Stroke after percutaneous transhepatic variceal obliteration of esophageal varix in Caroli syndrome

  • Here we present the case of an 11-year-old female patient diagnosed with Caroli syndrome, who had refractory esophageal varices. The patient had a history of recurrent bleeding from esophageal varices, which was treated with endoscopic variceal ligation thrice over a period of 2 years. However, the bleeding was not controlled. When the patient finally visited the Emergency Department, the hemoglobin level was 4.4 g/dL. Transhepatic intrajugular portosystemic shunt was unsuccessful. Subsequently, the patient underwent percutaneous transhepatic variceal obliteration. Twenty hours after this procedure, the patient complained of aphasia, dizziness, headache, and general weakness. Six hours later, the patient became drowsy and unresponsive to painful stimuli. Lipiodol particles used to embolize the coronary and posterior gastric veins might have passed into the systemic arterial circulation, and they were found to be lodged in the brain, kidney, lung, and stomach. There was no abnormality of the portal vein on portal venography, and blood flow to the azygos vein through the paravertebral and hemiazygos systems was found to drain to the systemic circulation on coronary venography. Contrast echocardiography showed no pulmonary arteriovenous fistula. Symptoms improved with conservative management, and the esophageal varices were found to have improved on esophagogastroduodenoscopy.

Waffle-Cone Technique Using Solitaire AB Stent

  • The waffle-cone technique is a modified stent application technique, which involves protrusion of the distal portion of a stent into an aneurysm fundus to provide neck support for subsequent coiling. The authors report two cases of wide necked basilar bifurcation aneurysms, which were not amenable to stent assisted coiling, that were treated using the waffle-cone technique with a Solitaire AB stent. A 58-year-old woman presented with severe headache. Brain CT showed subarachnoid hemorrhage and angiography demonstrated a ruptured giant basilar bifurcation aneurysm with broad neck, which was treated with a Solitaire AB stent and coils using the waffle-cone technique. The second case involved an 81-year-old man, who presented with dizziness caused by brain stem infarction. Angiography also demonstrated a large basilar bifurcation unruptured aneurysm with broad neck. Solitaire AB stent deployment using the waffle-cone technique, followed by coiling resulted in near complete obliteration of aneurysm. The waffle-cone technique with a Solitaire AB stent can be a useful alternative to conventional stent application when it is difficult to catheterize bilateral posterior cerebral arteries in patients with a wide-necked basilar bifurcation aneurysm.

Spontaneous Bilateral Supratentorial Subdural and Retroclival Extradural Hematomas in Association with Cervical Epidural Venous Engorgement

  • We describe a case of 36-year-old man who presented with a subacute headache preceded by a 1-month history of posterior neck pain without trauma history. Head and neck magnetic resonance imaging (MRI) studies disclosed bilateral supratentorial subdural and retroclival extradural hematomas associated with marked cervical epidural venous engorgement. Cerebral and spinal angiography disclosed no abnormalities except dilated cervical epidural veins. We performed serial follow-up MRI studied to monitor his condition. Patient's symptoms improved gradually. Serial radiologic studies revealed gradual resolution of pathologic findings. A 3-month follow-up MRI study of the brain and cervical spine revealed complete resolution of the retroclival extradural hematoma, disappearance of the cervical epidural venous engorgement, and partial resolution of the bilateral supratentorial subdural hematoma. Complete resolution of the bilateral supratentorial subdural hematoma was confirmed on a 5-month follow-up brain MRI. The diagnosis and possible mechanisms of this rare association are discussed.

모아레 측정법과 설문지 조사를 통한 경기도 S고등학교 재학생의 신체자각증상과 배부체형에 대한 실태조사 (A Study on the Back shape and self-conscious symptoms of the students in S High school Using the Moire measurement and Questionnaire investigation)

  • Objective : The purpose of this study was to investigate the back shape and self-conscious symptoms of the students in S High school using the phase-shifting scanning grating projection Moire interferometer and Questionnaire investigations. Methods : In this study the subjects consisted of 317 pupils[168 boys(53%), 149girls(47%)] attending S high school in Sungnamsi, Kyoungkido in 2002. Their ages ranged from sixteen to eighteen. With the phase-shifting scanning grating projection moire interferometer, the posterior views of the body were taken to see if there are correlation of remainder value of the height spot of left & right shoulder blade and gluteal region in Moire topography. And using questionnaire investigation, we investigated the self-conscious symptoms. Results : 1. In questionnaire investigation, we observed that the ratios of self-conscious symptoms of girls are more than that of boys. The ratios of headache, neck pain, lower back pain, digestive symptom were more than 70% in boys and girls. 2. In Moire topography, more frequent findings of scapular region were observed that left scapular area were higher than right(in boys 69.3%, 60%, 100%, aged 16,17,18, in girls 66.8%, 40.5%, 58.8% aged 16,17,18). 3. More frequent findings of gluteal region were observed that left scapular area were higher than right(in boys 75.2%, 60%, 36.4% aged 16,17,18, in girls 61.1%, 46.8%, 64.7% aged 16,17,18) 4. More frequent findings of reminder value of the vertical lines of cervical and buttock region in Moire topography were observed that the vertical lines of cervical region were inclined to left than the vertical lines of buttock(in boys 73.3%, 92.2%, 100% aged 16,17,18, in girls 72.2%, 77.2%, 73.5% aged 16,17,18) Conclusion : From these results, we found that the self-conscious symptoms were more than in girls than boys; the ratio of headache, neck pain, Lower back pain, digestive symptom was more than the others; the back shape of high school students were not balanced in scapular and buttock region; necks were inclined to left than buttock.

한국인 부정교합자의 측두하악장애(TMD) 유병율과 그 기여요인에 관한 연구 (The Prevalence Study of TMD and the Associated Factors in Korean Malocclusion Patients)

  • 서울대학교병원 치과 교정과에 내원한 6세 1개월에서 45세 8개월사이(평균 19세 6개월)의 부정교합자 205명(남 67명, 여 138명)을 대상으로 TMD 증상과 기여요인에 관한 설문조사와 Orthopantomogram, 경두개 방사선 사진(transcranial view)의 판독, 교정모형 검사 등을 시행하여 다음과 같은 결론을 얻었다. 1. Helkimo Anamnestic index는 Ai 0 $46.8\%$, Ai I $22.0\%$, Ai II $31.2\%$로 TND의 주관적 증상이 연령에 따라 증가하는 추세(p<0.001)를 보였으며 여성에서 높았다(p<0.05). 2. Orthopantomogram상 하악두의 이상소견은 flattening($4.4\%$)이 가장 많았으며 전체적으로 한가지 이상의 이상소견을 보인 경우는 $8.3\%$였다. 3. 목과 어깨의 동통(p<0.001) 이악물기, 입술을 깨무는 습관(이상 p<0.01). 두통(p<0.05)이 있는 경우 Ai가 높았다. 4. Angle II급에서 Orthopantomogram상 하악두 이상소견의 빈도가 높았고, 상악 대구치의 교모가 있을때 Ai가 높은 경향이 있었다(이상 p<0.05). 그외의 교합 요소는 TMD의 증상과 무관하였다. 5. 하악두 길이(Co'-Inc')와 하악지 길이(Co'-Go')가 짧고 하악두의 비율(Co'-Inc'/Co'-Go')이 낮을때, 그리고 좌우 하악지 길이 (Co'-Go'), 하악두 비율(Co'-Inc'/Co'-Go')이 다를때 Orthopantomogram상 하악두 이상소견이 많았다.

제왕절개술환자의 임상적 고찰 (Clinical Analysis of Cesarean Sections)

  • One hundred and fifty seven cesarean sectioned patients among 2010 deliveries, were reviewed with the following clinical charts from January, 1973 to December, 1974 at Taegu Presby-terian medical Center. The results are as follows; 1) The section rate was 7.8% 2) In age distribution, the most frequent age was 26~30ys and it~s rate was 37%, and others were as follows : 31~35ys 24.2%, 21~25ys 21%, 36~40ys 10.8%, 41~45ys 7% respective]y- 3) In indication, the cephalopelvic disproportion was the most prevalent and it's rate was 29.3%, and others were as follows : malpresentation 14% previous cesarean section 10.8% uttering dysfunction 9.6%, totemic 4%, elderly primigravida 4%, fetal distress 2.5% post maturity 3.2%, wanted cesarean section 1, 9%, pelvic deformity 1.2% twin 1.9%, abruptio placenta 1.2%, Vaginal stricture 1.2%, and others 2.5%. 4) In type of operation, tower cervical transverse type was 82.1%, classical type 17%, ana cesarean hysterectomy was 0.9% respectively. 5) General Anesthesia was major type and it's rate was 63%, and spinal anesthesia 17.2%, epidermal anesthesia 14% and other combined type was 5.8%. 6) Percental mortality was 11.4%. 7) There was no maternal death. 8) Among the cesarean section complications, wound infection was 8,4%, headache 4.4%, urinary tract infection 1.2% and one case in endometritis. 9) In combined surgery, tubal legation was 31.2%, appenectomy 7.6%, posterior repair 1.2%, salpingoop-horectomy 1.9%, and one case in resection of uttering septum. 10) Cesarean section was performed 61.3% in multipara and 38.2% in primipara. 11) 47 (30%) had received artificial abortions. 12) 28 (17.8%) had previously experienced cesarean section.

Intracranial Extension of Spinal Subarachnoid Hematoma Causing Severe Cerebral Vasospasm

  • Spinal subarachnoid hemorrhages (SAH) can extend into the intracranial subarachnoid space, but, severe cerebral vasospasm is rare complication of the extension of intracranial SAH from a spinal subarachnoid hematoma. A 67-year-old woman started anticoagulant therapy for unstable angina. The next day, she developed severe back pain and paraplegia. MRI showed intradural and extramedullar low signal intensity at the T2-3, consistent with intradural hematoma. High signal intensity was also noted in the spinal cord from C5 to T4. We removed subarachnoid hematoma compressing the spinal cord. The following day, the patient complained of severe headache. Brain CT revealed SAH around both parietal lobes. Three days later, her consciousness decreased and left hemiplegia also developed. Brain MRI demonstrated multiple cerebral infarctions, mainly in the right posterior cerebral artery territory, left parietal lobe and right watershed area. Conventional cerebral angiography confirmed diffuse severe vasospasm of the cerebral arteries. After intensive care for a month, the patient was transferred to the rehabilitation department. After 6 months, neurologic deterioration improved partially. We speculate that surgeons should anticipate possible delayed neurological complications due to cerebral vasospasm if intracranial SAH is detected after spinal subarachnoid hematoma.

Intracranial Fusiform Aneurysms : It's Pathogenesis, Clinical Characteristics and Managements

  • Objective : The objective of this study is to investigate clinical characteristics, management methods and possible causes of intracranial fusiform aneurysm. Methods : Out of a series of 2,458 intracranial aneurysms treated surgically or endovascularly, 22 patients were identified who had discrete fusiform aneurysms. Clinical presentations, locations, treatment methods and possible causes of these aneurysms were analyzed. Results : Ten patients of fusiform aneurysm were presented with hemorrhage, 5 patients with dizziness with/without headache, 4 with ischemic neurologic deficit, and 1 with 6th nerve palsy from mass effect of aneurysm. Two aneurysms were discovered incidentally. Seventeen aneurysms were located in the anterior circulation, other five in the posterior circulation. The most frequent site of fusiform aneurysm was a middle cerebral artery. The aneurysms were treated with clip, and/or wrapping in 7, resection with/without extracranial-intracranial (EC-IC) bypass in 6, proximal occlusion with coils with/without EC-IC bypass in 5, EC-IC bypass only in 1 and conservative treatment in 3 patient. We obtained good outcome in 20 out of 22 patients. The possible causes of fusiform aneurysms were regard as dissection in 16, atherosclerosis in 4 and collagen disease or uncertain in 2 cases. Conclusion : There is a subset of cerebral aneurysms with discrete fusiform morphology. Although the dissection or injury of internal elastic lamina of the cerebral vessel is proposed as the underlying cause for most of fusiform aneurysm, more study about pathogenesis of these lesions is required.

파종성 악성 핍지신경교종 환자에서 시행한 뇌실내 Methotrexate의 효과 - 증례보고 - (The Effect of Intraventricular Methotrexate in a Patient of Disseminated Anaplastic Oligodendroglioma - Case Report -)

  • Oligodendrogiomas account for about 4 per cent of intracranial gliomas and surgery is known to be an essential first step to establish an accurate diagnosis and when oligodendrogliomas recur with or without anaplastic features after initial resection, radiation and chemotherapy consisting of the administration of procarbazine, lomustine, and vincristine are usually indicated. We report our experience of an excellent result with intraventricular methotrexate chemotherapy for a patient with disseminated anaplastic oligodendroglioma. A 29-year-old male patient presented with diplopia and headache for two months. MRI showed a irregular, faintly enhanced mass in the posterior fossa. The hisotological diagnosis was an anaplaplastic oligodendroglioma and he was treated with chemotherapy of PCV regimen and radiotherapy followed by surgery. CSF dissemination was revealed by a follow-up MRI during the period. Intraventricular methotrexate(0.175mg/kg) was given twice a week for 4 weeks through ommaya reservoir and the size of the multiple tumors was decreased significantly on follow-up MRI. This case report suggests that an aggressive treatment involving intravent-ricular chemotherapy may be helpful even when anaplastic oligodendrogliomas disseminates to leptomeninges.

뇌지주막 낭종의 치료결과 분석 (Analysis of Treatment Result of Arachnoid Cyst)

  • Objective : The present study was performed to analyze treatment results for 22 cases of arachnoid cyst and to have appropriate surgical method in our department. Material and Methods : We performed a retrospective study in 22 cases in 11 years between 1989 to 2000 that could be followed up. The analysis was based on the results of patients age, sex distribution, developed area, clinical symptom, treatment method, and complication. Results : The age range of cyst development was between 7 months to 60 years with the average age of 21 years. As for sex distribution, 20 were male and 2 were female, with significantly more cyst development in males than females. Thirteen cases were developed in the sylvian fissure, 3 cases in the posterior fossa, 4 cases in the cerebral convexity of the supratentorial area, 1 case in the suprasella and 1 case in interhemiphere. Those cases with the sylvian fissure involvement included 6 cases of Type I, 4 cases of Type II, and 3 cases of Type III. As for the distribution according to hemisphere, more arachnoidal cysts were seen in the right hemisphere. The most common clinical symptom was headache, followed by seizure and speech disturbance. As for the treatment method in 22 cases, surgery was performed in 17 cases and conservative treatment in 5 cases. Fenestration was performed in 14 cases. 13 cases of them showed good outcome, and 1 case with delayed development showed no improvement. Cyst-peritoneal shunt was done in 2 cases. Both fenestration and cyst-peritoneal shunt were done in 1 case. Conclusion : Patients who perforemed fenestration were showed good outcome with few complication. We concluded that fenestration is the most appropriate surgical method for arachnoid cyst.
