Bone resorption surrounding tooth root causes tooth loss in periodontitis patients. Osteoclast has bone resorption activity. Effects of Artemisia asiatica on bone resorption induced by periodontopathogens, Porphyromonas gingivalis and Treponema denticola, were examined using co-culture systems of mouse osteoblasts and bone marrow cells. Addition of A. asiatica ethanol extract to bacterial sonicate abolished bacteria-induced osteoclastogenesis. To determine inhibitory mechanism of A. asiatica against osteoclastogenesis, effects of A. asiatica on expressions of osteoclastogenesis-inducing factors such as receptor activator of NF-${\kappa}B$ ligand (RANKL), prostaglandin $E_2\;(PGE_2)$, interleukin (IL)-1, and tumor necrosis factor (TNF)-${\alpha}$, in osteoblasts were examined. A. asiatica suppressed expressions of RANKL, $PGE_2$, IL-$1{\beta}$, and TNF-${\alpha}$ increased by each bacterial sonicate. These results suggest inhibitory action of A. asiatica against osteoclastogenesis is associated with down-regulations of RANKL, $PGE_2$ IL-$1{\beta}$, and TNF-${\alpha}$ expressions.
It is becoming increasingly apparent that periodontitis consists of mixture of diseases, most of which respond favorably to traditional mechanical therapy. Among these variants of the disease, some appear to be associated with unusual microbial infections and defective host defenses. Many of these fail to respond to conventional treatment. The recognition that some forms of periodontitis are refractory to standard periodontal therapy has given rise to a new classification of peridontitis. A series of 1692 subgingival microbial samples sent to a diagnostic microbiology laboratory included 738 samples that could be identified as compatible with a clinical diagnosis of refractory or recurrent periodontitis. In descending order of prevalence the associated microbiota included Bacteroides forsythus(85%) ,Fusobacterium species(78%), Spirochetes(67%), Campylobacter rectus(64%), Porphyromonas gingivalis(59%), Peptostreptococcus micros(58%), motile rods(46%), Prevotella intermedia(33%), Eikenella corrodens(13%), Capnocytophaga species(12%) ,and Actinobacillus actinomycetemcomitans(6%). Antibiotic resistance to tetracycline, penicillin G, or metronidazole was particularly noticeable for Fusobacterium species, Capnocytophaga species, and Actinobacillus actinomycetemcomitans. It was largely absent for Campylobacter rectus. No antibiotic data were obtained for Porphyromonas gingivalis or Bacteroides forsythus, as these species were detected by immunofluorescence. The results indicate that a substantial number of microorganisms associated with refractory periodontitis are variably resistant to commonly-used antibiotics. Diagnostic microbiology must be considered an essential adjunct to the therapist faced with periodontal lesions refractory to conventional treatment.
치주질환의 원인균인 Porphyromonas gingivlais (P. gingivalis)의 병인인자중 하나인 gingipain 효소에 대한 억제활성을 갖는 유산균을 개발하고자 본 연구실에 보관중인 30여종의 유산균 배양상등액의 활성을 검색하였다. 그 결과 발효식품에서 분리, 동정한 Weisiella cibaria SPM402와 Lactobacillus paracasei SMP412가 P. gingivalis에 대한 항균효과, gingipain 유전자 발현억제 및 gingipain 효소활성 억제능이 확인되었다. W. cibaria SPM402 의 배양 상등액 WC402는 10 mg/mL에서 kgp 유전자는 0.71±0.02배 발현되었고, rgpB 유전자는 0.71±0.14배 발현되 약간의 발현억제 효과가 확인되었다. 반면 L. paracasei SMP412의 배양상등액 LP412는 10 mg/mL 농도에서 kgp 유전자는 0.19±0.08배, rgpA 유전자는 0.09±0.02배, rgpB 유전자는 0.24±0.03배 발현되어 LP412의 뛰어난 gingipain 유전자 발현억제 효과를 확인하였다. 10 mg/mL농도에서, Kgp 효소 활성은 WC 402에 의해 78.65±3.58% (CAG), 82.45±1.22% (CFG), LP412에 의해 80.71±2.11% (CAG), 85.81±0.05% (CFG) 억제되었다. 동일한 농도에서 Rgp 활성은 WC402에 의해 78.6±1.01% (CAG), 86.78±0.47% (CFG), LP412에 의해 82.93 ±1.26% (CAG), 88.81±0.36% (CFG) 억제되었다. 이상의 결과들을 통해 WC402와 LP412는 P. gingivalis에 대한 항균효과 뿐 아니라 병인인자 gingipain을 효과적으로 억제하는 기능성 유산균임을 확인하였다.
Purpose: Although several reports have described the relationship between periodontal disease and cardiovascular disease, information about the association between periodontal disease and the progression of degenerative aortic stenosis (AS) is lacking. Therefore, we performed a retrospective, single-center, pilot study to provide insight into this potential association. Methods: Data from 45 consecutive patients (19 men; median age, 83 years) with mild or moderate degenerative aortic stenosis were analyzed for a mean observation period of 3.3±1.9 years. The total amount of Aggregatibacter actinomycetemcomitans and Porphyromonas gingivalis and titers of serum immunoglobulin G (IgG) against periodontal bacteria and high-sensitivity C-reactive protein (hs-CRP) were evaluated. Aortic valve area (AVA), maximal velocity (Vmax), mean pressure gradient (mean PG), and the Doppler velocity index (DVI) were evaluated. The change in each parameter per year ([ParameterLATEST-ParameterBASELINE]/Follow-up Years) was calculated from the retrospective follow-up echocardiographic data (baseline vs. the most recently collected data [latest]). Results: No correlation was found between the concentration of periodontopathic bacteria in the saliva and AS status/progression. The anti-P. gingivalis antibody titer in the serum showed a significant positive correlation with AVA and DVI. Additionally, there was a negative correlation between the anti-P. gingivalis IgG antibody titer and mean PG. The hs-CRP concentration showed positive correlations with Vmax and mean PG. Meanwhile, a negative correlation was observed between the anti-P. gingivalis IgG antibody titer and ΔAVA/year and Δmean PG/year. The hs-CRP concentration showed positive correlations with Vmax and mean PG, and it was significantly higher in patients with rapid aortic stenosis progression (ΔAVA/year <-0.1) than in their counterparts. Conclusions: Our results suggest that periodontopathic bacteria such as A. actinomycetemcomitans and P. gingivalis are not directly related to the status/progression of degenerative AS. However, inflammation and a lower immune response may be associated with disease progression.

Purpose: This study was undertaken to evaluate the clinical and microbiological effects of an erythritol powder air-polishing device (EPAP) as a supplement to scaling and root planing (SRP) therapy in patients with moderate chronic periodontitis. Methods: Clinical and microbiological evaluations were performed at 21 sites treated with SRP (control) and 21 sites treated with SRP+EPAP (test). All examinations were performed before treatment, 1 month after treatment, and 3 months after treatment. Results: There were no significant clinical differences between the test group and the control group. Microbiological analysis revealed that the relative expression level of Porphyromonas gingivalis was significantly lower in the test group than in the control group at 1 month after treatment. Clinical and microbiological results showed improvements at 1 month compared to baseline; in contrast, the results at 3 months after treatment were worse than those at 1 month after treatment. Conclusions: In this study, both SRP and SRP+EPAP were clinically and microbiologically effective as non-surgical periodontal treatments. In particular, the SRP+EPAP group showed an antimicrobial effect on P. gingivalis, a keystone bacterium associated with the onset of chronic periodontitis, in a short-term period. Periodic periodontal therapy, at intervals of at least every 3 months, is important for sustaining the microbiological effects of this treatment.
Black-pigmented bacteria have been implicated in the endodontic infections. This group of microorganisms includes Porphyromonas endodontalis, Porphyromonas gingivalis, Prevotella intermedia, and Prevotella nigrescens. The organisms display a wide variety of virulence factors that may be pertinent to acute endodontic infections. The aim of this study was to identify P. endodontalis, P. gingivalis, P. intermedia, and P. nigrescens by using special potency disk test, filter paper spot test, 165 rRNA gene-directed PCR, and API 32A. Microbial samples were collected from root canals of 33 intact teeth with necrotic pulp and/or apical periodontitis. Conventional laboratory methods were used for identification of the strains of black pigmented bacteria. Eighteen of 33 samples were positive for the growth of black-pigmented bacteria Five colonies were cultured from each pure cultured colonies from Brucella agar plate. Seventy seven colonies were positive for the growth of black-pigmented bacteria. Thirty three of 77(42.6%) were identifed as P. nigrescens, 10 of 77(12.9%)were P. gingivalis, 6 of 77(7.8%) were P. endodontalis, 10 of 77(12.9%) were P. intermedia. On the contrary the reference strains of P. nigrescens, experimental strains of P nigrescens was sensitive to kanamycin in special potency disk test. 165 rRNA gene PCR and API test after rapid presumptative identification methods, such as special potency disk test and filter paper spot test, would be accurate detection methods for black-pigemented bacteria.
Periodontitis is a chronic inflammatory disease and gums pull away from the teeth and form spaces. The main goal of treatment is to control the infection and anti-inflammation. Eucommia ulmoides Oliv. Bark (EU) is a traditional Korean herbal used for the treatment of arthritis and hypertension. In the present study, we investigated the anti-inflammatory properties of EU in Porphyromonas gingivalis-LPS (LPS-PG) induced YD-38 epithelial cells. We observed significant inhibition of nitric oxide (NO) production and the expression of interleukin-8 (IL-8), interleukin-6 (IL-6), tumor necrosis factor alpha ($TNF-{\alpha}$), cyclooxygenase-2 (COX-2) in LPS-PG-induced YD-38 cells. These results support that EU has an anti-inflammatory effects for the treatment of periodontitis.
치주질환과 구취를 유발시키는 중요한 원인균인 P. gingivalis에 대한 피톤치드의 항균효과와 항균작용은 이미 연구되어 있으나, 정상인의 구강상주균에 대한 연구는 아직 희귀한 편이다. 이에 본 연구에서는 건강한 정상인의 타액에 편백 피톤치드를 첨가하였을 때 사멸되지 않고 생존하는 타액세균을 분리하여 구강 유해균과 함께 배양한 후 구강 유해균에 대한 생존 타액세균의 억제효과를 파악함으로써 향후 프로바이오틱으로 작용할 수 있는 구강상주균의 균종을 동정하여 다음과 같은 결론을 얻었다. 1. 정상인의 전타액에 1% 피톤치드를 적용하였을 때 잔존 생균수는 감소하는 경향을 보였다. 2. 피톤치드 적용 후 생존한 주 세균종은 S. thermophilus (53%)로 나타났다. 3. 피톤치드 적용 후 생존한 균을 P. gingivalis A7A1-28과 P. gingivalis W83에 교차배양한 결과 생존균의 대부분(72.5%) 이 P. gingivalis A7A1-28과 P. gingivalis W83의 성장을 억제하였다. 4. 생존 S. thermophilus의 85.8%, S. sanguinis는 75.8%가 P. gingivalis 를 억제하는 것으로 나타났다. 이상의 결과로 미루어, P. gingivalis 등 구강 내 유해균을 직접 억제할 수 있는 것으로 알려진 피톤치드로 처리할 경우 피톤치드에 생존하는 구강상주균이 P. gingivalis에 대해 부가적으로 억제작용을 할 수 있기 때문에 피톤치드의 사용은 치주질환을 예방하고, 그 결과 치주질환 및 구취환자의 구강 환경을 크게 개선할 수 있을 것으로 생각된다.
Chronic/cyclic neutropenia, leukocyte adhesion deficiency syndrome, Papillon-$Lef{\grave{e}}vre$ syndrome, and $Ch{\grave{e}}diak$-Higashi syndrome are associated with severe periodontitis, suggesting the importance of neutrophils in the maintenance of periodontal health. Various Toll-like receptor (TLR) ligands are known to stimulate neutrophil function, including FcR-mediated phagocytosis. In the present study, the effect of TLR2 activation on the non-opsonic phagocytosis of oral bacteria and concomitant production of reactive oxygen species (ROS) by human neutrophils was evaluated. Neutrophils isolated from peripheral blood were incubated with Streptococcus sanguinis or Porphyromonas gingivalis in the presence of various concentrations of $Pam_3CSK_4$, a synthetic TLR2 ligand, and analyzed for phagocytosis and ROS production by flow cytometry and chemiluminescence, respectively. $Pam_3CSK_4$ significantly increased the phagocytosis of both bacterial species in a dose-dependent manner. However, the enhancing effect was greater for S. sanguinis than for P. gingivalis. $Pam_3CSK_4$ alone induced ROS production in neutrophils and also increased concomitant ROS production induced by bacteria. Interestingly, incubation with P. gingivalis and $Pam_3CSK_4$ decreased the amounts of ROS, as compared to $Pam_3CSK_4$ alone, indicating the possibility that P. gingivalis survives within neutrophils. However, neutrophils efficiently killed phagocytosed bacteria of both species despite the absence of $Pam_3CSK_4$. Although P. gingivalis is poorly phagocytosed even by the TLR2-activated neutrophils, TLR2 activation of neutrophils may help to reduce the colonization of P. gingivalis by efficiently eliminating S. sanguinis, an early colonizer, in subgingival biofilm.
Dental pulp infection is most commonly caused by extensive dental caries, and some bacterial species invade root canals; bacterial components and products are thought to be associated with the pathogenesis of periapical periodontitis. A principle driving force behind pulpal disease response appears to lie in the host immune system's to bacteria and their products. We examined the production of interleukin $1{\beta}$ (IL-$1{\beta}$) and tumor necrosis factor ${\alpha}$(TNF-${\alpha}$) from human peripheral mononuclear cells, lymphocytes and monocytes stimulated by heat-killed Acitnobacillus actinomycetemcomitans (ATCC 29523), Porphyromonas gingivalis (ATCC 33277) and Prevotella intermedia (ATCC 25611), and also by their sonicated bacterial extracts (SBE), respectively. The effects of three strains of heat-killed bacteria and their SBEs on the morphology of cultured blood cell lines HL-60 (KCLB 10240) and J774A.1 (KCLB 40067) were observed under the inverted microscope. Ultrastructural changes of J774A.1 exposed to heat-killed P. intermedia and its SBE were investigated using transmission electron microscopy. Production of IL-$1{\beta}$ was reduced in human peripheral mononuclear cells after stimulation by sonic bacterial extracts of A. actinomycetemcomitans, P. gingivalis, and P. intermedia. Heat-killed and sonic extract of P. gingivalis inhibited the production of TNF-${\alpha}$ in peripheral mononuclear cells. Production of TNF-${\alpha}$ was inhibited in peripheral monocytes after stimulation by sonic extracts of A. actinomycetemcomitans, P. gingivalis, and P. intermedia. HL-60 and J 774A.1 cells showed granular degeneration after treatment with heat-killed and sonic extracts of A. actinomycetemcomitans, P. gingivalis, and P. intermedia Chromatin margination and shrinkage were observed in 774A.1 treated with heat-killed P. intermedia. Cell wall structure and organelles were destroyed and vacuoles were formed in cytoplasm in J774A.1 treated with P. intermedia sonic extract. These results suggest that A actinomycetemcomitans, P gingivalis and P intermedia may have an important role in the formation and progression of pulpal diseases via both modulation of production of IL-$1{\beta}$ and TNF-${\alpha}$ from blood mononuclear cells and cytopathic effects.
