Diagnosis and treatment for ectopic eruption of permanent first molar (임상가를 위한 특집 3 - 제1대구치 이소맹출의 진단과 처치)

  • Ectopic eruption of the permanent first molar is defined as the eruption of the tooth in an abnormal position or orientation. It may causes distal root resorption and premature exfoliation of the adjacent primary second molar and uncontrolled space loss is followed. Prolonged partial impaction of the permanent molar may also cause undetected caries or abscess formation of the neighbor teeth. The purpose of this paper is to provide a brief review regarding the etiology, classification, and different management techniques for correcting ectopic eruption of permanent first molar.

Adults' concern for oral health and subjective oral health symptoms (성인의 구강건강관심 및 주관적 구강건강증상)

  • Objectives : The purpose of this study was to examine the self-rated oral health concern of adults over the age of 20 and from all over the country and their subjective oral health symptoms in a bid to provide some information on oral health policy setting for adults. Methods : The subjects in this study were 3,558 adults who voluntarily participated in a survey conducted by this researcher at the website of a company. After their answer sheets were analyzed, the following findings were given: Results : 1. 81 percent replied that they were very concerned about oral health. As to the relationship between their general characteristics and oral health concern, there were statistically significant differences in that regard according to their gender, age, purpose of seeing a dentist and occupation. 2. As for the relationship of their general characteristics to subjective awareness of oral health symptoms, there were statistically significant gaps in that aspect according to their gender, age, purpose of seeing a dentist, occupation and state of smoking. 3. In regard to the relationship between oral health concern and subjective awareness of oral health symptoms, those who were more concerned about oral health felt less subjective oral symptoms, but the difference between them and the others was not significant. The respondents who felt more symptoms of dental caries felt more symptoms of periodontal diseases as well. Conclusions : The above-mentioned findings indicate that in order to help adults promote or maintain their oral health, their concern for oral health should be stimulated by providing them with a lot of diverse information, and it seems that the development of programs that can eliminate their subjective oral symptoms of dental caries and peridontal diseases, the primary causes of tooth dysfunction, is required.

Anticariogenic Properties of the Ethanol Extract of Tribuli fructus against Streptococcus mutans (백질려 추출물이 Streptococcus mutans에 대한 항치아우식에 미치는 영향)

  • Streptococcus mutans is considered one of the primary etiologic agents of dental caries. we studied the effect of the ethanol extracts of Tribuli fructus (T. fructus) on the growth, biofilm formation, acid production, adhesion and insoluble glucan synthesis of S. mutans. The ethanol extracts of T. fructus showed concentration dependent inhibitory activity against the growth and acid production of S. mutans, and produced significant inhibition at the concentration of 0.025, 0.05, 0.1, 0.2, 0.3, 0.5 mg/ml compared to the control group. In the biofilm assay, the ethanol extracts of T. fructus inhibited formation of biofilm synthesized by S. mutans at the concentration of 0.05 mg/ml. The extracts markedly inhibited S. mutans adherence to HA treated with saliva, and cell adherence was repressed by more than 50% at the concentration 0.05 mg/ml. On the activity of glucosyltransferase which synthesizes water insoluble glucan form sucrose, ethanol extract of T. fructus showed more than 10% inhibition over the concentration of 0.025 mg/ml. Hence, we conclude that T. fructus might be a candidate of anticaries agent. Further studies are necessary to clarify the active constituents of T. fructus responsible for such biomolecular activities.

Effect of the Antimicrobial Peptide $\small{D}$-Nal-Pac-525 on the Growth of Streptococcus mutans and Its Biofilm Formation

  • Streptococcus mutans is the primary etiological agent of dental caries. The antimicrobial peptide $\small{D}$-Nal-Pac-525 was designed by replacing the tryptophans of the Trp-rich peptide Pac-525 with $\small{D}$-${\beta}$-naphthyalanines. To assess the effect of $\small{D}$-Nal-Pac-525 on cariogenic bacteria, the activity of $\small{D}$-Nal-Pac-525 on the growth of S. mutans and its biofilm formation were examined. $\small{D}$-Nal-Pac-525 showed robust antimicrobial activity against S. mutans (minimum inhibitory concentration of 4 ${\mu}g/ml$). Using scanning electron microscopy and transmission electron microscopy, it was shown that $\small{D}$-Nal-Pac-525 caused morphological changes and damaged the cell membrane of S. mutans. $\small{D}$-Nal-Pac-525 inhibited biofilm formation of S. mutans at 2 ${\mu}g/ml$. The results of this study suggest that $\small{D}$-Nal-Pac-525 has great potential for clinical application as a dental caries-preventing agent.

  • Talon cusp is cusp-like structure that develops additionally in crown of anterior tooth. And it is one of dental anomalies. Talon cusp can cause various diagnostic, functional, aesthetic problems depending on the size and configuration of cusp. there is a tendency for caries to occur in the developmental grooves, and advanced attrition, periodontal problems, irritation the tongue and temporomandibular pain, occlusal interference and displacement of affected tooth may result. Therefore, early diagnosis and appropriate treatment of each case is important so that it minimize local problems, such as caries, periodontal diseases, and malocclusion. These cases which are in permanent and primary anterior teeth are about gradual reduction forming reparative dentine and complete reduction of talon cusp and root canal therapy which is an alternative and effective form of treatment when gradual reduction of talon cusp may not be possible.

  • Vitamin D-resistant rickets(VDRR) is hereditary disease manifesting marked hypophosphatemia caused by renal tubular loss of phosphate into urine and an associated decrease in the calcium and phosphorous ion product. VDRR is identified by clinical symptoms, such as in the limbs, gait disturbance, dwarfism, familial occurrence, bowlegs, and knock-knees, as well as by laboratory findings. Dental findings are enlarged chambers and extension of the pulp horns into the cusp tips in both the primary and the permanent dentition in contrast to the vitamin D-deficient rickets. The major oral manifestations are multiple spontaneous abscesses in a caries-free dentition. We reported the clinical features and treatment of a 6-year-old boy with vitamin D-resistant rickets referred to the department of pediatric dentistry in Kyung Hee University for multiple spontaneous periapical abscesses and gingival fistula without severe dental caries.

Perception and Associated Factors of Primary Schoolchildren's Parents Concerning Approval on the Expansion Implementation of Community Water Fluoridation Program in Gimhae, Korea (김해시 수돗물불소농도조정사업 확대 시행에 대한 시민들의 인식과 찬성에 미치는 요인)

  • Objectives: The study sought to identify the perception of schoolchildren's parents concerning an approval on the expansion implementation of community water fluoridation program in Gimhae, Korea. Methods: Questionnaires were distributed to schoolchildren's parents in Gimhae in 2008. Responses were collected from 1,703 parents. The standard questionnaire of Ministry of Health and Welfare was used and items were socioeconomic variables, water used for drinking and cooking, awareness on the fluoridation program and approval on the expansion implementation of fluoridation program at the region of Myeongdong water plant. Significance was analyzed by chi-square test and multiple logistic analysis. Results: Of parents, 1.9% used plain tap water and 39.8% used boiled tap water for drinking and 50.0% used tap water for cooking. 58.0% of parents were aware of fluoridation program with 27.1% being aware of the implementation of the program in Gimhae. 74.5% approved the expansion implementation of the program to the region of Myeong-dong water plant. Significant variables associated to approving the expansion implementation of program were age, gender, residence period in Gimhae, occupation and awareness of the program. Conclusion: The expansion implementation of fluoridation program in Gimhae is desirable for caries prevention because of the majority of schoolchildren's parents' approval.

  • Ease of manipulation, adequate mechanical properties, long years of experience and economical cost are the factors which have established amalgam as the most widely used material for dental restorations. But amalgam restoration may require replacement because of secondary caries, fracture, "fall-out", dimensional change, tarnish or corrosion etc.. These failures of amalgam restorations seem to arise from failures during operations rather than from the inherent shortcomings of the material itself or of the patient's mismanagement. It is anticipated that notonly number of analgam restoration, but failures will be increase after more extensive utilization of the medical insurance which began in 1977. Then authors think that it would be helpful for the development of better treatment in daily dental practice, to know the duration of amalgam restorations and the reasons for their replacement. The data for this survey was compiled from 2, 856 out-patients of the Department of Dentistry, Ewha Woman's University Hospital from January 1975 to December 1977. 260 cases among 1,718 fillings were studied, of which 205 cases both had a single reason for replacement and recognized the date of the previous filling. The results obtained were as follows; 1. Amalgam fillings were 58. 5 percent of all dental restorative materials. Of these, 15. 13 percent of the amalgam restorations had to be replaced. 2. The first reason for replacement of amalgam restorations was secondary caries (56.10%), the second was fracture (23.80%) and the third was "fall-out" (8.78%). 3. Among those amalgms requiring replacement, 52.2 percent had been in place less than 3 years, 70.7 percent within 5 years and 89.8 percent had been in place less than 10 years. Only 10.2 percent had been in place more than 10 years.

TOOTH SURFACE CARIES PATTERNS IN THE PRIMARY DENTITION ACCORDING TO BREAST OR BOTTLE FEEDING (수유 요인에 따른 유치열의 치면별 우식패턴)

  • The purpose of this study was to investigate the tooth surface caries patterns in the primary dentition according to breast or bottle feeding. The subjects of study were 815 children, 36 to 71 months old, in Iksan, Cheongju and Ulsan cities. The caregivers of the children were asked which they fed between the breast milk and the infant formula during the 1st year after birth and the duration of feeding. There was no significant difference in dmfs between the breast milk group and the infant formula group(significance level 0.05, same below). Tooth groups, tooth surface groups, teeth, and tooth surfaces which had significantly higher dmfs in the breast milk group than in the infant formula group were upper anterior teeth(tooth groups), upper incisors' buccal and proximal surfaces(tooth surface groups), upper incisors and upper second molars(teeth), upper central incisors' buccal and distal surfaces, upper lateral incisors' buccolingual and proximal surfaces, upper 2nd molars'lingual, proximal, and occlusal surfaces, and lower 2nd molars' buccal and distal surfaces. In the breast milk group, tooth groups, tooth surface groups, teeth, and tooth surfaces which dmfs significantly increased as the duration of the feeding increased were upper anterior teeth and upper molars(tooth groups), upper anterior teeth's proximal surfaces, upper molars' buccolingual, proximal, and occlusal surfaces, lower molars' proximal surfaces(tooth surface groups), upper anterior teeth, upper molars, lower 2nd molars(teeth), upper anterior teeth's proximal surfaces, upper 1st molars'buccolingual, proximal, and occlusal surfaces, upper 2nd molars' buccal surfaces, and lower 2nd molars'mesial surfaces(tooth surfaces). In the infant formula group, dmfs increased after 3 years of feeding, but the difference was not significant. Caries prevention is necessary in case of breastfeeding more than two years.

THE ESTHETIC RESTORATION BY OPEN-FACED STAINLESS STEEL CROWN IN PREMATURE LOSS OF MAXILLARY PRIMARY INCISORS (상악 유중절치 조기 상실시 개창 금속관을 이용한 심미적 수복)

  • Premature loss of maxillary primary incisors often results from early childhood caries (ECC), trauma. While space maintenance in the posterior region is an important consideration when there is early loss of primary molars, the anterior segment appears to be stable, even with the early loss of several incisors, once the primary canines erupt. However, collapse of anterior arch integrity is evident in cases where incisor teeth are in a crowded dentition prior to extraction or lost before the eruption of the primary canines. So, when early loss of maxillary primary incisors, the aim of restoration is esthetics, speech problem, oral habit such as tongue thrusting than space maintenance. This paper reports that the esthetic problem due to premature loss of maxillary incisors can be successfully resolved by soldered open-faced stainless steel crown.
