경추 추간판 탈출증을 진단함에 있어 경추부 자기공명 영상 소견과 비교한 Spurling test의 진단적 타당성에 대한 연구 (A Study on Diagnostic Validity of Spurling Test Compared to Magnetic Resonance Imaging Findings in Diagnosing Herniated Cervical Intervertebral Disc)

  • Objectives : Spurling test is one of the most widely used physical tests to diagnose herniated cervical intervertebral disc. The purpose of this study is to analyse diagnostic validity of Spurling test. Methods : This study was carried out on 382 subjects who visited Bu-Chun Ja-Seng oriental hospital with the neck pain or radiating pain from neck to upper extremities from November, 2007 to October, 2008. All subjects underwent both Spurling test and cervical magnetic resonance imaging(MRI). The associations between Spurling test and herniated cervical intervertebral disc were analysed. Results : 1. The sensitivity of Spurling test for herniated cervical intervertebral disc was 64%. 2. The specificity of Spurling test for herniated cervical intervertebral disc was 86%. 3. The positive predictive value of Spurling test for herniated cervical intervertebral disc was 93%. 4. The negative predictive value of Spurling test for herniated cervical intervertebral disc was 44%. Conclusions : This study suggests that Spurling test is not useful in screening for herniated cervical intervertebral disc due to the test's relatively low sensitivity. Thus it is necessary to consult advanced tests such as diagnostic imaging to diagnose herniated cervical intervertebral disc.

암 환자에서 우울증의 선별도구로서 단축형 에딘버러 우울증 척도의 한국 표준화 (The Standardization of the Korean Version of Brief Edinburgh Depression Scale as a Screening Tool for Depression in Cancer Patients)

  • Objectives : Depression is a common psychiatric disorder in cancer patients. The Brief Edinburgh Depression Scale(BEDS), which is an abbreviated version of the Edinburgh Depression Scale, may serve as a useful tool in screening for the depression in patients with the medical illnesses. This report investigated the reliability and validity of the Korean Version of the BEDS(K-BEDS) for the depression in cancer patients. Methods : One-hundred cancer patients were enrolled in this study. All subjects completed the K-BEDS, the Hospital Anxiety Depression Scale(HADS), and the Karnofsky Performance Status Scale(KPSS). Reliability, validity and Receiver Operating Characteristic(ROC) curve analysis measures were assessed. Results : The K-BEDS showed good internal consistency(Cronbach ${\alpha}$=0.77) and test-retest reliability(0.94, p<0.001). All item-total correlations were above 0.3. Also, it revealed moderate correlation with the depression subscale of the HADS(r=0.617), but no correlation with the KPSS. Exploratory factor analysis produced only one factor, accounting for 47.1% of the total variance. The most valid cutoff value to screen for depression was a total score of 5 on the K-BEDS, which showed sensitivity of 62.5% and specificity of 86.4% with a positive predictive value of 4.60 and a negative predictive value of 0.43.

Validity of the Korean Developmental Screening Test for very-low-birth-weight infants

  • Purpose: The importance of the neurodevelopmental outcomes of very-low-birth-weight (VLBW) infants has been emphasized as their mortality rate has markedly improved. This study aimed to assess the validity of the Korean Developmental Screening Test (K-DST), a developmental screening tool approved by the Korean Society of Pediatrics, for the timely diagnosis of neurodevelopmental delay in VLBW infants. Methods: Subjects included VLBW infants enrolled in the Korean Neonatal Network database between January 2012 and December 2014. The collected data were analyzed for sensitivity, specificity, positive predictive value, and negative predictive value (NPV) in the K-DST compared to those in the Bayley Scales of Infant Development-II for VLBW infants. Results: A total of 173 patients were enrolled. Their mean gestational age and mean birth weight were $27.5{\pm}2.8weeks$ and $980.5{\pm}272.1g$, respectively. The frequency of failed psychomotor developmental index (PDI) <85 was similar to that in at least one domain of K-DST <1 standard deviation. Failure in more than one K-DST domain compared with a mental developmental index (MDI) <85 showed a sensitivity and NPV of 73.2% and 75.0%, respectively. Failure in more than one K-DST domain compared with PDI <85 showed a sensitivity and NPV of 60.3% and 71.6%, respectively. Each K-DST domain had a stronger correlation with predicting a failing MDI <85 than a failing PDI <85 (P<0.05). Conclusion: K-DST could be a useful screening tool for predicting mental developmental delay in VLBW infants and referring them for neurodevelopmental assessments.

태권도 공연 관람자의 인지된 가치 척도 개발 및 적용 (The Consumer Perceived Value in Taekwondo Performance Spectators: Scale Development and Validation)

  • 본 연구의 목적은 태권도 공연 관람자의 인지된 가치 척도를 개발하고 더 나아가 개발된 가치 척도에 대해 다양한 분석 방법을 이용하여 척도의 타당성, 신뢰성, 적용가능성을 평가하여 태권도 공연 관람자에 대한 후속 연구를 위한 기초 자료를 제공하는데 있다. 이에 따라 척도개발과 관련된 선행연구의 절차를 참고하여 8단계 방법인 (1) 선행연구 검토, (2) 예비 요인 및 문항 추출, (3) 문항의 정제 및 평가, (4) 예비조사, (5) 데이터 수집, (6) 문항의 타당성 확보, (7) 문항의 유효성 평가, (8) 문항의 적용가능성 검토를 실시하였다. 분석 결과 태권도 공연 관람자의 인지된 가치 척도는 문화적, 사회적, 쾌락적, 미적, 도덕적 및 실용적 가치 등 6가지 요인, 19문항으로 개발되었다. 한편, 개발된 가치의 적용가능성을 검토하고자 관람태도, 만족 및 미래 소비행동과의 관계를 분석한 결과 문화적, 사회적, 미적, 도덕적 가치는 관람태도에 유의한 영향을 미치는 것으로 나타났으며, 사회적, 쾌락적, 실용적 가치는 관람만족에 유의한 영향을 미치는 것으로 나타났다. 또한 문화적, 사회적, 미적 가치는 미래 소비행동에 유의한 영향을 미치는 것으로 나타나 개발된 가치 척도의 모든 요인은 소비자행동(태도, 만족 및 행동)에 영향을 미치는 요인임을 확인 하였다.

ICD-10을 이용한 ICISS의 타당도 평가 (Validation of the International Classification of Diseases 10th Edition Based Injury Severity Score(ICISS))

  • Objective : To compare the predictive power of International Classification of Diseases 10th Edition(ICD-10) based International Classification of Diseases based Injury Severity Score(ICISS) with Trauma and Injury Severity Score(TRISS) and International Classification of Diseases 9th Edition Clinical Modification(ICD-9CM) based ICISS in the injury severity measure. Methods : ICD-10 version of Survival Risk Ratios(SRRs) was derived from 47,750 trauma patients from 35 Emergency Centers for 1 year. The predictive power of TRISS, the ICD-9CM based ICISS and ICD-10 based ICISS were compared in a group of 367 severely injured patients admitted to two university hospitals. The predictive power was compared by using the measures of discrimination(disparity, sensitivity, specificity, misclassification rates, and ROC curve analysis) and calibration(Hosmer-Lemeshow goodness-of-fit statistics), all calculated by logistic regression procedure. Results : ICD-10 based ICISS showed a lower performance than TRISS and ICD-9CM based ICISS. When age and Revised Trauma Score(RTS) were incorporated into the survival probability model, however, ICD-10 based ICISS full model showed a similar predictive power compared with TRISS and ICD-9CM based ICISS full model. ICD-10 based ICISS had some disadvantages in predicting outcomes among patients with intracranial injuries. However, such weakness was largely compensated by incorporating age and RTS in the model. Conclusions : The ICISS methodology can be extended to ICD-10 horizon as a standard injury severity measure in the place of TRISS, especially when age and RTS were incorporated in the model. In patients with intracranial injuries, the predictive power of ICD-10 based ICISS was relatively low because of differences in the classifying system between ICD-10 and ICD-9CM.

An Improved Predictive Functional Control with Minimum-Order Observer for Speed Control of Permanent Magnet Synchronous Motor

  • In this paper, an improved predictive functional control (PFC) scheme for permanent magnet synchronous motor (PMSM) control system is proposed, on account of the standard PFC method cannot provides a satisfying disturbance rejection performance in the case of strong disturbances. The PFC-based method is first introduced in the control design of speed loop, since the good tracking and robustness properties of the PFC heavily depend on the accuracy of the internal model of the plant. However, in orthodox design of prediction model based control method, disturbances are not considered in the prediction model as well as the control design. A minimum-order observer (MOO) is introduced to estimate the disturbances, which structure is simple and can be realized at a low computational load. This paper adopted the MOO to observe the load torque, and the observations are then fed back into PFC model to rebuild it when considering the influence of perturbation. Therefore, an improved PFC strategy with torque compensation, called the PFC+MOO method, is presented. The validity of the proposed method was tested via simulation and experiments. Excellent results were obtained with respect to the speed trajectory tracking, stability, and disturbance rejection.

Is Transcranial Doppler Ultrasonography Old-fashioned?: One Institutional Validity Study

  • Objective : The purpose of this study is to investigate the correlation between various transcranial Doppler (TCD) ultrasonography parameters and clinical vasospasm after aneurysmal subarachnoid hemorrhage (SAH). Methods : This study enrolled 40 patients presented with aneurysmal SAH between September 2006 and August 2007. We measured differences of mean blood flow velocity (BFVm), highest systolic blood flow velocity (BFVh), and Lindegaard ratio (LR) in the middle cerebral artery on TCD examination. These parameters were evaluated for correlation with clinical vasospasm by univariate analysis and the receiver operating characteristic analysis. Results : Twelve patients (30%) developed clinical vasospasm. The best TCD parameters for the detection of clinical vasospasm were revealed to be differences of BFVm, BFVh, and LR values between $1^{st}$ TCD test and $3^{rd}$ TCD (7 cm/s. 11.5 cm/s, 0.45 respectively). The positive predictive value of anyone of three parameters was 60% and the negative predictive value was 100%. Conclusion : TCD is still considered a useful tool for screening clinical vasospasm. To confirm the predictive value of the above parameters. further prospective study will be needed.

A High Performance Permanent Magnet Synchronous Motor Servo System Using Predictive Functional Control and Kalman Filter

  • A predictive functional control (PFC) scheme for permanent magnet synchronous motor (PMSM) servo systems is proposed in this paper. The PFC-based method is first introduced in the control design of speed loop. Since the accuracy of the PFC model is influenced by external disturbances and speed detection quantization errors of the low distinguishability optical encoder in servo systems, it is noted that the standard PFC method does not achieve satisfactory results in the presence of strong disturbances. This paper adopted the Kalman filter to observe the load torque, the rotor position and the rotor angular velocity under the condition of a limited precision encoder. The observations are then fed back into PFC model to rebuild it when considering the influence of perturbation. Therefore, an improved PFC method, called the PFC+Kalman filter method, is presented, and a high performance PMSM servo system was achieved. The validity of the proposed controller was tested via experiments. Excellent results were obtained with respect to the speed trajectory tracking, stability, and disturbance rejection.

A Novel Modulation Method for Three-Level Inverter Neutral Point Potential Oscillation Elimination

  • A novel algorithm is proposed to regulate the neutral point potential in neutral point clamped three-level inverters. Oscillations of the neutral point potential and an unbalanced dc-link voltage cause distortions of the output voltage. Large capacitors, which make the application costly and bulky, are needed to eliminate oscillations. Thus, the algorithm proposed in this paper utilizes the finite-control-set model predictive control and the multistage medium vector to solve these issues. The proposed strategy consists of a two-step prediction and a cost function to evaluate the selected multistage medium vector. Unlike the virtual vector method, the multistage medium vector is a mixture of the virtual vector and the original vector. In addition, its amplitude is variable. The neutral point current generated by it can be used to adjust the neutral point potential. When compared with the virtual vector method, the multistage medium vector contributes to decreasing the regulation time when the modulation index is high. The vectors are rearranged to cope with the variable switching frequency of the model predictive control. Simulation and experimental results verify the validity of the proposed strategy.

Design and Experimental Validation of a Digital Predictive Controller for Variable-Speed Wind Turbine Systems

  • Advanced control algorithms must be used to make wind power generation truly cost effective and reliable. In this study, we develop a new and simple control scheme that employs model predictive control (MPC), which is used in permanent magnet synchronous generators and grid-connected inverters. The proposed control law is based on two points, namely, MPC-based torque-current control loop is used for the generator-side converter to reach the maximum power point of the wind turbine, and MPC-based direct power control loop is used for the grid-side converter to satisfy the grid code and help improve system stability. Moreover, a simple prediction scheme is developed for the direct-drive wind energy conversion system (WECS) to reduce the computation burden for real-time applications. A small-scale WECS laboratory prototype is built and evaluated to verify the validity of the developed control methods. Acceptable results are obtained from the real-time implementation of the proposed MPC methods for WECS.
