The purpose of this study was to review the literatures of experimental tracheal reconstruction. Although there have been significant advancements in the surgical treatment of the long circumferential tracheal injuries, there still has been a difficult problem with high morbidity and mortality. The method for tracheal reconstruction after circumferential resection is preferred end-to-end anastomosis for defects up to 6 cm in length, but larger tracheal defects require the use of tracheal allograft, various artificial prosthesis or autogenous organs. The tracheal allotransplantation has been widely used as there was significantly improved the method of surgical technique, preservation and immunosuppression. But it has been limited by a number of factors such as few donor, limited use of immunosuppressant, delayed revascularization and re-epitheliazation. Experimental studies on the tracheal prosthesis have a long history and they tried to use silicone, polytetrafluoroethylene, polypropylene mesh, Dacron, Marlex mesh, external or internal stents. Other experimental studies were reported the use of autogenous tissues that were cartilage. jejunum, aorta, skin, muscle, periostium or esophagus. But a great variety of these protheses have been resulted unsatisfactory in a significant Proportion of cases. Alternatively, the tissue-engineering technique has showed a new approach to reconstruct trachea and much progress in tissue-engineering bas been made recently. In conclusion, although the tracheal allotransplantation and the use of prosthesis and allograft have been reported a lot of limitation to overcome, we could sooner expect good result of ideal tracheal prosthesis.
전치부 치간 사이 공간은 치주 환자에게 흔히 발생되는 병적 치아 이동(PTM, pathological tooth migration)과 함께 자주 관찰된다. PTM에는 다양한 교합적 요인들이 기여하는데 특히 상악 전치부에서는 과도한 교합력이 있을 때 위치 문제로 인한 불리함 때문에 치간 이개가 쉽게 발생한다. 치주 지지 조직이 소실된 치아는 의도적인 연결 고정(splinting)을 통해 지지 면적을 확장하면 안정성이 증가될 수 있다. 또한 고정성 보철물로 수복 시에는 불리한 교합을 유리한 형태로 바꿀 수 있다. 임시보철물을 통해 안정적 교합을 확보 후 이것을 최종보철물로 옮기는 일은 CAD-CAM을 통하여 가능하다. 본 증례에서는 치간 접촉이 상실된 상악 전치부에서 정확한 진단과 치료를 통해 장기적으로 안정적인 결과를 보여 소개하고자 한다.
Purpose: This study was carried out for the purpose of obtaining basic data necessary for developing the future oral health promotion program of the elderly by surveying the actual condition of managing dental prosthesis in the elderly. Methods: It collected materials targeting 346 elderly brackets in over 65 years old who dwell in Seoul and Gyeonggi-do Province for about 10 months from September 2010 to June 2011, analyzed 312 copies except 34 copies, which are inappropriate for data processing due to being insufficient in response among these things, and obtained the following results. Results: In the actual condition of missing tooth, a case of missing tooth was indicated to be 78.8%. In case of missing tooth, the number of missing teeth was indicated to be 75.6% for under 5 pieces and 19.1% for over 21 pieces. In the actual condition of managing prosthesis, a case with dental prosthesis treatment was indicated to be 77.6%. A kind of prosthesis was indicated to be the highest in fixed partial dentures with 36.5%. In a method of rinsing dentures, 47.7% of the elderly responded as saying of washing it out with water. The appearance of removing dentures at night was indicated to be the highest in doing so with 63.1%. Satisfaction with prosthesis of being mounted now was indicated to be moderate with 50.4%. As for the appearance of having experience of education on how to manage prosthesis, it was indicated to be high with 50.4% in having none. In a kind of prosthesis depending on averagely monthly income, the ratio of total dentures was high when averagely monthly income is low. On the other hand, a case with high income was indicated to have high ratio in implants. The satisfaction with prosthesis was low when averagely monthly income is low. Also, in the experience of education on how to manage prosthesis, the ratio of responses as saying of having no experience of education on how to manage prosthesis was indicated to be high when averagely monthly income is low. Conclusion: Examining the above results, a developmental plan is considered to be necessary such as developing continuous and systematic oral health promotion program for managing elderly people's managing dental prothesis.
Partial edentulism has multiple implications in relation to function, esthetics and future rehabilitative treatment. This case report illustrates the management of a patient with extreme consequences of partial edentulism. The main clinical findings were unopposed remaining teeth, overeruption of the remaining teeth, loss of vertical dimension of occlusion, and significant disfigurement of the occlusal plane. Following the diagnostic procedure, a well-coordinated prosthodontic treatment involving liaison with other dental disciplines was indicated. The management involved an innovative combination of fixed and removable prostheses in conjunction with crown lengthening surgery and strategic implant placement. Series of provisional prostheses were applied to facilitate the transition to the final treatment.
Objectives: Patients with excessive tooth wear should first be diagnosed for the etiology of the tooth wear. Causes of tooth wear include bruxism, clenching, and taking medications for systemic diseases. After identifying the cause of tooth attrition, the final prosthesis should be restored with an appropriate vertical dimension. Methods: A 79-year-old man with worn out teeth desired a whole dental treatment. He was on medications for high blood pressure and asthma. The treatment proceeded with a consultation with a medical doctor. The medications for asthma evoked multiple teeth wear and a loss of the vertical dimension. After recovery of 3 mm of vertical dimension, 2 months of evaluation was followed by an interim prosthesis. Results: The increased vertical dimension caused no problem in function and esthetics, and the final restoration was performed with a full monolithic zirconia crown. Group function, adequate anterior guidance, and the occlusal plane were determined. Conclusions: After the final restoration, the patient was both esthetically and functionally satisfied, and a night guard splint was delivered to prevent prosthesis fracture. The patient was informed about the potential tooth wear associated with asthma drugs and educated to visit the clinic regularly.
최근에 열가소성 레진 클라스프를 이용한 가철성 장치의 사용이 빈번해졌다. 통상적으로 사용하는 아크릴릭 레진과 비교했을 때 열가소성 레진은 낮은 굴곡 강도와 탄성률을 가지고 있다. 그러므로 열가소성 레진으로 제작된 가철성 장치는 파절의 위험이 낮아 의치상이 얇고 가볍게 만들어져 환자들이 편하게 사용할 수 있다. 또한 장치가 치아 언더컷에 수동적으로 장착될 수 있어 지대치의 삭제가 최소이거나 거의 없다. 이 증례에서는 경도의 구개인두부전을 가진 44세 여환은 polyester 계열 열가소성 레진으로 제작된 구개거상장치로 치료를 받았다. 환자가 결손치가 없고 보존적인 치료를 원하기 때문에 열가소성 레진을 이용한 가철성 장치가 상대적으로 만족스러운 치료 결과를 보였다.
Total laryngectomy is the most useful procedure tor advanced laryngopharyngeal cancer, but it remains the major problem such as loss of voice. Voice restoration is essential for every patients who undergo a total laryngectomy. Ideal voice rehabilitation methods can resolve three factors. First, every laryngectomee can produce voice sufficient for communication, second every patient should be allowed to use both hands freely during phonation, and last, the voice restoration methods should be easy and safe without complication during and after treatment. Among various voice rehabilitation procedures during or after total laryngectomy, it can be divided electronic and pneumatic methods. In pneumatic methods, there are also divided both pulmonary air and non-pulmonary air methods. The non-pulmonary air methods include esophageal speech, buccal speech, and pharyngeal speech. Pulmonary air methods are divided into surgical and non-surgical such as pneumatic speech aid. In the surgical methods, there are neoglottic operation, tracheopharyngeal shunt, and tracheopharyngeal shunt operations. Recently, tracheoesophageal shunt with or without prosthesis are being recognized the most effective method. Blom-Singer low pressure prosthesis, Panje button, and Provox are well known types of prosthesis in the tracheoesophageal shunt operation. Amatsu method is a kind of famous tracheoesophageal shunt method without using prosthesis. Authors tried to review the published articles for evaluation of effectiveness and problems of tracheoesophageal shunt operation with or without prosthesis. In conclusion, indwelling type of prosthesis and pharyngeal myotomy and plexus neurectomy are recommended for higher success rate during tracheoesophageal puncture procedure. More over, Amatsu method is also one of the recommended voice rehabilitation procedure during total laryngectomy. In this situation, pharyngeal myotomy and plexus neurectomy may be helpful for better fluent communication.
Cementation is the last procedure and an important factor to access successful fixed prosthodontic treatment. Even though there are many kinds of luting materials in dental field, the resin cements are popularly used in now. Metals, polymers and ceramics are used as a material of fixed dental prosthesis. The bonding mechanisms between teeth and fixed restorations are composed of mechanical and chemical mechanism. In dentistry, we are relying on mechanical bonding, but we tried to get chemical bonding and many ways are introduced. So, we have to approach luting procedure differently by the materials of prosthesis for clinical success. In this article, let us think the proper cementation ways according to each prosthesis material.
Purpose: The purpose of this study is to provide basic data for the improvement of dental care service for patient care. Methods: The survey period was from December 10, 2018 to June 10, 2019. The subjects were 180 patients who were treated with dental implant in 20 dental hospitals and clinics in Daejeon and Gyeonggi province. A total of 145 questionnaires were analyzed except for 35 copies which were unreliable or difficult to analyze. Results: The most important care method for prolonging the life time of dental implant prosthesis was the highest rate of correct brushing (39.4%), and a majority in the subjects used oral hygiene products (91.0%). With regard to discomfort after dental implant prosthesis, food particles between the teeth showed the highest response (49.7%), and discomfort with gum infections and bleeding and reduced mastication increased significantly with age (p<0.05), whereas feeling of irritation decreased significantly with age (p<0.05). Conclusion: This study is meaningful in that it approached the perception and discomfort of prosthesis from the patients' point of view. It needs to establish the basic data to improve dental treatment service for patient care.

Alloplastic total replacement of the temporomandibular joint (TMJ) was developed in recent decades. In some conditions, previous studies suggested the rationale behind alloplastic TMJ replacement rather than reconstruction with autogenous grafts. Currently, three prosthetic products are available and approved by the US Food and Drug Administration. Among these products, customized prostheses are manufactured, via computer aided design/computer aided manufacturing (CAD/CAM) system for customized design; stock-type prostheses are provided in various sizes and shapes. In this report, two patients (a 50-year-old female who had undergone condylectomy for the treatment of osteochondroma extending to the cranial base on the left condyle, and a 21-year-old male diagnosed with left temporomandibular ankylosis) were treated using the alloplastic total replacement of TMJ using stock prosthesis. The follow-up results of a favorable one-year, short-term therapeutic outcome were obtained for the alloplastic total TMJ replacement using a stock-type prosthesis.
