Evaluation of Acute Respiratory Infections(ARI) Control Programme in a Korean Rural Community -The Patterns of Antibiotic Prescription- (한 농촌지역에서 실시한 소아 급성호흡기감염 관리사업의 평가 -항생제 사용을 중심으로-)

  • The purpose of this study was to evaluate the program for the control of acute respiratory infections(ARI) in children in a Korean rural area(Yonchon county). Evaluating the program, we focused on the pattern of prescription and appropriateness of antibiotics prescribed by the health personnel who had participated in the ARI Control Program. It was implemented at the primary health care setting in rural area, such as district hospital, health subcenters, and health posts. During six-months programme monitoring period, medical records were reviewed and collected data were analysed by the pediatrician, research coordinator of this study. The baseline data were collected from medical records of the same period(six months) of one year before the implementation of the ARI programme. The study results were as follow : 1. Common cold was the most prevalent disease(78.7%. 594 cases) among the all ARI cases (755 cases). The less frequent cases were bronchitis(11.9%), acute pharyngitis(5.2%), and pneumonia(1.8%). 2. Significant reduction in the use of antibiotics was observed after the programme implementation. Ninety three(15.7%) of 594 common cold cases were received antibiotics compared with 282(35.2%) of 802 in the baseline period. In the cases of bronchitis and acute pharyngitis, the reduction rates were 15.1% and 23.2% respectively compared to the baseline period. 3. Mean duration of antibiotics prescription was 1.81-1.75 days, similar to the baseline data. 4. The appropriateness rate of antibiotics prescriptions were 84.3%(common cold), 35.6% (bronchitis) and 28.2%(acute pharyngitis). In the case of pneumonia, the antibiotics prescription was compatible to the criteria developed. 5. Pediatrician prescribed antibiotics more appropriately for all cases than general practitioners in health sub-center, and nurse practitioners in health posts. 6. Antibiotics therapy was shown to be of no effect in the treatment of the all ARI cases. At the 5 and 10 days check-up of common cold cases after visits, proportion of improved patients were 58.3% in the antibiotics-used group and 51.4% in the control group. In the other cases of ARI, the patterns of response were similar to common cold. None of the differences in outcome between the antibiotics-used and control group was statistically significant. This ARI programme may have substantial a substantial impact on antibiotics use at the public health institutions(district hospital, health subcenters, health posts) which are of major domain for primary health care in Korean rural areas.

Clinical Availability of Rapid Strep Test in Children with Group A Streptococcal Pharyngotonsilitis (A군 연쇄구균 상기도 감염에 있어 신속검사의 유용성)

  • Purpose : The accurate diagnosis and proper treatment of group A streptococal infection should be emphasized concerning about possible development of late sequelae, such as acute rheumatic fever and acute glomerulonephritis. Inadequate & improperance of antobiotics have resulted in increased number of antibiotic-resistant bacteria. We would like to know the clinical usefulness of rapid strep test compared with conventional throat culture in out-patients with acute pharyngotonsilitis. Methods : From Sep. 2000. to Jan. 2001, rapid strep test(LINK 2 Strep A, USA) & throat culture were taken from 87 patients with clinically suspect pharyngotonsilitis from Masan Fatima hospital & kyunghee university hospital. Results : Of 87 cases with pharyngitis, 39 cases proved to have group A streptococci by throat culture. The positive predictive value of rapid test was 92.3%(36 of 39 cases) and sensitivity test was 81.8%(36 of 44 cases). The specificity of rapid test was 93.0%(40 of 43 cases) and negative predictive value was 83.3%(40 of 48 cases). Conclusion : The positive predictive value & specificity of rapid strep test is high. And so, this test will give the pediatricians practical guidance of antibiotic use in patients with pharyngitis. But more efforts should be made to prevent antibiotics abuse and correct diagnosis of pharyngitis.

Clinical features of Epstein-Barr Virus-associated Infectious Mononucleosis According to Age Group in Children

  • Objectives: Few studies of pediatric Epstein-Barr virus (EBV)-associated infectious mononucleosis (IM) have been conducted in Korea. We evaluated the clinical features of children with IM to define differences according to age. Methods: We conducted retrospective chart reviews of 68 children aged 0 to 15 years who were diagnosed by EBV-associated IM with EBV-Viral Capsid Antigen(VCA) IgM at laboratory test and were admitted between 2010 and 2014. The children were classified into four age groups: aged 0-3, 4-6, 7-9, and 10-15 years. Results: The age distribution of patients was as follows: 19 (27.9%) 0-3, 25 (36.8%) 4-6, 13 (19.1%) 7-9, and 11 (16.2%) 10-15. Fever was the most common presentation regardless of age. It was more common in the 0-3 group than the 4-6 group (P = 0.018). Pharyngitis was more common in the 7-9 group than the 0-3 group (P = 0.048), and myalgia was more common in the 10-15 group than the 0-3 group (P = 0.007). Pharyngitis was accompanied by lymphadenopathy, protracted fever, and rash. In the 0-3 age group, the prevalence of rash was higher while the percentage of atypical lymphocytes was lower, but there was no statistical support for this tendency. There were no differences in the frequency of hepatosplenomegaly or laboratory findings between age groups. Conclusions: IM is not uncommon in young children and its clinical presentation varies with age. Therefore, IM should be suspected in young febrile children with pharyngitis and rash despite low percentages of atypical lymphocytes.

  • Chrysanthemum zawadskii var. latilobum (Compositae), which is called Gu-Jul-Cho, has been used as traditional medicine in pneumonia, bronchitis, common cold, pharyngitis, bladder-related disorders, women's diseases, and hypertension, etc. In this study we have investigated various biological activities of methanol extracts and their main component, linarin isolated from this herb. The antipyretic effect of them was tested by Brewers yeast method and the antiinflammatory activities, with arachidonic acid, o-tet-radecanoylphorbol 13-acetate and carrageenan. Their antipyretic and antiinflammatory activities were more potent than acetaminophene and aspirin. They also showed stronger hepatoprotective activities than ones of silymarin in mice.

  • The PFAPA syndrome is characterized by periodic fever, malaise, aphthous stomatitis, pharyngitis and cervical lymphadenitis without long-term sequelae. It has been known as one of the cause of periodic fever in children. Because there is no specific laboratory findings, the diagnosis of PFAPA syndrome is not only made by exclusion of diseases which are related to the other causes of periodic fever, but also made by a careful history and characteristic clinical manifestations. The dramatic response to the single oral dose of corticosteroid is unique to this syndrome. The PFAPA syndrome tends to be resolved without any long-term adverse sequelae. We have reported a 3 year old boy who was diagnosed as having PFAPA syndrome by episodes of periodic fever for 3 months and by excluding other febrile diseases. He was treated with oral prednisolone and his symptoms had been improved dramatically.

Discordant Rate of Simultaneous Duplicate Throat Swab Culture for Discovering Beta-hemolytic Streptococcal Carrier from Normal School Children (초등학생의 베타용혈성 연쇄구균 보균자 검출에 있어서 인두부 중복배양(duplicate throat culture)의 유용성)

  • Purpose : The most patients with acute streptococcal pharyngitis lack of classic clinical manifestations, therefore diagnostic laboratory test such as the throat culture or a rapid antigen detection test are frequently employed in primary practices of developed countries. We'd like to know the accuracy of the throat swab culture as gold standard for diagnosis of streptococcal infection with studying the discordant and concordant rate of duplicate culture. Methods : The study included 89 normal school children (boys:50, girls:39) who were attending Uljin primary school in Uljin, Kyong Sang Buk Do on March 1996. We obtained simultaneous 2 times of throat swab from each subject, and plating and streaking on 5-7% of sheep blood agar separately. We counted the characteristic beta-hemolytic colonies after overnight incubation. Results : 1) The carrier rate of beta-hemolytic streptococci at first culture is 25.1% and second one is 29.2%. 2) Ten out of 89(11.2%) is discordant in duplicate culture. 3) Culture containing less than 50 colonies of beta-hemolytic streptococci (+2) in first culture is 70.4%, second one is 85.7%. 4) Number of colonies is less than 50 in all ten discordant children. Conclusions : The discordant rate of duplicate throat swab cullture for beta-hemolytic streptococci is 11.2%, even if the subjects are normal school children. About 5% of individuals harboring beta-hemolytic streptococci in the pharynx may be missed by a single throat culture. If we are trying to examine the patients with pharyngitis, the discordant rate will be much lower than this results.

A Case of PFAPA Syndrome Mimicking Cyclic Vomiting Syndrome (주기성 구토증으로 추적 관찰 중 진단된 PFAPA 증후군 1예)

  • PFAPA syndrome is characterized by periodic fevers associated with aphthous stomatitis, pharyngitis, and cervical adenitis and is unusual in infants and children. We report on a case of PFAPA syndrome mimicking cyclic vomiting syndrome in a 42-month-old girl. She had experienced multiple episodes of cyclic vomiting with abdominal pain from age 20 to 30 months. When she was 30 months old, periodic fever with pharyngitis was combined with cyclic vomiting, and when 40 months old, aphthous stomatitis and cervical adenitis were added. These periodic symptoms and signs were not treated with prokinetics or antibiotics. Symptom duration of an episode was 3 days. After cimetidine therapy (150 mg three times daily for 6 months), her febrile and cyclic vomiting episodes ceased. At the time of writing she had not received therapy for 10 months and has remained well without periodic attack.

Clinical and Bacteriologic Efficacy of Cefdinir on Pharyngitis and Pharyngotonsillitis Caused by Group A Beta Hemolytic Streptococci in Children (Group A-beta Hemolytic Streptococci에 의한 소아 인두편도염에 있어서 Cefdinir의 항균력과 임상적 및 세균학적 효과에 관한 연구)

  • Objective : To determine the clinical and bacteriologic efficacy and safety of Cefdinir in acute pharyngitis and pharyngotonsillitis caused by group A beta hemolytic streptococci in pediatric patients. Methods : Children aged 3 through 12 years who visited the hospitals enrolled in this study with signs and symptoms of pharyngitis or pharyngotonsillitis since May to December 2002, were taken throat culture and given Cefdinir(12 mg/kg/day, in three divided doses) for 7 days. Two hundred thirty five patients were enrolled and 90 patients who showed positive culture results were followed up for the signs and symptoms during the treatment to determine clinical efficacy. Follow-up culture were done at the end of the study and bacteriologic efficacy was determined Results : Ninety out of 235 patients who visited the hospitals with the signs and symptoms of pharyngitis showed positive growth on throat culture. Seventy nine patients were clinically and microbiologically assessable. The bacteriologic eradication rates of S. pyogenes were 100% in all the children treated with Cefdinir. Clinical cure rates were not different between less than 7 days-treated group(75%) and just 7 days-treated group(98.6%)(P=0.077). Two patients reported adverse reaction during Cefdinir treatment. Antimicrobial sensitivity of Cefdinir against S. pyogenes was 100% with range of MIC being less than 0.5 ${\mu}g/mL$. Conclusion : It seems that Cefdinir is one of reliable and well-tolerated drugs for the treatment of group A beta hemolytic streptococcal pharyngotonsillitis in children. It needs to be investigated short term efficacy in terms of improving drug compliance and impact of economic point of view. We can recognized that still high rate of the erythromycin resistant group A streptococci in our community should be monitored every year.

Association of a Provisional New emm Type Opacity Factor-Negative Group A Streptococci Strain ST4529 with Septicemia

  • Group A Streptococcus strain ST4529 is a provisional new ems type which has been recently reported in Malaysia (Jomal, et al. 1999. Energ. Infect . Dis. 5,10-14). This strain was found to be opacity factor (OF) negative with a Tl phenotype. Usually, OF negative strains with T1 phenotypes are associated with acute rheumatic fever. However, strain ST4529 was isolated from the blood of a patient with septicemia. Comparison of the deduced amino acid sequence of the mature hypervariable N-terminus of ST4529 showed only 43% identity with that of M5, the closest matched OF negative strain with a T1 phenotype. Thus, ST4529 most probably encodes a new serospecifically unique M protein which is associated with septicemia rather than pharyngitis infections. The strains with these phenotypes are very important because their sequences should be considered for developing any anti-streptococcal vaccines.

Clinical Study of Infectious Mononucleosis (전염성 단핵구증의 임상적 고찰)

  • Background: Infectious mononucleosis is a disease precipitated by Epstein-Barr virus(EBV) in mostly children, some seronegative adolescents and young adults comprising clinical symptoms such as fever, lymphadenopathy, and pharyngitis as well as laboratory findings such as hetero-phil antibodies and atypical lymphocytosis. It is confirmed by serologic test for EBV. Materials and Methods: A retrospective evaluation of 26 patients who diagnosed with infectious mononucleosis was peformed through the analysis of typical symptom, sign and laboratory findings. Results : Infectious mononucleosis occurs mostly at 3 to 10 years (74.9%), common symptoms and signs are fever, cervical lymphadenopathy, tonsillar enlargement and exudate. Positive ratio of atypical Lymphocyte(>10%) and hetrophil antibodies are 61.5%, 35.2% respectively, it is less diagnostic. EBV-viral capsid antigen(VCA) IgM are positive in all cases, so it is most diagnostic findings. Conclusion: Infectious mononucleosis should be considered as a cause of cervical lymphadenopathy and pharyngotonsillitis in children and young adults, the assessment of EBV-VCA IgM is necessary for the diagnosis.
