Prediction Model of Fatigue in Women with Rheumatoid Arthritis (여성 류마티스 관절염 환자의 피로 예측 모형)

  • Rheumatoid arthritis is a chronic systemic autoimmune disease. Although the joints are the major loci of the disease activity, fatigue is a common extraarticular symptom that exists in all gradations of rheumatoid arthritis. Fatigue is defined as a subjective sense of generalized tiredness or exhaustion and has multiple dimensions. Therefore fatigue is a common and frequent problem for those with rheumatoid arthritis. In fact, 88-100% of individuals with rheumatoid arthritis experience fatigue. Especially the degree of fatigue is higher in women than men with rheumatoid arthritis. Despite the importance of fatigue among the patients with rheumatoid arthritis, the mechanism that leads to fatigue in rheumatoid arthritis is not completely understood. This study was intended to test and validate a model to predict fatigue in women with rheumatoid arthritis. Especially it was intended to identify the direct and indirect effects of the variables of pain, disability, depression, sleep disturbance, morning stiffness, and symptom duration to fatigue. Data were collected by questionnaires including Multidimensional Assesment of Fatigue(Tack, 1991), numeric scale of pain, graphic scale of joints, Ritchie Articular Index, Korean Health Assessment Questionnaire(Bae, et al., 1998), Inventory of Function Status(Tulman, et al., 1991), Center for Epidemiologic Studies-Depression, and Korean Sleep Scale(Oh, et al 1998). The sample consisted of 345 women with a mean duration of rheumatoid arthritis for 10.06 years and a mean age of 49.64 years. SPSS win and Win LISREL were used for the data analysis. Structural equation modeling revealed the overall fit of the model. Pain predicted fatigue directly and indirectly through disability, depression, and sleep disturbance. Disability, sleep disturbance predicted fatigue only directly, while depression only indirectly through disability and sleep disturbance. Also morning stiffness and symptom duration predicted fatigue through disability and depression. All predictors accounted for 65% of the variance of fatigue. Depression, pain, and disability predicted sleep disturbance. Depression had reciprocal relationship with disability and they both were predicted by pain directly and indirectly. In summary, pain, depression, disability, sleep disturbance, morning stiffness, and symptom duration contributed to the fatigue of patients with rheumatoid arthritis. The best predictor of fatigue was pain. This finding indicates that the modification of pain, depression, disability, sleep disturbance, morning stiffness could be nursing intervention for relief or prevention of fatigue.

Prediction of Sensory Properties for the Stirred-type Fruit Yogurts by Instrumental Measurements (기계적 측정에 의한 호상요구르트의 관능특성 예측)

  • This experiment was carried out to predict the sensory properties of yogurt by instrumental methodology. Sensory attributes such as viscosity, mouth-feel, taste and quality were investigated. Instrumental parameters were measured with refractometer, viscometer, consistometer and rheometer. Sensory data showed that viscosity of peach yogurt was higher than that of strawberry and tropical-fruit-mixed (TFM) yogurts (p<.05). All instrumental parameters of peach yogurt were higher than those of strawberry and TFM yogurts, except cohesiveness and elasticity (p<.05). Viscosity measured by panelists was significantly correlated with instrumental viscosity, consistency, hardness, adhesiveness and gumminess in the fruit yogurts (p<.05). But mouth-feel and quality of yogurts showed poor relationships with instrumental parameters. The effective instrumental parameters for predicting sensory viscosity ($Y_{1}$) of yogurts were consistency ($X_{1}$), viscosity ($X_{2}$) and cohesiveness ($X_{3}$). And those for predicting mouth-feel ($Y_{2}$) were consistency. The estimated regression equations were as follows; $Y_{1}=4.968-0.0486X_{1}+0.00012X_{2}+0.0348X_{3},\;Y_{2}=5.701+0.0154X_{1}$.

Long-Term Performance Evaluation of Concrete Utilizing Oyster Shell in Lieu of Fine Aggregate (굴패각을 잔골재로 대체 사용한 콘크리트의 장기성능 평가)

  • To evaluate the practical application of oyster shells(OS) as construction materials, an experimental study was performed. More specifically, the long-term mechanical properties and durability of concrete blended with oyster shells were investigated. Test results indicate that long-term strength of concrete blended with 10% oyster shells is almost identical to that of normal concrete. However, the long-term strength of concrete blended with 20% oyster shells is appreciably lower than that of normal concrete. Thereby, concrete with higher oyster shell blend has the possibility of negatively influencing the concrete long-term strength. Elastic modulus of concrete blended with crushed oyster shells decreases as the blending mixture rate increases. Namely, the modulus is reduced to approximately 10∼15% when oyster shells are blended up to 20% as the fine aggregate. The drying shrinkage strain increases with an increasing crushed oyster shells substitution rate. In addition, the existing model code of drying shrinkage and creep do not coincide with the test results of this study. An adequate prediction equation needs to be developed. The utilization of oyster shells as the fine aggregate in concrete has an insignificant effect on fleering and thawing resistance, carbonation and chemical attack of concrete. However, water permeability is considerably improved.

Correlations Between the Physical Properties and Compression Index of KwangYang Clay (광양점토의 물리적 특성과 압축지수의 상관성)

  • The correlation equation empirically proposed to obtain compression indexes has been proposed to conveniently obtain the value using the soil parameter that can be obtained through simple tests when the number of time of consolidation testing is low or the distribution is large but most of the analyzed regions are limited to certain regions abroad or in the country and multiple data were integrated for use in many cases, thus it is not very reasonable to apply it. Therefore, to establish a new design method considering the uncertainty of the ground, it was selected the Kwangyang port area of which the data have been collected recently thus are relatively more reliable as the subject region of the study in order to maximally reduce the uncertainty of test data. After performing the verification of the normality of the consolidation test data obtained from the selected region and the transformation of variables, a prediction formula was proposed through the regression model with the transformed variables and the proposed regression model with transformed variables was compared with existing empirical equations to verify the suitability of the proposed model formula. After analyzing, it was confirmed that the coefficient of determination was increased after the Box-Cox variable transformation, thus the explanatory power was being enhanced and through the root-mean-square-error method, it was confirmed that the proposed model formula showed the most closed value to the test value.

Albumin-globulin Ratio for Prediction of Long-term Mortality in Lung Adenocarcinoma Patients

  • Background: Prior studies showed a relationship between serum albumin and the albumin to globulin ratio with different types of cancer. We aimed to evaluate the predictive value of the albumin-globulin ratio (AGR) for survival of patients with lung adenocarcinoma. Materials and Methods: This retrospective study included 240 lung adenocarcinoma patients. Biochemical parameters before chemotherapy were collected and survival status was obtained from the hospital registry. The AGR was calculated using the equation AGR=albumin/(total protein-albumin) and ranked from lowest to highest, the total number of patients being divided into three equal tertiles according to the AGR values. Furthermore, AGR was divided into two groups (low and high tertiles) for ROC curve analysis. Cox model analysis was used to evaluate the prognostic value of AGR and AGR tertiles. Results: The mean survival time for each tertile was: for the $1^{st}$ 9.8 months (95%CI:7.765-11.848), $2^{nd}$ 15.4 months (95%CI:12.685-18.186), and $3^{rd}$ 19.9 months (95%CI:16.495-23.455) (p<0.001). Kaplan-Meier curves showed significantly higher survival rates with the third and high tertiles of AGR in comparison with the first and low tertiles, respectively. At multivariate analysis low levels of albumin and AGR, low tertile of AGR and high performance status remained an independent predictors of mortality. Conclusions: Low AGR was a significant predictor of long-term mortality in patients with lung adenocarcinoma. Serum albumin measurement and calculation of AGR are easily accessible and cheap to use for predicting mortality in patients with lung adenocarcinoma.

  • Recently, to solve the problems associated with the neutralization and corrosion of reinforced concrete compression members, the structural configurations such as CFFT (Concrete Filled GFRP Tube) and RCFFT (Reinforced Concrete Filled GFRR Tube) have been developed and applied to main members of civil engineering structure. These members can increase structural performance in terms of structural stability, ductility as well as chemical resistance compared with conventional concrete structural members. Many researches in numerous institutions to predict the load carrying capacity of the concrete compression member strengthened with FRP materials have been conducted and they have been suggested an equation for the prediction of the load carrying capacity of the members. Through the review of the research results, it was found that their results are similar each other. Moreover, it was also found that the results are not directly applicable to our specimens since the results are largely depended upon the member configurations. Also, since the accurate design criteria for the RC members strengthened with FRP such as RCFFT have not been established properly, relevant theoretical and experimental investigations must be conducted for the application to the practical structures. In this study, structural behavior of RCFFT was evaluated through compressive and quasi-static flexural tests in order to formulate design criteria for the structural design. In addition, the RCFFT members were also investigated to examine their confinement effect and the equations capable of estimating the compressive ultimate strength and flexural stiffness of the RCFFT members were proposed.

Analysis of Numerical Experiment for Field Application of Cylindrical Slit Type Block Breakwater (실린더 슬릿형 소파블록 방파제의 실해역 적용을 위한 수치실험분석)

  • In order to evaluate applicability of cylindrical slit type block breakwater to the field water, which was designed from the previous physical model study, it is analyzed the calmness of harbor area by the numerical model experiment. For a small fishery port in southern coast of Korea a SWAN model using the wave action balance equation was formulated. The reflection and transmission coefficients induced by the physical model test were introduced to the numerical model. The model response with cylindrical slit type breakwater was compared with the impermeable breakwater case and the possibility of water quality improvement through the water circulation by the new structure was investigated. For numerical simulation, parameters of deepwater design wave from the prediction report II for overall deepwater design wave by KORDI were used and wind parameters from the 50years return period observed for 37years(1970~2006) were adopted in the numerical model. The response of west breakwater in Mijo port applying the NE and NNE waves, which were dominant in this area, was analyzed. It was found that the transmission characteristic of designed cylindrical slit breakwater was well presented in the numerical model.

Design of Sensor Network for Estimation of the Shape of Flexible Endoscope (연성 대장내시경의 형상추정을 위한 센서네트워크의 설계)

  • In this paper, a method of shape prediction of an endoscope handling robot that can imitate a surgeon's behavior using a sensor network is suggested. Unit sensors, which are composed of a 3-axis magnetometer and 3-axis accelerometer pair comprise the network through CAN bus communication. Each unit of the sensor is used to detect the angle of the points in the longitudinal direction of the robot, which is made from a flexible tube. The signals received from the sensor network were filtered using a low pass Butterworth filter. Here, a Butterworth filter was designed for noise removal. Finally, the Euler angles were extracted from the signals, in which the noise was filtered by the low path Butterworth filter. Using this Euler angle, the position of each sensor on the sensor network is estimated. The robot body was assumed to consist of links and joints. The position of each sensor can be assumed to be attached to the center of each link. The position of each link was determined using the Euler angle and kinematics equation. The interpolation was carried out between the positions of the sensors to be able to connect each point smoothly and obtain the final posture of the endoscope in operation. The experimental results showed that the shape of the colonoscope can be visualized using the Euler angles evaluated from the sensor network suggested and the shape of serial link estimated from the kinematics chain model.

An Ergonomic Analysis for Heavy Manual Material Handling Jobs by Fire Fighters (소방대원의 중량물작업에 대한 인간공학적 분석)

  • Modern fire fighting jobs have been expanded to include areas of rescue, emergency medical service as well as conventional fire suppression, so that load for fire fighting jobs has been increased. Specifically, musculoskeletal disorders (MSDs) such as low back injury have been considered as one of major industrial hazards in heavy manual material handling during fire fighting jobs. This study tried to evaluate risk levels and to prepare background for reducing risk levels associated with heavy manual material handling during fire fighting jobs. This study applied two major tools in evaluating heavy manual material handling jobs which were NLE (NIOSH Lifting Equation) and 3DSSPP (3D Static Strength Prediction Program). A risk index in terms of heavy manual material handling during fire fighting jobs was identified. This index consisted of seven risk levels ranged from nine points (the first level) to three points (the seventh level). There was no job associated with the first level (the highest risk level) of index. There was only one job (life saving job) belonging to the second level (the second highest risk level) of index. The third level had jobs such as usage of destruction equipment and lifting patient. A total of basic eighteen jobs was categorized into six different levels (2nd-7th levels) of index. The outcome of the study could provide a good basis for conducting job intervention, preparing good equipment and developing good education program in order to prevent and reduce MSDs including low back injury of fire fighting jobs.

Selection of Optimal Vegetation Indices for Estimation of Barley & Wheat Growth based on Remote Sensing - An Application of Unmanned Aerial Vehicle and Field Investigation Data - (원격탐사 기반 맥류 작황 추정을 위한 최적 식생지수 선정 - UAV와 현장 측정자료를 활용하여 -)

  • Unmanned Aerial Vehicle (UAV) imagery are being assessed for analyzing within field spatial variability for agricultural precision management, because UAV imagery may be acquired quickly during critical periods of rapid crop growth. This study refers to the derivation of barley and wheat growth prediction equation by using UAV derived vegetation index. UAV imagery was taken on the test plots six times from late February to late June during the barley and wheat growing season. The field spectral reflectance during growing period for the 5 variety (Keunal-bori, Huinchalssal-bori, Saechalssal-bori, Keumkang and Jopum) were measured using ground spectroradiometer and three growth parameters, including plant height, shoot dry weight and number of tiller were investigated for each ground survey. Among the 6 Vegetation Indices (VI), the RVI, NDVI, NGRDI and GLI between measured and image derived showed high relationship with the coefficient of determination respectively. Using the field investigation data, the vegetation indices regression curves were derived, and the growth parameters were tried to compare with the VIs value.
