  • This study examines the relationship between mother's stress and cortisol level during late pregnancy and how it changes into postpartum periods. Thirty one families were recruited from Daejeon and Incheon in South Korea. The major findings were as follows: (1) The level of mothers' cortisol followed the typical diurnal pattern of decline throughout the day. Further longitudinal analysis revealed that the level of Korean mothers' cortisol steadily lowered from the 8th prenatal month to the 6th postnatal month. (2) During pregnancy, mothers reported to experience changes in physical activities, daily routines, eating habits, types or amount of social activities and sleeping habits. Among these they were stressed the most by changes in physical activities and social activities. (3) The results of postpartum depression tests conducted at two months after giving birth showed mothers at this time were mostly stressed from difficulty sleeping and unnecessary self criticism. (4) The level of psycho social stress in mothers at six months after giving birth was mainly on social roles and the questions about self reliance marked the highest points. (5) Overall, there was a tendency for the level of maternal stress to decrease definitely from before and after the delivery and even though the level of stress was steadily decreasing up to 6th postpartum month it was not as sharp as those of before and after the birth of the babies. The level of cortisol analysis also showed the same obvious tendency.

  • The Purpose of this study was to build a substantive theory about the experience of postpartum depression. The qualitative research method used was rounded theory. The interviewees were eight others who had experienced postpartum depression. The data were collected through in-depth interviews with audiotape recording done by the investigator over a period of twelve months. he data were analyzed simultaneously by a constant comparative method in which new data were continuously coded into categories and properties according to Strauss and Corbin's methodology. Analysis the grounded data resulted in 28 concepts being identified. Eight categories emerged from the analysis. The categories were regret, loss of freedom, isolation of oneself, heartache, loss, emotional upset, avoidance, recovery. These substantive categories are consistent with precious research results. Causal conditions included : regret, loss of freedom. Phenomena : heartache, loss, emotional upset. Context : isolation oneself. Intervention condition : avoidance. Action/interaction strategies : desire for recovery. Consequences : recovery. These categories were synthesized into the core concept-The process of filling the empty loss of self. The process of the experienced postpartum depression was ① change after delivery, ② searching for a reason for depression, ③ effort to recover from postpartum depression, ④ recovery from postpartum depression and return to previous life. The process of recovery from postpartum depression was proceeded by ① support from others, especially husband, ② resolution of stressful life events, ③ reconstructing of life goals and resolution strategies, ④ acceptance of depression and seeking psychiatric treatment. Seven hypotheses were derived from the analysis. ① Mothers who experienced stressful life event and economic problem are more depressive. ② Mothers who have conflict with parents are more depressive. ③ The more somatic symptoms, the more depression. ④ Social support facilitates recovery from postpartum depression. ⑤ Mothers who have lower self-esteem are more depressive. ⑥ Mother's role overload disturbs recovery from postpartum depression. ⑦ Ideal maternal identity facilitates recovery from postpartum depression. Through this substantive theory, nurses can understand the importance of postpartum depression management.

  • Postpartum depression is one of the most serious problems in maternal health because it affects not only the mother but also her family. Postpartum depression disturbs the maternal-infant interaction and attachment. However, most postpartum depression patients ignore this problem and do not seek treatment. Many clinicians and researchers realiza there is a need to develop a postpartum depression scale. Thus, this study has been designed to development of a postpartum depression scale. Data were collected through a survey over a period of three months. Subjects who participated in the study were 167 Korean mothers in their postpartum period. The author used a convenience sampling method. The analysis of the data was done with SPSS PC/sup +/ for descriptive statistics, item analysis and factor analysis. Initially 62 items were generated from the interview data of eight postpartum depression patients and from a literature review. This preliminary scale was analyzed for reliability and validity. The results of this analysis are as follows. 1. Initially 62 items were analyzed through the Index of Content Validity(CVI) and 48 items were selected. 2. Seven factors were extracted through the principal component analysis, and these contributed 61% of the variance in the total score. Finally 46 items in the scale loaded .41∼ .84 on one of seven factors. 3. Each factor was labeled. Factor 1 was labeled 'emotional phenomena-emotional upset' and included 13 items, factor 2 was labeled' cognitive phenomena-self concept disturbance' and included seven items, factor 3 was labeled 'relationship to baby-negative feeling' and included six items, factor 4 was labeled 'relationship to baby- overload' and included eight items, factor 5 was labeled 'negative maternal identity' and included five items, factor 6 was labeled 'biophysiological phenomena-disturbance of physical functioning' and included four items, and factor 7 was labeled' interpersonal relationship phenomena-blamed others' and included three items. 4. Cronbach Coefficient Alpha for internal consistency was .95 for the total 46 items. Finally, the author suggests that this scale could be adequately applied in assessing the postpartum depression of mothers during the postpartum period. The results of this study can contribute to designing an appropriate postpartum depression prevention strategy.

  • Purpose: This study was designed to identify mother and infant related factors that influence child-care stress among the mother of newborn infants. Methods: Data were collected through self-administered questionnaires and descriptive statistics, correlation and multiple regression were used to analyze the data. The data survey was conducted with 957 conveniently selected mothers of infants when they visited a public health center in Seoul to have their children immunized. Results: The average item score for the Childcare Stress Inventory was 38.03, for the Edinburgh Postnatal Depression Scale, 9.31 and for the Degree of Bother Inventory, 23.42. The CSI was positively correlated to EPDS (r=.44, p<.001) and DBI (r=.40, p<.001). Also these two variables explained 30.0% of CSI in infants' mothers. Conclusion: These findings are expected to expand the understanding about postpartum mothers' child-care stress and can contribute to the development of comprehensive interventions based on community health nursing.

  • The longitudinal changes in the transitional milk yields of Korean lactating women(14 primiparae, 11 multiparae) have been studied by test-weighing method in the part of Kangwon Province form 7 day to 15 days postpartum. The transitional milk yields at 7, 10, 15 days postpartum were 531$\pm$148,598$\pm$156 and 639$\pm$169g, respectively. The mean milk yield was 589$\pm$162g from 7 to 15 days postpartum. The transitional milk yields between primiparae and multiparae appeared not significantly different but significantly different between mothers of boys and girl(p<0.05). The distribution of individual transitional milk yields were found 550-649g(28.0%), 450-549g(24.0%), 650-749g(13.3%) and 750-849g(13.3%). The transitional milk yields were not affected by mother's age, weight gain during pregnancy, gestational period and infant's weight at birth but affected by maternal height(p<0.05). Maternal weight reduction during the lactation had no correlation with the transitional milk yeilds.

  • Objectives Postpartum blues is known to be a major risk factor for postpartum depression and can be associated with the problems of language skills, behaviors or learning skills of their children. Therefore, it is very important for clinicians to evaluate precisely and control postpartum blues. Recent studies have found that music has an effect on depressive mood and the frontal EEG asymmetry of the patients with depression. The purpose of this study was to find out the effects of music on the frontal EEG asymmetry of the mothers with postpartum blues. Method Among one hundred and seventy mothers assessed with Korean version of the Edinburg Postnatal Depression Scale (EPDS), nine mothers with postpartum blues (EPDS ${\geq}$ 10) as postpartum blues group and nine non-depressive mothers (EPDS < 10) as non-depressive mother group were included. Ten non-labored, non-depressive women were also included as a normal control group. The subjects were evaluated with the State Trait Anxiety Inventory (STAI)-X1, the Visual Analogue Scale (VAS) and the Depression Adjective Checklist-Korean version (K-DACL) and EEG twice before and after the music sesssion with the length of twenty minutes and thirty two seconds. The statistical analyses were done for A1 score (log R - log L) which were computed from the alpha powers at F3 and F4. Results No significant difference was noted in demographic data among all three groups. The postpartum blues group had higher scores in the STAI-X1, the VAS and the K-DACL compared to the other groups at baseline, and their A1 scores were lower than those of only normal controls. There was a statistically significant increase of A1 score only in the postpartum blues group after the music session. Conclusion This study suggests that the mothers with postpartum blues may have a frontal EEG asymmetry which is possibly associated with their depressive mood, and the music session can affect the frontal asymmetry positively.

  • Purpose: There has not been decisional consensus in the concepts and the ways of postpartum cares between Western and non-Western cultures though the importance of postpartum period has been emphasized by the multitude of health professionals. In this study, we investigated the differences in recognition concerning postpartum care among Koreans, Korean residents in America, and Americans (hereafter, K, KA, A). Methods: We used questionnaire of women who had given birth from last 6 weeks to less than 13 years. The women consisted of three groups;104 K, 44 KA, and 44 A. Results: In the perception survey concerning postpartum care, every three group thought that postpartum care was important. However, there were differences in understanding how it would affect women's health, and what they did in the period. K significantly agreed with the content and the importance of postpartum care. The investigation concerning seven categories of postpartum care showed that K group performed the 6 categories except consuming medication beneficial for the postpartum care. KA group performed in a low rate and A group performed two categories in a significantly low rate two categories; avoiding cold stimulation and keeping the body warm. Conclusion: Based on this study, it should be suggested that there were common points and differences among the recognition concerning postnatal care in three groups. Therefore, health professionals make efforts in understanding diversities on postnatal care and providing the adequate services reflecting each mother's needs and recognition.
