특발성 척추측만증 환자의 척추 만곡 위치와 방향이 자세 균형에 미치는 영향성 평가 (Evaluation of the Effect of Location and Direction of the Scoliotic Curve on Postural Balance of Patients with Idiopathic Scoliosis)

  • 본 연구에서는 척추측만증 환자의 척추 만곡 위치와 방향이 자세 균형에 미치는 영향을 평가하였다. 총 15명의 실험대상자를 흉추 만곡 그룹, 요추 만곡 그룹, 이중 만곡 그룹으로 분류하여 연구를 진행하였다. 초음파 기반 동작 분석 시스템과 압력 분포 시스템을 사용하여 환자의 동적 체간 움직임(요추, 흉요추, 하흉추, 상흉추에서의 각도 변화)과 족저 압력 분포(최대힘, 최대압력)을 측정하였다. 측정 결과를 통해, 특발성 척추측만증 환자의 동적 체간 움직임과 족저 압력 분포 모두 척추 만곡의 발생 부위와 방향에 따라 비대칭적으로 각도와 압력이 증가하면서 자세 불균형이 발생하는 것을 알 수 있었다. 또한, 단일 만곡과 이중 만곡을 가진 그룹 간의 자세 균형 패턴에서의 차이를 확인할 수 있었다. 추후 연구에서는 본 연구에서의 결과를 기반으로 척추측만증 환자의 자세 조절 능력과 체간 균형을 향상시키고 척추측만을 치료하는데 도움을 줄 수 있는 재활 훈련 장치를 개발하고자 한다.

  • Purpose : The neurologically impaired patients have sensory, motor, cognition, perception problems which cause reduction of body schema, balance control and postural control provoke disfunction. The purpose of this case report was to evaluate effects on gait&balance upon improvement of postural control for left hemiplegia. Methods : This study has performed single subject design from March to April 2013 for 6 weeks. The subject of this study was a 67years old female patient with left hemiplegia. Timed Up and Go (TUG) test and Functional Reach (FR) test were used as evaluation tool. The subject was treated 5 times a week for 30 minutes each. Results : The result is that the walking velocity was decreased, the ability of gait was improved than before the training. The length of FR was increased from 8.33cm to 22.67cm. Conclusion : According to the results, the treatment improves subject's ability of walking in this study.

가상현실 프로그램이 만성 뇌졸중 환자의 선자세 균형에 미치는 영향 (Effects of Virtual Reality Program on Standing Balance in Chronic Stroke Patients)

  • The purpose of this study was to identity effects of virtual reality(VR) program related to standing postural control on balance, gait and brain activation patterns in chronic hemiplegic stroke patients. Subjects were assigned randomly to either VR group (n=12) or the control group (n=12) when the study began. Both groups received conventional physical therapy for 2 to 3 times per week. In addition to conventional physical therapies, VR group trained 3 types of virtual reality programs using IREX for standing postural control during 4 weeks (4 times/week, 30 minutes/time). Subjects were assessed for static and dynamic balance parameters using BPM, functional balance using Berg Balance Scale related to movement of paretic lower limb before and after 4 weeks of virtual reality training. The results of this study were as follows. 1. Following VR training, VR group demonstrated the marked improvement on dynamic mean balance, anteroposterior limits of stability (AP angle) and mediolateral limits of stability (ML angle). 2. Following VR training, both groups scored higher on Berg Balance Scale. However, a comparison of mean change revealed differences between groups. In conclusion, these data suggest that the postural control training using VR programs improve dynamic and functional balance performance in chronic hemiplegic stroke patients.

Balance Master를 이용한 TBA-G 착용 전후 균형능력 평가 비교 연구 (A Comparison Study of Postural Control Measures Between Before and After Applying Temporomandibular Joint Balance Appliance-Golf (TBA-G) Using Balance Master System)

  • Objectives: The purpose of this study was to examine whether Temporomandibular Joint Balance Appliance-Golf (TBA-G) can improve postural control ability of healthy adults. Methods: Twenty participants (10 male, 10 female) aged 20 to 39 years were involved. Postural control ability of all participants was assessed before and after applying TBA-G with Balance $Master^{(R)}$ system. Modified clinical test sensory interaction on balance (mCTSIB), unilateral stance, weight bearing and rhythmic weight shift were used to evaluate postural control ability. Results: After applying TBA-G, mCTSIB on a firm plate with eyes open increased from 0.2 to 0.23 (p<0.05) but directional control was improved in slow and moderate velocity of front/back rhythmic weight shift test (P<0.05). In two cases with postural imbalance, most of the postural control measures improved after applying TBA-G. Conclusions: The results suggest that TBA-G could improve balance control ability. A larger controlled trial is needed to determine more accurately the effect of TBA-G on balance control ability.

스마트폰을 이용한 이중과제 수행이 발목 불안정성을 가진 성인의 균형과 근 활성도에 미치는 영향 (The Effects of Dual Task Performance on Balance and Muscle Activity in Adults with Ankle Instability with Smartphones)

  • Purpose : Using a smartphone while walking districts attention and increases the risk of losing balance or falling. Ankle instability is caused by decreased muscle strength and decreased neuromuscular ability leading to postural control problems. Dual tasks increases the risk of falls by reducing postural control in adults with ankle instability. This study aimed to investigate the effect of performing a dual task on balance and muscle activity in adults with ankle instability using a smartphone. Methods : Forty-nine individuals with ankle instability participated in this study. A game of finding the wrong picture was performed using a smartphone in the dual task, and only looking at the blank screen of a smartphone was evaluated in the single task. The participants randomly performed single and dual task to evalutate balance and muscle atcitivy. Balance was evaluated using the Biodex balance system (BBS), and muscle activity was evaluated using surface EMG. Muscle activity of the gastrocnemius and tibialis anterior was measured at the same time as balance. Results : The results of this study showed that overall, anteior/posterior, and medial/lateral balance indices all showed significant differences when performing the dual task compared with those during the single task (p<.05). The muscle activity results showed a significant difference compared with that of the gastronemius muscle on the nondominant side during the dual task (p>.05). Conclusion : The results of this study showed that maintaining balance is more difficult when performing the dual task than during the single task, and only the muscle activity of the nondominant gastrocnemius muscle decreased. The dual task causes a decrease in concentration for postural control, which negatively affects postural stability. Individuals with ankle ankle instability should refrain from performing dual tasks, such as using smartphones, to prevent ankle damage.

농업인의 작업화 유형과 작업 부하가 정적 자세균형에 미치는 영향 (Effects of Footwear and Workload on Static Body Balance of Farmers)

  • Postural stability can reduce the likelihood of critical slip and fall accidents in workplaces. The present study aimed to analyze the effect of shoes type on the ability of postural control during quiet standing. The effect of workload on the body balance was also of primary concern. Thirteen healthy male undergraduate students participated voluntarily in the experimental study. Standing on a force plate with wearing slippers, sports shoes, or safety shoes, two-axis coordinate on subjects' center of pressures (COP) was obtained in the two levels, rest and workload. For the workload level, subjects performed treadmill exercise to reach the predetermined level of physical workload. By converting the position coordinates of COPs, the postural sway length in both anterior-posterior (AP) axis and medio-lateral (ML) axis was assessed. ANOVA results showed that, in AP direction, wearing slippers significantly increased the postural sway length compared to wearing sports shoes or safety shoes. No significant difference in the mean sway length in AP axis was observed between sports shoes and safety shoes. In ML direction, both the workload and the shoes type did not significantly affect the mean length of postural sway. However, the postural sway length increased marginally with the slippers especially during the workload condition. This study explains wearing slippers may interfere with the ability of postural control during quiet standing. Physical workload decreases the ability of postural stability further.

CORE 안정성 훈련이 뇌졸중 환자의 자세조절, 균형 및 상지기능에 미치는 효과 (The Effects of Core Stability on Postural Control, Balance and Upper Motor Function in Patients with Stroke)

  • Objectives : The purpose of this study was to evaluate the effects of core stability training on postural control and balance of hemiplegia patients who are difficult to control posture due to stroke. Methods : Subjects of the study were consisted of 25 adult hemiplegia patients(experimental 12, control 13) who were receiving rehabilitation therapy in hospital. Its group had a core stability training program by a physical therapists for 40 minutes, five times a week for nine-week period. Measurements of postural assessment scale for stroke(PASS), berg balance scale(BBS) and manual function test(MFT) were evaluated at initial presentation(pretest) and after completion of the each therapy program(posttest). Independent t-test and paired t-test was utilized to detect the mean difference between the groups. Results : Firstly, the result of PASS showed that postural control was significantly increased after the completion of core stability training(p<.01). Secondly, the result of BBS showed that balance control was significantly increased after the completion of core stability training(p<.01) and general physical therapy(p<.01). Lastly, the result of MFT showed that upper extremity's motor function was significantly increased after the completion of core stability training(p<.001). Conclusions : This study showed that core stability training is an effective treatment strategy on postural control, balance and upper extremity's motor function.

자세조절훈련이 만성 뇌졸중 환자의 균형과 보행 능력에 미치는 영향 (The Effect of Postural Control Training on Balance and Walking Ability in Patients with Chronic Stroke)

  • PURPOSE: The purpose of this study was to determine the effects of postural control training on balance and walking ability in chronic stroke patients. METHODS: Eighteen chronic stroke patients were allocated equally and randomly to an experimental group (n=9) or a control group (n=9). All participants received 60 minutes of comprehensive rehabilitation treatment, the experimental group additionally received a postural control training for 30 minutes, while the control group additionally performed a treadmill training for 30 minutes. These 30-minute training sessions were held five times per week for three weeks. Balance was assessed using Berg balance scale (BBS) and walking ability (gait speed, cadence, step length, and double limb support) was assessed using the GAITRite system. RESULTS: Improvement on all outcome measures was identified from pre-to-post intervention for both groups (p<.05). Post-intervention, there was a significant between-group difference on measured outcomes (p<.05). The experimental group exhibited greater improvement in the gait speed (p=.01; 95% CI .08-.16), cadence (p=.04; 95% CI .34-4.79), step length (p=.02; 95% CI 1.50-5.17), double limb support period (p=.04; 95% CI -2.18 to -.14), and BBS (p=.01; 95% CI 1.04-6.74) compared to the control group. CONCLUSION: The findings of this study suggest that postural control training may be beneficial for improving balance and walking ability of patients with chronic stroke.

Hyperlipidemia as a predictor of physical functioning for stroke

  • Objective: Elevated cholesterol levels contribute to changes of the arterial endothelial permeability. Hyperlipidemia promotes atherosclerosis and is associated with an increased risk of stroke incidence. The purpose of this study was to investigate the effects of having a history of hyperlipidemia prior to a stroke incidence on postural balance, anticipatory dynamic postural control, gait endurance and gait performance in individuals with hemiparetic stroke. Design: Cross-sectional study. Methods: Fifty-two adults who were diagnosed with stroke 6 months ago or more were enrolled in this study. They were divided into two different groups according to hyperlipidemia history before stroke. All participants were assessed with the Activities-specific into Balance Confidence (ABC) scale, Berg Balance scale (BBS), Dynamic Gait Index (DGI), Timed Up and Go test (TUG), and the 6-minute walk test (6MWT). An independent t-test was used to analyze the difference between the hyperlipidemia group and non-hyperlipidemia group. Results: After analysis, the BBS, TUG, and 6MWT scores were significantly different between the hyperlipidemia and non-hyperlipidemia group, but not the ABC and DGI scores. Conclusions: The results of this study show that having a history of hyperlipidemia before stroke affects static and dynamic postural balance performance, anticipatory dynamic postural balance, and gait endurance in individuals with chronic hemiparetic stroke. Based on the results of this study, we also suggest treatment for hyperlipidemia should be implemented throughout the therapeutic interventions, such as pharmacological or exercise programs, in order to restore the physical function of stroke survivors.

Effects of Sit-to-stand Training with Various Foot Positions Combined with Visual Feedback on Postural Alignment and Balance in Stroke Patients

  • PURPOSE: This study compared the effects of sit-to-stand training with various foot positions combined with visual feedback on the postural alignment and balance. METHODS: Thirty stroke patients were assigned randomly into three groups of standing with a symmetrical foot position (SSF) (n = 10), asymmetrical foot position with the affected foot at the rear (SAF) (n = 10), and visual feedback and asymmetrical foot position (SVAF) (n = 10). Sit-to-stand training with different foot positions was performed for 30 minutes a day, five times a week, for a total of four weeks, and the effects on postural alignment and balance were assessed. RESULTS: The angle between the midline and scapula peak of the affected side was decreased significantly at sitting and thigh-off in the SAF group and at sitting, thigh-off, and standing in the SVAF group (p < .05). The angle between the midline and scapula peak of the non-affected side was increased significantly at sitting and thigh-off in the SAF group and at sitting, thigh-off, and standing in the SVAF group, the difference in the angle between the scapular peaks of the left and right sides was decreased significantly at sitting and thigh-off in SSF group, and at sitting in SAF group (p < .05). In the SVAF group, the angle at sitting, thigh-off, and standing was decreased significantly (p < .05). A comparison of the balance ability showed that BSS in the SVAF group was improved significantly (p < .05). CONCLUSION: Based on these results, the postural alignment and balance ability were improved in stroke patients who participated in sit-to-stand with visual feedback and asymmetrical foot position training.
