Prevention in the United States Affordable Care Act

  • The Affordable Care Act (ACA) was signed into law on March 23, 2010 and will fundamentally alter health care in the United States for years to come. The US is currently one of the only industrialized countries without universal health insurance. The new law expands existing public insurance for the poor. It also provides financial credits to low income individuals and some small businesses to purchase health insurance. By government estimates, the law will bring insurance to 30 million people. The law also provides for a significant new investment in prevention and wellness. It appropriates an unprecedented $15 billion in a prevention and public health fund, to be disbursed over 10 years, as well as creates a national prevention council to oversee the government's prevention efforts. This paper discusses 3 major prevention provisions in the legislation: 1) the waiving of cost-sharing for clinical preventive services, 2) new funding for community preventive services, and 3) new funding for workplace wellness programs. The paper examines the scientific evidence behind these provisions as well as provides examples of some model programs. Taken together, these provisions represent a significant advancement for prevention in the US health care system, including a shift towards healthier environments. However, in this turbulent economic and political environment, there is a real threat that much of the law, including the prevention provisions, will not receive adequate funding.

국내 한의약공공보건사업 최근 연구 분석 (The Analysis of the Recent Research of Korean Medicine Public Health Promotion Programs in Korea)

  • Objective : The aim of this study is to analyze the recent research of Korean Medicine(KM) public health promotion programs in Korea. Method : We searched the study using Korean Medicine Information System, Research Information Service System and Korean Studies Information Service. We analyzed studies by research content, publication year and type of journal. Results : Analysis by research contents, it was divided into survey research, policy research, development research and evaluation research. Analysis by publication year, little research had been conducted from 1990 until 2000 and most of the research was done since 2000. Analysis by type of journal, 53% of studies published by journals related with KM. Conclusion : Development and evaluation research of KM public health promotion programs should be actively conducted. It is also need to establish the foundation in which KM public health promotion programs would be expanded not only KM field but also in other fields.

LonWorks-IP 가상 디바이스 네트워크상에서 예지 및 예방보전을 위한 DC 서보모터의 분산제어 (Distributed Control of DC Servo Motor on LonWorks-IP Virtual Device Network for Predictive and Preventive Maintenance)

  • LonWorks over IP(LonWorks-IP) virtual device network(VDN) is an integrated form of LonWorks device network and IP data network. In especially real-time distributed servo applications on the factory floor, timely response is essential for predictive and preventive maintenance. The time delay in servo control on LonWorks-IP based VDN has highly stochastic nature. LonWorks-IP based VDN induced transmission delay deteriorates the performance and stability of the real-time distributed control system and can't give an effective preventive and predictive maintenance. In order to guarantee the stability and performance of the system, and give an effective preventive and predictive maintenance, LonWorks-IP based VDN induced time-varying uncertain time delay needs to be predicted and compensated. In this paper new Pill control scheme based on Smith predictor, disturbance observer and band pass filter is proposed and tested through computer simulation about position control of DC servo motor. It is shown that how can the proposed control scheme be designed to minimize the effects of uncertain varying time delay and model uncertainties. The validity of the proposed control scheme is compared and demonstrated with the comparison of internal model controllers(IMC) based on Smith predictor with and without disturbance observer.

  • The purpose of this study is to investigate the actual conditions of herbal medicine products for the establishment of drug utilization review system. To accomplish of the purpose, we investigated medical treatment pay of insurance and consumption of herbal medicine products in pharmacy at jeollabuk-do, two oriental hospital etc. To gain valid and reliable the actual conditions of herbal medicine products, it needed close relationship with oriental hospital, society for manufacture of herbal medicines etc and further study classify herbal medicines by Korean Medicine.

잔여 유효 수명 예측 모형과 최소 수리 블록 교체 모형에 기반한 비용 최적 예방 정비 방법 (Cost-optimal Preventive Maintenance based on Remaining Useful Life Prediction and Minimum-repair Block Replacement Models)

  • Predicting remaining useful life (RUL) becomes significant to implement prognostics and health management of industrial systems. The relevant studies have contributed to creating RUL prediction models and validating their acceptable performance; however, they are confined to drive reasonable preventive maintenance strategies derived from and connected with such predictive models. This paper proposes a data-driven preventive maintenance method that predicts RUL of industrial systems and determines the optimal replacement time intervals to lead to cost minimization in preventive maintenance. The proposed method comprises: (1) generating RUL prediction models through learning historical process data by using machine learning techniques including random forest and extreme gradient boosting, and (2) applying the system failure time derived from the RUL prediction models to the Weibull distribution-based minimum-repair block replacement model for finding the cost-optimal block replacement time. The paper includes a case study to demonstrate the feasibility of the proposed method using an open dataset, wherein sensor data are generated and recorded from turbofan engine systems.

  • 현재 클러스터 시스템은 여러 분야의 문제들을 위하여 폭 넓게 이용되어지고 있으며 유용한 고성능 컴퓨팅 자원으로 인식되고 있다. 클러스터 시스템의 사용자가 늘어남에 따라 클러스터 시스템의 성능 개선 못지 않게 안정적인 운영을 유지하는 것도 중요한 상황이다. 하드웨어 예방 정비가 정상 운영을 위해서 중요한 것임에도 불구하고, 예방 점검 시간에 일반적인 클러스터 시스템을 위하여 사용될 수 있는 검사 도구는 주요 관심사가 되지 못했다. 본 논문에서는 하드웨어 예방 정비를 고려하여 클러스터 시스템을 위한 하드웨어 검사 도구를 제안한다. CTS(Cluster system Test Suite)로 명명된 클러스터 시스템 검사 도구는 메모리와 NIC를 점검하기 위한 두개의 검사 루틴을 가지고 있다. CTS를 설계시, CTS가 일반 적인 클러스터 시스템이 가지는 공통된 특징을 지원하도록 노력하였으며 검사 조건 설정에서 결과 조회까지 모든 작업은 통합 GUI 환경에서 진행될 수 있도록 하였다. 두 종류의 클러스터 시스템을 점검할 때, CTS를 사용하였고 클러스터 시스템을 관리하는데 유용한 정보가 제공됨을 확인하였다.

사업장 산업보건 정보관리 전산화 실태조사 (A Survey for Computerized Information System of Occupational Health Management at Worksite)

  • In order to develop the computerized information system of occupational health management at worksite, we surveyed actual states of computer use at worksites. We used a self-administrative questionnaire to the members of Korean Association of Occupational Health Nursing(KAOHN) from July 4 to August 21 in 1997. Among the members of KAOHN, 147 members answered. The worksites where they were employed were very diverse in aspect of jobs, locations, and size. Occupational health computerized system was used at 30(20.4%) worksites among 147 respondants. When they first introduced the computerized system the most difficult problem was the lack of support of manager. The programs that they have used mainly consist of drug management, health examination management, disease management, but the program of worksite environment management have been rarely used. Most users felt that the computerized system was effective, but there were problems in connection within programs. Many worksites have plans to take or expand the computerized information system within several years. It is necessary to develop the effective and integrated occupational health computerized system.

Probabilistic-based prediction of lifetime performance of RC bridges subject to maintenance interventions

  • In this paper, a probabilistic- and finite element-based approach to evaluate and predict the lifetime performance of reinforced concrete (RC) bridges undergoing various maintenance actions is proposed with the time-variant system reliability being utilized as a performance indicator. Depending on their structural state during the degradation process, the classical maintenance actions for RC bridges are firstly categorized into four types: Preventive type I, Preventive type II, Strengthening and Replacement. Preventive type I is used to delay the onset of steel corrosion, Preventive type II can suppress the corrosion process of reinforcing steel, Strengthening is the application of various maintenance materials to improve the structural performance and Replacement is performed to restore the individual components or overall structure to their original conditions. The quantitative influence of these maintenance types on structural performance is investigated and the respective analysis modules are written and inputted into the computer program. Accordingly, the time-variant system reliability can be calculated by the use of Monte Carlo simulations and the updated the program. Finally, an existing RC continuous bridge located in Shanghai, China, is used as an illustrative example and the lifetime structural performance with and without each of the maintenance types are discussed. It is felt that the proposed approach can be applied to various RC bridges with different structural configurations, construction methods and environmental conditions.

  • Purpose - This study surveyed real estate financial consumers and financial company staff regarding the components of the financial consumer protection system to seek detailed improvement plans for the Financial Consumer Protection Act. Design/methodology/approach - The Analytic Hierarchy Process (AHP) technique is applied. Findings - Both consumers and staff highly evaluated the importance of the preventive measures in the main classification factors. Regarding the sub-classification factors of preventive measures, consumers emphasized the responsible management of investment staff and financial institutions; however, the staff stressed the principle of effectiveness and efficiency. Regarding the elements of ex-post remedies, consumers answered that fast remedy would have a significant effect. At the same time, staff believed that punitive measures hinder free trading and investment activities. Regarding the sub-sub classification factors of prevention measures, the consumers value responsible management of staff and financial companies, while the staff tend to prefer the importance of the self-regulatory governance. Research implications or Originality - Based on the above results, financial regulatory authorities should find a balance between preventive and ex-post components once focusing on preventative measures. Our paper is one of the first research findings in this field of financial consumer protection system in Korea.

  • A data base design framework for predictive a preventive-maintenance system is presented in this paper in order to effectively control machines and reduce accident rates in the workplace. The data base is designed to meet general management requirements to evaluate different maintenance strategies. There are seven data files: the equipment list maintenace pesonnel, maintenance history, maintenance specification, spare part, maintenance equipment, and maintenance schedules. Each data base file has several record based upon data acquisition.
