선박사고 예방지수 설계에 관한 연구 (A Study On the Design of Preventive Index of Maritime Accidents)

  • 선박 항해기술 발달, 항해 교육 그리고 안전 법제 강화에도 불구하고 선박사고는 지속적으로 발생하고 있다. 이러한 해양사고의 저감을 목적으로 예방활동의 동기부여 및 촉진을 위해 예방활동을 정량적으로 평가하고 시각화한 선박사고 예방지수를 설계하였다. 제안된 예방지수는 지난 3년간(2010년~2012년)의 사고원인을 바탕으로 교육(Education), 기술(Engineering), 단속(Enforcement), 정보(Information) 영역에서 예방인자를 추출하여 평가모듈을 구성함으로서 예방지수를 산출하였다. 선박 종류별 예방지수를 측정한 결과 사고 발생률과 상관관계가 있는 유의한 결과를 얻었다. 평가모듈과 검증 및 조사과정을 보완하면 실제 선박사고의 예방에 활용이 기대된다.

  • Cancer is a disease which has the huge burden in worldwide, and cancer is the number one cause of death in Korea. At this point, the new framework for cancer monitoring index is required for regional cancer monitoring. Especially, cancer survivors are the important target which is rapidly increasing recently, also cancer survivor's quality of care should be considered in the cancer monitoring index framework. To develop the Multidimensional Cancer Monitoring Index considering cancer survivor's quality of care, we took into account cancer continuum which including prevention, detection, diagnosis, treatment, survivorship, assessment of quality of care and monitoring cancer patient, and end-of life care for stage. For target, components of health care delivery system such as patient, family, provider, payer, and policy maker are included. Also, Donabedian model which is a framework for examining health services and evaluating quality of health care such as structure, process, and outcome is applied to contents. This new cancer monitoring framework which includes multidimensional components could help to develop regional cancer monitoring index, and to make national cancer management and prevention policy in the future.

Non-linear Relationship Between Body Mass Index and Lower Urinary Tract Symptoms in Korean Males

  • Objectives: The purpose of this study was to evaluate the association between body mass index (BMI) and severe lower urinary tract symptoms (LUTS) in Korean males. Methods: This study was conducted on males aged ${\geq}50years$ who participated in the 2011 Korean Community Health Survey. LUTS severity was assessed using the Korean version of the International Prostate Symptom Score (IPSS) questionnaire, and was dichotomized as severe (IPSS >19) and non-severe ($IPSS{\leq}19$). BMI was divided into 6 categories: <18.5, 18.5-22.9, 23.0-24.9, 25.0-27.4, 27.5-29.9, and ${\geq}30.0kg/m^2$. To evaluate the relationship between BMI and LUTS, a survey-weighted multivariate Poisson regression analysis was performed to estimate prevalence rate ratios (PRRs). Age, smoking status, alcohol intake, physical activity, educational level, household income, and comorbidities were adjusted for in the multivariate model. Results: A U-shaped relationship was detected between BMI and severe LUTS. Compared with a BMI of $23.0-24.9kg/m^2$, the PRR for a BMI < $18.5kg/m^2$ was 1.65 (95% confidence interval [CI], 1.35 to 2.02), that for a BMI of $18.5-22.9kg/m^2$ was 1.25 (95% CI, 1.09 to 1.44), that for a BMI of $25.0-27.4kg/m^2$ was 1.20 (95% CI, 1.00 to 1.45), that for a BMI of $27.5-29.9kg/m^2$ was 1.11 (95% CI, 0.83 to 1.47), and that for a BMI ${\geq}30.0kg/m^2$ was 1.85 (95% CI, 1.18 to 2.88). Conclusions: This study showed that both high and low BMI were associated with severe LUTS.

오염물질기준지수(Pollutant Standards Index)를 이용한 대기질의 평가 -서울특별시 대기오염도에 대하여- (An Assessment of Air Pollution using Pollutant Standards Index (PSI) : $1983{\sim}1984$ in Seoul)

  • In order to assess the general health effect due to air pollution and to determine the critical pollutant which is meant by the worst effect to health among various pollutants in Seoul, air quality measurements at 10 sites during $1983{\sim}1984$ were analyzed using Pollutant Standards Index(PSI) : This index has been ultimately proposed to describe the comprehensive degree of the air pollution by U.S. Environmental Protection Agency(EPA) since 1976. Total average of PSI in Seoul during the two years was 139 that is described in 'unhealthful' The highest PSI appeared in Winter 183 through the 2 years, the lowest in Summer 99, and Spring 129 and Fall 150 respectively, PSI in Deungchon dong and Seongsu dong which are industrial areas were very high in terms of 'very unhealthful', while shinlim dong was shown in low in terms of 'good'. TSP was a priority pollutant in Seoul as the most frequently occuring critical pollutant. Its frequency was 76.4% in Spring, 86.0% in Summer, 78.0% in Fall and 44.2% in Winter during the 2 years. In Winter, $TSP{\times}SO_2$ was an important term as the critical pollutant in Seoul. Oxidant was the most frequently occuring critical pollutant in Kwangwhamoon through the whole seasons. It was recommended that the PSI could be used to assess the air pollution administratively and legally in context with publich health.

일부 제조업 근로자의 스트레스와 정신건강에 관한 연구 (Work Stress and Mental Health of the Industrial Workers)

  • Information on work stress and mental health status of industrial workers was required for optimum staffing and health care management. This study dealt with the relationship between mental health status examined by PSI(psychiatric symptom index) and socio-demographic variables. Especially this study attempted to find relevant determinants of the stress and mental health through multiple stepwise regression analysis based on data obtained from 687 occupational workers. The findings of this study were as follows; Mean scores of symptom dimension were higher in youth, female, more educated, unmarried and divorced, and workers in the noisy condition and short duration in job. And it showed high scores in long duration in job for anxiety; college educated for anger; unmarried for cognitive disturbance. The factors affecting the mean scores of psychiatric symptom index were varied according to the types of symptom: age, sex and duration in job for anxiety; age, sex, living with parent, education and marital status for anger; sex and marital status for depression; noisy condition and sex for cognitive disorder.

산업장 근로자의 직업성 스트레스와 정신건강에 관한 연구 (Research on work stress and mental health status of the industrial workers)

  • Recently concerns on work stress and mental health status of industrial workers were increased. The purpose of this study was to investigate the work stress measured by PSI (psychiatric symptom index) according to socio-demographic variables. The study population was consisted with two types of group; manual and clerical workers. The major findings of this study were as follows; Mean scores of psychiatric symptom in manual workers were higher in female, youth, educated, unmarried and divorced, noisy condition, that in clerical workers was higher in aged. Mean scores of psychiatric symptom were highly affected by degree of work autonomy, self-esteem, and self-efficacy in both groups. The higher the work autonomy, self-esteem, and self-efficacy, the lower the mean scores of work stress it showed. Psychiatric symptom index was explained with the work autonomy, self-esteem, self-efficacy through multiple stepwise regression. The annual admission rates were high in highly ranked groups on each psychiatric symptom dimension.
