TRANSPOSITION OF THE IMPACTED PRIMARY SECOND MOLAR AND THE TOOTH GERM OF SECOND PREMOLAR (매복된 제 2유구치와 제 2소구치 치배의 전위에 관한 증례 보고)

  • Most of impacted teeth reported are permanent teeth. Impaction of primary tooth caused by primary failure of eruption is rare and the most of impacted primary tooth are second primary molars. The etiology of a primary molar impaction would appear to be a physical barrier, early ankylosis of the tooth, abnormal development of the primary molar germ or malposition of the successor bud. Transposition has been described as an interchange in the position of two teeth within the same quadrant of the dental arch. It is most commonly seen with canine and lateral incisor teeth, but is rarely associated with the primary dentition. The two cases show the transposition of impacted primary molar and the tooth germ of second premolar in their clinical and radiographic examinations. In case 1, lower second primary molar was deeply impacted inferiorly to the tooth germ suspected to be a second premolar. In case 2, impacted all second primary molars were positioned inferiorly to the tooth germs suspected to be the successors. We extracted all second primary molars under general anesthesia and the extracted tooth germs appeared to be second premolars.

A Study of Root Canals Morphology in Primary Molars using Computerized Tomography (CT를 이용한 유구치 근관 형태에 관한 평가)

  • The purpose of this study is to identify the most common canal type in primary molars and the correlation between their roots and canals. A comprehensive understanding of root canal morphology will lead to more successful root canal treatment. Computed Tomography (CT) images from 114 children (81 boys, 33 girls) aged 3 - 7 years were obtained. The locations and numbers of roots and canals were evaluated, and the relationship between root and canal parameters was determined. The most commonly observed canal morphology in primary maxillary molars were mesio-buccal, disto-buccal and palatal canal. Primary mandibular molars most frequently contain mesio-buccal, mesio-lingual, disto-buccal and distolingual canal. All the roots of the primary maxillary molars except for the mesio-buccal root, each had 1 canal while there were 2 canals observed in each root of primary mandibular molars. Without exception, all mesial roots in primary mandibular 2nd molars had 2 canals. In case when either the palatal root in a primary maxillary molar or the distal root in a primary mandibular molar was separated into 2 roots, each root seemed to have its own canal. Even though the disto-buccal and palatal roots were fused into 1 root in primary maxillary molars, this fused root had tendency to have 2 separate canals. Primary mandibular molars showed similar canal morphologies between left and right molars in the same patient, while the primary maxillary molars did not.

  • A New ZVS(Zero Voltage Switching) and ZVZCS(Zero Voltage and Zero Current Switching) Three-Level Converter is proposed. The proposed converter presented in this paper used a phase shift control with a flying capacitor in the primary side to achieve ZVS for the all switch. A primary auxiliary circuit, which consists of one coupled inductor, is added in the primary to provide ZVZCS conditions to primary switches. Many advantages including simple circuit topology high efficiency, and low cost make this converter attractive for high power applications. The principle of operation, feature and design considerations are illustrated and verified through the experiment with a 2kW(27V, 74A) 40 kHz IGBT based experimental circuit.

  • The WHO separates odontogenic carcinomas into three groups : malignant ameloblastoma, primary intraosseous carcinoma(PIOC), and carcinomas arising from odontogenic epithelium including those arising from odontogenic cysts. In WHO criteria, primary intraosseous carcinoma is defined as a squamous cell carcinoma arising within the jaw, having no connection with the oral mucosa, and no developing from residues of odontogenic epithelium. This is a case of 52-year old man who had prolonged jaw pain and final diagnosis was primary intraosseous carcinoma(PIOC) on mandible. We obtained successful result after composite resection combined with hemimandibulectomy, RND, following reconstruction with latissmus dorsi myocutaneous flap, and postoperative radiation therapy.

  • Two-phase flow in a combustion chamber is experimentally analyzed according to the five different conditions in the Reynolds numbers of $1.02{\times}10^4$. As the height difference between the primary and secondary jets increases, the secondary has a little less effect on the primary one in the case on the same height difference, the primary jet is affects as the velocity of th secondary on increase. The primary-jet flow field cause the particle concentration since is controlled by the velocity of secondary jet, the height difference, and the angle of primary jet in the test section.

An Experimental Study on the NOx Formation of Fuel Staged Combustor (연료다단 연소기의 NOx 발생특성에 관한 실험적 연구)

  • The characteristics of NOx emission in multi fuel/air staged combustor have been experimentally studied. The design concept of multi fuel/air staged combustor is creation of two separate flame, a primary flame is act as a pilot flame for the secondary combustion stage combustion zone, where most of fuel burns. Experiments were performed on a semi-industrial scale (thermal input 0.233 ㎿) in a laboratory furnace and Liquefied Petroleum Gas(LPG) was used as primary and secondary fuels. The study included parametric study to identify the optimum operating conditions which are primary/secondary fuel ratio, primary/secondary air ratio, primary swirl intensity and secondary swirl intensity for reducing NOx emission. The test demonstrated that NOx emission can be reduced by >70% in accordance with operating conditions.

  • Multiple primary lung cancer is a rare disease entity and its clinical characteristics, treatment, and prognosis are poorly described. But the multiple primary lung cancer have a more favorable prognosis than locally recurrent or metastastic disease. Therefore, appropriate identification of multiple primary lung cancer will be very important. We have experienced a case of stage I multiple primary lung cancer in a 76-year-old male with two large mass in the right lower lobe without metastasis in the mediastinal lymph nodes with right mid and lower lobectomy. The microscopic pictures revealed adenocarcinoma in the one & small cell carcinoma in another. The post-operative courses were in uneventful for 4 months & but he was treated with chemotherapies, 2 times for complete remission of small cell carcinoma to now after discharge.

Prediction of Seasonal Variations on Primary Production Efficiency in a Eutrophicated Bay (부영양화해역의 내부생산효율에 대한 계절변동예측)

  • The Primary Production of phytoplanktons produces organic matter in high concentration in eutrophicated Hakata Bay, Japan, even during the winter season in spite of low water temperature. Phytoplanktons are considered to have any biological capabilities to keep activities of photosynthesis under the unfavorable conditions, and this affects water quality of the bay. In this study, seasonal variations in primary production efficiency were predicted by using a simple box-type ecosystem model, which introduced the concept of efficiency for absorption of solar radiation energy in relation to growth of phytoplanktons under the low solar radiation intensity. According to the simulation result of primary production, it was organic pollution comes from dissolved organic carbon (DOC) throughout the year, DOC of which is originated from the primary production of phytoplanktons on biological response of the seasonal variation of ambient conditions.

PANENDOSCOPIC EXAMINATION OF THE UPPER AERODIGESTIVE TRACT FOR THE DETECTION OF SECOND PRIMARY CANCERS IN HEAD & NECK CANCER PATIENTS (두경부암종 환자에서 상부 호흡소화기관에 병발한 원발성 중복암의 진단적 종합내시경검사)

  • The increasing incidence of multiple primary carcinomas occuring in the upper aerodigestive tract is well documented, with the accepted incidence being as high as 20-30%. The fiberoptic endoscopy has also enabled visualization of areas previously inaccessible without general anesthesia. A prospective panendoscopic examination of the upper aerodigestive tract was peformed on 104 patients with squamous cell carcinoma of head & neck in our hospital between 1989 and 1994. Five second primary cancers (4.8% :2 stomach, 2 esophagus, 1 lung cancers) were detected endoscopically. These finding should reinforce the belief that head & neck cancer is a panmucosal disease of the aerodigestive tract that silent second primary cancers are not uncommon. So every effort should be done to detect second primary cancers in head & neck squamous cell carcinomas. Panendoscopy has proved valuable in achieving that.
