A comparative review of epinephrine and phenylephrine as vasoconstrictors in dental anesthesia: exploring the factors behind epinephrine's prevalence in the US

  • This review paper delves into the comparative study of epinephrine and phenylephrine as vasoconstrictors in dental anesthesia, exploring their histories, pharmacological properties, and clinical applications. The study involved a comprehensive literature search, focusing on articles that directly compared the two agents in terms of efficacy, safety, and prevalence in dental anesthesia. Epinephrine, with its broad receptor profile, has been a predominant choice, slightly outperforming in the context of prolonging dental anesthesia and providing superior hemostasis, which is crucial for various dental procedures. However, the stimulation of beta-adrenergic receptors caused by epinephrine poses risks, especially to patients with cardiovascular conditions. Phenylephrine, a selective alpha-1 adrenergic agonist, emerges as a safer alternative for such patients, avoiding the cardiovascular risks associated with epinephrine. Moreover, its vasoconstrictive effect may not be as deleterious as that of epinephrine, due to its selective action. This review reveals that despite the potential benefits of phenylephrine, epinephrine continues to dominate in clinical settings, due to its historical familiarity, availability, and cost-effectiveness. The lack of commercially available pre-made phenylephrine dental carpules in most countries, except Brazil, and a knowledge gap within dental academia regarding phenylephrine, contribute to its limited use. This review concludes that while both agents are effective, the choice between them should be based on individual patient conditions, availability, and the practitioner's knowledge and familiarity with the agents. The underuse of other vasoconstrictors like levonordefrin and the unavailability of phenylephrine in pre-mixed dental cartridges in many countries highlights the need for further exploration and research in this field. Furthermore, we also delve into the role of levonordefrin and examine the rationale behind the exclusion of phenylephrine from commercially available pre-mixed local anesthetic carpules, suggesting a need for a responsive approach from pharmaceutical manufacturers to the distinct needs of the dental community.

A Review of an Enhancement Course for College Students' Standardized English Test

  • This study reviews an English enhancement course implemented for medical and nursing college students of 2011 to improve their standardized English test scores. A total of 117 first-year students underwent the pre-test and the post-TOEIC in November 2011. After the pre-test, lectures on listening enhancement strategies and problem solving tactics on reading comprehension were supplemented with 4 times of enhancement-tests. Their pre- and post-TOEIC results were compared to see if such enhancement strategies were effective to improve their TOEIC scores. It was further to investigate, through survey questionnaires, how the subjects felt about a number of pre-test-type TOEIC exercise with what-to-listen and how-to-infer listening strategies and pattern-drill practices for reading comprehension. It was found that the scores of listening comprehension improved significantly for both major students whereas reading scores showed slight decrease among nursing majors. It is concluded that administering numerous pre-test-type-TOEIC exercise is effective for students to improve their listening scores supplemented with listening strategies Also, students' response showed positive toward the enhancement course.

The Impact of Indigenous People's Pre-existing Information on Rice Farming: Findings from Laos

  • Dissemination of information can enhance smallholder farmers' agricultural outcomes and incomes in developing countries. However, the impact evaluation for new information can be inaccurate without considering pre-existing information that the indigenous people have used. This study explores qualitative causal links between existing agricultural information used by Lao smallholder farmers on rice yield and selling price with 180 household data. We categorized the pre-existing information into weather, farming technique, input, intermediate trader, and sales price. The source of each piece of information is used as an instrumental variable to overcome the endogeneity issue between information use and agricultural outcomes. Using farming technique information positively affects rice yields by 57.1% compared to those without that information. Moreover, intermediate trader and crop sales information result in 64.5% and 60.0% higher selling prices than non-user groups. A statistically significant causal relationship exists with agricultural outcomes. The more genuine impact should be measured with a newly updated impact evaluation approach that considers this pre-existing agricultural information.

New Obligations of Health Insurance Review and Assessment Service: Taking Full-fledged Action Against the COVID-19 Pandemic

  • In 2020, the coronavirus disease 2019 (COVID-19) pandemic has caused unprecedented disruptions to global health systems. The Korea has taken full-fledged actions against this novel infectious disease, swiftly implementing a testing-tracing-treatment strategy. New obligations have therefore been given to the Health Insurance Review and Assessment Service (HIRA) to devote the utmost effort towards tackling this global health crisis. Thanks to the universal national health insurance and state-of-the-art information communications technology (ICT) of the Korea, HIRA has conducted far-reaching countermeasures to detect and treat cases early, prevent the spread of COVID-19, respond quickly to surging demand for the healthcare services, and translate evidence into policy. Three main factors have enabled HIRA to undertake pandemic control preemptively and systematically: nationwide data aggregated from all healthcare providers and patients, pre-existing ICT network systems, and real-time data exchanges. HIRA has maximized the use of data and pre-existing network systems to conduct rapid and responsive measures in a centralized way, both of which have been the most critical tactics and strategies used by the Korean healthcare system. In the face of new obligations, our promise is to strive for a more responsive and resilient health system during this prolonged crisis.

예비 물리 교사들의 학생평가 문항 제작 수행 수준 조사 - 형성 평가를 위한 선다형 물리 개념 검사 문항 분석 중심으로 (A Study on Performance Level of Pre-service Physics Teachers in Constructing Questions for classroom assessment - Focused on Analysis of Multiple Choice Question about Physics Conceptest for Formative Assessment)

  • 성취평가제 도입에 따라 교사들이 이에 부합하는 평가 문항을 제작하는 능력의 중요성이 강조되고 있다. 본 연구는 예비 물리 교사들의 학생평가 전문성 신장을 위한 기초 자료를 제공하기 위해 예비 물리 교사들이 제작한 형성 평가 문항을 살펴보았다. 물리교육학과 2학년에 재학 중인 17명의 예비교사들이 이 연구에 참여하였다. 이 연구를 위해 우리는 예비교사들에게 형성 평가도구로써 선다형 물리 개념 문항을 제작하게 하고 예비교사들이 제작한 문항을 서로 검토하는 과정을 관찰하였다. 예비교사들은 5시간의 검토 과정을 통해 그들의 문항을 수정해 갔으며 16명의 예비교사들이 최종 완성된 문항을 제출하였다. 연구 결과, 예비교사들은 다소 낮은 수행 능력을 보였다. 문항을 제작하고 검토함에 있어 예비교사들은 다양한 관점을 가지지 못하였으며 대부분이 학생들이 가진 개념에 가한 정보를 얻는 것을 고려하지 않았다. 이러한 연구결과는 예비교사들이 문항 제작에 있어 어려움이 있음을 나타내 주고 성취평가제 도입의 성공적인 정착을 위해 예비교사들의 학생평가 전문성 신장을 위한 특별한 방안이 필요함을 보여준다.

Cardiovascular effect of epinephrine in endodontic microsurgery: a review

  • Epinephrine is one of the most widely-used vasoconstrictors in dental treatment including endodontic microsurgery. However, the systemic safety of epinephrine has been in debate for many years because of its potential risk to cause cardiovascular complications. The purpose of this review was to assess the cardiovascular effect of epinephrine use in endodontic microsurgery. Endodontic microsurgery directly applies epinephrine into the bone cavity, and the amount is reported to be much larger than other dental surgeries. Moreover, when considering that systemic potency of intraosseous application is reported to be comparable to intravenous application, the systemic influence of epinephrine could be increased in endodontic microsurgery. Besides, pre-existing cardiovascular complications or drug interactions can enhance its systemic influence, resulting in increased susceptibility to cardiovascular complications. Although clinical studies have not reported significant complications for patients without severe systemic complications, many epinephrine-induced emergency cases are warning the cardiovascular risk related with pre-existing systemic disease or drug interactions. Epinephrine is a dose-sensitive drug, and its hypersensitivity reaction can be fatal to patients when it is related to cardiovascular complications. Therefore, clinicians should recognize the risk, and the usage of pre-operative patient evaluation, dose control and patient monitoring are required to ensure patient's safety during endodontic microsurgery.

  • To review the theoretical background of regulatory approaches to cross-border transfer of personal data, this paper divides major digital trade participating countries into four types according to the OECD - non-regulatory, post-intervention, pre-supervision, and national control. It then analyzes the US, Japan, the EU, and China respectively that belong to each type. South Korea, which is currently about to pass the amendment by the National Assembly, has identified that it is in the middle of post-intervention and pre-supervision, and needs to evolve into pre-supervision norms like the EU while it has to participate more actively in the process of establishing international digital trade rules. Korea first needs to sign digital trade agreements and promote mutual certification projects more actively from the standpoint of a medium-sized open country with growing digital companies and digitally-open consumers. Second, the government should fully consider the interests of not only companies but also various trade stakeholders including domestic consumers, when drafting and implementing trade policies. To this end, 'a single window approach' is needed not only at the Ministry of Trade, Industry, and Energy, but also at the level of the entire government which require an integrated form of digital trade policy governance.

The Literature Review for Postmodern Furniture Aesthetics of communication called New Design Furniture

  • This study explored the literature review for intending to make contemporary furniture expressed as postmodern aesthetics accessible to the broadest possible public, particularly through an examination and realization of art and everyday life, using qualitative analysis about the related literature as the principal methodology. The postmodern furniture aesthetics of communication expressed through double-coding, which was seen by Charles Jencks as the defining element of Post-Modernism, is characterized by the use of ideas from the traditions of craft. Hence, I dealt with pre-Modern furniture and Modern-furniture aesthetics as the background of postmodern furniture called New design furniture. As a result, contemporary furniture called New design furniture represented the use of ornament, craftsmanship, or beauty from the traditions of craft, which was a main source for communication value in postmodern furniture aesthetics.

UAP56- a key player with surprisingly diverse roles in pre-mRNA splicing and nuclear export

  • Transcripts contain introns that are usually removed from premessenger RNA (MRNA) in the process of pre-mRNA splicing. After splicing, the mature RNA is exported from the nucleus to the cytoplasm. The splicing and export processes are coupled. UAP56 protein, which is ubiquitously present in organisms from yeasts to humans, is a DExD/H-box family RNA helicase that is an essential splicing factor with various functions in the prespliceosome assembly and mature spliceosome assembly. Collective evidence indicates that UAP56 has an essential role in mRNA nuclear export. This mini-review summarizes recent evidence for the role of UAP56 in pre-mRNA splicing and nuclear export.
