-410 form for progressive muscle training). Progressive muscle relaxation training according to Jacobson's Theory was done by messaged word from biofeedback. The data was analyzed using Chronbach' ${\alpha}$ and t-test of the SPSS program and the significance level of statistics was 5%. The results of the study were : 1) The progressive muscle relaxation training using biofeedback was effective for the reduction of symptoms of stress(t=-4.248, p<.001) under clinical practice stress conditions. 2) The progressive muscle relaxation training using biofeedback was not effective for the values of epinephrine(t=-1.294, p=.206). 3) The progressive muscle relaxation training using biofeedback was effective for the reduction of systolic blood pressure (t=-2.757, p=.01). 4) The progressive muscle relaxation training using biofeedback was effective for the reduction of diastolic blood pressure (p=-2.032, 0=.05). 5) The progressive muscle relaxation training using biofeedback was not effective for the reduction of pulse rate(t=-15, p=.988). 6) The progressive muscle relaxation training using biofeedback was effective for the maintenance of natural killer cells (t=2.381, p=02). The first clinical rotation for student nurses is a stressful experience as seen by the rise in the SOS in the control group. Biofeedback using progressive muscle relaxation were effective in preventing the rise of symptoms of stress and the blood pressure means when comparing the pre to post clinical experience, The mean natural killer cell count was depressed in the control group but not significantly different in the experimental groups, It is proposed here that stress via the hypothalamic - pituitary - adrenal axis suppressed the NK cell count whereas the relaxation methods prevented the rise in stress and the resulting immune depression. We recommend relaxation techniques using biofeedback as a health promotion technique to reduce psychological stress. In summary. the progressive muscle relaxation training using biofeedback was effective for the reduction of symptoms of stress under clinical practice stress conditions.
