국내 암환자의 오심구토에 적용한 비약물적 중재효과의 메타분석 (A Meta-Analysis of Intervention Studies on Nausea and Vomiting in Cancer Patients)

  • Purpose: This study was designed to analyze the characteristics and effect size of published intervention studies related to nausea & vomiting among cancer patients. Methods: A total of 1,083 studies were retrieved. From these studies, 20 studies met the inclusion criteria with a total of 698 participants. Two authors independently extracted data from the selected studies and assessed the methodological quality. The data were analyzed by the RevMan 5.0 program of Cochrane library. Results: The twenty studies utilized various non-pharmacological interventions: Nine studies on acupuncture (45%), Five studies utilized massage (25%), Two studies used oral cryotherapy (10%) and Four studies used other therapies. In the twenty studies the effect size of the intervention studies reported a higher effect sizes for massage (d=-1.62) and acupuncture (d=-0.89). Conclusion: This study suggests that non-drug therapy can reduce the levels of nausea and vomiting intensity. Massage and acupuncture interventions studies were more numerous and this may account for the higher effect rate.

불면증의 바이오피드백 치료 (Biofeedback Treatment for Insomnia)

  • 불면증은 다양한 신체적, 정신적 원인에 의해 생기는데, 특히 과잉각성은 불면증에 중요한 영향을 미친다고 알려져 있다. 불면증 환자에서 약물치료에 한계가 있으며 심리적 요인이 불면증의 발생과 경과에 큰 영향을 미친다는 사실이 알려지면서, 여러 가지 비약물치료적 방법이 도입되었다. 바이오피드백을 포함한 비약물치료는 불면증 치료에 효과적이며 치료 효과도 오래 유지되는 것으로 보고되었다. 바이오피드백은 이완요법과 함께 사용 시 불면증 환자의 각성 수준을 낮춰 수면의 시작 및 유지에 도움이 되므로, 각성 수준이 높거나 약물 사용에 제한이 따르는 불면증 환자에게 매우 유용할 수 있다.

Degradation or aggregation: the ramifications of post-translational modifications on tau

  • Tau protein is encoded in the microtubule-associated protein tau (MAPT) gene and contributes to the stability of microtubules in axons. Despite of its basic isoelectric point and high solubility, tau is often found in intraneuronal filamentous inclusions such as paired helical filaments (PHFs), which are the primary constituent of neurofibrillary tangles (NFTs). This pathological feature is the nosological entity termed "tauopathies" which notably include Alzheimer's disease (AD). A proteinaceous signature of all tauopathies is hyperphosphorylation of the accumulated tau, which has been extensively studied as a major pharmacological target for AD therapy. However, in addition to phosphorylation events, tau undergoes a number of diverse posttranslational modifications (PTMs) which appear to be controlled by complex crosstalk. It remains to be elucidated which of the PTMs or their combinations have pro-aggregation or anti-aggregation properties. In this review, we outline the consequences of and communications between several key PTMs of tau, such as acetylation, phosphorylation, and ubiquitination, focusing on their roles in aggregation and degradation. We place emphasis on the structure of tau protofilaments from the human AD brain, which may be good targets to modulate etiological PTMs which cause tau aggregation.

A Tale of Two Models: Mouse and Zebrafish as Complementary Models for Lymphatic Studies

  • Lymphatic vessels provide essential roles in maintaining fluid homeostasis and lipid absorption. Dysfunctions of the lymphatic vessels lead to debilitating pathological conditions, collectively known as lymphedema. In addition, lymphatic vessels are a critical moderator for the onset and progression of diverse human diseases including metastatic cancer and obesity. Despite their clinical importance, there is no currently effective pharmacological therapy to regulate functions of lymphatic vessels. Recent efforts to manipulate the Vascular Endothelial Growth Factor-C (VEGFC) pathway, which is arguably the most important signaling pathway regulating lymphatic endothelial cells, to alleviate lymphedema yielded largely mixed results, necessitating identification of new targetable signaling pathways for therapeutic intervention for lymphedema. Zebrafish, a relatively new model system to investigate lymphatic biology, appears to be an ideal model to identify novel therapeutic targets for lymphatic biology. In this review, we will provide an overview of our current understanding of the lymphatic vessels in vertebrates, and discuss zebrafish as a promising in vivo model to study lymphatic vessels.

Suppression of Human Fibrosarcoma Cell Metastasis by Phyllanthus emblica Extract in Vitro

  • Phyllanthus emblica (PE) is known to exhibit various pharmacological properties. This study aimed to evaluate the antimetastatic potential of a PE aqueous extract. Cytotoxicity to human fibrosarcoma cells, HT1080, was determined by viability assay using the 3-(4,5-dimethylthiazol,2-yl)-2,5-diphenyltetrazolium bromide (MTT) reagent. Cell migration and invasion were investigated using chemotaxis chambers containing membranes precoated with collagen IV and Matrigel, respectively. Cell attachment onto normal surfaces of cell culture plates was tested to determine the cell-adhesion capability. The molecular mechanism of antimetastatic activity was assessed by measuring the gene expression of matrix metalloproteinases, MMP2, and MMP9, using reverse transcription-polymerase chain reaction (RT-PCR) assay. The mRNA levels of both genes were significantly down-regulated after pretreatment with PE extract for 5 days. Our findings show the antimetastatic function of PE extract in reducing cell proliferation, migration, invasion, and adhesion in both dose- and time-dependent manners, especially growth arrest with low $IC_{50}$ value. A decrease in the expression of both MMP2 and MMP9 seems to be the cellular mechanism for antimetastasis in this case. There is a high potential to use PE extracts clinically as an optional adjuvant therapeutic drug for therapeutic intervention strategies in cancer therapy or chemoprevention.

칼륨채널 활성물질 Lemakalim의 고칼륨혈증 및 저칼륨혈증에서의 심기능에 대한 영향 및 항 고혈압작용 (Pharmacodynamics of Potassium Channel Opener Lemakalim in the Isolated Rat Hearts and Conscious SHRs under Hyperkalemic and Hypokalemic Condition)

  • Pharmacological effects of lemakalim on cardiovascular system were investigated using isolated rat hearts and conscious SHRs subjected to hyperkalemic and hypokalemic condition. In the isolated hearts perfused with normal physiological salt solution(4.7 mM KCI), lemakalim increased cardiac function and coronary flow, and these effects were significantly potentiated under hypokalemic(1.2, 2.5 mM KCI), but attenuated under hyperkalemic(IO mM KCI) condition. In conscious SHRS, lemakalim(0.1, 0.2, 0.3mg/kg, p.o.) produced a dose-related decrease in systolic blood pressure, the maximal hypotensive effect being reached around 0.5 hr after dosing. The intensity and the duration of hypotensive effect of lemakalim were significantly increased when administered in combination with dihydrochlorothiazide (2 mg/kg, p.o.), but decreased with triamterene(32 mg/kg, p.o.). It appears that the differential effects of two types of diuretics on the hypotensive action of lemakalim are due to their hypokalemic and hyperkalemic action, respectively. It is conclued that the concomitant use of $K^{+}$ channel openers and hypokalemic diuretics may be an appropriate model of combination therapy in the treatment of hypertension.

  • Efficacy and general pharmacology of recombinant human erythropoietin (rHu-EPO), were investigated. Efficacy studies were conducted in normal, cisplatin- and acute hemorrhage-induced anemic rats. Normal and anemic animals were treated intravenously with rHu-EPO for 5 days and the changes in the numbers of red blood cells (RBC), hemoglobin (Hb), hematocrit (Hct) and reticulocytes (Ret) were examined. In normal rats, rHu-EPO significantly increased RBC, Hb, Hct and Ret at doses of 50∼ 1250 IU/kg/day. Cisplatin-induced anemic rats showed significant increase of RBC, Hb, Hct and Ret in a dose-dependent manner after administration of rHu-EPO (50∼200 lU/kg/day). And in acute hemorrhage-induced anemic rats, rHu-EPO(4∼100 Iu/kg/day) accelerated recovery of anemia with significant increase in Ret. These changes disappeared gradually after cessation of the treatment. The general pharmacological effects of rHu-EPO were investigated in mice, rats, guinea-pigs, rabbits and cats. rHu-EPO had no influences on central nervous, cardiovascular, gastrointestinal and blood coagulation system. It also had no influence on the contraction of phrenic nerve-diaphragm preparation. rHu-EPO produced significant increase of urine volume at a dose of 7000 IU/kg. These results suggest that rHu-EPO might be useful for the therapy of anemia without serious side effect.

Temporomandibular joint disorder from skull-base osteomyelitis: a case report

  • Skull-base osteomyelitis is a rare disease affecting the medulla of the temporal, sphenoid, and occipital bones. In general, it occurs due to external ear canal infections caused by malignant external otitis. Skull-base osteomyelitis usually affects elderly diabetic patients. The patient, a 58-year-old man, was referred for evaluation and management of the left jaw. Clinical examination of the patient revealed pain in the left jaw and mouth-opening deflection to the left. The maximum active mouth opening was measured to about 27 mm. Panoramic, CT, and CBCT revealed bone resorption patterns in the left condyle. Through control of diabetes, continued pharmacological treatment, arthrocentesis, and occlusal stabilization appliance therapy were carried out. The extent of active mouth opening was increased to 45 mm, and pain in the left jaw joint was alleviated. This was a case wherein complications caused by failure to control diabetes induced skull-base osteomyelitis. There is a need for continued discussion about the advantages and disadvantages of arthrocentesis with lavage for patients with skull-base osteomyelitis and other treatment options.

월경전 불쾌기분 장애에 대한 임상적 고찰 (Premenstrual Dysphoric Disorder : A Clinical Review)

  • 월경전 불쾌기분 장애(premenstrual dysphoric disorder, PMDD)는 여성의 약 $4{\sim}5%$에서 발병하며 섹스 스테로이드 호르몬 양의 변화에 대한 증가된 반응성 및 세로토닌, 멜라토닌, GABA의 감소가 원인으로 생각된다. 우울증, 계절성 정동 장애, 공황장애, 신경성 폭식증은 질병 특성 상 공통점이 있어서 월경전 불쾌기분 장애와 상호 연관성을 암시한다. DSM-IV-TR의 진단 기준에 따라 월경전 불쾌기분 장애를 진단하려면 반드시 정신적 증상이었어야 한다. 흔한 정신적 증상은 이자극성, 분노, 우울, 기분 스윙, 정동 불안정, 긴장, 불안, 피로, 음식 갈망이다. 현재로서 최선의 치료 약물은 세로토닌 재흡수 차단제이며, leuprolide, danazol, estradiol, spironolactone, bromocriptine도 사용될 수 있다. 경증 및 중증도 환자에 대한 보조적 치료로는 식이요법, 운동, 광선요법, 정신치료, 일기 쓰기 등이 있다.

Effects of Intramuscular Electrical Stimulation Using Inversely Placed Electrodes on Myofascial Pain Syndrome in the Shoulder - A Case Series

  • Myofascial pain syndrome (MPS) is one of the common musculoskeletal conditions of the shoulder which may develop sensory-motor and autonomic dysfunctions at the various level of the neuromuscular system. The pain and dysfunction caused by MPS were primarily treated with physical therapy and pharmacological agents in order to achieve painfree movements. However, in recent years intramuscular electrical stimulation (IMES) with conventional electrode placement was used by researchers to maximise therapeutic values. But, in this study an inverse electrode placement was used to deliver electrical impulses intramuscularly to achieve neuro-modulation at the various level of the nervous system. Nine patients with MPS were treated with intramuscular electrode stimulation using inversely placed electrodes for a period of three weeks. All nine subjects recovered from their shoulder pain and disability within the few weeks of intervention. So, this inverse electrode placement may be more appropriate for chronic pain management.
