Analysis of Basic Constituents in Korean Earthworm About Lamnodrilus gotai Hatai (한국산 토룡의 기초성분 분석 -Lamnodrilus gotai Hatai를 대상으로-)

  • The earthworm(Annelid) is a herbicine which has traditionally been used in the treatment of infectious fever, jaundice, infection of middle ear, laryngitis, pharyngitis, nephritis, headache, toothache and certain urinary tract infections from the olden times. Before the isolation and purification of biologically active components we analyzed the basic constituents(proteins, amino acids, mineral, etc.) with lyophilized powder of Lamnodrilus gotai Hatai. The results were as follows: Minerals detected and quantitatively analyzed were $Ca^#$, $Mg^#$, Fe, Zn, Mn, Cu, Co, Cr, Ni, Ge and Se. Amino acids detected were alanine, phenylalanine, leucine, glutamic acid, tyrosine, threonine, arginine, aspartic acid, methionine, lysine, serine, histidine, isoleucine, glycine, proline and etc. The constituents of proteins, fat, fiber, ash and phosphorous were measured. These constituents were compared and discussed with those of other investigations.

Clinical Manifestation Patterns and Trends in Poststreptococcal Glomerulonephritis

  • Poststreptococcal glomerulonephritis (PSGN) is one of the most recognized diseases in pediatric nephrology. Typical clinical features include rapid onset of gross hematuria, edema, and hypertension, and cases are typically preceded by an episode of group A ${\beta}$-hemolytic streptococcus pharyngitis or pyoderma. The most common presenting symptoms of PSGN are the classic triad of glomerulonephritis: gross hematuria, edema, and hypertension. However, patients with PSGN sometimes present with unusual or atypical clinical symptoms that often lead to delayed diagnosis or misdiagnosis of the disease and increased morbidity. Additionally, the epidemiology of postinfectious glomerulonephritis (PIGN), including PSGN, has changed over the past few decades. This paper reviews atypical clinical manifestations of PSGN and discusses the changing demographics of PIGN with a focus on PSGN.

A Case of Unexpected Clinostomum complanatum Infection Initially Presenting as Foreign Body in Pharynx

  • A 46-year old man visited our outpatient clinic with complaint of foreign body sensation in throat after consuming raw freshwater fish 5 days ago. Laryngoscopic examination revealed a motile worm attached on posterior pharyngeal wall. The worm was removed using biopsy forceps under transnasal endoscopy and evidently identified as Clinostomum complanatum after microscopic examination. Patient's subjective foreign body sensation of throat and hyperemia of laryngeal mucosa remained for approximately 2 weeks post-removal, which were eventually resolved after administration of non-steroidal anti-inflammatory drug and anti-refluxant drug for 2 weeks. Treatment was ended at three weeks since the first visit. C. complanatum infections in humans are rare, and only four cases have been reported in Korea. Symptoms resembling pharyngitis or laryngitis occurs by consumption of raw, infected freshwater fish and treatment is done by mechanically removing the parasite.

The First Isolation of Chalamydia pneumoniae from a Korean Patient (한국인에서 처음 분리된 Chlamydia pneumoniae)

  • Background : Chlamydia pneumoniae is one of common causes in upper and lower respiratory infections. Isolating C. pneumoniae from clinical specimens is very difficult due to the characteristics of the organism. Recently, we succeeded in isolating C. pneumoniae from a Korean patient, who suffered from acute pharyngitis. This is the first isolate from a clinical specimen in Korea. Methods : We attained a nasopharyngeal swab from a 22-year-old female patient, and inoculated it on a monolayer of the Hep-2 cell line. After 8 passages, we found the inclusion bodies of C. pneumoniae by an immunofluorescence(IF) test. The species-specific monoclonal antibody IF staining and species-specific PCR were done to confirm the species of the isolate, and electron microscopy was used to characterize the morphology. Results : The isolated was confirmed to be C. pneumoniae by species-specific IF and PCR, and the strain was named LKK-1. The shape of the elementary body was round and with a narrow periplasmic space, as shown by electron microscopy, which is similar to the Japanese strain, but not the Western strain. Conclusion : We succeeded in isolating C. pneumoniae from a 22-year-old patient with acute pharyngitis, which is the first isolate in Korea. In the future, this Korean strain will be useful to the study of C. pneumoniae.

Clinical analysis of febrile infants and children presenting to the pediatric emergency department (소아응급실에 내원한 발열 환아에 대한 연구 분석)

  • Purpose : We studied clinical patterns of the febrile infants and children presenting to the Pediatric Emergency Department(ED) in the general pediatric population. Method : We analyzed 1,764 infants and children who had a history of measured body temperature of $38.3^{\circ}C$ or higher, before or after arrival at Pediatric ED of Ilsan Paik Hospital from September 2004 to August 2005. We analyzed their clinical patterns, such as the frequency of febrile illness, age distribution, monthly distribution, diagnosis. Result : The frequency of the febrile illness at Pediatirc ED was 19.9 percent (1,764/8,881 cases). The percentage of admission into hospital was 26.4 percent. The most common age group presenting to Pediatric ED with febrile illness was the 12-23 months group(22.8 percent) followed by the 4-11 months group and the 2 years group. In May, the most numerous patients presented to the Pediatric ED with febrile illness. The frequency of febrile illness was significant higher in the 'Spring & Summer group' compared to the 'Autumn & Winter group'. The ratio of male to femal was 1.4:1. The most common diagnosis was acute pharyngitis(42.5 percent), followed by acute gastroenteritis (15.3 percent), pneumonia(7.0 percent). Conclusion : The frequency of the febrile illness at Pediatirc ED was 19.9 percent. The most numerous patients visited Pediatric ED in May. And, the most common age group was 12-23 months. The proportion of non-urgent disease, such as acute pharyngitis, acute gastroenteritis, acute nasopharyngitis and acute bronchitis, was high. There is a need to educate parents and improve the medical system in Korea.

The Efficacy of Immediate Diet for Reducing Local Adverse Events of Inhaled Corticosteroid: A Pilot Study

  • Background: Local adverse events associated with inhaled corticosteroid use, including dysphonia, pharyngitis and oral candidiasis, can affect adherence for treatment. 'Mouth rinsing method' has been used for reducing local adverse events, but it cannot ensure complete prevention. The goal of this pilot study was to identify whether the 'immediate diet method' can reduce local adverse events in a limited number of patients. Methods: The study was conducted in a total of 98 patients, who had been prescribed a medium-dose fluticasone propionate for the first time, from January to October in 2010. One training nurse had performed the education on how to use the inhaler, including the mouth rinsing method. And with follow-ups at one month intervals, any patient who experienced such adverse events were educated on the immediate diet method, having a meal within 5 minutes after using an inhaler and they were checked on any incurrence of adverse events with one month intervals for 2 months. Results: The mean age of patients was 65.9 years old. The local adverse events had incurred from 18.4% of the study subjects. When performed the follow-up observation in 18 patients with local adverse events after education on the immediate diet method, 14 patients (77.8%) had shown symptomatic improvements. Three of 4 patients did not show any improvement, in spite of implementing the immediate diet method. The other 1 patient did not practice the immediate diet method properly. Conclusion: The immediate diet method may be useful in reducing the local adverse events, caused by the use of inhaled corticosteroid.

Comparison of the Scrophulariae Radix of Gyeongbuk's with that of Chinese: Content of Harpagoside and Inhibitory Effect of Nitric Oxide (경북산 현삼과 중국산 현삼의 비교: Harpagoside 함량 및 Nitric Oxide 저해활성)

  • Scrophulariae Radix (Scrophulariaceae) has been traditionally used to treat pharyngitis, laryngitis, tonsillitis, fever, and edema. To compare the quality of the Scrophulariae Radix produced from Gyeongbuk with that from China, a quantitative analysis on the major component, E-harpagoside, was performed by reversed-phase HPLC, and a relative inhibitory activity of nitric oxide (NO) was assayed in the Raw 264.7 cells. Using a Luna $C_{18}$(2) column, we carried out quantitative analysis with a gradient of 0.1% phosphoric acid in water and acetonitrile. Our results demonstrated that the content of E-harpagoside in Gyeongbuk's Scrophulariae Radix were three times higher than that of Chinese. Moreover, the inhibitory activity of Scrophulariae Radix of Gyeongbuk's in the lipopolysaccharide (LPS)-induced NO production was higher than that of Chinese.

  • Inflammatory bowel disease (IBD) is a well-recognized risk factor for thrombotic events in adults but data on children are scarce. In the great majority of adult patients, thrombotic events are usually deep vein thrombosis and pulmonary embolism. Other sites such as jugular veins are extremely rare. We present a case of Lemierre syndrome in an adolescent girl with active ulcerative colitis and discuss possible risk factors. This is the first reported case of severe Lemierre syndrome with thrombus extension to cranial veins in a patient with ulcerative colitis. Early recognition of Lemierre syndrome in patients who present with rapidly worsening symptoms of neck pain, fever and signs of pharyngitis is imperative because it increases a chance of favorable prognosis. It is important for pediatricians treating IBD patients not to underestimate possible thrombotic events in children with IBD. Recognition of additional risk factors is crucial for prompt diagnosis and adequate treatment.

Recent Clinical Research on Effect of Acupuncture on Sore Throat (인후통의 침치료에 대한 국외 임상 연구 동향)

  • Objectives The purpose of this study is to investigate recent clinical studies on the effect of acupuncture on sore throat. Methods Based on the PubMed search with the key search terms of 'sore throat, acupuncture', dated from 1992 to 2015, 7 controlled trials and 8 case series was found, and were analyzed for this study. Results 1. The most commonly used acupoints were Quchi (LI11), and Hegu (LI4). 2. The most commonly used meridians were The Lung Meridian of Hand-Taiyin and The Large Intestine Meridian of Hand-Yangming. 3. Acupuncture treatment was effective for both acute and chronic sore throat. 4. More clinical studies are needed to prove the effectiveness of the acupuncture on sore throat. To be more objective on the study results, we can measure temperature drops on or amount of analgesic consumption for sore throat after acupuncture treatment.

The Effects of Nebulizer Therapy with Normal Saline on Postoperative Thirst and Sore Throat (생리식염수를 이용한 흡입요법이 수술 후 갈증과 인후통에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effect of nebulizer therapy with normal saline on thirst and sore throat among postoperative patients. Methods: A quasi-experimental nonequivalent control group, pretest-posttest design was used with 50 participants. The experimental group received nebulizer therapy with normal saline every two hours for 10 minutes three times and wet gauze (n= 25), and the control group received only wet gauze (n= 25). Data were collected using the Numeral Rating Scale for Thirst and Sore Throat. Measurements were calculated before applying nebulizer therapy and total five times for 12 hours after treatment. Data were analyzed using the repeated measured ANOVA. Results: Thirst for the experimental group using the nebulizer therapy with normal saline improved more than for the control group (F= 3.06, p= .043). Sore throat was not significantly different between the two groups (F= 0.63, p= .565). Conclusion: Study results indicate that using nebulizer therapy with normal saline can reduced thirst for postoperative patients. So nurses can apply nebulizer therapy with normal saline to reduce thirst and to improve comfort for postoperative patients.
