Effectiveness Analysis of Preliminary Evaluation of Public Library Establishment Feasibility Based on the Revised Library Act (도서관법 개정에 따른 공공도서관 설립 타당성 사전평가의 실효성 분석)

  • The purpose of this study is to determine whether the pre-evaluation of the feasibility of establishing a public library, which is mandatory according to the revision of the Library Act, can improve the quality of the library. Therefore, the effectiveness of the preliminary evaluation was analyzed through the case analysis of the preliminary evaluation of the public library construction support project, which was implemented for the same purpose before the revision of the Library Act. In the case of libraries built after preliminary evaluation, most of the minimum standards for legal librarians required by the Enforcement Decree of the Library Act were secured, which helped secure basic library personnel. However, there were few libraries that secured and operated as much manpower as originally planned, and in some cases, the application applied to operate the library directly, but it was changed to consignment operation at the time of opening, so the pre-review details were not faithfully reflected. This study proposed re-establishing realistic librarian placement standards according to the revision of the law and introducing a post-evaluation using the library registration system so that it would not be a formal pre-process through case analysis.

A Numerical Study on the Mitigation Effect of Water Curtain for SiCl4 Toxic Gas Release (SiCl4 누출 시 수막설비의 방재효과에 대한 수치 해석 연구)

  • Silicone tetrachloride (SiCl4) leak accidents cause enormous human and environmental damage because it is highly toxic. Some handling facilities use water curtains to reduce the impact range of SiCl4. Although the water curtain is known as one of the most efficient technologies for post-release mitigation, its effect on reducing SiCl4 concentration needs to be investigated scientifically and quantitatively. In this study, three-dimensional computational fluid dynamics (CFD) was used to investigate the physical and chemical effects of water curtains as a release-mitigation system for SiCl4. SiCl4 is released and dispersed five seconds prior to the operation of the water curtain. Once the water curtain works, the SiCl4 reacts chemically with the water and its concentration decreases rapidly; it reaches an emergency response planning guidelines level 2 (ERPG-2) of 5 parts per million (ppm) at about 570 m. We observed, however, that the physical effect of water curtains on reducing SiCl4 concentration is insignificant when the chemical effect is eliminated. These results are crucial since they can be a scientific and quantitative basis for the 'technical guidelines for estimating the accident affected range'. In order to protect the public from chemical accidents, more toxic gas mitigation technologies need to be developed.

Initiating Liver Transplantation at a Public Hospital in Korea

  • Background: Although there more than 1,000 liver transplantations (LTs) are performed in Korea annually, their immense cost remains a great hurdle. Hence, in an attempt to reduce the medical costs of LT, a program was initiated at a public hospital affiliated with the Seoul National University Hospital. Methods: A total of 11 LTs have been successfully executed since the first LT performed at Seoul Metropolitan Government Seoul National University Boramae Medical Center in July 2011 through December 2014. Results: Nine patients (81.8%) were male and two (18.2%) were female. The mean age of patients was 53.4±11.4 years. Hepatitis B virus-related liver disease (n=6, 54.5%) was the most common causative disease, followed by alcoholic liver disease (ALD) (n=4, 36.4%). The actuarial 3-year survival rate was 90.9%. The median total medical cost of LTs was US $41,583 (calculated from operation to discharge), but only $11,860 was actually charged for patients with health insurance coverage. One female patient who had undergone deceased donor LT for alcoholic liver cirrhosis died during follow-up. This patient was non-compliant with the medical instructions after discharge, and finally expired due to septic shock at 10 months post-LT. Conclusions: In the public hospital, LT was successfully performed at a much lower cost. However, LT guidelines and peritransplant management protocols for patients with ALD must be established before escalating LT at public hospitals since ALD with poor compliance is one of the most common causes of complications at public hospitals.

Comparison of Outcomes at Trauma Centers versus Non-Trauma Centers for Severe Traumatic Brain Injury

  • Objective : Traumatic brain injury (TBI) is one of the most common injuries in patients with multiple trauma, and it associates with high post-traumatic mortality and morbidity. A trauma center was established to provide optimal treatment for patients with severe trauma. This study aimed to compare the treatment outcomes of patients with severe TBI between non-trauma and trauma centers based on data from the Korean Neuro-Trauma Data Bank System (KNTDBS). Methods : From January 2018 to June 2021, 1122 patients were enrolled in the KNTDBS study. Among them, 253 patients from non-traumatic centers and 253 from trauma centers were matched using propensity score analysis. We evaluated baseline characteristics, the time required from injury to hospital arrival, surgery-related factors, neuromonitoring, and outcomes. Results : The time from injury to hospital arrival was shorter in the non-trauma centers (110.2 vs. 176.1 minutes, p=0.012). The operation time was shorter in the trauma centers (156.7 vs. 128.1 minutes, p=0.003). Neuromonitoring was performed in nine patients (3.6%) in the non-trauma centers and 67 patients (26.5%) in the trauma centers (p<0.001). Mortality rates were lower in trauma centers than in non-trauma centers (58.5% vs. 47.0%, p=0.014). The average Glasgow coma scale (GCS) at discharge was higher in the trauma centers (4.3 vs. 5.7, p=0.011). For the Glasgow outcome scale-extended (GOSE) at discharge, the favorable outcome (GOSE 5-8) was 17.4% in the non-trauma centers and 27.3% in the trauma centers (p=0.014). Conclusion : This study showed lower mortality rates, higher GCS scores at discharge, and higher rates of favorable outcomes in trauma centers than in non-trauma centers. The regional trauma medical system seems to have a positive impact in treating patients with severe TBI.

  • In this study, the village development promotion system was reorganized for Gimhae-si, and improvement plans and post-operation management plans were prepared through expert surveys. Through the Village Development Project, the landscape and environment of the village were improved, and the village and residents were positively changed. The Village Development Project laid the foundation for regional revitalization, giving the next generation hope for the sustainable development of the village. While maintaining the framework of the existing Rural Development Projects guidelines through case analysis of other regions, we operate educational programs for each stage by reflecting regional characteristics, and utilize and communicate with intermediate support organizations, systematically operate, and actively intervene by Eup·Myeon administration. In addition, as a follow-up management program, various follow-up management programs were being operated, such as support for capacity building such as consulting, as well as facility maintenance and support for village managers. As a result of the expert survey to review the adequacy of the improvement plan, the improvement plan derived from the overall average of 4.28 points and the standard deviation of 0 to 0.94 points for the process is judged to be appropriate. In particular, the item of 'self-reliance stage follow-up management' was very important as all experts responded with 5 points. By establishing the village development performance goals, we tried to check whether the qualitative/quantitative goals were achieved by year, and to secure quantified status data in response to changes such as resetting performance goals or adding new performance indicators. In this study, in order to improve the capacity to achieve regional development in response to the rural agreement, we prepared our own sustainable village development promotion system, governance establishment, and follow-up management plan. In addition, it is expected to be used as a reference material for the selection of new districts and the follow-up management of completed districts by establishing the village development goals.

A Convergence Study on the Effects of Workplace Spirituality on Infection Control Knowledge, Performance, and Job Stress of Dental Hygienists in the COVID-19 Pandemic (일터 영성이 COVID-19 팬데믹 상황에서 치과위생사의 감염관리 지식, 수행과 직무 스트레스에 미치는 영향에 관한 융복합 연구)

  • This study aimed to analyze the Influence of workplace spirituality on job stress and infection control performance of dental hygienists in the COVID-19 pandemic situation Questionnaire was conducted for 149 dental hygienists from February to March 2021. COVID-19 knowledge and practice of infection control, job stress. and workplace spirituality. Survey data were analyzed t-test, ANOVA, Pearson's correlation using statistical programs of PASW Statistics ver. 21.0. Workplace spirituality was investigated to increase infection control performance and reduce job stress in a pandemic situation. The group with high infection control knowledge and performance showed low levels of job instability, organizational system, and stress. As for job stress, those with less than 2 years of experience reported relationship conflict, those with 3 to 5 years of experience showed high levels of job instability, organizational system, inadequate compensation, and workplace culture. In a pandemic situation, workplace spirituality was investigated to increase infection control performance and reduce job stress, so a plan to improve the quality of medical care was required for holistic and systematic organizational operation in preparation for the post-coronavirus.

Determination and evaluation of dynamic properties for structures using UAV-based video and computer vision system

  • Buildings, bridges, and dams are examples of civil infrastructure that play an important role in public life. These structures are prone to structural variations over time as a result of external forces that might disrupt the operation of the structures, cause structural integrity issues, and raise safety concerns for the occupants. Therefore, monitoring the state of a structure, also known as structural health monitoring (SHM), is essential. Owing to the emergence of the fourth industrial revolution, next-generation sensors, such as wireless sensors, UAVs, and video cameras, have recently been utilized to improve the quality and efficiency of building forensics. This study presents a method that uses a target-based system to estimate the dynamic displacement and its corresponding dynamic properties of structures using UAV-based video. A laboratory experiment was performed to verify the tracking technique using a shaking table to excite an SDOF specimen and comparing the results between a laser distance sensor, accelerometer, and fixed camera. Then a field test was conducted to validate the proposed framework. One target marker is placed on the specimen, and another marker is attached to the ground, which serves as a stationary reference to account for the undesired UAV movement. The results from the UAV and stationary camera displayed a root mean square (RMS) error of 2.02% for the displacement, and after post-processing the displacement data using an OMA method, the identified natural frequency and damping ratio showed significant accuracy and similarities. The findings illustrate the capabilities and reliabilities of the methodology using UAV to evaluate the dynamic properties of structures.

A Study on the Determination of the Optimal Parameter for the Evaluation of the Effective Prestress Force on the Bonded Tendon (부착식 텐던의 유효 긴장력 평가를 위한 최적의 매개변수 결정에 관한 연구)

  • The bonded tendon was adopted to the reactor building of some operating nuclear power plants in Korea and the assessment of the effective prestress force on the bonded tendon is being issued as an important pending problem for continuous operation beyond their design life. The sensitivity analysis of various parameters was carried out to evaluate the effective prestress force using the system identification technique and the optimal parameters were determined for SI technique in this study. The 1/5 scaled post-tensioned concrete beams with the bonded tendon type were manufactured and in order to investigate the relationship of the natural frequency and the displacement to the effective prestress force, impact test, SIMO sine sweep test and bending test using the optical fiber sensor and the compact displacement transducer were carried out. As a result of tests, both the natural frequency and the displacement show the good relationship with the effective prestress force and both parameters are available for the SI technique to estimate the effective prestress force.

A comparative study Program Outcome between nursing college students before and After Integrated Clinical Practice (간호대학생의 통합임상실습 전·후 학습성과의 차이)

  • This study was attempted to be used as basic data for efficient clinical practice education operation by identifying differences in program outcomes before and after integrated clinical practice of nursing students. Data collection was conducted before and after 6 weeks of integrated clinical practice for 38 nursing college students from July 19 to September 10, 2021 with a single group pre- and post-design. The collected data were analyzed by frequency, Wilcoxon sign rank, and Pearson correlation using SPSS/Win 23.0. After the integrated clinical practice, 1st program outcomes(Care integration)(Z=-4.63, p<.001), 2nd(Core practice)(Z=-3.99, p<.001), 6th(Critical thinking)(Z=-3.60, p<.001) and 11th(Research practice)(Z=-2.76, p=.005) increased significantly compared to before practice. Since integrated clinical practice was found to be an effective practice to improve program outcomes, it is suggested to actively utilize it as a strategy for improving program outcomes.

  • The AR/VR-based education and training system is expected to contribute greatly to accident prevention and budget reduction as well as practical training effects similar to the battlefield environment. Research to use AR/VR for learning is ongoing, and technology can be improved without experiencing failures that can occur in the real world. Major advanced countries in defense recognized the advantages of AR/VR technology early on, and developed and utilized systems using them in various fields, from mastery of individual weapon system operation to comprehensive combat training systems, war history education, and post-traumatic stress treatment. Therefore, the purpose of this study is to examine the cases of AR/VR application education and training in advanced defense countries and to draw implications for the South Korean military.
