A Clinical Study on Softening E.C.T. & Comparison of Propofol and Pentothal as Anaesthetic Agents on Seizure Duration (연성 전기 경련 요법의 임상적 고찰과 마취제 pentothal과 propofol에 따른 경련기간의 비교에 관한 연구)

  • The authors performed this preliminary study to investigate the effect of softening E.C.T. and propofol was compared to pentothal for induction of anaesthesia for E.C.T. on seizure duration. The results were follows ; 1) E.C.T. was performed in 60 psychiatric inpatients who were admitted during the study period. Of them 51.7% were diagnosed as schizophrenia, 21.6% as major depressive disorder, 16.7% as bipolar I disorder, manic and 10% of others. 2) Mean number of E.C.T. was 12.2 times a patient. 3) The most common target symptoms were persecutory delusion in schizophrenia, psychomotor retardation or agitation in major depressive disorder, and violent aggressive behavior in bipolar I disorder, manic. 4) Pre-ECT medication usually used were atropine $0.0093mgkg^{-1}$, pentothal $2.76mgkg^{-1}$ or propofol $1.42mgkg^{-1}$. 5) The duration of seizure, as measured clinically, was reduced with propofol(20.5 sec) in comparison with pentothal (35.7 sec)(p<0.001). This suggests the possibility that additional treatments may be needed for the same clinical effect in psychiatric illness when propofol is used as the induction agent.

  • Objectives : In the psychiatry, structured interview is very important tool to be used in epidemiological or psychopharmacological studies. However, investigators often find that the administration of comprehensive diagnostic interviews is time-consuming and expensive to be used in clinical or research settings. Considering these points, Sheehan and his colleagues developed MINI (Mini International Neuropsychiatric Interview) to meet the need for a brief, reliable, and valid structured diagnostic interview for psychiatric disorders. The MINI has been translated into many languages and used in many countries. Therefore, we translated the MINI into Korean and determined its validity in this study. Methods : Twohundred seventy patients and normal subjects participated in the validation of the MINI versus an expert's professional opinion. Schizophrenia and other psychotic disorders, mood disorders including major depressive disorder and bipolar disorder, anxiety disorders including panic and other phobic disorders, and alcohol dependence were included in this study. The validity was obtained by examination whether MINI based diagnoses were compatible with diagnoses by expert psychiatrists. Results : The range of Kappa values was 0.22 (somatoform disorder) to 0.93 (bipolar disorder - past). Overall agreement between MINI and expert's diagnoses were good. The Kappa values for anxiety disorders seemed to be higher than those for other disorders. Conclusion : The MINI Korean version has the good validity. It also has potential applications as a diagnostic tool for psychiatric disorders.

Regional Cerebral Perfusion Abnormalities in Autistic Disorder : Statistical Parametric Mapping Analysis (자폐 장애에서의 국소 대뇌관류 이상)

  • Objectives : Autism is a well-known psychiatric disorder that is presumed to have a neural basis. To investigate the underlying neurofunctional abnormalities of autism, the authors performed single photon emission computed tomography (SPECT) on children with autism. Methods : Fifty-five children with untreated autism (47 boys and 8 girls, mean age=$50.6{\pm}20.28$ months) were selected from among the patients visiting the child and adolescent psychiatric clinic of Seoul National University Hospital. Psychiatrists had diagnosed the participants according to the DSM-IV criteria for autistic disorder and the Childhood Autism Rating Scale (CARS) criteria for a diagnosis of autism. All participants were examined using 99mTC-HMPAO Brain SPECT. Using statistical parametric mapping (SPM) analysis, we compared the participants' SPECT images to standardized SPECT images of normal children, which had been retrospectively selected by the authors, on a voxel by voxel basis. Voxels with a p-value less than .001 were considered to be significantly different. Results : The autistic group showed significant hypoperfusion in the right medial frontal gyrus, right precentral gyrus, and left precuneus gyrus. In addition, they showed no significant hyperperfusion areas when compared to the control group. Conclusion : The findings of hypoperfusion in the medial-frontal lobe and precuneus are accord with hemodynamic abnormalities that have been already reported. Therefore, these findings are compatible with the recently suggested "theory of mind" hypothesis and the disturbances in attention shifting that have been observed in autistic children.

Neurocognitive Dysfunction in Patients with Obsessive-Compulsive Disorder in Association of Duration of Untreated Illness : A Preliminary Study (강박장애의 치료받지 않은 유병기간에 따른 신경인지기능 이상 : 예비연구)

  • Objectives Obsessive-compulsive disorder (OCD) is a chronic and disabling psychiatric disorder. The duration of untreated illness (DUI) has been suggested as one of the predictors of clinical course and outcome in various psychiatric disorders. There is increasing evidence that cognitive dysfunction is associated with the prognosis of OCD. The aim of this study was to investigate the influence of DUI on the neurocognitive functions in patients with OCD. Methods Sixty-two patients with a DSM-IV diagnosis of OCD from the outpatient clinic were included in this study. We defined the short DUI if the DUI was 2-year or less and the long DUI if it was longer than 2-year. Neurocognitive functions were assessed by visuospatial memory function test and 4 subsets of K-WAIS such as vocabulary, arithmetic, block design and picture arrangement. Differences in neurocognitive functions as well as clinical variables between OCD patients with short DUI and those with long DUI were investigated. Correlation analyses were also performed to determine the correlation between DUI and neurocognitive functions. Results Compared with the short DUI group, the long DUI group performed worse in the block design test, which measures executive function. The long DUI group also had a higher level of compulsive symptom severity than the short DUI group. However, the DUI was not correlated with neurocognitive functions. Conclusions Findings in this preliminary study suggest that the long DUI in patients with OCD is associated with more severe executive dysfunction. Studies with larger samples and longitudinal design are needed to further confirm the prognostic role of the DUI in OCD.

A Review of Prevention and Treatment of the Crimes on the Characteristics of Mental Illness (정신질환의 특성에 따른 범죄 예방과 치료에 관한 소고)

  • This study reviewed the important preceding studies of prevention and treatment of the crimes and therapies on the characteristics of mental illness, trying to investigate and identify more effective ways to prevent, decrease the crimes of the mentally ill and treat them. The main results were as follows. First, relationships between mental illness and crimes have been inconsistently reported, to investigate environmental stresses and triggers of the mentally ill additionally. Second, the term 'mentally illness' was to be defined more specifically, such as severe psychiatric disabled, antisocial personality disorder, intellectual disability, anger·impulse control disorder, and drug abuse. If medication and psychosocial treatments were properly treated to the severe psychiatric disabled, their crime incidence and recidivism will be decrease enough to live with their neighbors in community. But the ways and processes of treating antisocial personality disorder, intellectual disability, anger·impulse control disorder, and drug abuse were very different from it, requiring more intense psychological, medical and biological interventions and social seclusion. The national campaigns and projects for their human rights and community care will be needed to promote the therapeutic effects for them. The limitation and future tasks were discussed.

Influences of mental health characteristics and admission experiences on perceived coercion (정신장애 특성과 입원과정의 경험들이 지각된 강요에 미치는 영향)

  • Coercive treatment in mental health has undergone an immense period of philosophical and clinical debate and yet it remains as a highly important issue in which ideology and practice contradict each other. this study focused on the perceived coercion of the persons with mental disorder and analyzed how the characteristics of mental disorder(psychiatric symptoms, psycho-social functions, insight, and the degree of awareness on the need for treatment) and experiences in the process of hospitalization (legal status, coercive measures, and procedural justice) can predict perceived coercion. The participants of this study were 302 patients that has been hospitalized in the psychiatric ward within the period of 4 weeks. 195 participants(64.6%) were male and 106(35.1%) participants were female. MAES, BPRS, GAF, Insight, Legal Status, Coercive Measures, and Need for Treatment were measured. Regression analysis was used to analyze how much perceived coercion can be predicted by characteristics of mental disorder such as the patients' BPRS, GAF, insight, and need for treatment. As a result it showed that among the characteristics of mental disorder insight and awareness of the need for treatment were the main predictors and the characteristics of experiences during hospitalization such as procedural justice, coercive measures, and legal status all displayed significant predictability. As well as implications of results in a practical method of intervention to reduce perceived coercion, the paper discussed issues for limitations and future consideration.

Factorial Validity of the Korean Version of the Illness Intrusive Rating Scale among Psychiatric Outpatients Mainly Diagnosed with Anxiety or Depressive Disorders (불안 및 우울장애를 주요 진단으로 하는 정신건강의학과 외래환자 대상 한국판 질병침습도 평가척도의 요인 타당도 연구)

  • Objectives : The Illness Intrusiveness Rating Scale (IIRS) is a well-validated self-report instrument for assessing negative impact of chronic illness and/or adverse effects of its treatment on everyday life domains. Although extensive literature probed its psychometric properties in medical illness, little attention was paid for its validity for psychiatric population. This study aimed to test factorial structure of the Korean Version of the IIRS (IIRS-K) in a consecutive sample of psychiatric outpatients. Methods : Data set of 307 first-visit patients of psychiatric clinic at Guri Hanyang univ. Hospital were used. Exploratory and confirmatory factor analysis, internal consistency were tested in IIRS-K. We also checked Spearman's correlation analysis between IIRS-K, Zung's self-report anxiety scale and Zung's self-report depression scale. Results : 76.9% of the patients were with anxiety disorder and depressive disorder. The principal component factor analysis of the IIRS-K extracted three-factor structure accounted for 63.2% of total variance that was contextually similar to the original English version. This three-factor solution showed the best fit when tested confirmatory factor analysis compared to the original IIRS, two-factor model of IIRS-K suggested from medical outpatients, and one-factor solution. The IIRS-K also showed good internal consistency (Cronbach's α=0.90) and good convergent validity with anxiety and depression scales. Conclusions : The IIRS-K showed the three-factor structure that was similar but not identical to original version. Overall, this study proved factorial validity of the IIRS-K and it can be used for Korean clinical population.

The Study on Korean Medical Pattern Differentiation of Sleep-Wake Disorders by DSM-V Classification (DSM-V 분류에 따른 수면-각성장애의 한의학적 변증 연구)

  • This study covers pattern differentiation based on Korean medical references, research trend and modern clinical applications about Sleep-Wake disorders of Diagnostic and Statistical Manual of Mental Disorders(DSM-V) published by American Psychiatric Association. Insomnia disorder is mostly caused by yin deficiency of liver-kidney or liver qi depression and main patterns are heart-kidney non-interaction, deficiency-excess complex pattern containing phlegm-heat due to qi stagnation and blood stasis. Hypersomnolence disorder is more due to yang deficiency rather than yin deficiency and it's major pattern is spleen-kidney yang deficiency. Cataplexy is main feature in narcolepsy and corresponds to depressive psychosis or fainting in terms of Korean Medicine and narcolepsy is assumed to be relevant to liver wind. Breathing-related sleep disorders are related with phlegm-fluid retention brought on spleen deficiency with dampness encumbrance. Pattern of circadian rhythm sleep-wake disorders is combined with yin deficiency of liver-kidney or liver qi depression of insomnia disorder and spleen-kidney yang deficiency or dampness-phlegm of hypersomnolence disorder. Yin deficiency with effulgent fire brought on drugs or alcohol is one of main patterns of substance/medication-induced sleep disorder and combined patterns with yin deficiency of liver-kidney and blood stasis or dampness-phlegm-heat are mostly applied clinically. This study drew major and frequently applied patterns of sleep-wake disorders based on Koran medical literature and modern clinical applications. And that can be the groundwork for the task ahead like clinical practice guideline of sleep-wake disorders containing pattern differentiation, diagnosis and prescriptions.

  • Attention Deficit-Hyperactivity Disorder(ADHD) is the most common psychiatric disorder in school-age children which is increasing gradually. It is hard for ADHD children to focus on particular stimulus for a long time. ADHD children are characterized by hyperactivity which shows in behaviors like, talking too much, getting up from their seats without permission, running around, keep on moving their arms and legs. ADHD children have problems with impulse controlling that they tend to fall and get into an accident very often. Thus dentist's attention and care must increase when dental treatment is performed with ADHD children since behavior control, oral hygiene problems, and trauma might be expected. By using sedative drugs and psychological behavior control, chief complaint was resolved in a 8-years-old female diagnosed for ADHD.

A Study on Somatization in Psychiatric Patients (정신과에 자문 의뢰된 환자의 신체화 경향에 대한 연구)

  • To evaluate the simplicity and efficacy of the 7-symptom screen test for somatization disorder, the authors tried 7-symptom screen test to find out the easiness in diagnosing the somatization disorder and to evaluate the simplicity and efficacy of it from Mar 1991 to Feb 1992. The objects were 135 female outpatients who visited Department of Psychiatry, College of Medicine, Yeungnam University. The results were as follows : The discriminant index was over 3.0 for all 7 symptoms and two item accuracy was 89%(sensivity 99%, specificity 77%), three item accuracy was 87%(sensitivity 83%, specificity 90%). In discriminant analysis, the cut off score for the criteria of somatization disorder was 87% when three or more symptoms were checked for 7-symptoms. This result means that 7-symptom screen test is the simple and accurate method for screening and diagnosing the somatization disorder.
