A Study on Design and Application of Tissue Compensator for 6MV X-rays (6MV X-선에 대한 조직 보상체의 제작 및 응용에 관한 연구)

  • A radiation beam incident on an irregular or sloping surface produces the non-uniformity of absorded dose. The use of a tissue compensator can partially correct this dose inhomogeneity. The tissue compensator is designed based on the patient's three dimensional contour. After required compensator thickness was determined according to tissue deficit at $25cm\pm25cm$ field size, 10cm depth for 6MV x-rays, tissue deficit was mapped by isoheight technique using laser beam system. Compensator was constructed along the designed model using 0.8mm lead sheet or 5mm acryl plate. Dosimetric verification were peformed by film dosimetry using humanoid phantom. Dosimetric measurements were normalized to central axis full phantom readings for both compensated and non-compensated field. Without compensation, the percent differences in absorbed dose ranged as high as $12.1\%$ along transverse axis, $10.8\%$ along vertical axis. With the tissue compensators in place, the difference was reduced to $0\~43\%$ Therefore, it can be concluded that the compensator system constructed by isoheihnt technique can produce good dose distribution with acceptible inhomogeneity, and such compensator system can be effectively applied to clinical radiotherapy.

An Applicable Method of an Electromagnetic Wave Absorber for SAR Reduction in the Human Head Exposed to Electromagnetic Fields Radiated by a Cellular Phone (휴대폰 전자파에 노출된 두부내 SAR 저감을 위한 전자파 흡수체 적용 방법 연구)

  • In order to reduce the specific absorption rate(SAR) in a human head exposed to electromagnetic fields radiated by a cellular phone, we have analyzed an electromagnetic wave absorber attached to the handset. A manufactured electromagnetic wave absorber was composed of Mn - Zn, which had complex relative permittivity of 7.30-j0.05 and permeability of 2.20-i1.55. The SAR value from the electromagnetic wave absorber attachment was calculated by using the nonuniform finite difference time domain(FDTD) algorithm and measured by phantom model at 835 MHz. The SAR reduction due to the electromagnetic wave absorber are about 18 % at 835 MHz. The V.S.W.R and radiation pattern of antenna are good agreement with the normal antenna. The gain reduction due to the electromagnetic wave absorber are only 0.3 dB at 835 MHz. But the sensitivity of cellular phone generally improves about 1 dB.

A Case Study of Application of Exposure Index in Computed Radiography by Using Human Chest Phantom (인체 흉부 모형 팬텀을 이용한 컴퓨터방사선영상에서 노출지수의 적용 사례 연구)

  • As the use of digital radiographic system has been expanded, there are some concerns an increase about in patient of radiation dose. Therefore, International Electro-technical Commission (IEC) has been proposed a standard foe exposure index (EI). In this study, the EI was measured on human chest model using computed radiography (CR). Radiation quality used RQA5 of IEC62494-1. After acquiring the chest anterior posterior image (Chest AP) by using the phantom, the EI was obtained by applying the system response. In this study, we have analyzed the images with the detector size (Full filed ROI) and the optimized image (Fit filed ROI). The EI increased proportionally with radiation dose increase. Due to the discrete increase in pixel value, the EI showed an exponential increase. The discrete increase in noise equivalent quanta (NEQ) resulted in a discrete increase in the EI. The EI of the two images used in this study increased with increasing NEQ but showed different increments. For the measurement of the EI, IEC standards must be followed. The EI should be used as an index to evaluate the image quality for quality control of X-ray image rather than as an indicator of exposure dose. When calculating the EI, the system response should be applied depending on whether or not the grid is used. The size of the field should be obtained by including only the necessary parts.

New thyroid models for ICRP pediatric mesh-type reference computational phantoms

  • As part of the ICRP Task Group 103 project, we developed ten thyroid models for the pediatric mesh-type reference computational phantoms (MRCPs). The thyroid is not only a radiosensitive target organ needed for effective dose calculation but an important source region particularly for radioactive iodines. The thyroid models for the pediatric MRCPs were constructed by converting those of the pediatric voxel-type reference computational phantoms (VRCPs) in ICRP Publication 143 to a high-quality mesh format, faithfully maintaining their original topology. At the same time, we improved several anatomical parameters of the thyroid models for the pediatric MRCPs, including the mass, overlying tissue thickness, location, and isthmus dimensions. Absorbed doses to the thyroid for the pediatric MRCPs for photon external exposures were calculated and compared with those of the pediatric VRCPs, finding that the differences between the MRCPs and VRCPs were not significant except for very low energies (<0.03 MeV). Specific absorbed fractions (target ⟵ thyroid) for photon internal exposures were also compared, where significant differences were frequently observed especially for the target organs/tissues close to the thyroid (e.g., a factor of ~1.2-~327 for the thymus as a target) due mainly to anatomical improvement of the MRCP thyroid models.

The Relationship between Adult Interpersonal Traumatic Experience and Posttraumatic Growth : The Multiple Mediating Effect of Optimism and Quality of Relationship (성인의 대인외상경험과 외상 후 성장의 관계 : 낙관성과 관계의 질의 다중매개효과)

  • This study is to research the mediating effects of optimism and quality of relationship which interpersonal traumatic experience influences posttraumatic growth. It focuss on verifying the impact of interpersonal traumatic experience to optimism and quality of relationship and the influence of optimism and quality of relationship to posttraumatic growth. And also it is to verify dual mediating effect between optimism and quality of relationship. Data for this study were collected through the use of a survey instrument completed by 405 interpersonal traumatic experience, 30-60 age group. Collected data were analysed by AMOS program, Structure Equation Model(SEM) was implemented to verify the mediating effect between optimism and quality of relationship. Finally, Phantom Variable was utilized for verification of indirect influence of a multi mediating effects. The findings of this study were as follows, First, the result shows that the higher level interpersonal traumatic experience, the higher posttraumatic growth they have. Second, it shows that optimism and quality of relationship have partial mediating effect between interpersonal traumatic experience and posttraumatic growth. Third, it was analyzed that optimism and quality of relationship had double mediating effect. Based on these findings, the research discussion reinforced the importance of intervene with its optimism and quality of relationship in the site of those who experience interpersonal trauma. Theological, political and practical implications of this study are as follows. First, it has a series of significance in terms of that this study confirmed its influence considering the quality of relationship from interpersonal experience among relevant factors of traumatic accident, optimism among individual factors, quality of relationship among environmental factors based on 'the crisis of life and the theory of individual growth'. econd, even though optimism and quality of relationship were proved as major predictors for posttraumatic growth of those who experience interpersonal traumatic, psychological support center for traumatic experience is not operated systematically in Korea. Third, this study implies that it should intervene mainly with its optimism and quality of relationship in the site of those who experience interpersonal trauma such as any abuses including sexual abuse, bullying, and divorce.

Radiation Dose during Fluoroscopy at the Organ from Extracorporeal Shock Wave Lithotripsy (체외충격파쇄석술에서 투시 시 주요 장기별 방사선 피폭선량)

  • We measured the radiation exposure for 55 persons (male: 36, female: 19) who was diagnosed with kidney and ureter stones and received ESWL. The absorbed dose was measured at the organ which is expected to absorb relatively much radiation (kidney, bladder, liver). The radiation dose measurement voltage 80kVp, current of 5mA as a fixed model of the human body by using the Rando phantom with Radiophotoluminescent Glass Dosimeter. Absorbed dose was measured for two times (5 minute and 10 minute, each) and converted to effective dose. Mean number of treatment was 1.8 times (1~4) per patient was the mean time of radiation exposure533 seconds (248-2516). For the treatment of right renal stone, the effective dose of right kidney, left kidney, liver and bladder was 2.458mSv, 0.152mSv, 1.404 mSv and 0.019mSv, respectively. For the treatment of left renal stone, the effective dose of right kidney, left kidney, liver and bladder was 2.496mSv, 0.252mSv, 0.178 mSv, and 0.017mSv, respectively. For the treatment of distal ureter stone, the effective dose of right kidney, left kidney and bladder was 0.009mSv, 0.01mSv and 3.742mSv, respectively.

Algorithm for the design of a Virtual Compensator Using the Multileaf Collimator and 3D RTP System (다엽콜리메터와 삼차원 방사선치료계획장치를 이용한 가상 선량보상체 설계 알고리듬)

  • The virtual compensator which are realized using a multileaf collimator(MLC) and three-dimensional radiation therapy Planning(3D RTP) system was designed. And the feasibility study of the virtual compensator was done to verify that it can do the function of the conventional compensator properly. As a model for the design of compensator, styrofoam phantom and mini water phantom were prepared to simulate the missing tissue area and the calculated dose distribution was produced through the 3D RTP system. The fluence maps which are basic materials for the design of virtual compensator were produced based on the dose distribution and the MLC leaf sequence file was made for the realization of the produced fluence map. Ma's algorithm were applied to design the MLC leaf sequence and all the design tools were programmed with IDL5.4. To verify the feasibility of the designed virtual compensator, the results of irradiation with or without a virtual compensator were analyzed by comparing the irradiated films inserted into the mini water phantom. The higher dose area produced due to the missing tissue was removed and intended regular dose distribution was achieved when the virtual compensator was applied.

A Study of Signal Intensity of MRA in Flow Phantom of Fusiform Aneurysm (방추형 동맥류 모형에서 자기공명 혈관조영술의 신호강도에 대한 연구)

  • Purpose : Authors correlated the three-dimensional time-of-flight MRA signal intensity characteristics and flow profile simulated by computer in an experimental flow phantom model. Materials and Methods : The three-dimensional time-of-flight MRA was performed in a fusiform flow phantom and analyzed the flow signal. computer assisted flow simulation was performed in same flow geometry. The MRA signal intensity and flow velocity distribution and direction was compared. Results : The flow was depicted as homogeneous signal internsity in inlet and outlet area and inhomogeneous signal intensity in fusiform area. Typically, the flow was depicted as target appearance in transition area to outlet. Whereas mean signal internsity decreased slowly in fusiform area, it rapidly dropped and resumed in transition area to outlet. In computer assisted flow simulation, Whereas there were flow velocity decrease and flow direction change to peripheral in entrance to fusiform area, ther were rapid flow velocity resuming and flow direction change to central in transition area to outlet. Conclusion : The signal loss and target appearance in transition area to outlet is characteristic of fusiform flow. These signal changes correlate with abrupt flow velocity and direction change well.

Utilization of Tissue Compensator for Uniform Dose Distribution in Total Body Irradiation (전신방사선조사시 균등한 선량분포를 이루기 위한 조직보상체의 이용)

  • Purpose : This study was performed to verify dose distribution with the tissue compensator which is used for uniform dose distribution in total body irradiation(TBI). Materials and methods : The compensators were made of lead(0.8mm thickness) and aluminum(1mm or 5mm thickness) plates. The humanoid phantom of adult size was made of paraffin as a real treatment position for bilateral total body technique. The humanoid phantom was set at 360cm of source-axis distance(SAD) and irradiated with geographical field size(FS) $144{\times}144cm^2(40{\times}40cm^2$ at SAD 100cm) which covered the entire phantom. Irradiation was done with 10MV X-ray(CLINAC 1800, Varian Co., USA) of linear accelerator set at Department of Therapeutic Radiology, Chonnam University Hospital. The midline absorbed dose was checked at the various regions such as head, mouth, mid-neck, sternal notch, mid-mediastinum, xiphoid, umbilicus, pelvis, knee and ankle with or without compensator, respectively. We used exposure/exposure rate meter(model 192, Capintec Inc., USA) with ionization chamber(PR 05) for dosimetry, For the dosimetry of thorax region TLD rods of $1x1x6mm^3$ in volume(LiF, Harshaw Co., Netherland) was used at the commercially available humanoid phantom. Results : The absorbed dose of each point without tissue compensator revealed significant difference(from $-11.8\%\;to\;21.1\%$) compared with the umbilicus dose which is a dose prescription point in TBI. The absorbed dose without compensator at sternal notch including shoulder was $11.8\%$ less than the dose of umbilicus. With lead compensator the absorbed doses ranged from $+1.3\%\;to\;-5.3\%$ except mid-neck which revealed over-compensation($-7.9\%$). In case of aluminum compensator the absorbed doses were measured with less difference(from $-2.6{\%}\;to\;5.3\%$) compared with umbilicus dose. Conclusion : Both of lead and aluminum compensators applied to the skull or lower leg revealed a good compensation effect. It was recognized that boost irradiation or choosing reference point of dose prescription at sternal notch according to the lateral thickness of patient in TBI should be considered.

Prediction of Entrance Surface Dose in Chest Digital Radiography (흉부 디지털촬영에서 입사표면선량 예측)

  • The purpose of this study is predicted easily the entrance surface dose (ESD) in chest digital radiography. We used two detector type such as flat-panel detector (FP) and IP (Imaging plate detector). ESD was measured at each exposure condition combined tube voltage with tube current using dosimeter, after attaching on human phantom, it was repeated 3 times. Phantom images were evaluated independently by three chest radiologists after blinding image. Dose-area product (DAP) or exposure index (EI) was checked by Digital Imaging and Communications in Medicine (DICOM) header on phantom images. Statistical analysis was performed by the linear regression using SPSS ver. 19.0. ESD was significant difference between FP and IP($85.7{\mu}Gy$ vs. $124.6{\mu}Gy$, p=0.017). ESD was positively correlated with image quality in FP as well as IP. In FP, adjusted R square was 0.978 (97.8%) and linear regression model was $ESD=0.407+68.810{\times}DAP$. DAP was 4.781 by calculating the $DAP=0.021+0.014{\times}340{\mu}Gy$. In IP, adjusted R square was 0.645 (64.5%) and linear regression model was $ESD=-63.339+0.188{\times}EI$. EI was 1748.97 by calculating the $EI=565.431+3.481{\times}340{\mu}Gy$. In chest digital radiography, the ESD can be easily predicted by the DICOM header information.
