Purpose: This study compares and examines the effects of proprioceptive neuromuscular facilitation (PNF) on patients with chronic low back pain through systematic literature review and meta-analysis. Methods: Domestic literature was searched with combinations of keywords including "proprioceptive neuromuscular facilitation," "PNF," "back pain," and "low back pain" using the Research Information Sharing Service (RISS), Korean Studies Information Service System (KISS), and Korean Medical Database (KMbase). Six studies (n=148) were finally included in the analysis through a selection and exclusion process. The quality of the studies was evaluated using the PEDro scale. Results: According to the meta-analysis results, the low back pains of the PNF group and the contrast group showed a standardized mean difference (SMD) of 2.21 (95% CI: -3.35, -1.07, p=0.01, $I^2=83%$) after intervention. Thus, the PNF group showed a statistically significant decrease in low back pain compared with the control group. In addition, the SMDs of the Oswestry Disability Index (ODI), lung function, and the Roland and Morris Disability Questionnaire (RMDQ) were -1.34 (95% CI: -1.88, -0.79, p<0.01, $I^2=35%$), 1.14 (95% CI: 0.49, 1.79, p=0.01, $I^2=0%$), and -1.59 (95% CI: -2.56, -0.62, p=0.01, $I^2=46%$), respectively. Thus, the PNF group showed statistically significant differences from the control group. Conclusion: At present, there is some limit to obtaining definite results about effect sizes because there are relatively few randomized controlled experiments that analyze the effects of PNF exercise in patients with chronic low back pain. Therefore, continuous efforts should be made to conduct randomized clinical trials and long-term efficacy studies in the future.
이 연구의 목적은 14주 고유수용성 저항운동이 사회복지시설 이용 여성 노인의 근지구력, 동적 평형성 및 보행능력에 미치는 영향이었다. 이 연구에 참여한 대상자는 평균연령이 70세 전후의 30의 여성 노인을 운동집단 15명과 통제집단 15명으로 구성하였다. 대상자들은 운동프로그램 전과 후에 근지구력(30초 의자 앉았다 일어서기), 동적 평형성(3m 왕복 걷기)과 보행능력(10m 걷기, 400m 걷기)을 검사하였다. 운동집단은 주 3회 60분간 스위스 볼을 이용한 고유수용성 저항운동을 하였다. 연구 결과 하체 근지구력에서는 유의한 개선이 있었고, 동적 평형성을 평가하는 3m 왕복 걷기에서도 유의한 개선이 있었다. 보행능력인 10m 걷기와 400m 걷기에서도 유의한 개선이 있었다. 결론적으로 고유수용성 저항운동에 의한 동적평형성과 보행능력의 개선은 여성 노인의 낙상 관련 가능성을 감소시킬 것이다.
Background: The purpose of this study was to identify the effects of underwater and ground proprioceptive neuromuscular facilitation lower extremity pattern exercise on trunk control and lower extremity muscle activity of chronic stroke patients. Design: Pretest-posttest design: single blind. Methods: The subjects were 28 patients (experimental group, n=14 or control group, n=14) diagnosed with chronic stroke. The experimental group performed underwater proprioceptive neuromuscular facilitation lower extremity pattern exercise. The control group performed ground proprioceptive neuromuscular facilitation lower extremity pattern exercise. Training was conducted once a day for 30 minutes, five days per week for six. Trunk control was measured using the Trunk Impairment Scale. Lower extremity muscle activity was measured using surface electromyography. Results: As a result of comparison within groups, the experimental and control group showed significant difference for trunk control after the experiment (p<0.05). In comparison between two groups, the experimental group showed more significant difference in trunk control than the control group (p<0.05). In comparison within groups, the experimental group showed significant difference for lower extremity muscle activity after the experiment (p<0.05). In comparison between two groups, the experimental group showed more significant difference in lower extremity muscle activity than the control group (p<0.05). Conclusion: Based on these results, underwater proprioceptive neuromuscular facilitation lower extremity pattern exercise effectively improved the trunk control and lower extremity muscle activity of chronic stroke patients.
목적: 고유 수용성 감각 기능은 견관절의 안정성에 중요한 역할을 하는 것으로 알려졌으나 회전근개 파열 환자에 대하여는 크게 알려진 바가 없다. 본 연구의 목적은 회전근개 파열 환자의 고유 수용성 감각 기능에 대하여 조사하고자 하였고 특히 파열크기, 견갑하근의 파열 유무 등에 따라 조사하고자 하였다. 대상 및 방법: 2011년 7월부터 2012년 2월까지 회전근개의 파열로 본원에서 수술적 치료가 시행된 76예(남자 28명, 여자 48명)를 대상으로 하였고 평균 나이는 61.7세(범위, 38세~76세)였다. 수술전 고유 수용성 감각 기능 평가를 위하여 외회전 및 내회전에 대한 위치 감각(joint position sense)를 측정하였고 방식은 능동적 재위치법(active re-positioning)을 사용하였다. 정해진 회전 각도와의 편차의 절대값을 측정하였다. 회전근개의 파열 크기, 견갑하근의 파열 유무, 통증(pain visual analogue scale), 견관절 기능 점수(American society of elbow and shoulder score), 견관절 운동 범위를 조사하였으며 이들 변수에 따른 차이를 비교하였다. 결과: 외회전의 경우 정상 관절은 $4.9^{\circ}{\pm}2.9^{\circ}$, 이환된 관절은 $4.9^{\circ}{\pm}3.0^{\circ}$의 절대 편차를 보였고 이는 통계적으로 유의하지 않은 차이를 보였다(p=0.87). 내회전의 경우 정상관절은 $4.0^{\circ}{\pm}2.7^{\circ}$인데 비하여 이환된 관절은 $4.8^{\circ}{\pm}3.7^{\circ}$로 편차가 증가하였으며 이는 통계적으로 유의하였다(p=0.043). 회전근개의 파열 크기에 따라 파열이 더 큰 경우(중파열 이상)가 파열이 더 작은 경우(부분파열 및 소파열)에 비하여 내회전의 고유감각기능 감소가 더 뚜렷한 경향을 보였으며(5.0 vs. 4.0, p=0.061), 견갑하근의 파열이 있는 환자군에서 내회전의 고유감각 기능 감소가 더 뚜렷한 경향을 보였다(4.8 vs. 4.0, p=0.065). 통증이 클수록, 견관절 기능 검사 점수가 낮을수록 내회전에 대한 고유감각기능이 감소하였으며(각각 p=0.04 및 p=0.005) 견관절 운동 범위의 제한이 있는 환자군에서 내회전 고유감각기능이 더욱 감소되는 것이 관찰되었다(5.3 vs. 3.7, p=0.041). 결론: 회전근개 파열 환자에서 내회전에 대한 고유감각기능 감소가 관찰되었으며 특히 이는 회전근개의 파열 정도가 심하고 견갑하근의 파열이 동반된 경우 뚜렷한 경향을 보였다. 또한 통증이 심하거나 견관절 기능 점수가 낮으며 운동 범위의 제한이 있는 경우 내회전에 대한 고유감각기능이 감소하는 것으로 관찰되었다.
The purpose of this review are that what is the concept of sport P. N. F and to give the information about proprioceptive neuromuscular facilitation technique when the sport physical therapy will be needed in field. Technique of proprioceptive neuromuscular facilitation are methods of placing specific demands in order to secure a desired response. Greatest emphasis was placed on the application of optimal resistance throughout the range of motion, using many combinations of motions which were related to primitive patterns and employment of postural and righting reflexes. The treatment after sport injury patient is required that two component actions of muscles as well as permitting action to occur at two or more joint. The effect of P. N. F and of sprot P. N. F are reviewed. Implications for treatment of sport disability are suggested.
Purpose: This study examined the effects of an exercise program involving the Proprioceptive Neuromuscular Facilitation (PNF) lower-extremity patterns on the Bone Mineral Density (BMD), balance and lower muscular strength in patients with osteoporosis. Methods: The participants were allocated randomly to 2 groups: experimental group and control group. To evaluate the effects of exercise, the subjects were evaluated using the BMD and Time Up & Go Test and One Leg Stance Test for balance, and the Sit To Stand for lower muscular strength. Results: After the 8 week study, the experimental group showed a significant difference in all items compared to the control groups. Conclusion: These results suggest that a PNF exercise program is effective in patients with osteoporosis. Furthermore, an exercise program involving these patterns can be used in a follow-up program for patients with osteoporosis. Overall, more study will be needed to develop and apply more efficient exercise programs for such patients.
Although sensory deficits caused by stroke have been occasionally reported, dysfunctions of discriminative sensation have seldom been studied in patients with strokes. With the use of specifically designed methods, discriminative sensations including texture discrimination and position sense were tested in 67 patients with acute unilateral stroke. Thirty-two age and sex-matched healthy subjects were used as controls. Impaired discriminative sensation was common in patients with unilateral stroke (detected in 57 out of the 67 patients) regardless of the lesion location except for patients with lateral medullary stroke. Proprioceptive discriminative sensation remained intact in all except for three out of 25 patients who were initially diagnosed as having pure motor stroke on the bases of conventional sensory tests. However, tactile discriminative sensation remained intact in only 17 out of 25 patients. Discriminative sensory disturbances are common in patients with unilateral stroke even in those with intact sensory function on routine examination. The subtle disturbances of this sensation may explain, at least in part, the clumsiness of the patients that is not readily explained by conventional neurological tests.
Background: To determine the effect of proprioceptive exercise(PE) on muscle activation and balance of healthy subjects. Methods: 20 subjects were randomly assigned to proprioceptive exercise(PE, n=10) and general exercise group(GE, n=10). To measure muscle activation of the lower limb and balance. For evaluation of muscle activation(Tibialis anterior, Gastrocnemius lateral and medial head), the Electromyogram was used, and balance was measured using the one-leg standing test, OLST. Results: Tibialis anterior(TA), Gastrocnemius medial head(GM) and Gastrocnemius lateral head(GL) muscle activations were significantly (p<0.05) improved in PE group and GE group. and significantly difference between groups(p<0.05). Regarding balance, eye open and close OLST were significantly (p<0.05) increased in PE group and GE group. and significantly difference between groups(p<0.05). Conclusion: Proprioceptive exercise can improve muscle activation of the lowe limb and balance in subjects.
Proprioception means the ability to perceive the sensation of position and movement of body. As it is transmitted to central nervous system and used in feed-back or feed-forward motor control, proprioception allows us to keep our normal movement and normal balance activity. However, the conditions such as injury, disease, aging and fatigue can damage the proprioceptiive sensation of position, movement and lead to a functional impairment and additional damages in musculoskeletal system, because they alter the amount of proprioceptive ability that transfer into the central nervous system. The purpose of this study was to identify the definition and the function of proprioception, to look into variations in injury, disease, aging and fatigue that can be easily met in clinical application and eventually to provide valuable aid for assessment and treatment.
