비스포스포네이트 관련 악골괴사의 진단 및 치료에 대한 임상적 연구 (Clinical study of diagnosis and treatment of bisphosphonate-related osteonecrosis of the jaws)

  • Introduction: Bisphosphonates is used widely for the treatment of the Paget's disease, multiple myeloma, bone metastases of malignant tumors with the prevention of pain and their pathological fracture. However, it was recently suggested that bisphosphonates related osteonecrosis of the jaw (BRONJ) is a side effect of bisphosphonate use. Materials and Methods: Twenty-four individuals, who were referred to the Department of Oral and Maxillofacial surgery, Dankook University Dental Hospital, were selected from those who had exposed bone associated with bisphosphonates from January, 2005 to December, 2009 according to the criteria of American Association of Oral and Maxillofacial Surgeons (AAOMS) for BRONJ. The patients group consisted of 7 males and 17 females between the age of 46 to 78 years (average 61.8 years). Each patient had panoramic imaging, computed tomography (CT), whole body bone scanning performed for a diagnosis and biopsy sampling from the necrotizing tissue. C-terminal cross-linking telopeptide of type I collagen (CTX) level of patients who had undergone surgical intervention was measured 7 days before surgery. Results: The main cause of bone exposure was post-extraction (15), chronic periodontitis (4), persistent irritation of the denture (3). Twenty people had undergone BRONJ treatment for two to eight months except for 4 people who had to maintain the bisphosphonates treatment to prevent a metastasis and bone trabecular pain with medical treatment. When the bisphosphonate treatment was suspended at least for 3 months and followed up according to the AAOMS protocols, the exposed necrotizing bones were found to be covered by soft tissue. Conclusion: Prevention therapy, interruption of bisphophonates for at least 3 months and cooperation with the physician for conservative treatment are the essential for treating BRONJ patient with high risk factors. The CTX level of BRONJ patients should be checked before undergoing surgical intervention. Surgical treatments should be delayed in the case of a CTX level <150 pg/mL.

척추의 중심점과 Modified U-Net을 활용한 딥러닝 기반 척추 자동 분할 (Deep Learning-based Spine Segmentation Technique Using the Center Point of the Spine and Modified U-Net)

  • Osteoporosis is a disease in which the risk of bone fractures increases due to a decrease in bone density caused by aging. Osteoporosis is diagnosed by measuring bone density in the total hip, femoral neck, and lumbar spine. To accurately measure bone density in the lumbar spine, the vertebral region must be segmented from the lumbar X-ray image. Deep learning-based automatic spinal segmentation methods can provide fast and precise information about the vertebral region. In this study, we used 695 lumbar spine images as training and test datasets for a deep learning segmentation model. We proposed a lumbar automatic segmentation model, CM-Net, which combines the center point of the spine and the modified U-Net network. As a result, the average Dice Similarity Coefficient(DSC) was 0.974, precision was 0.916, recall was 0.906, accuracy was 0.998, and Area under the Precision-Recall Curve (AUPRC) was 0.912. This study demonstrates a high-performance automatic segmentation model for lumbar X-ray images, which overcomes noise such as spinal fractures and implants. Furthermore, we can perform accurate measurement of bone density on lumbar X-ray images using an automatic segmentation methodology for the spine, which can prevent the risk of compression fractures at an early stage and improve the accuracy and efficiency of osteoporosis diagnosis.

성인여성의 식습관과 영양섭취상태와 골밀도 및 골무기질함량과의 관계 (The Relationship between Food Habit, Nutrient Intakes and Bone Mineral Density and Bone Mineral Content in Adult Women)

  • Bones are important parts in sustaining the shape of the body, but they are also metabolic organs which undergo bone remodeling by constant bone resorption and formation. Osteoporosis, the typical metabolic bone disease, is characterized by a reduction in bone mineral density (BMD). Women more than men are at risk fir osteoporosis-related fractures, especially in the lumbar spine, wrist, and hip region. Risk of fracture depends on one's BMD, which open determined by the peak bone mass value achieved at skeletal maturity and followed by subsequent age-and menopause-related bone loss. Genetic and environmental factors are known to play a key role in bone metabolism and diet is considered as one of the important environmental factors. The purpose of the present study was to assess the status of BMD and bone mineral content(BMC) to clarify the relationships between dietary intakes and the risk of osteoporosis in adult women in Taegu. Subjects were 130 healthy females in between 20 and 69 years of age. BMD and BMC of the lumbar spine(venebrae L2-4) of the subjects were measured by dual energy X-ray absorptiometry. The average age of the subjects was 47.4${\pm}$11.7 years old, the average weight was 57.2${\pm}$8.4kg, the average age of menarche was 16.6${\pm}$1.9 years old and the average age of menopause was 48.4${\pm}$5.3 years old. The nutrient intakes of the subjects measured by the convenient method were generally lower than the level of RDA. The result of nutrient intake assessed has shown that the average energy intake was 1701${\pm}$316kca1 which is 85.1% of the RDA and the average calcium intake was 485.4${\pm}$172.3mg which is 69.3% of the RDA. The intakes of protein, vitamin A, vitamin B$_1$, niacin were greater than the RDA, whereas the remaining nutrient intakes were lower than the RDA. The average BMD of the subjects was shown to be 1.06${\pm}$1.09g/$\textrm{cm}^2$. The highest BMD of 1.24${\pm}$0.14g/$\textrm{cm}^2$ was noticed in the subjects of 30s compared to 20s, 40s, 50s, 60s. The BMD values were compared by the relative body weight(R3W) of the menopause subjects, and it was found that the underweight group had significant lower BMB while the rest of the groups did not have any differences in BMD. The most strongly correlated nutrient with BMD among the menopause subjects appeared to be calcium. The women whose Ca intakes were higher than 500mg showed the significantly higher BMD than those with Ca intakes lower than 500mg. This study suggests that the most effective way to prevent osteoporosis and to reduce the incidences of fractures seems to be minimizing bone loss through the adequate intake of calcium as well as avoiding underweight, especially in menopausal women. Inhibitory Effects of Curcuma xanthorrhiza Supercritical Extract and Xanthorrhizol on LPS-Induced Inflammation in HGF-1 Cells and RANKL-Induced Osteoclastogenesis in RAW264.7 Cells

  • Periodontal disease is triggered by the host immune response to pathogens in the microbial biofilm. Worsening of periodontal disease destroys the tooth-supporting tissues and alveolar bone. As oral inflammation can induce systemic diseases in humans, it is important to prevent periodontal disease. In this study, we demonstrated that Curcuma xanthorrhiza supercritical extract (CXS) and its active compound, xanthorrhizol (XAN), exhibit anti-inflammatory effects on lipopolysaccharide (LPS)-treated human gingival fibroblast-1 cells and anti-osteoclastic effects on receptor activator of nuclear factor kappa B ligand (RANKL)-treated RAW264.7 cells. LPS-upregulated inflammatory factors, such as nuclear factor kappa B p65 and $interleukin-1{\beta}$, were prominently reduced by CXS and XAN. In addition, RANKL-induced osteoclastic factors, such as nuclear factor of activated T-cells c1, tartrate-resistant acid phosphatase, and cathepsin K, were decreased in the presence of CXS and XAN. CXS and XAN inhibited the mitogen-activated protein kinase (MAPK)/activator protein-1 (AP-1) signaling pathway. Collectively, these results provide evidence that CXS and XAN suppress LPS-induced inflammation and RANKL-induced osteoclastogenesis by suppressing the MAPK/AP-1 pathway.

발효 시금치 추출물의 무기인산염에 의해 유도된 혈관 석회화 저해 효과 (Inhibitory Effect of Fermented Spanish Extract on Inorganic phosphate-induced Vascular Calcification in ex vivo Aortic Rings)

  • Spinach (Spinacia oleracea L.), a green leafy vegetable, is well known as a functional food due to its biological activities. Vascular calcification is associated with several disease conditions including atherosclerosis, diabetes, and chronic kidney disease (CKD), and is known to raise the risk of cardiovascular diseases related morbidity and mortality. However, there are no previous studies that have investigated the effects of fermented spinach exract (FSE) against aortic and its underlying mechanisms. Therefore, this study investigated the effects and action of possible mechanisms of FSE on inorganic phosphate (PI)-induced vascular calcification in ex vivo mouse aortic rings. PI increased vascular calcification through calcium deposition in ex vivo aortic rings. FSE inhibited calcium accumulation and osteogenic key marker, runt-related transcription factor 2 (Runx2), and bone Morphogenetic Protein 2 (BMP-2) protein expression in ex vivo aortic rings. And, FSE inhibited PI-induced extracellular signal-regulated kinase (ERK) and p38 phosphorylation in ex vivo aortic rings. These results show that FSE can prevent vascular calcification which may be a crucial way for the prevention and treatment of vascular disease association with vascular calcification.

갱년기 유도 흰쥐에서 감태 추출물이 골 대사 지표물질의 변화에 미치는 영향 (The Effect of Ecklonia cava Extracts on Bone Turnover Markers in Ovariectomized Rats)

  • 갱년기의 여성에는 여러 폐경 증후들이 나타나는데, 특히 에스트로겐의 감소로 인한 골 손실의 증가 등으로 골다공증질환의 발병율이 높아지게 된다. 이러한 갱년기 증후군을 치료하기 위해 호르몬 대체 요법으로 치료를 하고 있으나 유방암과 자궁암 등의 부작용에 대한 논쟁의 여지가 있어 갱년기 증후군을 치료하는데 양면성을 가진다. 그러므로 본 연구에서는 쉽게 섭취 할 수 있으며, 부작용이 없는 해조류인 감태 추출물을 이용하여 in vivo 실험을 통해 갱년기 장애 시 나타날 수 있는 골 손실에 미치는 영향을 검토하였다. 난소 절제술을 시행한 군들에서 전체적으로 체중이 증가하였으나, 난소절제 후 감태 추출물을 투여에 의한 체중 감소는 나타나지 않아 감태 추출물이 체중 감소에는 크게 영향을 미치지 않은 것으로 나타났다. 난소절제 시(OVX-CON) 에스트로겐 결핍으로 bone turnover가 증가되어 비 난소절제(SHAM)군에 비해 혈장 중의 ALP의 활성이 증가되었으나, 난소 절제 후 감태 추출물을 투여한 군에서 그 활성이 유의적으로 감소하는 경향이 나타났다. 이것은 난소 절제 후 에스트로겐의 분비가 감소되는데 반해 감태 추출물 투여가 에스트로겐 대체 작용을 함으로써 난소절제로 인한 골 손실 정도를 완화시켜 준 것으로 추측되어진다. 골 용해 지표인 혈액 중의 CTx는 난소절제군이 비 절제군에 비해 높은 수준을 보여 난소 절제군에 있어 골 용해가 증가되었음을 나타내었다. 반면 감태 추출물의 투여로 인해 CTx 함량이 감소하였으며, EC-200군의 경우 SHAM과 비슷한 수준으로 나타났다. 이는 난소절제로 인해 bone turnover가 증가된 상태에서 감태 추출물의 투여로 bone turnover가 감소하여 골 흡수 저해 효과를 타나낸 것으로 추정된다. 또한 결합조직 중의 collagen 함량은 난소절제로 인하여 감소하였으나, 감태 추출물의 투여에 의해 점진적으로 회복되는 경향을 나타내었다. 이상의 결과로부터 estrogen 부족으로 인한 골 손실의 예방에 감태가 유익한 효과를 가지며, 이를 활용한 기능성식품 개발도 가능할 것으로 기대되어 진다.

사람의 체내에서 타우린의 역할에 관한 연구 (Studies on the Function of Taurine: Review)

  • Taurine is an abundant amino acid in many animals, including humans. Relatively large amounts of taurine are found in leukocytes, heart, muscles, retinas, kidneys, bones, and liver. Taurine has antioxidant effects; it reacts with hydrogen peroxide to prevent oxidation of the cell membrane. Taurine enhances the effects of anticancer drugs, while also reducing side effects, and taurolidine, a taurine derivative, has been shown to exhibit anti-cancer effects without notable side effects in several types of cancer. Taurine aids in cholesterol metabolism by increasing the rate of synthesis of bile acids, and, thus, reduces triglyceride levels. In addition, taurine is involved in the growth and differentiation of nerve cells and is associated with some neurological disorders. Taurine aids in bone formation and prevents bone dissolution. Moreover, taurine prevents liver damage from a variety of drugs and, thus, protects the liver. Taurine is involved in the development and function of the retina and lens. It also has anti-atherosclerotic and anti-thrombotic effects that protect against cardiovascular disease. Taurine may have additional physiological functions, and warrants further investigation.

Evaluation of prognosis related to compliance with supportive periodontal treatment in patients with chronic periodontitis: a clinical retrospective study

  • Purpose: The purpose of this study was to evaluate the prognostic effect of patient compliance with supportive periodontal treatment (PC-SPT). Chronic periodontitis patients were classified based on their compliance level, and factors affecting PC-SPT and the prognosis of PC-SPT were investigated. Methods: This study selected 206 patients who started SPT after receiving periodontal treatment between 2010 and 2012. Patients who continued SPT through February 2016 were included. The patients were classified according to whether they exhibited complete compliance (100% of visits), excellent compliance (${\geq}70%$ of visits), incomplete compliance (<70% of visits), or non-compliance (only 2 visits). Patient characteristics that could affect PC-SPT, such as age, sex, distance of the clinic from their residence, implantation, and periodontal treatment, were investigated. The number of newly decayed and extracted teeth, alveolar bone level changes around the teeth and implants, and implant removal were examined to evaluate the prognosis of PC-SPT. Results: Sex and the presence of an implant significantly affected PC-SPT. Additionally, the number of newly decayed and extracted teeth and changes in alveolar bone levels around the teeth and implants were significant prognostic factors related to PC-SPT. Conclusions: PC-SPT in chronic periodontitis patients will help maintain periodontal health and prevent further periodontal disease.

Human immunodeficiency virus에 감염된 환자의 악교정수술 : 증례보고 (Orthognathic surgery of human immunodeficiency virus infected patient : A case report)

  • Human immunodeficiency virus is a retrovirus that causes acquired immunodeficiency syndrome. Acquired immunodeficiency syndrome is defined in terms of "either the occurrence of specific diseases in association with a HIV infection or a CD4 cell count below 200cells/ul" by centers for disease control and prevention(CDC). When performing the surgery of human immunodeficiency virus infected patients, several factors should be considered. First, standard precautions should be performed to prevent infection. It is safe to treat human immunodeficiency virus infected patients if we follow the standard precautions. Second, when making a surgical plan, surgeons have to take account of delayed bone healing and postsurgical infection. This case report presents a case of orthognathic surgery of human immunodeficiency virus infected patient.

측두하악관절에서 발생한 건활막 거대세포종 - 증례보고 - (Tenosynovial Giant Cell Tumor of the Temporomandibular Joint - Case Report -)

  • A rare case of tenosynovial giant cell tumor of invading left temporomandibular joint(TMJ) extending the middle cranial fossa is reported. Report of this entity in the temporomandibular joint is rare. A 39-year old male patient presented with mild discomfort and swelling in preauricular area. MR image demonstrated a destructive lesion of the left TMJ, with bony erosion of temporal bone and extension into middle cranial fossa. The patient underwent radiotherapy as adjuvant therapy to prevent recurrence, followed by resection. The histological and radiological features of this tumor are discussed along with a brief description of the disease entity.
