  • Objective : This study was carried out to analyze the trends of pharmacopuncture used to treat Herpes Zoster related articles that have been published in the Korean medicine journal.Method : We studied 14 research papers treated with pharmacopuncture for Herpes Zoster in Korean medicine journal. We analyzed for type of pharmacopuncture used, treatment point and clinical type. Results : 1. The number of searched journal is 14 papers. 2. The most common clinical type is herpes zoster generalisatus. Others types are postherpetic neuralgia, ramsay hunt syndrome, herpes zoster ophthalmicus, postherpetic paralysis 3. Pharmacopunctures used to treat herpes zoster are BV, Hwangreonhaedok-tang pharmacopuncture, Ginseng Pharmacopuncture, CF, JsD, Immuno-yakchim, etc. The most frequently used type of harmacopuncture is BV. 4. The most used part as a treatment point is a-shi point. Conclusions : It is needed to improve the cure rate through a comprehensive analysis of the Herpes zoster treatments. It is necessary to comprehensively analyze the treatments to increase the cure rate of about Herpes zoster.

  • Background: The use of neurolytic agents to control neuropathic pain has been described from the last century Phenol and ethyl alcohol have been widely used as neurolytic agents, however, their neurolytic effect is variable in efficacy and duration of action, and infrequently accompanied with grave complications. It has been found that 5% lidocaine causes irreversible conduction blockade in animal studies. The goal of this study was to evaluate the neurolytic effect of 5%o lidocaine on various neuropathic pain syndromes for prolonged analgesia. Methods: Twenty-five patients with a diagnosis of neuropathic pain including trigeminal neuralgia (n = 7), postherpetic neuralgia (n = 10), and postsurgical neuralgia (n = 8) were selected after failure of routine therapeutic regimens. After performing a diagnostic nerve block with 1% lidocaine and 5% lidocaine was injected. The patients were followed for 6 months. Visual analog scale (VAS) scores and side effects were recorded for each patients. Results: A significant decrease in pain scores after neurolytic blockade with 5% lidocaine was seen in all of three pain groups. All the patients reported immediate and prolonged pain relief lasting from 4 weeks to 6 months. None of patients exhibited any appreciable side effects or complications. Conclusions: We suggest that 5% lidocaine may be used safely and effectively for the purpose of prolonged analgesia in selected patients with intractable neuropathic pain syndromes.

  • Gabapentin is an oral antiepileptic agent with an unknown mechanism of action. There have been many proposed uses for gabapentin, including neuropathic pain, reflex sympathetic dystrophy, postherpetic neuralgia, midscapular pain secondary to radiation myelopathy and migraine prophylaxis. This report presents patients who were treated with gabapentin when other pharmacologic interventions failed to relieve neuropathic pain 3 patients with neuropathic pain were included among these cases. All patients were started on 200 mg gabapentin. The maximum dose required for pain relief was between 800 mg and 2400 mg. Gabapentin may be a useful adjunct for treating neuropathic pain with minimum of side effects.

  • Herpes zoster is a viral disease characterized by skin rash and persistent pain. Early treatment with epidural analgesia provides very effective pain relief and reduces the incidence of postherpetic neuralgia. However, epidural analgesia in elderly, deliberated or hypovolemic patients may complicate circulatory depression such as hypotension and bradycardia. Even if temporary, a major decrease in blood pressure may decrease coronary blood flow of patients with arteriosclerosis and ischemic accident may occur. We experienced a case of unstable angina pectoris after lumbar epidural blockade in a herpes zoster patient with chronic stable angina pectoris.

  • A case of severe complications following intrathecal administration of 45 ml of hypertonic saline solution for the treatment of postherpetic neuralgia was presented. Transient immediate complications included were tachycardia, hypertension, neck stiffness and muscle twitch. Pulmonary edema, paralytic intestinal obstruction, and the cauda equina syndrome including sphincter disorder with atonic urinary bladder developed shortly after the injection. Tenesmus and sensory abnormality around perineum and soles were the longlasting complications.

  • Lidocaine has been studied for many chronic pain conditions, including postherpetic neuralgia (PHN) and recently it has also been increasingly used in transdermal drug delivery systems. In this study, pharmacokinetics of a lidocaine patch was studied in four hairless male rats. The plasma concentration was determined by a validated LC/MS/MS method after applying a $3{\times}2cm^2$ (30mg) patch for 12 hours. From the plasma lidocaine concentration vs time curves, $AUC_{0-20h}$, Cmax, and Tmax of lidocaine patch were $2,926.32{\pm}335.28ng{\cdot}h/ml$, $256.86{\pm}29.63ng/ml$, and $6.00{\pm}2.31h$, respectively.

  • ;The Varicella zoster virus can cause two infectious diseases. The primary infection of this virus leads to Chicken pox and it goes into a latent period until it reveals itself again. When secondarily shown. it is called Herpes Zoster. Herpes Zoster can usually be seen between T-3 and L-2 of immunodepressant patients but in 13%. it is seen in the trigeminal nerve branch. During the predrominal stage. burning sensation, itching. and sharp pain can be experienced unilaterally where the nerve branch is distributed. Patients can also express tooth pain in this stage. After 2-4 days the acute stage begins with its characteristic unilateral macule showing vesicle formation. In this stage fever and fatigue can be experienced. Continuous pain even after the acute stage is called postherpetic neuralgia; deep pain and burning sensation can be expressed by the patient. The sequelae of this disease consists of rare osteomyelitis. necrosis of the jaws. or loss of teeth.teeth.

  • Objectives : This study reports a case of Soeumin patients with Herpes zostersMethods : We examined progress of Soeumin Herpes zoster patient's symptoms (skin blisters, pain, insomnia, etc.) and complications when during and after the treatments. Results : Soeumin Herpes zoster patient was diagnosed as Yang Depletion. Yongdamsagan-tang, Sweet BV, were effective for decrease the patient's skin blisters and pain. Soeumin Bojungikgi-tang prevented the patient's aftereffects. Conclusions : Soeumin Herpes zoster patient who had severe pain, blisters, and sleep disorder, was expected high risk of postherpetic neuralgia. But diagnosed Yang Depletion symptomatology and treated with Soeumin Bojungikgi-tang, patient recover completely without sequelae.

  • Herpes zoster (HZ) is an acute, unilateral inflammatory viral infection characterized by a rash with painful blisters in a localized area of the body. HZ is often associated with intense pain in the acute phase and presents postherpetic neuralgia in the chronic phase. During the prodromal stage of the HZ from the trigeminal nerve, however, the only presenting symptom may be odontalgia, which could be particularly difficult to diagnose. This distinctive syndrome occurs predominantly in the immunocompromised or elderly individuals. In this article, we report a case of HZ developed in the trigeminal nerve of a 60-year-old immunocompromised female patient, whose symptoms including atypical, non-odontogenic odontalgia had improved after series of antiviral treatments.

  • Tricyclic antidepressant (TCA) is a useful drug for treating neuropathic pain. However, tremors are one of the relatively frequent side effects of TCA. A female patient, who was suffering from postherpetic neuralgia, was treated with amitriptyline starting with 10 mg/day. She developed resting tremors on the second day after increasing the dose to 30 mg/day. This case highlights the need for the careful use of amitriptyline in the treatment of neuropathic pain in elderly patients.
