The Effects of the Systemic Follow up Health Care Program on the Health Promotion and the Risk Reduction in Premature Infants and Their Mothers (체계적 건강관리프로그램이 모성과 미숙아의 건강증진 및 질병예방에 미치는 영향에 관한 연구)

  • Purpose: This research was conducted to evaluate the effects of asystemic follow-up care program on health promotion and risk reduction in 64 high-risk infants(HRI) including premature infants and their mothers. Method: The intervention consisted of systemic NICU education, tele-counseling and 3 home visits in 6 months. The subjects were divided into either the intervention group or the control group receiving the conventional NICU education without the tele-counseling and home visiting. Infant health promotion was measured using physical assessment, types of health problems, reflexes, OPD visiting history, DDST, immunization, feeding assessment, Infant death rate, etc. Maternal self-esteem, postpartum depression and family function were measured using the maternal self-report inventory(MRI), EPDS, and family apgar score(Fapgar), retrospectively. Result: All premature infants in the intervention group were in the normal range of growth and development, and the regular vaccination schedule. The health problems in the intervention group were addressed early so not to develop into adverse effects. The follow-up program for 6 months showed beneficial effects on MRI, EPDS, and Fapgar. Conclusion: A systemic follow-up health care program is beneficial on health promotion and risk reduction in 64 HRI including premature infants and their mothers.

  • 본 연구는 소방대원의 현장 작업자세 평가와 분석을 통하여 소방대원의 근골격계 부담작업 유해요인을 분석하고자 한다. B시에 소재하고 있는 소방서의 남자 소방대원을 대상으로 소방 현장에서 많이하는 자세 중 요구조자 이송작업자세, 방수작업자세, 유압구조장비 작업자세, 만능도끼작업 자세를 인간공학적 평가 기법인 Rapid Entire Body Assessment(REBA), Rapid Upper Limb Assessment(RULA)와 NIOSH Lifting Equation(NLE)를 이용하여 평가하고 분석하였다. 자세 분석결과, RULA 분석에서 모든 작업이 최고 점수인 7점으로 평가되었고 REBA는 5점에서 10점까지로 RULA분석에 비하여 낮은 점수를 나타내었다. 따라서, 소방대원들의 작업환경과 자세에 문제가 많다는 것을 알 수 있었다.

Musculoskeletal Workload Evaluation in Physical Therapist (물리치료사의 근골격계 부담작업 유해요인 조사)

  • This study investigated the symptoms of work-related musculoskeletal disorder(WMSDs) for physical therapists who have different work duties. We analyzed the symptoms in the musculoskeletal system and the degree of work-harmfulness by the survey of the symptom in the musculoskeletal system and rapid upper limb assessment(RULA) for pain control group(n=56), adult exercise group(n=53), and pediatric exercise group(n=22). As a result, 69.6% of the pain control group, 84.9% of the adult exercise group, 81.8% of the pediatric exercise group show the observable symptom. The adult exercise group has the biggest work load per hour in the final wrist & arm score of the ergonomic risk assessment using RULA. The action level of the pain control group is $3.0{\pm}0.9$; the Action Level of adult exercise group is $3.3{\pm}0.6$; the Action Level of the pediatric exercise group is $3.2{\pm}0.8$, and so it is shown that the adult exercise group has a problem of working posture. It is considered that devices and education system for preventing from WMSDs should come into wide use.

An Analysis of Pre-Service Science Teachers' PCK for Lessons Using Analogies (예비과학교사의 비유 사용 수업에 대한 PCK 분석)

  • In this study, we investigated pre-service science teachers' design for lessons using analogies in the perspectives of PCK. Three pre-service science teachers at a college of education in Seoul participated in this study. After the workshop of instructional analogies in science education, they practiced lessons using analogies in teaching practices. We observed their lessons and collected all of the teaching-learning materials. Semi-structured interviews were also conducted. The analyses of the results reveal that they dealt with mapping and unshared attribute only when using main analogies in their lessons and these processes were teacher-centered. There were some cases where they failed to adequately deal with analogies including concepts beyond the curriculum. When dealing with unshared attributes, they did not tend to accept students' opinions although they thought that unshared attributes are strongly related to misconceptions. Their understanding of assessment using analogies was not high. Assessment was relatively well done when they use student-centered analogies such as physical analogies or role-playing analogies. On the bases of the results, we suggest some educational implications for pre-service science teacher education.

Pain management and Pain knowledge of Nurses (간호사의 암환자 통증관리 및 통증지식에 관한 연구)

  • Purpose: This study is part of questionnaire survey concerned with the views of nursing staff for cancer patients. This study was done to describe cancer pain management problem, pain management knowledge. Methods: The participants were 188 nurses at cancer ward. Data were collected from July 2003 to August 2003. Data were collected with multiple-choice items and one open-ended question, which were constructed structured questionnaire. The data were analyzed by means of SPSS statistical software and content analysis. Results: Experiences of pain education are 53.7%, pain assessment sheet not use 86%, pain management status are partial. Education need of pain management was 87.8% patients in pain were very often(23.9%) or rather often (35.1%) cancer patients. The nurses are respond to open question. Pain management problems as assessed by nurses are categorized 11 item. The health professional problems are Knowledge deficit pain management, Incontrollable pain, Doctor's busy. The patient problems afraid of narcotics for addiction, side effect, distinguishing between physical and suffering, economical problem. The scores of knowledge about pain was average 16.7 score. The pain knowledge showed significant correlation education need(r=.180, p=.013). The effective variable was need of $education({\beta}=.163)$. Conclusion: It was found that pain knowledge was middle score and pain management problem was multiple. The study highlights the need to increase pain education for health professional. Therefore, This study suggest that health professionals education should be done to improve pain management problem.

Review of Meta-analysis Research on Exercise in South Korea (국내 운동 관련 메타분석 논문의 질 평가)

  • Purpose: The purpose of this study was to evaluate the quality of meta-analysis regarding exercise using Assessment of Multiple Systematic Reviews (AMSTAR) as well as to compare effect size according to outcomes. Methods: Electronic databases including the Korean Studies Information Service System (KISS), the National Assembly Library and the DBpia, HAKJISAand RISS4U for the dates 1990 to January 2014 were searched for 'meta-analysis' and 'exercise' in the fields of medical, nursing, physical therapy and physical exercise in Korea. AMSTAR was scored for quality assessment of the 33 articles included in the study. Data were analyzed using descriptive statistics, t-test, ANOVA and ${\chi}^2$-test. Results: The mean score for AMSTAR evaluations was 4.18 (SD=1.78) and about 67% were classified at the low-quality level and 30% at the moderate-quality level. The scores of quality were statistically different by field of research, number of participants, number of databases, financial support and approval by IRB. The effect size that presented in individual studies were different by type of exercise in the applied intervention. Conclusion: This critical appraisal of meta-analysis published in various field that focused on exercise indicates that a guideline such as the PRISMA checklist should be strongly recommended for optimum reporting of meta-analysis across research fields.

Quality Evaluation for the Diagnosis and Management of Hypertensives by Pubilc Health Doctors (공중보건의의 고혈압 진단 및 치료과정 평가)

  • Background : Little work has been carried out regarding quality assessment research in a primary care setting, comparing with that of hospitals. This study aims to evaluate the process of diagnosis and management of hypertension by public health doctors on the basis of pre-established clinical guideline, and to identify several modifying factors associated with them. Methods : Hypertension was selected as the target disease, because it is a chronic disease which is of great public health importance. Self-administered questionnaires were mailed to public health doctors practicing at health centers and health subcenters across the nation. The response rate was 20.9%. The questionnaire included the diagnosis and management process such as measuring blood pressure, history taking, physical examinations, and treatment approches and potentially modifying factors such as level of training, duration of practice as a public health doctor, and education on management of hypertension. Results : Public health doctors pay little attention in measuring BP, hypertension related history taking, performing physical examination and laboratory examination. But they devoted much effort in diagnosing hypertension exactly and giving nonpharmacological treatment. Among various antihypertensive drugs, calcium-channel blockers were the most preferred agent(50.9%). Level of training, duration of practice ad a public health doctor, and education on management of hypertension made no difference on quality of care(p>0.05). Conclusion : These public health doctors showed poor compliance with the pre-established clinical guidelines, which leaves much to be desired in diagnosing and managing hypertensive patients by public health doctors. This study might be able to contribute to develop some strategies, such as educational programs, which would be able to improve the process of care in hypertensives.

Nursing Need and Satisfaction of Families with their Hospitalized Children (입원환아 가족의 간호요구도와 만족도)

  • This study was done to evaluate the need and satisfaction for nursing care of the families with their hospitalized children. The data were collected through the questionnaire from March 15, 2002 to April 7. Subjects were the 103 families caring for their hospitalized children at pediatric ward in two university hospitals in Daegu. The nursing need instrument was developed by Seo(1999) and modified by the researcher of this study based on the classification of nursing care area(nursing assessment, direct nursing, education and counseling, and facilities and environment). The nursing satisfaction instrument was developed by Wandelt & Ager(1974) and modified by Park(1994) based on classification of nursing care area (psychosocial care, physical care, general care, professional care, and communicative care). The data were analyzed for mean, percent, t-test, ANOVA, and Pearson correlation coefficient using SAS program. The results are summarized as follows: 1. The scores in the nursing need showed in the Direct nursing(3.41±.42), Facilities and Environment(3.38±.46), Education and Counseling (3.35±.40), and Nursing Assessment(3.14±.41) area in order. 2.The scores in the nursing satisfaction showed in the Psychosocial care(3.70±.74), Commu- nicative care(3.60±.72), General care(3.42±.76), Professional care(3.38±.82), and Physical care(3.32±.70) area in order. 3.General characteristics of families which influence on the satisfaction showed a significant difference according to their educational(F=5.63, p=.001) and economical level(F=4.47, p=.006), and hospitalized experience(t=2.30, p=0.02). 4. There was no correlation between the nursing need and the nursing satisfaction.

  • [Purpose] Blood glucose and insulin resistance were lower following hypoxic exposure in previous studies. However, the effect of hypoxia as therapy in obese model has not been unknown. [Methods] Six-week-old mice were randomly divided into chow diet (n=10) and high-fat diet (HFD) groups (n=20). The chow diet group received a non-purified commercial diet (65 % carbohydrate, 21 % protein, and 14 % fat) and water ad libitum. The HFD group was fed an HFD (Research Diet, #D12492; 60% kcal from fat, 5.24 kcal/g). Both groups consumed their respective diet for 7 weeks. Subsequently, HFD-induced mice (12-weeks-old) were randomly divided into two treatment groups : HFD-Normoxia (HFD; n=10) and HFD-Hypoxia (HYP; n=10, fraction of inspired=14.6%). After treatment for 4 weeks, serum glucose, insulin and oral glucose tolerance tests (OGTT) were performed. [Results] Homeostatic model assessment values for insulin resistance (HOMA-IR) of the HYP group tended to be lower than the HFD group. Regarding the OGTT, the area under the curve was 13% lower for the HYP group than the HFD group. [Conclusion] Insulin resistance tended to be lower and glucose uptake capacity was significantly augmented under hypoxia. From a clinical perspective, exposure to hypoxia may be a practical method of treating obesity.

Effects of aerobic exercise training on the risk factors for liver diseases in elderly women with obesity and impaired fasting glucose: A pilot study

  • [Purpose] In the present pilot study, we aimed to investigate the effects of the Silverrobics exercise program, which is similar to aerobic dance, on the factors related to glucose metabolism and liver enzymes. [Methods] Eight elderly women with obesity and impaired fasting glucose participated in the Silverrobics exercise program (60 minutes per session for five times a week for 8 weeks). The program was conducted at 50-60% of the heart rate reserve at 1 to 2 weeks and at 60-80% of the heart rate reserve at 3 to 8 weeks. To verify the effect of this 8-week exercise program on glucose metabolism and liver enzymes, blood analysis at pre- and post-training was performed. [Results] After the Silverrobics exercise program, there were significant decreases in the glucose (p<0.05), glycated hemoglobin A1c (p<0.05), 1,5-anhydroglucitol (p<0.05), and insulin levels (p<0.01) and homeostatic model assessment of insulin resistance score (p<0.05). However, there were no significant effects on the liver enzymes, except for alkaline phosphatase. The alkaline phosphatase level increased after the Silverrobics exercise program (p<0.05). [Conclusion] Although the Silverrobics exercise program had no beneficial effects on the liver enzymes, it may play an important role in preventing liver diseases considering the effects on glucose metabolism.
