임신 중 만성스트레스가 생후 4주 유서(幼鼠)의 피부장벽에 미치는 영향 (The Effect of Stress During Pregnancy on the Skin Barrier of Mice Four Weeks After Birth)

  • Background and Objectives: The skin barrier protects skin against a harmful environment. Its function includes an antimicrobial barrier as well as a physical barrier. Stress is harmful to the skin barrier and there are many studies on this, but there are few about the effect of stress during pregnancy on the skin barrier of offspring. The aim of this study was to investigate the effect of stress during pregnancy on the skin barrier of offspring by examination with the naked eye, cortisol, weight, TEWL and histologic tests. Materials and Methods: Male hairless mice ten weeks old were coupled with females for three days. After twelve days the females were divided into two groups. We stressed one group and not the other group. The offspring from the non-stressed (N group) and stressed (S group) grew up until four weeks. The S group was exposed to chronic mild stress using the chronic stress model protocol modified method of Wilner and Towell for a week. We made examinations with the naked eye, assessment of cortisol, weight, TEWL, and histologic test to contrast the S group with the N group. Statistical analysis was performed by using Mann-Whitney and Friedman test. Statistical significance was achieved if the probability was less than 5% (p<0.05). Results: 1. By making an examination with the naked eye, S group showed papules, creases and dryness on their skin, but N showed no change compared with normal skin. 2. There was no statistically significant difference between S group and N group in cortisol (p>0.05). 3. There was no statistically significant difference between S group and N group in weight. N group had less weight than S group (p<0.05). 4. There was no statistically significant difference between S group and N group in TEWL (p>0.05). 5. There was no statistically significant difference between S group and N group in TEWL recovery (p>0.05). 6. In TEWL and TEWL recovery, there was statistically significant difference by time between S group and N group (p<0.005). Histologically, S group showed a thicker epidermis and epidermal crest than N group. Conclusions: We found that stress during pregnancy has negative effects on the skin barrier of offspring. Though there were no statistically significant differences between S group and N group in TEWL recovery, we found evidence that the recovery of skin barrier function is interrupted by stress during pregnancy by examination with both the naked eye and histology.

Responses and adverse effects of carboplatin-based chemotherapy for pediatric intracranial germ cell tumors

  • Purpose: Cisplatin-based chemotherapy has been commonly used for the treatment of intracranial germ cell tumors (IC-GCTs). However, this treatment exhibits some adverse effects such as renal problems and hearing difficulty. Carboplatin-based chemotherapy was administered to pediatric patients with IC-GCTs from August 2004 at the Samsung Medical Center. In this study, we assessed the responses and adverse effects of carboplatin-based chemotherapy in pediatric IC-GCTs patients according to the risk group, and compared the results with those of the previous cisplatin-based chemotherapy. Methods: We examined 35 patients (27 men and 8 women) diagnosed with IC-GCTs between August 2004 and April 2008 and received risk-adapted carboplatin-based chemotherapy at the Samsung Medical Center. Patients were divided into either low-risk (LR) or high-risk (HR) groups and a retrospective analysis was performed using information from the medical records. Results: Although hematological complications were common, hearing difficulties or grade 3 or 4 creatinine level elevation were not observed in patients who underwent carboplatin-based chemotherapy. The frequency of febrile neutropenia did not differ between the risk groups. The overall survival was 100% and event-free survival (EFS) was 95.7%. The EFS rate was 100% in the LR group and 90% in the HR group, respectively. Conclusion: Despite their common occurrence in high-risk patients, no lethal hematological complications were associated with carboplatin-based treatment. The current carboplatin-based chemotherapy protocol is safe and effective for the treatment of pediatric patients with IC-GCTs.

IPTV 도입 과정에 대한 지대추구론적 분석 (Rent Seeking Analysis of IPTV Introduction Process in Korea)

  • 이 연구는 신매체 도입 과정에서 나타나는 이해관계자들의 갈등과 그로 인한 정책결과를 경제이론인 지대추구론에 입각하여 분석한 논문이다. 방송산업 분야에서 신매체 도입은 사업권 허가를 통한 진입규제가 이루어지는 만큼 어떤 미디어정책보다도 정부와 사업자의 지대주구 가능성이 높은 분야이다. 광의의 지대추구개념은 이익집단이 정부를 대상으로 요구하는 협의의 지대추구개념에서 나이가 관료나 정부가 주도적으로 지대추출을 하는 활동까지를 포함하며 지대추구라고 정의하고 있기 때문이다. 신매체 도입 과정에서 가장 심각한 갈등을 초래하였고 갈등의 양상이 사업자와 정부 주제가 모두 관여된 2008년 IPTV 도입 사례를 대상으로 지대추구의 유형 및 특징을 분석하고 그러한 행위가 궁극적으로 정책이나 시장에 어떤 결과를 가져왔는지 평가해보았다. 연구 결과 IPTV의 도입은 사업자와 규제기관 모두가 각자의 지대를 추구하였으며, 그 과정에서 나타난 각종 로비와 자원 낭비 등을 고려할 때 기존 연구와 달리 신매체 도입 과정에서 나타나는 정책 참여자들의 지대추구행위는 긍정적이기보다는 부정적 결과를 가져왔다고 결론 내릴 수 있다.

2-7세 영유아 자녀의 IPTV 동영상 시청 행태와 이용에 대한 어머니의 인식 (Analysis of IPTV VOD Usage Behavior of 2 to 7 Years Old Children and Mother's Perceptions about Their Child's Usage)

  • 본 연구는 최근 가입자가 급격하게 늘어나고 있는 IPTV를 대상으로 가정에서 IPTV 유아 동영상 이용 실태와 자녀의 IPTV 유아 동영상 이용에 대한 어머니의 인식에 대해 살펴보고, 어머니의 배경 변인이 자녀의 IPTV 유아 동영상 이용 시간에 영향을 주는지 여부를 분석해보았다. 이를 통해 유아 자녀의 IPTV 유아 동영상 이용 현황을 진단하고, 주 양육자인 어머니가 IPTV를 좀 더 교육적으로 활용할 수 있는 지도방안을 마련하기 위한 기초자료를 제공하고자, 전국의 IPTV 이용자 중 2세~7세 유아 자녀를 둔 어머니 500명을 대상으로 자료를 수집하고 분석하였다. 연구 결과로는 첫째, 자녀가 IPTV 유아 동영상을 처음 이용한 시기는 만 1세가 36.6%로 가장 높게 나타나 영아 때부터 IPTV를 보여주는 것을 알 수 있고, 가정에서 IPTV 유아 동영상을 이용하는 가정이 93.4%로 매우 높게 나타났고, 유아 동영상을 일주일에 4일 이상 이용하는 경우가 40.8%로 높게 나타나 매우 많이 활용하는 것으로 조사되었다. 이용 시간을 보면 주중에는 주로 오후에, 주말에는 주로 오전에 이용하는 경향을 보여주었고, 유아 동영상의 주 이용 장르는 애니메이션이 43.4%로 가장 높게 나타났다. 둘째, IPTV 유아 동영상 이용에 대한 어머니의 인식에 대한 조사 결과, 가정에서 IPTV 유아 동영상을 이용하는 가장 큰 이유는 자녀가 원하기 때문이라고 응답한 경우가 59.6%, 유아 동영상 시청시 어머니들이 가장 우려하는 사항은 과도한 TV의존이 66.0%로 조사되어, 대부분의 어머니들은 자녀의 TV의존에 대한 염려를 가지고 있음에도 불구하고 유아 자녀의 요구에 의해 IPTV 유아 동영상을 보여주고 있는 것을 알 수 있다. 셋째, 어머니의 학력 및 월평균 가계소득과 IPTV 유아 동영상이용 시간과 유의한 관계가 있는 것으로 나타났다.

fMRI를 이용하여 수지굴신운동(手指屈伸運動)과 조해(照海)(KI6) 자침(刺鍼)에 의(依)한 대뇌운동피질(大腦運動皮質)의 활성변화(活性變化)에 관(關)한 비교(比較) 연구(硏究) (The New Finding on BOLD Response of Motor Acupoint KI6(照海) by fMRI)

  • Introduction : Recent studies Suggested that there is a strong correlation between acupuncture stimulation and its related cortical activation. Anther study showed that either positive or negative BOLD effects could be observed depending on anatomical structure in acupuncture stimulation. In ttis study, we investigated a new acupoint $KI_6$ (照海), which was known as motor-related acupoint and obtained an evidence that the stimulation of $KI_6$ resulted in either negative or positive BOLD response to stimulation. Methods & Results : 1. Subjects and paradigms : Two separate stimulation paradigms were performed on five healthy (aged 22-23 yrs) in this study. First, the paradigm of acupuncture stimulation was that the acupuncture needle was inserted in acupoints $KI_6$, which is located in lateral side of the foot and then continuously twisted(補瀉를 除外한 捻轉法) for 70 seconds for 10 cycles of activation. During rest period (70 seconds), the needle was completed removed from acupoint. Total 60 cycles were performed and 10 images were obtained per cycle. Second, nonacupoint was randomly selected and the same paradigm was performed as acupoint stimulation. The stimulation protocol comprised 10 cycles of alternating. activation and rest (10 images per cycle). Total 60 cycles were performed and each cycle take about 1.5 sec for motor task. Subjects take an at least 15 minutes break before starting anther paradigm. 2. fMRI mapping : Multi-slice functional images were obtained on a 1.5T Magnetom Vision MRI scanner (Simens Medical, Erlangen, Germany) equipped with high performance whole-body gradients. The BOLD T2 * - weighted images were acquired with acho planar imaging sequence (TR = 1.2 sec, TE = 60 msec, and flip angle = $90_{\circ}$). The other sequence parameter are : FOV = 210 mm, matrix=$64{\times}128$ or $64{\times}64$, slice number=10 and slice thickness = 5 or 8 mm. the anatomic images were obtained with Spin-echo T1-weighted images. The resulting images were then anaiyzed with STIMULATE (CMRR, U. of Minnesota) to generate functional maps using a student T-test (p < 0.005) and cluster analysis. Both positive and negative response were evaluated. Conclusions : We have observed the activation of the motor cortex by stimulating motor-related acupoint ($KI_6$). Among five subjects, negative BOLD response was shown in four and positive response in one. All subjects showed positive response to conventional finger flexion-extension task. To understand the detailed mechanisms of correlation between acupuncture stimulation and BOLD fMRI changes and two typs of response, further study strongly required.

광학 현미경을 이용한 산화 그래핀 이미지 분석 조건에 관한 연구 (A Study on Image Analysis of Graphene Oxide Using Optical Microscopy)

  • 광학 현미경 관찰을 통해 산화 그래핀의 형상, 크기 및 두께를 쉽게 파악할 수 있는 광학 관찰을 위한 최적 조건을 확보하고자 하였다. 본 연구에서는 $SiO_2$ 절연막이 300 nm 두께로 도포된 실리콘 기판 위의 산화 그래핀을 하이드라진 증기 환원을 통하여 본래의 모폴로지를 유지한 채 환원된 산화 그래핀의 이미지의 선명도를 증가시켰고, 녹색 필터를 사용한 관찰을 통해 이미지의 대비값을 보다 증대시켰다. 추가적으로 얻어진 광학 이미지를 RGB 채널별로 분리하는 방법을 제안하고 이를 통해 이미지를 분석하였다. 그 결과 하이드라진 증기 환원 처리 및 녹색 파장에서의 광원 하에서 고대비의 이미지 확보가 가능하였으며, 더불어 광학 이미지의 RGB 채널 분리만으로도 선명한 그래핀 이미지를 얻을 수 있음을 알아내었다.

다지역 임상시험의 계획 및 설계에 대한 국제 제도적 동향 분석 (Regulations and Guidelines for Planning and Design of Multi-regional Clinical Trials)

  • Objective: Multi-regional clinical trials have been widely used for accelerating global drug development by multinational pharmaceutical companies. In this study, we aimed to review and analyze the international trends in regulations and guidelines on multi-regional clinical trials by regulatory authorities and international organizations, such as International Conference on Harmonisation, for referring to policies, including development of domestic guidelines for multi-regional clinical trials. Methods: The policies, regulations, and guidelines published by the US Food and Drug Administration, European Medicines Agency, Pharmaceuticals and Medical Devices Agency (Japan), and China Food and Drug Administration were searched, and the International Conference on Harmonisation E17 draft guideline was reviewed. Results: The regulatory authorities in developed countries have developed and implemented regulations and guidelines on multi-regional clinical trials to promote simultaneous global drug development and evaluate the regional differences in drug safety and efficacy. International Conference on Harmonisation developed the draft guideline for planning/designing of multi-regional clinical trials in 2016, which recommends the general principles for strategy-related issues and design of multi-regional clinical trials, and for protocol-related issues, such as consideration of regional variability, subject selection, dose selection, endpoints, comparators, overall sample size, allocation to regions, collecting information on efficacy and safety, and statistical analysis. Conclusion: It is important to understand the international regulatory requirements for designing and planning of multi-regional clinical trials for global drug development. Moreover, it is necessary to prepare multi-regional clinical trial guidelines in accordance with the Korean regulation for clinical trials and drug administration.

체외수정시술을 위한 과배란유도시 난소낭종의 크기에 따른 임상적 반응에 대한 연구 (A Study on Clinical Response to Controlled Ovarian Hyperstimulation of In Vitro Fertilization and Embryo Transfer According to the Size of Baseline Ovarian Cyst)

  • Objective: This study was performed to compare the clinical response to controlled ovarian hyperstimulation (COH) of in vitro fertilization and embryo transfer (IVF-ET) according to the size of baseline ovarian cyst. Method: From February 1992 to March 1999, a retrospective analysis was done of 272 cases who underwent COH using mid-luteal phase long protocol of gonadotropin-releasing hormone agonist (GnRH-a) for IVF-ET. These cases were divided into four group; group 1 (n=63) had cysts with mean diameters between 20.0 and 29.0 mm on their baseline ultrasound on cycle day 3, group 2 (n=57, $30.0{\sim}49.0mm$), group 3 (n=68, >50.0 mm) and control group (n=84). Cases were excluded according to the following criteria; pure male factor infertility, the presence of only one ovary, high CA-125 level and previous endometriosis. Results: There were no statistically significant differences between cases with baseline ovarian cyst <50.0 mm in diameter and control group in any of the parameters. However, cases with baseline ovarian cyst>50.0 mm in mean diameter needed more amount of human menopausal gonadotropin (hMG), showed significantly lower estradiol ($E_2$) level, the number of follicle >15.0 mm on the day of human chorionic gonadotropin (hCG) administration, the number of oocytes retrieved, the number of mature oocytes, and pregnancy rate compared with control group. Conclusion: This study suggests that cases with baseline ovarian cyst <50.0 mm in diameter do not adversely impact on IVF-ET outcome. However, cases with baseline ovarian cyst >50.0 mm in diameter had adverse effects on various parameters. Therefore, to improve the outcome of IVF-ET in these cases, ovarian cyst aspiration prior to initiating COH may be required.

임프란트 식립시 이식된 탈회골기질을 함유한 이식재의 골형성에 대한 연구 (A STUDY ON THE BONE FORMATION OF GRAFT MATERIAL CONTAINING DEMINERALIZED BONE MATRIX WITH A SIMULTANEOUS INSTALLATION OF IMPLANT)

  • Purpose: The aim of the present study is to evaluate the effect of autogenous bone and allograft material coverd with a bioresorbable membrane on bone regeneration after a simultaneous installation of implant. Materials and methods: Twelve healthy rabbits, weighing about $3{\sim}4$ kg, were used in this experiment. Following impalnt(with 3.25 mm diameter and 8 mm length) site preparation by surgical protocol of $Oraltronics^{(R)}$, artificial bony defect, 5mm sized in height and depth, was created on femoral condyle using trephine drill(with 5 mm diameter and 5 mm length). Then implant was inserted. In the experimental group A, the bony defect was filled with autogenous particulated bone and coverd with $Lyoplant^{(R)}$ resorbable membrane. In the experimental group B, the bony defect was filled with allograft material(Orthoblast $II^{(R)}$) containing demineralized bone matrix and covered with $Lyoplant^{(R)}$. In the control group, without any graft materials, the bony defect was covered with $Lyoplant^{(R)}$. The experimental group A and B were divided into each 9 cases and control group into 3 cases. The experimental animals were sacrificed at 3, 6 and 8 weeks after surgery and block specimens were obtained. With histologic and histomorphometric analysis, we observed the histologic changes of the cells and bone formation after H-E staining and then, measured BIC and bone density with KAPPA Image $Base^{(R)}$ system. Results: As a result of this experiment, bone formation and active remodeling process were examined in all experimental groups and the control. But, the ability of bone formation of the experimental group A was somewhat better than any other groups. Especially bone to-implant contact fraction ranged from 12.7% to 43.45% in the autogenous bone group and from 9.02% to 29.83% in DBM group, at 3 and 8 weeks. But, bone density ranged from 15.67% to 23.17% in the autogenous bone group and from 25.95% to 46.06% in DBM group at 3 and 6 weeks, respectively. Although the bone density of DBM group was better than that of autogenous bone group at 3 and 6weeks, the latter was better than the former at 8 weeks, 54.3% and 45.1%, respectively. Therefore these results showed that DBM enhanced the density of newly formed bone at least initially.

이종 망에서의 지연경보 경로차단 스케줄러를 이용한 MPTCP성능 개선방안 (Improving the performance of Multipath TCP using Delay Alerted Path-blocking Scheduler in Heterogeneous Networks)

  • Multipath TCP (MPTCP)는 다중 경로를 동시에 사용하여 전송효율과 안정성을 얻을 수 있는 4계층 프로토콜로 현재 IETF를 통해 표준화되어 사용되고 있다. MPTCP는 비슷한 특성의 다중 경로를 망에서 사용하는 경우 단일 TCP보다 좋은 성능을 얻을 수 있지만 특성이 다른 망에서 사용하는 경우 다중 경로 간 지연시간의 차이로 인해 다중경로간의 패킷 도착시간의 차이가 발생하여 수신버퍼에서 패킷을 reordering하는 문제가 발생한다. 이러한 패킷 reordering 문제로 인해 이종 망에서의 MPTCP는 단일 TCP를 사용하는 것보다 성능이 저하되는 문제점을 가지고 있다. 따라서 본 논문에서는 MPTCP 각 경로상의 지연시간을 측정하여 지연시간이 크게 증가하는 경로를 차단하여 수신버퍼의 패킷 reordering을 줄이고, 차단한 경로에 작은 크기의 복제 패킷을 보내어 지속적인 지연시간측정을 하여 해당 경로의 네트워크의 혼잡이 줄어들면 차단을 해제하는 지연경보 경로차단 스케줄링을 제안하였다. 제안한 스케줄링의 성능분석을 위해 리눅스커널에 제안한 스케줄링을 구현 설치하고 테스트베드를 구성하여 실험을 통해 제안 스케줄링이 이종 망에서 MPTCP의 성능저하를 감소시키는 것을 확인하였다.
