양전자 방출 단층촬영에서 폐암으로 의심되었던 고립 폐 결절 형태의 폐흡충증 1예 (A Case of Pulmonary Paragonimiasis Presented as Solitary Pulmonary Nodule and Suspected as Lung Cancer on 18F-Fluorodeoxyglucose Positron Emission Tomography)

  • 폐흡충증은 폐흡충(Paragonimus westermani)의 중간 숙주인 민물 게, 민물 가재 등의 섭취를 통해 인체에 감염되는 기생충 질환으로, 기흉, 흉수, 낭성 종괴 등 다양한 임상 양상을 보인다. 고립 폐 결절로 형태로 발현하는 경우에는, 폐결핵 및 폐암 등과의 구분이 중요하다. 양전자 방출 단층촬영 검사가 감별 진단에 도움이 되지만, 폐결핵 및 폐 히스토플라스마종 등의 진균 감염에서의 위양성이 보고된 바 있다. 저자들은 48세 남자로 단순흉부촬영상 우연히 발견된 고립 폐 결절에 대한 평가 위해 내원한 환자에게서, 흉부 CT 및 FDG-PET에서 좌폐상엽의 폐문주위에 FDG 섭취가 증가된 고립 폐 결절 양상을 보여 폐암으로 의심하였으나, 폐엽 절제술을 시행 후 폐흡충증으로 진단하여 치료하였기에 문헌 고찰과 함께 보고하는 바이다.

동시계수 양전자 소멸 측정에 의한 양성자 조사된 Si 구조 특성 (Proton Irradiated Cz-Si by the Coincidence Doppler Broadening Positron Annihilation Spectroscopy)

  • 동시 계수 도플러 넓어짐 양전자 소멸 분광법으로 p형과 n형 실리콘 시료에 0, 4.0 MeV 에너지의 1.0, $2.0{\times}10^{13}$ protons/$cm^2$ 양성자 빔 조사에 의한 결함을 측정하여 실리콘 구조 특성에 대하여 조사하였다. 양전자와 전자의 쌍소멸로 발생하는 감마선 스펙트럼의 수리적 해석 방법인 S-변수와 W-변수를 사용하여, 시료의 구조 변화를 측정하였다. 본 연구에서 측정된 S-변수는 시료에 조사된 양성자의 빔 에너지에 따라 변하지 않고 거의 일정한 값을 보인 반면, 양성자 조사량의 변화에 따라 결함이 증가하였으며, 그 이유는 양성자 조사 에너지에 따른 Bragg 피크 때문에 에너지는 시료의 특정 깊이에 주로 결함을 형성하여 시료전체에는 결함으로 잘 나타나지 않기 때문으로 판단된다. 그리고 빔의 조사량에 따른 결함의 영향이 큰 것으로 나타났다.

The Role of $^{18}F$-Fluorodeoxyglucose Positron Emission Tomography in the Treatment of Brain Abscess

  • Objective : The purpose of this study was to evaluate whether $^{18}F$-fluorodeoxyglucose positron emission tomography (FOG-PET) can be used to assess the therapeutic response of brain abscess. Methods : A study was conducted on 10 consecutive patients with brain abscess, Magnetic resonance imaging (MRI) with diffuse-weighted imaging (DWI) was performed at 3 and 6 weeks after surgical treatment and intravenous antibiotics therapy and FOG-PET at 6 weeks after treatment. The extent of the abscess, signal changes on MRI, and FOG-PET standardized uptake values were analyzed and correlated with the response to therapy. Results : Aspiration or craniotomy with excision of the abscess followed by intravenous antibiotics for 6-8 weeks resulted in good recovery with no recurrence. In 10 patients, two had low signal intensity on the DWI; one had no uptake on FOG-PET imaging after 6 weeks antibiotics and discontinued intravenous treatment, but the other patient had diffuse, increased uptake on FOG-PET imaging after 6 weeks antibiotics and underwent an additional 2 weeks of intravenous antibiotics. The remaining eight patients had high signals on the DWI. Four had no uptake on FOG-PET imaging and the treatment period varied from 6 to 8 weeks (mean, 6.75 weeks). Among the other four patients, FOG was accumulated in a diffuse or local area corresponding to a high signal area within the DWI and 2 weeks of intravenous antibiotics was added. Conclusion : MRI plus FOG-PET improved the accuracy of assessing therapeutic responses to antibiotics treatment of brain abscess and aided in optimizing therapy.

The Unnecessity of Positron Emission Tomography Computed Tomography in the Etiologic Evaluation of Neurodevelopmental Delay in Craniosynostosis Patients

  • Background: In evaluation of craniosynostosis patients in terms of neurodevelopmental delay, positron emission tomography computed tomography (PET-CT) scan can be used to assess brain abnormalities through glucose metabolism. We aimed to determine the unnecessity of PET-CT in this study. Methods: Thirty-eight patients diagnosed with craniosynostosis who underwent distraction osteogenesis from October, 2010 to November, 2013 were reviewed. Magnetic resonance imaging (MRI) and PET-CT scan were carried out for evaluation of the brain structure and function, whereas X-ray and CT scan were taken for evaluation of the skull. Results: Nine patients reported abnormal MRI findings which were not significant, and five patients showed local problem on brain on PET-CT scan. No correlation was found among them. Conclusion: PET-CT evaluation of possible abnormal brain findings do not affect surgical planning or require additional therapy. Preoperative PET-CT scan is not the essential study to get any etiologic information of the disease consequences or to establish the treatment plan.
