Legal Status of Medical Personnel on Medical Records (환자의 의무기록 관련 의료인의 법적 지위)

  • This study is a paper reviewed legal status of medical personnel and issues of law on recently discovered medical records. As the increase of medical personnel who have gone through the administrative disposal in regards to the medical records, it is needed to examine the legal issue or dispute on the medical records under the current law. Medical records are the statement on patient's medical conditions made by the medical personnel. This records are used as important source for patient's further treatment. This becomes the communication route between the patients and the other medical personnel, and it provides the patients a right to find out their medical information. According to the Medical Service Act (Article 21), a medical personnel shall prepare respectively a record book of medical examination and treatment. And medical personnel shall make a signature. Furthermore, the medical personnel or the opener of the medical institutions must preserve the record book (including an electronic medical record). Meanwhile, the issues of a ban on false entry, additional record, revision or manipulation on the medical record have been recently on the rise. This paper briefly examined the major issues in regards to the medical records. It especially clarified the legal duty on medical records and its major-contentious-issues. At the same time, it pointed out the problems of the unreasonable over interpretation of the law. Furthermore, this suggested the guidelines for the further discussion and review.

The effects of Hamstring Stretching Exercise on Angle of Lumbar Flexion and Pain Reduction with Chronic Low Back Patients (슬괵근 스트레칭운동이 만성요통 환자의 요부 굴곡각도와 통증감소에 미치는 효과)

  • The purpose of this research is to study how the stretching exercise along with heat and electric therapy can effect on Angle of Lumbar Flexion and Reduction of Pain against the Hamstring Construction Symptom that can be found in the Chronic Low Back Patients. The target of research was 28 person selected out of the patients diagnosed as Lumbar Sprain and Lumbar Syndrome, who have positive from the active test of Hamstring construction and passive test. Their age range is 20 to 60 years old, 14 male and 14 female. As a research method, simple heat and electric therapy was applied to counter part while Hamstring Stretching Exercise including simple heat and electric therapy was applied to the experimental part. After 6 weeks treatment for each part, the results of effect on Angle of Lumbar Flexion and Pain Reduction was compared and analysed on the basis of the gravity angle meter and Visual Analogue Scale (VAS). The outcome of research is as follows: 1. Angle of Lumbar Flextion increased effectively not in the conventional counter group but the experimental group to which Hamstring Stretching was applied simultaneously. 2. remarkable effect was shown in the experimental part applied Hamstring Stretching. Consequently from the results of this study, we could find out the fact that the therapy combined preserve treatment and hamstring stretching not only produced increasing Lumbar Flexion Angle but also was more effective on reducing pain of the patients according to it than the conventional or preserve therapy.

  • To preserve straw sandals excavated at Goongnamji site, this study examined the weight change rate by applying the methods of Primal MC-76 and vacuum freeze-drying used mostly as a earth-layer hardening material among PEG and acrylic resin, which are applied widely for preservation of waterlogged archaeological wood, as a means to preserve plant-based cultural properties along with the examination of the subject material. An experiment was also performed on moisture absorption after treatment. The materials of sandals were identified as Typha spp. The weight change experiment revealed that the vacuum freeze-drying after being treated with PEG 2-Step showed the most stabilized changes in weight, while it offered the smallest change in color as well.

  • Just as culture is a collection of lifestyles, Cultural Properties is what is made over a long period of time and is left around us, whether it takes form or not. Many countries around the world have their own standards and are trying hard to preserve those with precious values. And in the case of Modern Architecture, because they are still in use In our daily lives and for the lack of recognition, they are continuously threatened during urban changes and developments. The purpose of the research is not to preserve Modern Architecture as done In the museum, but to seek a way for it to coexist with the changing environments of contemporary cities. Therefore, it is to recognize Modern Architecture as valuable assets, which expresses historicity in urban settings, and to seek the design characteristics of Modern Architecture. This is a process in order to find a new method for conservation of historicity. The result are as followings. Modern Architecture have their design characteristics and these can be applied as a design guideline. Therefore the form, material, and the colors of individual Modern Architecture can be analyzed so as to create a design guideline according to their styles. According to the analysis of Registered Cultural Properties in Seoul, they can be categorized into 6 uses and found the design characteristics of individual uses.

Binary Image Watermarking for Preserving Feature Regions (특징영역을 보존한 이진영상의 워터마킹)

  • In this paper, an effective digital watermarking method for copyright protection of binary image data is proposed. First a binary image is grouped into feature regions which has geometrical features and general one. The watermark for authentication is embedded in general regions in order to preserve geometrical features regions. We have used run-length code and special runs for grouping feature regions and general one. For invisibility of watermark, we have embedded the watermark considering transition sensitivity of each pixel in general regions. The proposed method is applied some binary image such as character, signature, seal, and fingerprint image to evaluate performance. By the experimental results, the proposed method preserve feature regions of original image and have higher invisibility of watermarks.

Subarachnoid Hemorrhage from a Dissecting Aneurysm of the Posterior Cerebral Artery in a Child : Rebleeding after Stent-Assisted Coiling Followed by Stent-Within-Stent Technique

  • Pediatric patients with dissecting aneurysms usually present with ischemia rather than bleeding. We report a case of a 15-year-old boy with a dissecting aneurysm of the posterior cerebral artery (PCA) presenting with hemorrhage. He was first treated with stent-assisted coil embolization, in an attempt to avoid trapping of the PCA and preserve the perforators. After the procedure, he recovered well from general anesthesia, but rebleeding occurred from the same lesion 6 hours after the procedure, therefore endovascular segmental occlusion of the parent artery was performed secondarily. Apparently, a reconstructive method of stent-assisted coiling is worth trying to preserve the parent vessel and perforators, but it is not always efficient and durable for dissecting aneurysms.

Structure and Management Plan of the Spontaneous Herbaceous Communities in Midongsan Arboretum, Chungcheongbuk-do (충청북도 미동산수목원의 자생 초본군락 구조 및 관리방안)

  • The purpose of this study was to examine the ecological characteristics of herbaceous communities by systematic and scientific analysis of their structure and diversity in the Midongsan Arboretum and to offer raw data for a long-tenn monitoring study. The importance value and diversity index of species appearing in twenty plots from July to September, 2004 were analyzed and a management plan for these communities is presented. Vascular plants were represented by 60 taxa of 23 families, 51 genera, 50 species and 10 varieties. Based on the results of importance value analysis, the most dominant species was Artemisia princeps var. orientalis, followed by Setaria viridis and Erigeron canadensis. The diversity index analysis showed that plot no. 5 had the highest H' and H'_{max}$(2.0135 and 2.6391). It's species composition was comparatively more diverse and it's structure more stable than other plots. Artemisia montana and Dactylis glomerata showed the highest correlation between species. Because herbaceous communities are important biological habitats and provide important function in environmental conservation, it is important to properly preserve these communities. At the same time, in order to preserve genetic resources and improve spatial function, it may be necessary to consider removing herbaceous communities in certain areas. In the future, the relations between physicochemical soil properties and herbaceous communities should be examined and community movement should be studied.

A General Plan for the Preservation and Practical Use of Junkyung and Youngkyung Tombs at Samcheok (삼척 준경묘와 영경묘의 보존 및 활용을 위한 기본계획)

  • This research aims to make the general plan for the preservation and practical use about Junkyung and Youngkyung tombs at Samcheok. The general plan is classified according to three large territories. First, it is necessary for these two tombs to preserve territories and strengthen the historical identity in order to restore the spoiled building such as Subokbang and Surabang by verification and excavation. Second, to preserve the nature and environment of territories surrounding the two tombs is essential for the extension of the essence about these cultural heritage to improve the familiarity and effective utilization by trial programs, which are related to the historical background of the tombs and so forth. Third, supporting and making convenient facilities in the neighborhood near cultural assets is required to install public parks and parking lots for visitors. Additionally, it is needed to advertise successively about these two tombs as the beginning of Chosun Dynasty tombs Also, it is necessary to consider implementing programs because Junkyung and Youngkyung tombs along with Chosun royal tombs in the north of Korea might be designated as World Culture Heritage if Korea should be a unified nation.

Surgical Strategies for Prevention and Treatment of Airway Aspiration in Head and Neck Cancer Patients (두경부암 환자에서 기도 흡인의 예방과 치료를 위한 수술 전략)

  • Postoperative airway aspiration is not uncommon in patients with head and neck cancer. Airway aspiration has serious consequences, such as swallowing disorders, nutrition-related health problem, or reducing the quality of life due to maintenance of tracheal or nasogastric tubes. The postoperative oropharyngeal defect due to the surgery may interfere with normal swallowing reflex, or the laryngeal dysfunction caused by radiation therapy may cause severe airway aspiration, which may lead to complications such as dyspnea and pneumonia. Complete removal of the disease is also important in the treatment of head and neck cancer, but it is necessary to select a method to avoid and predict the occurrence of airway aspiration according to the treatment method. The most important factor to prevent airway aspiration after surgery is to preserve the proper volume of the oropharynx and to preserve at least one of the cricoarytenoid joint function. It is also the most effective way to reduce additional complications by seeking appropriate surgical treatment according to airway aspiration status. The purpose of this study is to review the operative methods that can induce airway aspiration and consider the prevention and treatment strategy through review of the literature.

DIFFUSION PIECEWISE HOMOGENIZATION VIA FLUX DISCONTINUITY RATIOS

  • We analyze piecewise homogenization with flux-weighted cross sections and preservation of averaged currents at the boundary of the homogenized domain. Introduction of a set of flux discontinuity ratios (FDR) that preserve reference interface currents leads to preservation of averaged region reaction rates and fluxes. We consider the class of numerical discretizations with one degree of freedom per volume and per surface and prove that when the homogenization and computing meshes are equal there is a unique solution for the FDRs which exactly preserve interface currents. For diffusion submeshing we introduce a Jacobian-Free Newton-Krylov method and for all cases considered obtain an 'exact' numerical solution (eight digits for the interface currents). The homogenization is completed by extending the familiar full assembly homogenization via flux discontinuity factors to the sides of regions laying on the boundary of the piecewise homogenized domain. Finally, for the familiar nodal discretization we numerically find that the FDRs obtained with no submesh (nearly at no cost) can be effectively used for whole-core diffusion calculations with submesh. This is not the case, however, for cell-centered finite differences.
