치과용 생체세라믹스의 강화

  • 인체 경조직인 뼈와 치아는 콜라겐 단백질과 인산칼슘 무기질 성분으로 구성된 최적의 복합체라 볼 수 있다. 이러한 인산칼슘 무기질 성분은 결정학적, 화학적으로 hydroxyapatite (HA, Ca10(PO4)6(OH)2)와 거의 유사하여 높은 생체활성과 골전도성을 보이는 것으로 알려져 있다. 또한, hydroxyapatite의 고온 분해 산물로 볼 수 있는 tricalcuim phosphate(TCP, Ca3(PO4)2)는 체내에 이식시 체액에 용해되어 신생골을 유도하는 생체흡수성 세라믹스로 알려져있다. 이러한 HA와 TCP는 우수한 생체친화성에도 불구하고 기계적 특성이 낮아, non-load bearing 부위에 적용이 되어 다공질의 골수복재 용도로 활용되거나, load-bearing부위의 적용을 위한 금속 implant 등의 생체친화성 보완을 위한 표면 코팅재로 사용되고 있는 실정이다. 본 발표에서는 생체특성이 매우 우수한 인산칼슘 생체세라믹스의 load-bearing part 적용을 위하여 강도 및 파괴인성의 기계적 특성을 증진시킨 사례를 살펴보고, non-load bearing part에 실제 사용되는 인산칼슘 생체세라믹스의 다공질(porous) 골수복재(bone graft)의 역학 특성 증진을 위한 아이디어와 함께, 역학 특성이 매우 뛰어난 지르코니아 생체세라믹스의 강화-소결법에 대하여 소개하고자 한다.

A STUDY ON SHEAR BOND STRENGTH OF INTERFACE BETWEEN BONE AND TITANIUM PLASMA SPRAYED IMZ IMPLANT IN RABBITS (가토에 이식된 Titanium plasma Sprayed IMZ 임프란트와 골의 계면 접촉 양상에 따른 결합력에 관한 연구)

  • In recent years immediate implantation has been tried by a few clinicians. This study placed IMZ implants in the rabbit femur with and without bony defects around the implant for simulating fresh extraction site. And one group with bony defects used porous hydroxyapatite ganules(HA) to fill if and the other group left the bony defects around the implant. The purpose of this study was to compare the shear bond strength and the bony contact and formation around the implant. Fifteen rabbits were divided into three groups and placed 10 IMZ implants to each group. Implant sites were surgically prepared with IMZ drills kit and implants were placed(Control), artificial bony defect was created with Apaceram drills kit around the implant sites and implants were placed(Experimental I), bony defect was filled with porous hydroxyapatite granules(Experimental II). Thereafter, rabbits were sacrificed at 8th week and specimens were prepared and pushout tested for shear bond strength of bone-implant interface immediately. Undecalcified and decalcified specimens were prepared with Vilanueva and hematoxylin-eosin stain for light microscopic finding. The results of this study were as follows. 1. In the control group, mean shear strength of bone-implant interface was $2.614{\pm}0.680$ MPa, experimental I was $0.664{\pm}0.322$ MPa, and experimental II was $2.281{\pm}0.606$ MPa. There was significant difference between control and experimental I, between experimental I and experimental II, but did not show significant difference between control and experimental II statistically. 2. In the bony formation surrounding IMZ implant of the three groups, that of cortical bone is more advanced than cancellous bone area. 3. In the histological findings of undecalcified specimens, control and experimental II showed more than 50% of bony or osteoid formation at the bony-implant interface. 4. In the histological findings of undecalcified specimens, experimental I showed less than 50% of bony or osteoid formation at the interface, and observed partial bony defect in the coronal zone. 5. In the experimental II group, were observed direct bony contact to hydroxyapatite granules, and infiltration of a few giant cells. 6. No inflammatory responses were seen around the titanium implants and the hydroxyapatite granules.

The Effect of Silk Fibroin/Nano-hydroxyapatite/Corn Starch Composite Porous Scaffold on Bone Regeneration in the Rabbit Calvarial Defect Model (가토 두개골 결손 모델에서 실크단백과 나노하이드록시아파타이트, 옥수수 녹말 복합물을 이용한 골 이식재 개발)

  • Purpose: This study evaluated the capability of bone formation with silk fibroin/nano-hydroxyapatite/corn starch composite scaffold as a bone defect replacement matrix when grafted in a calvarial bone defect of rabbits $in$ $vivo$. Methods: Ten New Zealand white rabbits were used for this study and bilateral round-shaped defects were formed in the parietal bone (diameter: 8.0 mm). The silk fibroin 10% nano-hydroxyapatite/30% corn starch/60% composite scaffold was grafted into the right parietal bone (experimental group). The left side (control group) was grafted with a nano-hydroxyapatite (30%)/corn starch (70%) scaffold. The animals were sacrificed at 4 weeks and 8 weeks. A micro-computerized tomography (${\mu}CT$) of each specimen was taken. Subsequently, the specimens were decalcified and stained with Masson's trichrome for histological and histomorphometric analysis. Results: The average ${\mu}CT$ and histomorphometric measures of bone formation were higher in the control group than in the experimental group at 4 weeks and 8 weeks after surgery though not statistically significant ($P$ >0.05). Conclusion: The rabbit calvarial defect was not successfully repaired by silk fibroin/nano-hydroxyapatite/corn starch composite scaffold and may have been due to an inflammatory reaction caused by silk powder. In the future, the development of composite bone graft material based on various components should be performed with caution.

Fabrication and characterization of porous hydroxyapatite scaffolds with PMMA addition using tertiary-butyl alcohol based freeze casting method (삼차부틸알코올 기반 동결주조 공정을 이용한 PMMA 첨가 다공질 수산화아파타이트 지지체의 제조 및 특성 평가)

  • In order to prepare porous scaffolds capable of pore control, PMMA powder serving as a pore-forming agent was added to HA powder to synthesize a slurry containing TBA as a solvent. And then, porous HA scaffolds where pillarshaped pore channels interconnected with each other were fabricated by freeze-casting and sintering. The crystal structure of the HA scaffolds according to the addition amount of PMMA powder was measured by XRD and the surface and inner cross section of the scaffolds were analyzed through SEM. It was found that removal of PMMA during sintering affects the internal structure of the scaffolds and the crystallinity of the HA powder. Furthermore, through evaluating the physical and mechanical properties of the scaffolds, it was confirmed that the porosity, pore size and compressive strength can be controlled by controlling the addition amount of the pore-forming agent. It was also found that the HA scaffolds produced in this study were similar in structure and properties to the natural cancellous bone. This suggests that porous HA scaffolds with PMMA can be used as an alternative to autogenous bone for tissue engineering as an artificial bone scaffold.

Fabrication and Characterization of Porous TCP coated Al2O3 Scaffold by Polymeric Sponge Method

  • A porous $Al_2O_3$, scaffold coated with tricalcium phosphate(TCP) was fabricated by replica method using polyurethane(PU) foam as a fugitive material. Successive coatings of $Al_2O_3$ and hydroxyapatite(HAp) were applied via dip coating onto polyurethane foam, which has a slender and well interconnected network. A porous structure was obtained after sequentially burning out the foam and then sintering at $1500^{\circ}C$. The HAp phase was changed to TCP phase at high temperature. The scaffold showed excellent interconnected porosity with pore sizes ranging from $300{\sim}700{\mu}m$ in diameter. The inherent well interconnected structural feature of PU foam remained intact in the fabricated porous scaffold, where the PU foam material was entirely replaced by $Al_2O_3$ and TCP through a consecutive layering process. Thickness of the $Al_2O_3$ base and the TCP coating was about $7{\sim}10{\mu}m$ each. The TCP coating was homogeneously dispersed on the surface of the $Al_2O_3$ scaffold.

  • The plasma-spray technique is currently the most frequently used method to produce calcium phosphate coatings. Hydroxyapatite(HAp), one form of calcium phosphate, is preferred by its ability to form a direct bond with living bone, resulting in improvements of implant fixation and faster bone healing. Recently, concerns have been raised regarding the viable use and long-term stability of plasma-spray HAp coatings due to its nature of comparatively thick, porous, and poor bonding strength to metal implants. Thin layers (maximum of few microns) of calcium phosphate were formed by an e-beam evaporation with and without ion bombardments. The Ca/P ration of film was controlled by either using the evaporants having the different ration of Ca/P with addition of CaO, or adjusting the ion beam assist current. The Ca/P ration had great effects on the structure formation after heat treatment and the dissolution bahavior. The calcium phosphate films produced by IBAD exhibited high adhesion strength.

Evaluation of Bioactivity of Ti-6Al-7Nb Alloys with Various Hydrothermal Treatment Times (열수처리 시간에 따른 Ti-6Al-7Nh 합금의 생체활성 평가)

  • This study was to investigate whether the bioactivity of the anodized and hydrothermally treated Ti-6Al-7Nb alloy were affected by the time of hydrothermal treatment. Anodizing was performed at current density 30 $mA/cm^2$ up to 300 V in electrolyte solutions containing $DL-{\alpha}-glycerophosphate$ disodium salt hydrate $(DL-{\alpha}-GP)$ and calcium acetate (CA). Hydrothermal treatment was done at $300^{\circ}C$ for 30 min, 1 hr, 2 hrs, and 4 hrs to produce a thin film layer of hydroxyapatite (HA). The bioactivity was evaluated from HA formation on the surfaces in a Hanks' solution with pH 7.4 at $36.5^{\circ}C$ for 10, 20, and 30 days. Anodic oxide films were porous with pore size of $1\sim4{\mu}m\;and\;3\sim4{\mu}m$ thickness. The anodic oxide films composed with strong anatase peak with presence of rutile peak, and showed the increase in intensity of anatase peak after hydrothermal treatment. It was shown that the intensity of anatase peak increased with increasing the time of hydrothermal treatment but was no difference in rutile peak. The corrosion voltage was the highest in the group of hydrothermal treatment for 2 hrs (Ecorr: -338.6 mV). The bioactivity in Hank's solution was accelerated with increasing the time of hydrothermal treatment.
