Knowledge, Attitude, and Practice of Obstetric Nurses in Relation to Breast Cancer and Breast Self-examination (산과 간호사의 유방암과 유방자가검진에 대한 지식, 태도 및 실천)

  • Purpose: This study aims to determine obstetrics nurses knowledge, attitude, and practice about breast cancer and breast self-examination (BSE), and to contribute to the early detection of breast cancer during breastfeeding periods. Methods: For the survey, 163 individuals (obstetric nurses) were chosen by convenience sampling and agreed to participate in the study. Their knowledge, attitude, and practice about breast cancer and BSE. Results: Knowledge and attitude averaged $70.45{\pm}10.90$ (of 100) and $3.64{\pm}0.27$ (of 5), respectively. While most nurses (99%) recognized the importance of BSE, only 58.9% experienced BSE. BSE practice level averaged $8.35{\pm}1.96$ (of 12). Only 20.2% had recommended BSE to their clients. Practice level varied significantly for different marital status, breastfeeding experience, and education, while knowledge and attitude remained independent. Nurses who had experienced mammogram or breast ultrasonogram themselves scored higher in knowledge. Attitude was higher for nurses who received recommendation for BSE, performed BSE, received BSE education, or recommended BSE to clients. Practice level was higher for nurses who received BSE education or willing to perform BSE in future. Practice level had a positive correlation with attitude but no correlation to knowledge. Conclusion: Obstetric nurses need continuing education for practicing BSE. Practical BSE education can not only promote preventive behavior of nurses, but it can also improve the breast health management of obstetrical clients.

A Study on Health Promotion Behavior and It′s Related Factors of Industrial Workers (산업근로자의 건강증진 행태에 관한 관련요인분석)

  • From September 2, 1995 through October 31, this questionnaire was made by the 1, 200 industrial workers who work for 15 enterprises of 5 types of business. And it was for helping to devise a policy and to develop a program for industrial workers' health promotion by understanding the consciousness behavior level of industrial workers in our country and the related factors which are under the influence of it and health promotion behavior. The summary and conclusions are as follows. 1. In health promotion score level by related factors, the health diagmosis level score was 7. 37(81.9/100) of the perfect score 9, nutrition level score was 7.00(77.8/100), health education behavior level score was 6.00(66.7/100), exercise behavior level score was 6.01(66.8/100), occupational disease knowledge level score was 6.00(66.7/100). 2. Health diagnosis behavior level was significantly high when the age is older, when the occupation term is longer, when the economic status is better. And wjem tje satosfoed degree for vocational aptitude, working environment, and the education contents. 3. Nutrition (dietary habit) status level was high in men, in the age group of 40 over, in the group of having their spouse, in the group of being paid over one million won a month, in the upper economic classes (P〈0.001). It was also high in graduates middle school and in daytime workers (P〈0.05). 4. Health education behavior level was high in the older ahe hroup, in a single life (separation by death, divouce, separation) and in the longer occupation term(P〈0.001). 5. Exercises behavior level was high in men, in the workers who are paid 500~990 thousand won a month, in the better economic classes(P〈0.01). 6. Knowledge level on an occupational disease was high in men, in the older age group, in the group of having a spouse, In the workers who are paid 500~990 thousand won a month, in the group of having a longer occupation term, and in the residents living not in a large city(P〈0.01). 7. When health status was higher, health promotion behavior, behavior level, health diagnosis (P〈0.001), nutrition(P〈0.05), health education behavior (P〈0.05), exercise behavior(P〈0.01) and the knowledge level on an occupation disease was high. 8. The main factors which are under the influence on the degree of practicing healthy life were the level of knowledge and behavior, sex, his/her health status, and the satisfied degree of working environment. These variables could explain it 18.0%. 9. The factors which are under the influence on health promotion behavior and behavior levels were the variables of the satisfied degerr of education contents, sex, health knowledge, economic status, health status, occupation terms, monthly income, working tiredness. These variables could explain it 21.3%. By these results, it is inportant for industrial workers' health promotion to level up the health diagnosis behavior, dietary habit considering nutrition, behavior on health education, behavior for exercise, and knowledge on an occupational disease. Especially we should develop the proper program considered sex, health status, satisfied degree of working environment and education contents, economic status, eccupation terms, knowledge on health, and behavior level. Because health promotion business gies in gear with productivity promotion.

  • Fixed prosthetic appliance is the restorations of damaged teeth with cast metal or porcelain, and of replacing missing teeth with fixed, or cemented prosthesis. Successfully treating a patient by means of fixed prosthodontics requires the thoughtful combination of dentist and dental laboratory technician. In order to know the satisfaction of dentisits about fixed prosthetic appliances-porcelain, precious and non precious metal-, this study was conducted for 69 practicing dentisis in Pusan during Fed. to Mar. 1988 using a questionnaire method and was analyzed by the use of percentage, $X^2$-test The results are obtained as follow: 1. General characteristics of the respondents, The rate of age distribution of practcing dentists responded was as follow : 31$\sim$35 age group is 29.0%, 51$\sim$55age group 18.8%, 41$\sim$45 age group and 46$\sim$50 group each 11.6%, 36$\sim$40 age group and 56$\sim$60 age group 8.7%, over 60 age group 7.3%, below 30 age group 4.3% respectively. The tate of term of practice experience distribution of dentists responded was as follow : 6$\sim$10 year group 26.1%, 25$\sim$25year group 18.8%, 11$\sim$15 years group and 16~20 years group each 13.0%, below 5 years group and over 30 years group each 10.2%, 26$\sim$30 years group 8.7% in order. The rate of respondents’ degree was follow : bachelor group 81.2%, doctor group 13.0%, master group 5.8% in order. The rate of respondents’completed medical specialist course was as follow : non-complete group 75.4%, complete group 24.6%. The rate of having own laboratory in respondents’clinic was as follow : no having own laboratory group 58.0%, having own labiratory group 42.0% The rate of being lab. technician in respondent;s clinic was as follow: non-being lab. technician group 91.3%, being lab. technician group 8.7%. The rate of transactional commercial dental lab. numbers was as follow : I dental lab. group 40.6%, 2 dental lab. group 30.4%, 3 dental lab. group 20.3%, production at own lab. group 8.7% 2. Cognitions about the respondent's job, About cognition of patient numvers, there were neither many nor few group 62.3%, few group 17.4%, many group 15.9%, too many group 4.4% in order.(P<.01) As compared with the ratio of dentists to patient prothetic treatment, there were moderate level group 46.4%, high level group 33.6%, low level group 18.8% in order.(P<.01) By job satisfaction of respondents, the highest percentage group was satisfied group 44.9%, neither astisfied nor disstified group 33.3%, dissatisfied group 14.5%, much satisfied group 7.3%(P<.01). As compared with the level of prosthetics price, the were moderate level group 71.0%, low level group 23.2%, high level group 5.8%(P<.01) 3. Satisfaction and assessment of dissatisfied factor of the fixed prosthetic appliance, As compared with the satisfaction of al prosthetic appliance, there were neither satisfied nor dissatisfied group 59.4%, satisfied group 20.3% dissatisfied group 18.8%, much satisfied group 1.5% in order.(P<.01). About precious and non precious metal crown and bridge, there were neither satisfied nor dissatisfied group 60.9%, satisfied group 24.6%, dissatisfied group 11.6%, much satisfied group 2.9% in order.(P<.01) The reat of respondent;s satisfaction about porcelain crown and bridge was as follow : neither satisfied nor dissatisfied group 43.5%, dissatisfied group 33.3%, satisfied group 21.7%, much satisfied group 1.5%(P<.01). The rate of respondent's dissatisfied factor item of precious and non precious metal crown and bridge was as follow : inadequate margin fit item 39.1% respondent, inadequate occlusion item 24.6%, inadequate adaptation item 23.2%, poor anat omic form item 15.9%, inadequate contact item 31.9%, improper polishing item 4.4%, the rest item 11.6% respectively. The rate of respondent's dissatisfied factor item of porcelain crown and bridge was as follow : inadequate margin fit item 52.2%, inadequate occlusion item 2.9%, inadequate adaptation item 10.1%, poor anatomic form item 11.6%, improper polishing item 1.5%, shade mismatching item 71%, the rest item 7.2% respectively. The rate of cognition of respondents’ about cause of dissatisfied factor on fixed prosthetic appliance by item was as follow : deficiency of sincerity item 55.1%, deficiency of knowledge about work item 47.8%, deficiency of experience item 15.9%, poor quality of using material item 5.8%, the rest item 2.9% respectively.

Effects of the Breast Cancer Risk Appraisal on Breast Self-Examination Practice in Adult Women (유방암위험사정이 일 여성의 유방자가검진(BSE)실행에 미치는 영향)

  • Breast cancer ranks as one of the major health problems of adult women. Studies have shown that Breast Self-Examination (BSE) is effective in detecting breast cancer in its early stages. To motivate women BSE practicing, a nonequivalent control group pretest-posttest design was conducted. Women 40 years or older who participated in the public education program for BSE from March 11 to April 6, 1996, were randomized in an experimental group(N=50) which received a letter about the Breast Cancer Risk Appraisal and in a control group(N=50) which didn't received a letter about the Breast Cancer Risk Appraisal. After 6 months, a follow-up phone survey was taken on all participants to measure the effectiveness of the intervention. Collected data was analyzed by one, two, and three-way ANOVA with an SAS program. The results are summarized as follows : 1. The level of the Breast Cancer Risk Appraisal of the subjects is high risk(17%), moderate risk(12%), boderline risk(14%), no increased risk (57%). 2. The intervention was effective in increasing the practice scores of BSE for women(F=5.12, P<.05). 3. BSE practice scores according to breast cancer risk appraisal level of the experimental group was not significantly increased after the intervention as contrasted with the control group (F=2.33, P>.05). 4. BSE practice scores according to educational level of the experimental group was significantly increased after the intervention, as contrasted with that of the control group (F=10.09, P<.001) .On the basis of this study it can be concluded that the Breast Cancer Risk Appraisal increases practice of BSE.

A Study on Correlations Between Lifestyle and Health Status in High School Students (남녀고등학생의 생활양식과 건강수준간 상관성 분석)

  • Health can be maintained and promoted by pursuing a healthy lifestyle. A healthy lifestyle implies keeping healthy habits such as regular exercise, a balanced diet, weight control, and stress management etc. The objectives of this study are: (1) To find out patterns in the lifestyle and health status of high school students. (2) To analyze the factors that affect a healthy lifestyle. (3) To compare the level of understanding of a healthy lifestyle and its practice between male students and female students. The study population wag 388 high school students. The data was analysed through a % total, a $X^2$ test, and a multiple classification analysis. The lifestyle assessment questionnaire was devided into ten sections. The results are as follows; (1) There was a positive correlation between health status and a healthy lifestyle. (2) There existed strong positive correlations between independent variables (age, group) and dependent variable (nutritional practices, physical activity, stress management, serve of purpose). (3) The level of understanding of a healthy lifestyle was not high for most of the students. Female students showed a higher understanding than male students in nutrition practice, while male students group showed a higher and under standing than female students for physical activity. The other result were similar in their practice of a healthy lifestyle. With all these above considerations, the level of understanding and practice of a healthy lifestyle in students was not higher than the adult group. Students should have more educational opportunities and take a more systematic education in practicing a healthy lifestyle.

Variation in radial head fracture treatment recommendations in terrible triad injuries is not influenced by viewing two-dimensional computed tomography

  • Background: We analyzed association between viewing two-dimensional computed tomography (2D CT) images in addition to radiographs with radial head treatment recommendations after accounting for patient and surgeon factors in a survey-based experiment. Methods: One hundred and fifty-four surgeons reviewed 15 patient scenarios with terrible triad fracture dislocations of the elbow. Surgeons were randomized to view either radiographs only or radiographs and 2D CT images. The scenarios randomized patient age, hand dominance, and occupation. For each scenario, surgeons were asked if they would recommend fixation or arthroplasty of the radial head. Multi-level logistic regression analysis identified variables associated with radial head treatment recommendations. Results: Reviewing 2D CT images in addition to radiographs had no statistical association with treatment recommendations. A higher likelihood of recommending prosthetic arthroplasty was associated with older patient age, patient occupation not requiring manual labor, surgeon practice location in the United States, practicing for five years or less, and the subspecialties "trauma" and "shoulder and elbow." Conclusions: The results of this study suggest that in terrible triad injuries, the imaging appearance of radial head fractures has no measurable influence on treatment recommendations. Personal surgeon factors and patient demographic characteristics may have a larger role in surgical decision making. Level of evidence: Level III, therapeutic case-control study.

A Study on the Methods of Improving the Lifelong Learning City Project Based on the Community Development Theory (지역사회개발론에 근거한 평생학습도시 사업 개선 방안 탐색)

  • The Lifelong Learning City Project has made quantitative expansion as well as qualitative growth since 2001 but the project has been criticized by academic scholars and field practitioners. The Lifelong Learning City Project is a national policy project which has been promoted by the Ministry of Education and Human Resources Development and should be required to make production profits proportional to the amount of public finance. The Lifelong Learning City Project is a community development project intended to promote growth and progress by supporting the community in lifelong learning endeavors. Therefore, the community development theory could offer guidelines to the Lifelong Learning City Project. Based on this assumption, this study intends to investigate the Lifelong Learning City Project at the national, city, and county levels using the community development theory. The improvement methods of the Lifelong Learning City Project are role allotment between national and wide level projects supporting organizations, and the establishment of a system and a long term project policy. In addition, the project is to have a more systematic performance. It is to enhance opportunities for community members' participation, and practice in planning, performance of learning, and the proper performance in regard to the community conditions and specificity. The most important goal of the Lifelong Learning City Project is to support the empowerment of community members by making opportunity planning, practicing and sharing lifelong learning more accessible.

A Study on the Demand for Nurse Anesthetists in Korea (우리나라 종합병원과 일반병원의 마취 전문간호사 수요분석)

  • Purpose: This descriptive study was conducted to project the number of nurse anesthetists needed in hospital settings, up to the year 2015. Method: Necessary data and information were collected from various funded reports, professional literature, web sites and personal visits to national and private institutions. The number of nurse anesthetists needed was projected after considering the total number of cases requiring anesthesia including deliveries, workload ratio for caesarean section to total number of deliveries, and percent of deliveries requiring and anesthesia. Result: The projected number of nurse anesthetists needed for hospital settings are as follows: 1. The number of registered anesthesia personnel in Korea in 2002 was 2,481 anesthesiologists and 543 nurse anesthetists but only 60% of anesthesiologists and 30% of nurse anesthetists (147) were actually practicing in the field of surgery. 2. By the year 2015, the total number of projected nurse anesthetists needed in hospital settings will be between 214 and 265. Conclusion: In order to match the supply to the need, the professional organizations should direct efforts towards enacting legislation. Educational systems should identify strategies to initiating an adequate number of nurse anesthetist programs at the master's level as well as standardizing curriculums across programs.

Priorities of Nursing Research Areas in Korea (우리나라에서의 간호연구 우선순위)

  • This study was to identify priority areas of nursing research in Korea. Method: A data analysis was done for the PhD subjects (n=133) who responded at the national level two-rounds delphi survey. Subjects were asked to assign a score from 1 to 7 for 29 nursing research areas regarding 3 aspects: the degree of nurses research leadership; the degree of contribution to nursing professions; and the degree of contribution to health and welfare of patients/clients. Result: For the overall three aspects, research area on clinical nursing practice was identified as the first priority, followed by nursing education, home health nursing, nursing research, and chronic disease. For nursing research leadership, research area on clinical nursing practice, nursing education, home health nursing, nursing research, and nursing diagnosis were identified. Concerning contribution to the nursing profession, the first priority was nursing education, followed by clinical nursing practice, nursing research, nursing policy, and home health nursing. Concerning contribution to the health/welfare of patients, clinical nursing practice, home health care, geriatric nursing, chronic disease, and symptom management were ranked as top 5 research priorities. Conclusion: The findings of this national survey will assist in building insights into the research needs of nurses practicing in Korea.

A Design Support System for the Structural Design of Ships Based on an Expert System Development Shell (범용 전문가시스템 쉘을 이용한 선박의 구조설계 지원 시스템)

  • Conventional computer programs developed and used by practicing engineers can be considered to contain expert knowledge and design experience. If these conventional programs are converted into expert systems, the difficult and time consuming process of knowledge acquisition can be simplified. Also the constructed knowledge-base can have higher confidence level than that constructed by the usual knowledge acquisition method of interviews. An existing computer program which is being used by ship structural designers has been reformulated as a design expert system by applying an expert system development shell-Nexpert. Utilizing the callable interface provided by the development shell, external design tools have also been integrated. The interfaced external functions are a graphical user interface (GUI) for the design process control, and graphics functions for the visualization of design results. It is observed that the developed system for design support is useful in two aspects. The trace-back function shows what portion of design rules are applied in arriving at certain design decisions. Also the knowledge-base can be conveniently updated as design rules of the classification societies are updated.
