Decomposition of a stereo signal into the primary and ambient components is a key step to the stereo upmix and it is often based on the principal component analysis (PCA). However, major shortcoming of the PCA-based method is that accuracy of the decomposed components is dependent on both the primary-to-ambient power ratio (PAR) and the panning angle. Previously, a modified PCA was suggested to solve the PAR-dependent problem. However, its performance is still dependent on the panning angle of the primary signal. In this paper, we proposed a new PCA-based primary-ambient decomposition algorithm whose performance is not affected by the PAR as well as the panning angle. The proposed algorithm finds scale factors based on a criterion that is set to preserve the powers of the mixed components, so that the original primary and ambient powers are correctly retrieved. Simulation results are presented to show the effectiveness of the proposed algorithm.

Objectives: Although the prevalence of depressive disorders in South Korea's general population is known, no reports on the prevalence of depression among patients who visit primary care facilities have been published. This preliminary study was conducted to identify the prevalence of depressive disorder in patients that visit two primary care facilities. Methods: Among 231 consecutive eligible patients who visited two primary care settings, 184 patients consented to a diagnostic interview for depression by psychiatrists following the Diagnostic and Statistical Manual of Mental Disorders-IV criteria. There were no significant differences in sociodemographic characteristics such as gender, age, or level of education between the groups that consented and declined the diagnostic examination. The prevalence of depressive disorder and the proportion of newly diagnosed patients among depressive disorder patients were calculated. Results: The prevalence of depressive disorder of patients in the two primary care facilities was 14.1% (95% confidence interval [CI], 9.1 to 19.2), with major depressive disorder 5.4% (95% CI, 2.1 to 8.7), dysthymia 1.1% (95% CI, 0.0 to 2.6), and depressive disorder, not otherwise specified 7.6% (95% CI, 3.7 to 11.5). Among the 26 patients with depressive disorder, 19 patients were newly diagnosed. Conclusions: As compared to the general population, a higher prevalence of depressive disorders was observed among patients at two primary care facilities. Further study is needed with larger samples to inform the development of a primary care setting-based depression screening, management, and referral system to increase the efficiency of limited health care resources.

Purpose: The purpose of this study was to provide a survey of patients with dementia registered and managed by primary health care posts. Method: Computation of 2016 dementia data registered in Health Care Center programs of 14 municipalities in ChoongNam province was analyzed. Data collection was done based on a pretest for dementia prevention and general management of registered dementia patients. Results: Results showed; Screening tests for dementia, 40% of population 60 or over, average number of cases, 174, average number of dementia registrants, 3.1, programs for prevention, approximately 70% special policy measures and 28% cognitive rehabilitation programs, counseling and education operating well overall, average number of dementia registrants/clinic 11.8, with women accounting for 70%, elders with less than 3 years of education, 75%, residence type cohabitation by married couples, 41%, and elders with Alzheimer type dementia, 64%. Conclusion: During early detection of dementia and follow-up examinations, high-risk groups (women, elders) should receive a dementia examination. In management of dementia there is a need to develop various programs including physical, economic, and emotional support not only for patients, but also for families. Health care managers also need systematic education to give them expert knowledge of dementia and management of dementia.

Background: Prognostic biomarkers in breast cancer are routinely investigated in the primary tumors to guide further management. However, it is proposed that the expression may change during the disease progression, and may result in a different immune profile in the metastatic nodes. This work aimed to investigate the expression of breast prognostic biomarkers in primary tumors and in its axillary nodal metastasis, to estimate the possible discordant expression. Materials and Methods: 60 paired primary and axillary nodal metastasis samples were collected from patients with primary breast cancer with positive nodal deposits, diagnosed at the Maadi Military Hospital, Cairo, Egypt, during the year 2013. ER, PR and HER2 expression was assessed by immunohistochemistry in all samples Results: 48.3% of the included cases showed concordant results for both ER and PR receptors between the primary tumor and its nodal metastasis while 51.7% showed discordant results and the discordance level was statistically significant. On the other hand, 70% of the cases showed concordant Her2 results between the primary tumors and the nodal deposits, 30% showed discordant results and the difference was significant. Conclusions: The study indicated that the discordance in ER and PR receptor expression between the primary breast tumor and their nodal metastasis may be significant. The possible switch in the biomarker status during the disease progression is worth noting and may change the patient therapeutic planning. So, whether the treatment selection should be based on biomarkers in the lymph node is a topic for further studies and future clinical trials.
Objective: Recurrent lumbar disc herniation has been reported to occur in 5% to 15% of surgically treated primary lumbar disc herniation cases. We investigated the molecular biologic characteristics of primary herniated discs and recurrent discs to see whether the recurrent discs has the similar biological features with primary herniated discs. Methods: Primary hemiated disc and recurrent disc cells were obtained by discectomy of lumbar disc patients and cells were isolated and then taken through monolayer cultures. We compared chondrogenic and osteogenic mRNA gene expression, and western blot between the two groups. Results: The mRNA gene expression of recurrent disc cells were increased 1.47* times for aggrecan, 1.38 times for type I collagen, 2.04 times for type II collagen, 1.22 times for both Sox-9 and osteocalcin, and 1.31 times for alkaline phosphatase, respectively, compared with the primary herniated lumbar disc cells (*indicates p < 0.05). Westem blot results for each aggrecan, type I collagen, type II collagen, Sox-9, osteocalcin, and alkaline phosphatase were similar between the primary herniated disc cells and recurrent disc cells. Conclusion: These results indicate that the recurrent disc cells have similar chondrogenic and osteogenic gene expression compared to primary herniated disc cells. Therefore, we assumed that the regeneration of remaining discs could fill the previous discectomy space and also it could be one of the factors for disc recurrence especially in the molecular biologic field.

In cognitive radio networks, acquirement of position information of primary user is very important to secondary network since localization information of primary users can be utilized for improving the spectrum efficiency of secondary network and for avoiding harmful interference to primary users by using proper power control. Among various location methods, Received Signal Strength (RSS)-based localization has been widely used for distance measurements in the location detection process despite its inherent inaccuracy because it can be easily implemented without any additional hardware cost. In the RSS-based localization, the distance is measured by the received signal strength, and distance error can be caused by many factors such as fading, shadowing and obstacle between two nodes. In the paper, therefore we propose a localization scheme based on estimation of path-loss exponent to localize the location of primary users more accurately. Through simulations, it is shown that the proposed scheme can provide less localization error and interference rate to primary users than other schemes.
Purpose: The purpose of this study was to describe the prevalence of clonorchiais and analyze the knowledge level and intention of the participants to change their behavior related to clonorchiais. Method: The participants were 514 people from 7 community health posts along the Geum River. The formalin-esther sedimentation technique was used to detect the Clonorchis Sinensis (CS) eggs and a questionnaire for data on related factors. The study was carried on from December 2008 to January 2009. Results: The prevalence of CS averaged 9.3%, with a range from 0% to 24.6%. The significant factors were sex, habit of eating raw fish, and habit related to smoking and alcohol consumption. The level of knowledge was not high and not significantly different between the CS positive group and CS negative group. Intention to change their habits of eating raw fish showed various stages of change and attitudes to raw fish eating habit of others were not positive. Conclusions: These results suggest that a CS control program needs to be developed by community health services in the Geum River area to decrease the prevalence of clonorchiais.

Purpose: The purpose of this study was to provide basic data for nursing interventions to assist primary caregivers of young children by determining the degree stress and fatigue that caregivers were under due to parenting young children. Methods: Participants were primary caregivers (181 mothers and 160 grandmothers) of young children. Results: 1) Mean score for degree of parenting stress was 2.61 (${\pm}.69$) and for fatigue, 1.97 (${\pm}.54$). 2) There was a significant difference in degree of parenting stress according to primary caregivers' age, employment status, income, whether the children were wanted babies, health status of the children and caregivers, personality of the children, and benefits from, and satisfaction with costs of childcare. Likewise, there was a significant difference in degree of fatigue according to relationship of primary caregivers to the children, primary caregivers' age, religion, income, whether the children were wanted babies, health status of the children and caregivers, personality of the children, and benefits from, and satisfaction with costs of childcare. 3) Stress and fatigue perceived by the primary caregivers were significantly correlated (r=.554 p<.001). Conclusion: To alleviate more effectively primary caregivers' parenting stress and fatigue, early nursing intervention and educational programs need to be developed.
This study was carried out to investigate the growth and branching pattern of stolons at Korean lawn(Zoysia japonica Steud.) in the field condition. The results were summarized as follows ; 1.About 80% of observed lawns had one primary stolon. Among the lawns with several primary stolons, 30% of them had two primary stolons, 63% had three, and 7% had four. 2.The angles between the primary stolon and the shoot were increased from 0˚ to 52˚ according to the node order from the terminal shoot apex, and reached maximum angle at the 7th node. 3.The internode length was the longest in the middle position of stolon, and its growing rate which depends on months was increased from May to September. 4.The branching angles between primary and secondary stolon were from 44˚ to 53˚ in each node. The average left branching angle was about 48.20, right branching angle 47.8˚, and the total average branching angle was 48.00. 5.The rate of initial brabch was the highest at the 10th node in May, the 7th node in July and the 5th node in September. But, the initial branching rate at the 7th node in July was higher than any other that at the 10th node in May and the 5th node in September. 6.The distribution rate of secondary stolon in each node of primary stolon was the highest at the 7th node. 7.when the terminal shoot apex of primary stolon was damaged mechanically, the branching rate at the first node after the damaged region was highly increased to 62%. The results of this study may be suggested that the secondary stolon begins to branch with the angle of 480 from the 7th node of the normal primary stolon, and those may be used as a basic data for the branching simulation in lawn.

Cognitive radio technology enables to share the spectrum dedicated to primary users. In CR network, it is of primary concern to protect the primary users. Thus, it is required to periodically sense the spectrums occupied by primary users and adapt the communication parameters used by CR users to protect the primary users. However, it is inevitable to experience the hidden node problem due to the primary users, that are not detected by spectrum sensing. To perfectly protect the primary users, it is essential to address the hidden node problem in CR network. In this paper, we propose a new approach to handle the hidden node problem and evaluate the performance of proposed scheme.
