경부 종류의 세침 흡인 세포학적 검사에 대한 임상적 고찰 (A Clinical Observation of Fine Needle Aspiration Cytology in the Neck Mass)

  • Neck mass is common neoplasms, but it poses a diagnostic dilemma for the physician. The differential diagnosis include neoplastic, inflammatory and developmental causes. The FNAC is one of the most valuable tests in the initial assessment and differential diagnosis of the neck mass. FNAC was performed with 267 cases of the neck mass, during the period from April, 1988 to October, 1990 at the department of General Surgery, Soon Chun Hyang. University Hospital. Thyroid lesions were excluded from this analysis. Final diagnosis was based on resection histology in 58 cases, and surgical specimens were compared with FNAC. The following results were obtoired ; 1) Of 267 cases, there we re 9 cases(3.4%) of congenital lesion, 74 cases(27.7%) of inflammatory lesion, 40 cases(15.0%) of benign tumor, 12 cases(4.5%) of primary malignant tumor, 37 cases(13.8%) of metastatic tumor, 75cases(28.1%) of reactive hyperplasia, 20 cases(7.5%) of unsatisfactory. In the pathologic classification, inflammatory lesion was the most common. 2) In the 58 cases of excisional biopsy, sensitivity 93.8%, specificity 95.2%, false positive 11.8%, false negative 2.4%, positive predictive value 88.2%, negative predictive value 97.6%, accuracy 94.8%. 3) The most common disease was the tuberculous lymphadenitis (53 cases, 19.8%). sensitivity 57.9%, specificity 100.0%, false positive 0.0%, false negative 17.0%, positive predictive value 100.0%, negative predictive value 83.0%, accuracy 86.2%.

요 세포 검사의 진단적 가치; 액상세포검사와 고식적 방법의 비교 (Diagnostic Value of Urine Cytology in 236 cases; a Comparison of Liquid-Based Preparation and Conventional Cytospin Method)

  • Urine cytology is an important screening tool for urinary tract neoplasms. Liquid-based preparation methods, such as $ThinPrep^{(R)}$, have been introduced for non-gynecological samples. We aimed to assess the diagnostic accuracy of liquid-based preparations in urine cytology by comparing the results of the conventional Cytospin preparation method for the same samples. A total of 236 cases subject to urine cytology were enrolled in this study from January 2005 to December 2005. All cases were subjected to cystoscopy and if a malignancy was suspected, a biopsy was performed. Urine cytology slides were made using the $ThinPrep^{(R)}$ preparation method and the conventional Cytospin and/or direct smear method from the individual samples. The results of urine cytology were compared with the final cystoscopic or histological diagnoses. We analyzed the sensitivity, specificity, positive predictive value, negative predictive value and accuracy of both cytology preparation methods. A total of 236 slides made using the liquid based method were satisfactory for slide quality, whereas 5 slides (2.1%) prepared by conventional methods were unsatisfactory because of air-drying, a thick smear, or a bloody or inflammatory background. The $ThinPrep^{(R)}$ method showed 53.1% sensitivity, 92.6% specificity, a 92,6% positive predictive value, a 94.1% negative predictive value and 85,6% accuracy, while the conventional method showed 51% sensitivity, 98.4% specificity, a 92.6% positive predictive value, a 98.4% negative predictive value and 88,6% accuracy. Although the diagnostic values were equivalent between the use of the two methods, the quality of the cytology slides and the time consumed during the microscopic examination for a diagnosis were superior for the $ThinPrep^{(R)}$ method than for the conventional method. In conclusion, our limited studies have shown that the use of the liquid based preparation method is beneficial to improve the quality of slides and reduce the duration for a microscopic examination, but did not show better sensitivity, accuracy and predictive values.

생체 간이식 공여자에서 비알코올성 지방간 질환의 진단에 있어서 초음파검사의 타당도 연구 (Validity of Ultrasonography in the Diagnosis of Non-alcoholic Fatty Liver Disease in Living Liver Donors)

  • 생체 간이식 공여자를 대상으로 시행되는 간생검 조직소견의 지방변성 정도와 복부초음파검사 결과를 분석하여, 지방간 질환의 진단에 있어서 초음파검사의 타당성을 알아보고자 하였다. 총 지방 함유량 10% 기준으로 범주화 하여 10% 이하를 음성, 10% 이상을 양성으로 하였을 때 초음파검사의 민감도는 64.6%이었고, 특이도는 68% 이었으며, 양성 예측도와 음성 예측도는 각각 76.8%, 54% 이었다. 초음파검사 결과 정상과 경증을 음성으로, 중등도를 양성으로 조작하였을 때 초음파검사의 민감도는 26.8% 이었고, 특이도는 100% 이었으며, 양성 예측도와 음성 예측도는 각각 100%, 45.5% 이었다. 총 지방 함유량 10% 이상을 상태변수(State variable)로 ROC curve 분석을 시행 하였을 때. 간/신장 명도비의 곡선하면적(Area under curve, AUC)은 0.859로 지방간을 예측하는데 좋은 지표로 나타났으며, 95% 신뢰구간(CI: 0.795~0.922)이 통계적으로 유의한 값을 보였다(p<0.001). 지방간진단에 있어서 복부초음파검사는 간생검 병리학적 결과를 예측하는데 높은 타당도를 보였다.

Statistical Methods for Comparing Predictive Values in Medical Diagnosis

  • Evaluating the performance of a binary diagnostic test, including artificial intelligence classification algorithms, involves measuring sensitivity, specificity, positive predictive value, and negative predictive value. Particularly when comparing the performance of two diagnostic tests applied on the same set of patients, these metrics are crucial for identifying the more accurate test. However, comparing predictive values presents statistical challenges because their denominators depend on the test outcomes, unlike the comparison of sensitivities and specificities. This paper reviews existing methods for comparing predictive values and proposes using the permutation test. The permutation test is an intuitive, non-parametric method suitable for datasets with small sample sizes. We demonstrate each method using a dataset from MRI and combined modality of mammography and ultrasound in diagnosing breast cancer.

Positive and negative predictive values by the TOC curve

  • Sensitivity and specificity are popular measures described by the receiver operating characteristic (ROC) curve. There are also two other measures such as the positive predictive value (PPV) and negative predictive value (NPV); however, the PPV and NPV cannot be represented by the ROC curve. Based on the total operating characteristic (TOC) curve suggested by Pontius and Si (International Journal of Geographical Information Science, 97, 570-583, 2014), explanatory methods are proposed to geometrically describe the PPV and NPV by the TOC curve. It is found that the PPV can be regarded as the slope of the right-angled triangle connecting the origin to a certain point on the TOC curve, while 1 - NPV can be represented as the slope of the right-angled triangle connecting a certain point to the top right corner of the TOC curve. When the neutral zone exists, the PPV and 1-NPV can be described as the slopes of two other right-angled triangles of the TOC curve. Therefore, both the PPV and NPV can be estimated using the TOC curve, whether or not the neutral zone is present.

Clinical predictors of therapeutic laparotomy in anterior abdominal stab injuries: a multicenter study from low-income institutions in Ethiopia

  • Purpose: Despite the high incidence of abdominal stab injuries, the rate of nontherapeutic laparotomies and the predictors of therapeutic laparotomies have rarely been studied in low-income settings. Methods: This multicenter retrospective study involved three of the largest academic medical centers in central Ethiopia. All patients who sustained an anterior abdominal stab injury and underwent exploratory laparotomy, regardless of the intraoperative findings, were included over the 3-year course of the study. Results: Of the 117 patients who underwent exploratory laparotomy, 35 patients (29.9%) underwent nontherapeutic laparotomies. Three factors predicted therapeutic laparotomy: hollow viscus evisceration (adjusted odds ratio [AOR], 5.77; 95% confidence interval [CI], 1.16-28.64; P=0.032), localized and generalized peritonitis (AOR, 4.77; 95% CI, 1.90-11.93; P=0.001), and white blood cell count ≥11,500/mm3 (AOR, 2.77; 95% CI, 1.002-7.650; P=0.049). The overall positive predictive value of the therapeutic predictors was 80.2%, while the negative predictive value of all predictor-negative patients was 58.1%. The predictors would have prevented 51.4% of the nontherapeutic laparotomies. Conclusions: Close to one-third of the patients had a nontherapeutic laparotomy. The clinical predictors of therapeutic laparotomy were shown to have a high positive predictive value despite a lower negative predictive value. Further prospective studies that involve all patients who sustain anterior abdominal stab injuries are needed to potentially improve on the negative predictive value of the predictors suggested by our study.

대퇴 외과 절삭 징후(cut-off sign) - 원판형 외측 반월상 연골 예에서 나타나는 새로운 단순 방사선 소견 - (Femoral Condyle Cut-off Sign - New Indirect Sign of Radiologic Finding in Knee with Discoid Lateral Meniscus -)

  • 목적 : 단순 방사선 사진에서 슬관절 외측 원판형 연골판을 진단하는데 도움이 되는 새로운 방사선 소견인 대퇴외과 절삭 징후(cut-off sign)를 관찰하여 이를 보고하고, 이의 진단적 의의를 특이도, 민감도, 양성 예측도, 음성 예측도와 함께 보고하고자 한다. 대상 및 방법 : 관절경 시술로 확진 된 50예의 완전 원판형 연골군과 50예의 정상 대조군을 대상으로 하였으며, Tunnel view에서 보이는 대퇴 외과와 내과의 내측부의 최장 길이를 측정하여 비율을 비교 분석하였다. 결과 : 원판형 연골군에서 대퇴 내과부 길이와 외과부 길이의 비율의 평균은 0.716 이였으며, 정상 대조군에서는 0.902 이었다. 이를 T-test로 분석했을 때, t-value는 -11.13 (p<0.0001)로 두 군간에 매 우 유의 한 차이를 나타내었으며, 또한 경계값(cut point)을 0.8로 하여 chi-square test로 분석했을 때 통계적으로 두 군간에 유의한 차이가 있었으며, 민감도 $76\%$, 특이도 $100\%$였으며, 양성 예측도 $100\%$, 음성 예측도 $80.6\%$로 나타났다. 절삭 징후는 육안적으로도 뚜렷한 차이를 보여 원판형 외측 반월상 연골을 단순 방사선 사진에서 진단할 수 있는 매우 좋은 간접적인 소견이라고 생각한다. 결론 : 대퇴 외과 절삭 징후는 단순 방사선 검사의 Tunnel view에서 원판형 연골을 진단할 수 있는 좋은 방사선 소견이며, $100\%$의 특이도와 $100\%$의 양성 예측도를 나타내는 우수한 진단소견으로 판단된다.

Preoperative prediction of the location of parotid gland tumors using radiographic anatomical landmarks

  • Introduction: The location of parotid gland tumors in the superficial or deep lobes can affect the time and difficulty of operations. Therefore, accurate preoperative evaluation of the tumor location is important for surgical outcomes. Materials and Methods: A total of 16 patients with parotid gland tumors and who underwent a parotidectomy between April 2003 and March 2011 were retrospectively reviewed in terms of demographic background, tumor location, surgical treatment, and treatment outcomes. Tumor location was estimated by four landmarks on contrast enhanced computerized tomography scans, which were Conn's arc, the facial nerve (FN) line, the Utrecht line, and the retromandibular vein. Tumor location was confirmed by relative position depending on the facial nerve during surgery. It was assumed positive since the tumor lies in the superficial lobe of the parotid gland, the sensitivity, specificity, positive predictive value, negative predictive value, and efficiency of each landmark were evaluated. Results: Our result revealed that the facial nerve line had a sensitivity of 83.3%, specificity of 100%, positive predictive value of 100%, negative predictive value of 71.4%, and efficiency of 87.5%. Some would be more efficient preoperative evaluation methods of the relationship of parotid gland tumors to the facial nerve than others. Conclusion: In our study, the FN line was found to be the most reliable analysis method.

The effect of metal artifacts on the identification of vertical root fractures using different fields of view in cone-beam computed tomography

  • Purpose: The aim of this study was to investigate the effects of metal artifacts on the accurate diagnosis of root fractures using cone-beam computed tomography (CBCT) images with large and small/limited fields of view (FOVs). Materials and Methods: Forty extracted molar and premolar teeth were collected. Access canals were made in all teeth using a rotary system. In half of the teeth, fractures were created by the application of mild pressure with a hammer. The teeth were then randomly put into a wax rim on an acryl base designed in the shape of a mandible. CBCT scans were obtained using a Newtom 5G system with FOVs of $18cm{\times}16cm$ and $6cm{\times}6cm$. A metal pin was then placed into each tooth, and CBCT imaging was again performed using the same fields of view. All scans were evaluated by two oral and maxillofacial radiologists. The specificity, sensitivity, positive predictive value, negative predictive value, and likelihood ratios (positive and negative) were calculated. Result: The maximum levels of sensitivity and specificity (100% and 100%, respectively) were observed in small-volume CBCT scans of teeth without pins. The highest negative predictive value was found in the small-volume group without pins, whereas the positive predictive value was 100% in all groups except the large-volume group with pins. Conclusion: The specificity of CBCT decreased with the presence of a pin in the large-volume group, but not in the small-volume group.

극상근 파열에 대한 Empty Can Test의 진단적 가치 - 관절경 소견과 비교 - (Empty Can Test for Diagnosis of Supraspinatus Tear - Compare with Arthroscopic Finding -)

  • Object: To determine the sensitivity, specificity, and positive and negative predictive values of an empty can test for diagnosing supraspinatus tendon tear. Methods: We reviewed 146 patients who have shoulder pain with limited active range of motion. We evaluated patients according to empty can test. Sensitivity, specificity, and positive and negative predictive values for supraspinatus test was estimated using arthroscopic evaluation. Results: A hundred and twenty-four of 127 cases with positive empty can test had supraspinatus tendon tear. Of 19 samples with negative empty can test, 15 had no supraspinatus tear. Sensitivity, specificity, and positive and negative predictive values for the empty can test were 97.6%, 83.3%, 97.6%, and 78.9%, respectively. Conclusions: Empty can test was found to have a high sensitivity and good positive predictive value in identifying the tear of rotator cuff tendon. We concluded that empty can test of the shoulder is a reliable diagnostic method which could be used for the diagnosis of rotator cuff tear.
