Altered Functions of Adrenoceptors in Splanchnic Vascular Beds in Portal Hypertensive Rat Model: Effect of Propranolol (문맥 고혈압 흰쥐에 있어서 내장혈관의 아드레나린성 수용체의 기능변동과 이에 대한 Propranolol의 효과)

  • Alterations in splanchnic circulatory hemodynamics along with reactivities to the alpha adrenoceptor agonists were assessed in association with the preventive effects of propranolol 10 days after portal ligation. Decreases in precapillary resistance (Ra) and postcapillary resistance (Rv) along with increases in mesenteric blood flow (MBF) and capillary pressure (CP) were observed in conjunction with an increment of splenic pulp pressure (SPP). Dose-dependent increase in Rv in response to noradrenaline, increases in Ra and RV to adrenaline, and increases in superior mesenteric arterial pressure (SMAP), Ra and Rv to phenylephrine observed in sham group were significantly attenuated by portal vein stenosis. In PPL-3 group (propranolol 3 mg/kg, i.p. three times daily for 10 days), MBF was significantly decreased in association with decrease in mesenteric venous pressure (MVP) when compared with those of protal ligated (PL) group, and decreased Ra and Rv in PL group were recovered toward the values of sham group. Likewise, in PPL-1 group (propranolol 5 mg/kg, i.p. once daily for 10 days), the pressor response of Rv to adrenaline was recovered up to the level of sham group. Thus, it is suggested that decreases in Ra and Rv in association with increases in MBF and CP may have a close relevance to the increased SPP, and the changes in circulatory hemodynamics and vascular reactivities were effectively reversed by longterm propranolol treatment. Based on these results, it is concluded that these changes observed in portal hypertension are closely related with the altered functions of the adrenoceptors in the splanchnic vascular beds.

Effect of Cimetidine on the Hepatic Blood flow -On the Basis of Pharmacokinetics of Indocvanine Green in Rats- (시메티딘이 간혈류량에 미치는 영향 - Rat에 있어서 Indocyanine Green의 체내 동태를 중심으로 -)

  • The influence of cimetidine pretreatment(100mg/kg, single i.p.) on the hepatic blood flow was investigated using pharmacokinetic parameters of indocyanine green(ICG) in the rat on the basis of hepacc perfusion-limited model. ICG(1mg/kg) was respectively administered via femoral and portal vein to the control and to the cimetidine-pretreated rats. The rate constant K12, K20 and the systemic clearance(CLt) of ICG were significantly(p<0.05) decreased ill the cimetidine-pretrea-to(B rats, but no significant diffirences were observed in hematocrit and liver weight. The biliary excretion rates of ICG were also decreased regardless of the route of administration in the cimetidine-pretreated rats. And also the hepatic blood flow in rats was decreased about $16\%$ by cimetidine. It may be concluded that the decreased hepatic blood flow with cimetidine mainly contributed to the decreased hepatic uptake and the decreased systemic clearance of ICG.

Hepatic Cell Membrane Changes of Rats in the Early Postmortem Period

  • To investigate the postmortem changes in hepatic cell membrane, the rats were sacrificed with cervical dislocation and kept in an incubator at $25^{\circ}C$, 70% of humidity for 12 hours. The biochemical experiments in postmortem were done at 2, 4, 8 and 12 hours. The degree of rigor mortis and algor mortis were increased with the time during 12 hours. The contents of hepatic malondialdehyde were rapidly increased ai 2 hours, and gradually decreased afterward. In histological findings, after 8 hours, the clotted blood was seen in central vein and sinusoids, and especially portal veins were dilated a1though the structure of hepatic lobules was preserved well. Furthermore, both in the histochemical and enzymatic examinations, membrane bounding alkaline phosphatase activities were gradually decreased with the time. In conclusion, the activity of membrane bounding alkaline phosphatase was linearly decreased with time in the early postmortem period and so it might be referred to the possibility fur the estimation of death time in the early postmortem period.

Renal Action of Furosemide in the Chiken (Furosemide의 닭의 신장(腎臟)에 대(對)한 작용(作用))

  • The renal action of furosemide was investigated in the chicken, a species which has poorly developed loops of Henle and only rudimentary counter-current system in the kidney. Furosemide was infused into a hindleg vein, which is known to lead to the peritubular capillaries, forming renal portal system. A dose of 0.03 mg/kg/20 min. furosemide elicited a profound diuresis with saluresis, limited only to the infused side. This action rests on the inhibition of sodium reabsorption in the tubules, as the GFR remained unchanged or even decreased. It is thus inferred that the action of furosemide on Henle's loop contributes to the overall diuretic action only a negligible degree.

  • The existence of oxytocin in the pituitary gland of chicken has been ascertained, but its physiological roles are still obscure. In the study the action of oxytocin on renal function of the chicken was investigated during water diuresis, utilizing clearance and the Sperber technique. The results obtained are summarized as follows: Oxytocin, like in many species of mammals, elicited a profound diuretic response in the chicken. Urine flow, excretion of electrolytes, as well as glomerular filteration rate increased, with intravenous infusion of $3{\sim}10\;m{\mu}/kg/min$. Oxtocin, infused into the renal portal circulation via hindleg vein in a dose of $3{\sim}13\;m{\mu}/kg/min$. elicited marked increase in urine flow, glomerular filteration rate and sodium excreted in the urine. The diuretic effect was more pronounced in the infused side. It is suggested that diuretic response to oxytocin in the chicken results from dual action of oxytocin: increase of GFR and inhibition of sodium reabsorption on the renal tubule. The possibility that oxytocin might act through some endogenous substances could be ruled out.

  • Effects of Ssang Wha Tang (SWT), a blended Chinese traditional medicine, on the pharmacokinetics of sulfobromophthalein (BSP) were studied in the rats. BSP was administered via portal vein to the control and the SWT-treated rats. The in vitro distribution of BSP to blood cells and the hemato-physiological conditions, liver weight, GOT. GPT activity were also examined. The systemic clearance $(CL_s)$ of BSP was increased with the administration of SWT, but no significant differences were observed in the liver weight and in vitro distribution of BSP to blood cells. These results suggest that the intrinsic clearance of free BSP of the liver is increased with the administration of SWT in the rats.

  • Portosystemic shunts in 3 Dogs with respiratory sign of cough and with dementia signs characterized by hypersensitivity, salivation, and seizure were diagnosed at veterinary teaching hospital of Seoul National University. In radiographs, microhepatica was observed. In abdominal ultrasonography, abnormal intrahepatic connections between the portal vein and the systemic vessels and tortuous vascularity were found. There was no complication such as ammonium urate urolith in kidney or urinary bladder, These dogs were treated with medicine and protein-restricted diet.

  • This study was performed to examine the clinical, serological, ultrasonographic and pathological findings in dogs with acute liver disease induced by tetrachlorethylene at 4 times of anthelminthic oral dose. The results obtained through this experiment could be summarized as follows: 1. The dogs administered with tetrachlorethylene, revealed decreased body weight, and showed lethargy and depression. 2. In serological findings, bilirubin values slightly increased, AST and ALT was decreased at 1∼3 days, and after that time increased according to the lapse of days, and revealed the highest at 5 days, and decreased to normal values at 6 days. 3. In ultrasonographic findings, branches of the portal vein were increased, the echodensity of the liver parenchyma was decreased at early stage, and increased at mid stage, and decreased at last stage. 4. In histopathological findings, necrosis of parenchymal cell, and perivascular hemorrhage were observed more severely at 6 days, as compared with 3 days. There results suggest that ultrasonographic examination is considered to be a more simple, rapid, non-invasive and useful diagnostic method for acute liver parenchymal lesion.

Solid and Papillary Epithelial Neoplasm of the Pancreas in Children (소아에서의 췌장의 고형유두상 상피성 종양)

  • Four children with solid and papillary epithelial neoplasm of the pancreas are reported. Three were girls. Mean age at operation was 12 years and 7 months (10-13 years). Clinical presentation included nausea, vomiting, and apalpable mass. One had hemoperitoneum due to tumor rupture. In two cases, tumors were in the body of the pancreas, and one the body and tail, and in one,the tail. Mean diameter of the tumors was 10.8 cm (8-15cm). Surgical procedures were distal pancreatectomy and splenectomy in 2 cases, distal pancreatectomy in one, and subtotal pancreatectomy and splenectomy in one. Mean follow-up period was 61 months (6-121 months). Three patients are still alive without any recurrence. However, in the one case of ruptured tumor, portal vein thrombosis and liver metastasis developed after subtotal pancreatectomy and splenectomy during the course of postoperative adjuvant chemotherapy.

Recent Trends in the Endoscopic Management of Variceal Bleeding in Children

  • Variceal bleeding results in significant morbidity and mortality in both children and adults. The guidelines for the management of variceal bleeding are well established in adults but not in children as there have been insufficient pediatric studies of this disorder. In addition, the adult guidelines for treatment of variceal bleeding cannot be applied directly to children as the etiology and natural course of this disease differs between children and adults. Examples of recommended treatments in children include endoscopic variceal ligation as secondary prophylaxis for biliary atresia whereas a meso-Rex shunt operation for extrahepatic portal vein obstruction. In this review, we discuss prophylaxis options and some technical aspects of endoscopic management for variceal bleeding in children.
