Dedifferentiated Chondrosarcoma of the Rib Masquerading as a Giant Chest Wall Tumor in a Teenage Girl: An Unusual Presentation

  • Chondrosarcoma of the chest wall is a rare primary neoplasm found to occur in elderly men. Patients present with an enlarging, painful, anterior chest wall mass arising from either the vicinity of the costochondral junction or the sternum. Treatment includes wide resection with appropriate chest wall reconstruction. We report an unusual presentation of this uncommon tumor occurring as a huge chest wall mass in a young teenage girl.

  • From May 1965 to December 1990, 78 patients with chest wall tumors were operated on. The mean age of the patients was 31.5 years with 50 male and 28 female patients. Forty-nine cases[62.8%] were developed at bony or cartilaginous wall and 29 cases[37.2%] at soft tissue of chest wall. Thirty-two of them[41.0%] were malignant, either primary or metastatic, and 46 tumors[59.0%] were histologically benign. For 55 patients who were operated on since 1982, 6 surgical biopsies. 39 tumor excisions, and 11 wide excisions with chest wall reconstruction were done. Preoperative factors favoring diagnosis of malignant neoplasm were; 1] old-aged male patient, 2] bone or cartilaginous tumors, 3] involvement of multiple ribs, 4] complaint of pain, 5] large size on palpation[larger than 4cm]. With proper diagnosis and management plan, we think, operations of chest wall tumors can give good results.

Primary Uterine Lymphoma: A Case Report

  • Primary lymphoma of the uterus is a rare disease, the reported characteristic MR imaging findings being homogeneous intermediate signal intensity of the indistinct mass on T1- and T2-weighted images, and the preservation of endometrial lining and uterine architecture. We report a case of primary uterine lymphoma which showed tumoral necrosis, endometrial disruption and diffuse anterior vaginal wall involvement.

원발성 흉벽종양의 임상적 고찰 (Clinical Study of Primary Chest Wall Tumors)

  • 1979년 1월부터 1996년 8월까지 전북대학교병원 흉부외과에서 치료한 원발성 흉벽종양은 51례이다. 원발성 종양을 가진 환자의 평균 나이는 36.1세이고 남자는 32명, 여자는 19명이었다. 양성종양이 40례(78.4%), 악성종양이 11례(21.6%)이었다. 증상은 국소 종괴가 가장 흔한 증상으로 32(62.7%)례에서 있었다. 골 및 연골 조직에서 발생한 경우는 32례(62.7%), 연부조직에서 발생한 경우는 19례(37.3%)이다. 양성 종양인 경우 3례에서는 광범위 절제술 및 흉벽 재건술이 시행되었고 37례에서는 절제술만 시행되었으며 현재까지 재발이나 종양관련 사망은 관찰되지 않았다. 악성 종양인 경우 광범위 절제술 및 재건술을 시행한 경우는 8례에서 시행되었다. 광범위 절제술이 시행된 5례에서는 Marlex Mesh, Prolene Mesh와 Teflon felt을 이용하여 흉벽 재건술이 시행되었다. 전 예에서 수술과 관련된 사망은 없었다. 광범위 절제술을 받은 악성종양 환자 중 5례(71.4%)가 생존하고 있다.

Surgical Resection and Polypropylene Mesh Reconstruction for Canine Chest Wall Soft Tissue Sarcoma

  • A 6-year-old spayed female French Bulldog presented with a left-sided chest wall tumor. Physical examination revealed that the tumor was firmly adhered to the chest wall. A preoperative punch biopsy of the tumor revealed a grade 2 soft tissue sarcoma (STS). On computed tomography, the tumor's dimensions were assessed as 6.5 × 5.7 × 3.5 cm, and it exhibited invasiveness near the tissue surrounding the ninth rib. The tumor size was large in comparison to the dog's chest wall area. Hence, if the traditional wide-margin resection surgery were to be performed, primary wound closure seemed impractical and could potentially result in respiratory function complications. Therefore, considering the extent of tumor invasion and grade, deep margins were established to include the removal of the eighth to tenth ribs, and a 1-cm lateral margin was designated to enable primary wound closure. To reconstruct the chest wall, polypropylene mesh was attached to the adjacent ribs and the remaining muscles were sutured and covered over the mesh. The dog exhibited a rapid recovery beginning the day after the operation. Postoperative biopsy confirmed that the tumor was a grade 2 STS, and the surgical margins were evaluated as incomplete. The owner chose to pursue follow-up observation instead of chemotherapy. In this study, the surgical approach was chosen based on the importance of functional recovery after surgery. Recent research indicates that the tumor grade is more critical for postoperative prognosis than the extent of surgical margins when removing an STS.

  • The SWTR(Surface Water Treatment Rule) requires achieving removal 99.9% Giardia and 99.99% Virus. To remove Giardia and Virus, disinfection process is essential process. CT is important index for Disinfection performance. CT value is expressed by CXT. C is represented by disinfection concentration and T is represented by $T_{10}$ which means 90% contact time in clearwell. To improve Disinfection performance, it is desirable to increase $T_{10}$. Primary factor affected $T_{10}$ is flow Pattern. Because guide wall change that flow pattern is uniform, we have installed guide wall to increase $T_{10}$. In this paper, we have performed numerical analysis for clearwell installed symmetric guide wall and center divided guide wall using CFD. We have analyzed flow pattern and $T_{10}$ in each clearwell. And. we compare symmetric guide wall with center divided guide wall in flow pattern and $T_{10}$.

Molecular Cloning and Sequencing of Cell Wall Hydrolase Gene of an Alkalophilic Bacillus subtilis BL-29

  • A DNA fragment containing the gene for cell wall hydrolase of alkalophilic Bacillus subtilis BL-29 was cloned into E. coli JM109 using pUC18 as a vector. A recombinant plasmid, designated pCWL45B, was contained in the fragment originating from the alkalophilic B. subtilis BL-29 chromosomal DNA by Southern hybridization analysis. The nucleotide sequence of a 1.6-kb HindIII fragment containing a cell wall hydrolase-encoding gene was determined. The nucleotide sequence revealed an open reading frame (ORF) of 900 bp with a concensus ribosome-binding site located 6 nucleotide upstream from the ATG start codon. The primary amino acid sequence deduced from the nucleotide sequence revealed a putative protein of 299 amino acid residues with an M.W. of 33, 206. Based on comparison of the amino acid sequence of the ORF with amino acid sequences in the GenBank data, it showed significant homology to the sequence of cell wall amidase of the PBSX bacteriophage of B. subtilis.

Pseudo-dynamic approach of seismic earth pressure behind cantilever retaining wall with inclined backfill surface

  • Knowledge of seismic earth pressure against rigid retaining wall is very important. Mononobe-Okabe method is commonly used, which considers pseudo-static approach. In this paper, the pseudo-dynamic method is used to compute the distribution of seismic earth pressure on a rigid cantilever retaining wall supporting dry cohesionless backfill. Planar rupture surface is considered in the analysis. Effect of various parameters like wall friction angle, soil friction angle, shear wave velocity, primary wave velocity, horizontal and vertical seismic accelerations on seismic earth pressure have been studied. Results are presented in terms of tabular and graphical non-dimensional form.

Performance of retaining walls with and without sound wall under seismic loads

  • The seismic characteristics of two semi-gravity reinforced concrete cantilever retaining walls are examined via an experimental program using an outdoor shake table (one with and the other without concrete masonry sound wall on top). Both walls are backfilled with compacted soil and supported on flexible foundation in a steel soil container. The primary damages during both tests are associated with significant lateral displacements of the wall caused by lateral earth pressure; however, no collapse occurs during the tests. The pressure distribution behind the walls has a nonlinear trend and conventional methods such as Mononobe-Okabe are insufficient for accurate pressure estimation.
