《상한론(傷寒論)》에서 환제의 크기와 복용방법 (The Size and Administration Method of Pill preparation in Treatise on Cold Damage Diseases)

  • Objects : In this study, I aimed to determine the size and administration Method of pills preparation in Treatise on Cold Damage Diseases. Methods : I investigated the Treatise on Cold Damage Diseases, Newly Revised Materia Medica, Variorum of the Classic of Materia Medica, The Korean Herbal Pharmacopoeia, and recent studies. Results : There were four kinds of pills preparation. Each size of pill preparation was equal to the Odongja, Tanwhan, Yolk, and quarter of daily dosage of Decoction. One pill of Tanwhan was equivalent to 16 pills of Odongja, and one Yolk was equal to 40 pills of Odongja. The Diameter of Odongja was 6.5 mm, Tanwhan was 16.37897 mm, and Yolk was 22.11484 mm. Jeodangwhan was equal to one quarter daily dosage of Jeodangtang. Maximum size to swallow with water was Odongja and daily dosage, which was 16 pills, includes one square-inch-spoon of powder preparation. When we made the pill preparation, it is difficult to make 16 pills using one square-inch-spoon of powder. Therefore, Zhang Zhongjing made one dosage started from 10 pills to 20 pills to find out optimum dose. Due to the difficulty of making precise size of pills and considerable amount of daily dosage, there was no smaller size than Odongja. When the size of Pill preparation was larger than Odongja, it was ground or boiled slightly to make suspensions. Conclusions : These results suggest that the sizes of Pill preparation in the Treatise on Cold Damage Diseases were true compared with reality.

혼합 양파환이 과체중 및 비만 남성의 혈중 지질 농도에 미치는 영향 (Effect of Mixed Onion Pills on Plasma Triglyceride and Total Cholesterol Levels in Overweight or Obese Men)

  • The effect of mixed onion pills on body composition, blood pressure, and concentrations of blood glucose and lipids were studied in 10 men ($38.11{\pm}2.82$ years old). The mixed onion pills was produced by mixing quercetin, onion, mulberry leaves and buckwheat. Ten volunteers (male, BMI 23.0~35.4 kg/$m^2$) were chosen for this study. The volunteers consumed 1.2 g of mixed onion pills (containing about 30 mg of quercetin) per day for 2 months. The administration of the mixed onion pills did not produce any weight and BMI changes in the subjects. The systolic blood pressure and the level of blood glucose were slightly lowered by the supplementation of the mixed onion pills. After 2 months both the concentration of plasma triglycerides and total cholesterol of the subjects were lowered by 12.4% and 5.0%, respectively (triglyceride; pre $251.67{\pm}45.63$ mg/dL vs. post $220.44{\pm}44.67$ mg/dL and total cholesterol; pre $207.25{\pm}10.39$ mg/dL vs. post $196.63{\pm}10.47$ mg/dL, p<0.05). These results suggest that the mixed onion pills may possibly lower the level of plasma lipids in overweight or obese men (BMI greater than 23 kg/$m^2$).

미혼모의 응급피임약에 대한 지식과 태도 (Unmarried Mother's Knowledge and Attitudes toward Emergency Contraceptive Pills)

  • Purpose: This study was conducted to identify relationships among knowledge and attitudes of unmarried mothers toward emergency contraceptive pills. Methods: Data were collected through structured questionnaires from 135 unmarried mothers enrolled in 7 single mothers' facilities nationwide. Data were analyzed using the SPSS/WIN 17.0 program for descriptive statistics, t-test, ANOVA, Scheffe-test, and Pearson correlation coefficients. Results: For knowledge about emergency contraceptive pills, there were significant differences among who live with her before pregnancy, experience of past pregnancies, state of present pregnancy and preparation in using contraceptives. For attitude toward emergency contraceptive pills, there were significant differences according to age, education level and religion. There were significant positive relationships between knowledge and attitudes toward emergency contraceptive pills. Conclusion: The results of this study suggest that unmarried women should be better informed about emergency contraceptive pills, and reassured about their safety. Efforts are needed to disseminate up-to-date information to experts in sex education including nurses.

Associations of the Neighborhood Environment With Substance Use: A Cross-sectional Investigation Among Patients in Compulsory Drug Detention Centers in Thailand

  • Objectives: To identify the associations of characteristics of the neighborhood environment with substance abuse among clients receiving treatment for drug abuse in Thailand. Methods: A cross-sectional study was conducted of 1128 drug addicts from 28 neighborhoods who were receiving treatment at all 7 compulsory drug detention centers in Thailand. A trained interviewer conducted structured interviews with the subjects about substance use and the perceived neighborhood environment in their community. A multilevel logistic regression model was applied to estimate the effects of the neighborhood environment on substance use. Results: The majority of participants, 53.8% only used methamphetamine pills, 31.3% used other illicit drugs as well as methamphetamine pills, and 14.9% used an illicit drug other than methamphetamine. Three neighborhood characteristics were associated with substance use. A 1-unit increase in the perceived neighborhood cohesion score was associated with a 15% reduction in methamphetamine pill use and an 11% reduction of the use of both methamphetamine pills and another illicit drug. Conversely, a 1-unit increase in perceived neighborhood crime predicted 19 and 14% increases in the use of methamphetamine pills and the use of both methamphetamine pills and another illicit drug, respectively. In addition, a 1-unit increase in the scores for stigma surrounding addiction corresponded to a 25% increase of the use of methamphetamine pills and a 12% increase in the use of both methamphetamine pills and another illicit drug. Conclusions: Substance use among drug addicts was influenced by characteristics of the neighborhood environment. Therefore, prevention and intervention strategies should be designed based on a consideration of the impact of neighborhood context on substance use behaviors.

딥러닝을 활용한 알약인식 및 복용관리 시스템 (Systems for Pill Recognition and Medication Management using Deep Learning )

  • 알약 구매 후 조제약 봉투나 포장지를 잃어버린 경우 약의 효능을 알기 어렵고 많은 사람들이 시중에서 판매하는 약을 구매하여 복용한 후 보관 시 용도를 구분해서 보관하지 않는 경우가 많다. 또한 알약 부작용에 대한 정보의 접근성이 낮아 약을 오남용하는 상황이 발생하기도 한다. 기존의 알약 정보를 검색하여 정보를 알려 주는 대부분의 서비스 및 어플리케이션들은 사용자가 알약의 상세정보를 직접 입력하거나 선택해야 하는 번거로움이 존재한다. 본 논문에서는 이러한 불편한 상황들은 개선하고 보다 나은 서비스를 제공하기 위해 약학정보원에서 제공하는 알약 22,000종의 사진에 대해 제형과 색상을 학습시킨 딥러닝 모델을 구축하였다. 구축된 정보를 활용하여 촬영된 사진으로 알약을 검색하고 알약 정보를 알려주며 사용자의 약 복용을 관리해주는 기능을 갖는 시스템을 개발하였다.

로즈마리를 첨가만 유지 코팅 생약제 환의 품질안정성 (Quality Stability of the Herb Pill Coated with Edible Oils Containing Rosemary Essential Oil)

  • Quality stability of the herb pill coated with edible oils containing rosemary was investigated. Herb pills were made of herb powders such as Panax ginseng, Cinnamomum cassia, Lycium chinense, Zyzyphus jujuba and Zingiber officinale. Rapeseed oil and lubriol were used as edible coating oil. After herb pills coated with edible oils with or without rosemary were stored at $40^{\circ}C$ for 180 days, the microbial viable cell counts and peroxide values(POV) of the herb pill were investigated. After 180 day storage, POVs of herb pills with only rapeseed oil or lubriol were 0.51 and 0.49 meq/kg, respectively. However, when rosemary was added in herb pills the POVs were decreased to 0.30 and 0.39 meq/kg, respectively. The addition of rosemary to the rapeseed oil and lubriol tended to decrease the microbial viable cell counts of the herb pill. The microbial viable cell counts of rapeseed oil and lubriol were 940 and 820CFU/g, respectively after 180 days of storage. However, these levels were suppressed to 720 and 640CFU/g by the resemary addition. On the other hand, the ginseng saponin content of herb pills was not affected by the rosemary addition during storage.

스마트 폰의 영상 처리를 통한 알약 인식 시스템 (Pill Identification System Using Image Processing in Smart Phone)

  • 본 논문에서는 노인의 의료 서비스를 보조하기 위하여 영상 처리를 통해 알약을 구분하고 정보를 제공하기 위한 시스템을 개발하였다. 노인의 경우에는 가정에 보관하고 있는 알약의 종류가 다양하고 어떤 약인지 구분하기 힘들어 사용하지 못하고 버리거나 방치해 두는 경우가 많다. 본 논문에서는 안드로이드 폰을 사용하여 알약을 촬영하고 영상처리를 하여 약의 모양과 분할선, 색 등의 특징을 구분하고, 서버와 연동하여 약의 이름과 효과, 주의사항 등의 정보를 제공함으로써 노인들이 약을 쉽게 구분할 수 있도록 하였다.

알약 자동 인식을 위한 딥러닝 모델간 비교 및 검증 (Comparison and Verification of Deep Learning Models for Automatic Recognition of Pills)

  • When a prescription change occurs in the hospital depending on a patient's improvement status, pharmacists directly classify manually returned pills which are not taken by a patient. There are hundreds of kinds of pills to classify. Because it is manual, mistakes can occur and which can lead to medical accidents. In this study, we have compared YOLO, Faster R-CNN and RetinaNet to classify and detect pills. The data consisted of 10 classes and used 100 images per class. To evaluate the performance of each model, we used cross-validation. As a result, the YOLO Model had sensitivity of 91.05%, FPs/image of 0.0507. The Faster R-CNN's sensitivity was 99.6% and FPs/image was 0.0089. The RetinaNet showed sensitivity of 98.31% and FPs/image of 0.0119. Faster RCNN showed the best performance among these three models tested. Thus, the most appropriate model for classifying pills among the three models is the Faster R-CNN with the most accurate detection and classification results and a low FP/image.

한방초콩환의 제조단계에 따른 추출물의 생리활성 변화 (Changes in Biological Activities of Extracts from Herbal Chokong Pills by Manufacturing Stages)

  • Objectives : This study was conducted to evaluate the antioxidative and anticancer activity of the water and ethanol extracts from pickled soybean products by manufacturing stage. Methods : Yakkong (YK) was pickled for 15 days to prepare Chokong (CK) and Chokong pills (CKP) were mixed Chokong (CK) powder with vinegar. Herbal Chokong pills (HCKP) were made by addition five kinds of medicinal herbs to Chokong. Pickled soybean products by each manufacturing process were extracted with distilled water and 70% ethanol, and the extracts were tested for their antioxidative and anticancer activities. Results : The highest electron donating abilities (EDA) of the water and ethanol extracts were from HCKP among pickled soybean products and those were 81.2% and 91.5% at 1,000 $\mu$g/mL, respectively. The ethanol extracts of pickled soybean products had higher activities of EDA and superoxide dismutase (SOD) than water extracts. The highest anticancer activity was the water extracts of HCKP against both of MDA and A549 cells. Conclusions : These results suggest that herbal Chokong pills (HCKP) can be used as natural antioxidant to prevent oxidative damage in normal cells probably because of their antioxidative characteristics.

심적환$^{(R)}$이 흉통 흉민에 대하여 미치는 영향에 대한 다기관 무작위배정 이중맹검 임상연구 (Effect of Cardiotonic Pills$^{(R)}$ on Chest Pain and Discomfort: A Multi-center Double-blind Randomized Controlled Trial.)

  • Objectives: This was a double blinded, randomized, placebo-controlled clinical study for evaluation of safety and effective dose finding of Cardiotonic Pills$^{(R)}$ in patients with chest pain and discomfort. Cardiotonic Pills$^{(R)}$ are composed of Salviae Miltiorrhizae Radix (丹蔘), Notoginseng Radix (三七根) and Borneolum (龍腦). Major effects of Salviae Miltiorrhizae Radix and Notoginseng Radix are vasodilatation, sedation and analgesic action. Borneolum has an antibacterial effect, and can stimulate the central nervous system. All of these substances are oriental herbs that have been used for a long time in east Asia. Cardiotonic Pills fi received Investigational New Drug (IND) approval from the Food and Drug Administration (FDA) in the USA and 40 million people in the world take this pill. We performed a phase IV clinical study to confirm its efficacy and safety in patients who have probable cardiogenic or psychogenic chest pain or chest stifling. Methods: This study was planned for a multi-center clinical trial including four university hospitals of oriental medicine in Korea. This was the first time to evaluate the 'planning treatment according to diagnosis (辨證施治)' of chest pain or chest discomfort according to oriental medical guidelines. The patients who were included in this trial were adult volunteers from 20 to 70 years old who had chest pain or chest discomfort more than twice during a recent month, and we received written consent to participate in this study from all of them. After administration of Cardiotonic Pills$^{(R)}$ for 8 weeks, number of occurrences, duration, appearance and degree of chest pain or chest discomfort was observed and degree of symptoms (severity of illness, global improvement) were measured using a patient's global assessment composite scale. Results: In the patient's global assessment scale, the severity of illness of the Cardiotonic Pills$^{(R)}$ group (n=25) was 14/25=0.56 but of the placebo group (n=25) was 7/25=0.28 (p-value=0.0449). This result indicates Cardiotonic Pills$^{(R)}$have a positive effect on the symptoms of chest pain and discomfort. However, the global improvement of the Cardiotonic Pills$^{(R)}$group was 23/25=0.92, and of the placebo group was 22/25=0.88 (p-value=0.6374). The total symptom score of the Cardiotonic Pills$^{(R)}$ group was $1.68\pm20.06$, and of the placebo group was $16.76\pm72.l4$(p-value=0.2285). The number of symptom events of the Cardiotonic Pills$^{(R)}$ group was $72\pm29.78$, and of the placebo group (n=25) was $10.80\pm38.42$ (p­value=0.3660). We could not find any effects on the other factors examined besides the severity of illness, beyond the difference of standard deviations. Conclusions: Cardiotonic Pills$^{(R)}$ significantly reduced chest pain and chest discomfort in patients. Therefore, we expect that Cardiotonic Pills$^{(R)}$ will be helpful for patients with chest pain and chest discomfort not only caused by heart disease but also by other diseases.
