저소득층 노인을 위한 맞춤영양관리 프로그램의 개발과 시범 적용 연구 (A Study on Customized Nutrition Intervention Program Design and Application for the Low-Income Elderly)

  • This study aimed to plan nutrition support programs for the elderly living alone whose nutrition status were seriously concerned, conducted seven stages nutrition intervention program on a trial basis, and evaluated the effectiveness of the program of the Elderly Nutrition Support Project. Subjects were selected for personalized nutrition management based on nutritional risk score and nutrition intervention were tailored to the problems occurred. The elderly nutrition support program targets were 44 senior citizens who lived alone with low income. The 33 (as Type 1) of the subjects with whom milk, tofu, seaweed, eggs, black beans have been supported, and also provide nutrition education, and the rest 11 persons (as Type 2) to whom food was not supported but provide nutrition education programs. As a result, all subjects showed that compared with pre and post program implementation, their daily exercise time and milk and protein consumption level were increased and some improvement was observed regular meals consumption and low-salt diets. Their nutrient intake level such as calories, protein, calcium, iron improved after implementation. In addition, NSL DETERMINE scores significantly improved from 13.21 to 7.24 in Type 1 and 11.27 to 9.91 in Type 2. As positive dietary behavioral changes were observed as in that they purchased more protein and calcium rich foods.

개인별 목표 설정과 스마트폰 기반 영양상담을 통한 20-30대 성인의 영양지수 및 식이 자아효능감 향상 (Improving the nutrition quotient and dietary self-efficacy through personalized goal setting and smartphone-based nutrition counseling among adults in their 20s and 30s)

  • 본 연구에서는 20-30대 성인을 대상으로 개인별 식습관 목표를 설정하고 인스턴트 메신저를 활용한 스마트폰 기반의 영양상담이 대상자의 영양지수, 식이 자아효능감, 영양소 섭취상태를 향상시킬 수 있는지를 평가하고자 하였다. 총 30명을 대상으로 모바일 메신저의 1:1 채팅방을 통해 주 1회 1시간 내외로 8주간 비대면 영양상담을 진행하였다. 상담 첫째 주에는 사전 온라인 설문조사와 식사섭취조사를 실시하였고, 두 번째 주에는 사전 조사 결과를 바탕으로 2가지의 식습관 목표를 설정하였고, 목표 달성 여부를 매일 기록할 수 있는 체크리스트를 제공하였다. 3주차부터는 상담영양사가 체크리스트를 바탕으로 피드백 메시지와 식생활 지침에 대한 정보를 카드뉴스 형식으로 제공하였다. 7주차 상담 후에는 사후 온라인 설문조사와 식사섭취조사를 실시하였고, 8주차에는 그 동안의 변화된 식습관을 살펴보고 상담 과정 평가에 대한 온라인 설문조사를 실시하였다. 영양상담 전후의 결과를 분석한 결과, 영양상담 후 대상자의 영양지수와 주위 환경 자극 조절 효능감 점수가 영양상담 전보다 유의하게 높았다. 영양지수 문항별로는 과일, 우유 및 유제품, 견과류, 채소류, 패스트푸드, 라면, 달고 기름진 빵류, 가당음료류의 섭취 빈도 점수가 영양상담 후 유의하게 높았다. 단백질, 비타민 A, 티아민, 리보플라빈, 엽산, 칼슘, 철의 섭취량과 비타민 A, 리보플라빈, 엽산, 칼슘, 철의 INQ도 영양상담 전보다 후에 높았다. 또한 참가자들은 영양상담 프로그램과 제공되는 영양정보에 대해 높은 만족도를 보였다. 이와 같이 개인별 목표 설정과 스마트폰 기반 영양상담은 20-30대 성인의 영양지수, 주위 환경 자극 조절 효능감, 그리고 영양소의 양적, 질적 향상에 효과적인 것으로 나타났으며, BMI에 따라 분류한 저체중·정상체중군과 과체중·비만군 모두 유사한 결과를 얻었다. 본 연구 결과 개인별 목표 설정, 자기 모니터링, 목표 달성에 대한 피드백과 격려가 행동 변화에 긍정적인 효과를 주는 것으로 보이며, 스마트폰을 편하게 사용하는 젊은 성인에게는 대면 상담이 아닌 스마트폰 채팅 기반 영양상담도 효과적인 방법이라고 사료된다.

서울시 일부 취약계층 노인에서 맞춤형 영양중재 프로그램에 따른 식품안정성 확보 및 건강·영양상태 개선 효과 (The effects of a personalized nutrition intervention program on food security, health and nutritional status of low-income older adults in Seoul city)

  • 본 연구에서는 서울시 일부 취약계층 노인을 대상으로 맞춤형 영양교육과 보충식품으로 구성된 영양꾸러미를 제공하여 식품안정성 확보와 건강·영양 상태 개선에 대한 효과를 평가하고자 하였다. 연구 대상자는 서울시 일부 자치구 내 방문건강관리사업 대상자 중 65세 이상 노인 372명이었다. 맞춤형 영양중재 프로그램은 맞춤 영양교육과 보충식품 제공인 2가지 요소로 구성되었고, 영양중재는 약 4개월간 이루어졌다. 프로그램 효과평가를 위해서 식품 및 영양소 섭취량, 식품안정성, 빈혈 여부, 간이영양평가도구 (MNA)를 활용한 영양불량 여부, 만성 질환관리 수준, 허약 여부가 조사되었다. 영양중재 프로그램 참여 이후, 대상자의 모든 영양소 섭취량이 프로그램 참여 이후 유의적으로 증가하였고 권장섭취기준 대비 50%에 미치지 못했던 대부분의 영양소 섭취는 프로그램 참여 이후 50%를 넘게 섭취하였다. 프로그램 참여 이전 모든 대상자가 식품불안정 상태였으나 프로그램 참여 이후 37.1%가 식품안정성을 확보하였다. 영양 상태에서는 불량과 위험은 감소하였고, 정상이 0%에서 29.8%로 증가하였으며, 빈혈에서는 정상이 18.7%에서 28.5%로 증가하였다. 또한, 질환관리 수준에서는 정기 및 집중관리 수준의 대상자 비율은 감소하였고, 자기역량 관리 수준이 0%에서 4.6% 증가하였으며, 허약 상태에서는 허약 및 허약 위험 비율은 감소한 반면, 정상 비율이 0%에서 9.7% 증가하였다. 연령별로 분석하였을 때, 80세 이상의 고령인 노인일수록 특히 만성질환수준의 관리와 허약 수준에 대한 개선이 미비한 것으로 나타났다. 본 연구 대상자는 질환을 갖고 있는 방문건강관리사업의 대상자로 경제적으로나 건강상태로나 매우 취약한 대상자로서 이들의 질환이 더 악화되지 않도록 유지하거나 개선시키는 부분은 매우 중요하며, 본 연구는 대상자들의 질환 관리 및 영양상태 개선을 위하여 질환별 보충식품의 제공 및 영양교육 등의 전문적인 관리를 통해서 영양 상태 향상뿐만 아니라 질환 개선까지의 가능성도 보여주었다.

미래 식품 경향과 조리과학 (Future food and the role of food cookery Science)

  • The environment which surrounds us such as climates, technology change rapidly. Forecasting the future of food by reviewing the subject at the moment would be meaningful, because we could understand the impact of new technology, set the new goal and create the future by speaking out on issues of our concern and by our choices. There are two points of view on our future, one is pessimistic view due to the food and the water shortage, as well as environmental contamination. Another is optimistic view mainly because of innovative technologies. Biotechnology would enable to develope the personalized food using nutrigenomics, nutrigenetics, and nutraceuticals. Nanotechnology and shelf-life extension techniques would contribute great deal to the development of the future food. Consumer's concern for the health and the wellness as well as palatability would affect the development of future food so that the involvment of food cookery scientists are more important to meet the consumer's needs and trends.

인공지능기반 건강기능식품 추천서비스 사용의도에 미치는 영향요인 분석 (Analysis of the Influence Factors on Intention of Use for Artificial Intelligence-Based Health Functional Food Recommended Service)

  • The health functional food market continues to grow, and according to that trend, the subdivision sales of personalized health functional foods, which have been legally prohibited, will be operated as a special regulatory pilot project. Personalized health functional food recommendations have a variety of personalized indicators to consider, and it is believed that algorithmic methods will be needed to proceed in a customized manner considering all of them. This study aims to contribute to the development of the AI-based health functional food recommendation service by studying factors that affect the use of the AI-based health functional food recommendation service. This paper analyzed the intention of use for AI-based health functional food recommendation service based on the information system success model and Technology Acceptance Model. This study considered information quality factors, service quality factor, and system quality factor as independent variables influencing perceived usefulness, perceived ease of use and trust. For empirical analysis, 406 questionnaires were used and the collected data were performed using AMOS 22.0 and SPSS 22.0. Research has shown that the accuracy, timeliness, empathy and availability have a positive effect on usefulness. Understandability and availability has been shown to have a positive effect on ease of use. The accuracy, understandability, empathy and availibility has been shown to have a positive impact on Trust. Usefulness, ease of use and trust all have been shown to have a positive influence on intention of use.

영양관리과정에 의한 영양상담이 제 2형 당뇨병 환자의 식사요법 실천과 혈당 조절에 미치는 영향 (Effect of Nutrition Counseling by Nutrition Care Process on Diet Therapy Practice and Glycemic Control in Type 2 Diabetic Patients)

  • Objectives: This study examined the effects of nutrition counseling by the nutrition care process (NCP) on diet therapy practice and glycemic control in patients with type 2 diabetes mellitus. Methods: The survey was conducted on 49 patients whose hemoglobin A1c (HbA1c) level ranged from 6.5% to below 10% among patients aged 30~60s with type 2 diabetes mellitus. Nutrition counseling by the NCP process was carried out twice: first nutrition counseling and follow up counseling. The questionnaires were composed of 54 questions in five fields (general characteristics, health-related behaviors, diet therapy-related items, dietary life, diet therapy-related knowledge, diet therapy-related barriers). Nutrition intervention in nutrition counseling was performed based on the individualized diagnosis of NCP. Results: All the subjects practiced self-monitoring of their blood glucose levels, regular exercise, and diet therapy after NCP-based nutrition counseling. Diet therapy-related knowledge and practice by the subjects were improved after nutrition counseling. While the intake of boiled white rice decreased, the intake of boiled brown rice and barley rice in the subjects increased significantly. After nutrition counseling, the weight and HbA1c of the subjects decreased. Conclusions: These results suggest that personalized nutrition counseling by NCP process is effective for diet therapy compliance and glycemic control of type 2 diabetic patients.

전북 일부 지역 초등학교 고학년 대상 '영양·식생활 가이드북'을 활용한 영양교육 효과 (Effects of Nutrition Education Using Dietary Guidebook in Higher Grade Elementary Students of Jeonbuk Area)

  • Objectives: This study was conducted to examine the effects of nutrition education with a dietary guidebook for children on dietary attitude, nutrition knowledge and nutrient intakes. Methods: The subjects were 54 higher grade elementary students (27 educated vs. 27 non-educated). The educated group was provided individual and/or group lessons (40 min/lesson/week, 4 week) using a dietary guidebook of Children developed by The Korean Society of Community Nutrition (KSCN) & Korean Food and Drug Administration (KFDA). The contents were Balanced Diet, Smart Food Choices, Cooking a Healthy Snack and Building a Healthy Body. We examined the differences in nutrition knowledge, dietary attitudes and dietary intake between the educated group and non-educated group. Results: After education, the educated group improved dietary attitude, nutrition knowledge and qualitative nutrient intakes compared to the non-educated group. Specifically, among dietary attitudes, 'taking a meal with salty and spicy food' increased, while among nutrition knowledge, 'functions of protein', 'functions of fat', 'foods with carbohydrates', 'foods with fat', 'foods with vitamins', and 'foods with minerals' were increased. Nutrition adequacy ratio (NAR) scores for vitamin C, iron, and zinc were increased. Conclusions: Nutrition education using a dietary guidebook for children developed by the KSCN & KFDA had positive effects on nutrition knowledge and qualitative nutrient intakes. These findings suggest that nutrition education focused on personalized daily energy and nutrient requirements may improve dietary attitude and quantitative nutrient intakes of children.

Optimizing Nutrition Support in Cancer Care

  • Involvement of a multidisciplinary team in cancer care may have added benefits over the existing system of patient management. A paradigm shift in the current patient management would allow more focus on nutritional support, in addition to clinical care. Malnutrition, a common problem in cancer patients, needs special attention from the early days of cancer care to improve quality of life and treatment outcomes. Patient management teams with trained oncology dietitians may provide quality personalized nutritional care to cancer patients.

임신부의 영양교육 경험에 관한 연구 (A Study on Pregnant Women's Experience about Nutrition Education)

  • This study was conducted to investigate pregnant women's experience about nutrition education for pregnant women in order to improve nutrition education programs. The questionnaires were distributed to 185 women with children whose age of under 24 months. About 46% of respondents participated in nutrition education for pregnant women. Major reasons for nonparticipation was 'no information(47%)' and 'lack of time(32%)'. About 40% of women attended to education operated by health centers, 34% maternity hospitals, 26% companies of formula or baby supplies. Participation rate in nutrition education showed significant differences(p<0.05) with age and household income. Women in their forties and with monthly income over three million Won showed higher rates than those of women in other groups. The subjects of education were nutrient supplements for pregnant women(21%), pregnancy complications and health(19%), abnormal symptoms of pregnancy and nutrition (18%), weight gain during pregnancy(17%), dietary guideline and directions for pregnancy (15%), relationship between nutrition of pregnant woman and baby's health(10%) in order. Teaching method which was used most frequently was lecture(35%). About 74% of women were not satisfied with the education. Nutrition management for pregnancy was the subject which pregnant women wanted to learn but not been taught enough. About 80% of women wanted more education and preferred personalized education such as personal counselling (30%), home visitation(26%), telephone(16%) and internet(15%) counselling. These results showed nutrition education for pregnant women was unhelpful for practical life. Therefore, nutrition education programs for pregnant women has to reflected pregnant women's individual needs to heighten the effectiveness of nutrition education.

장기간 경관급식을 공급받는 노인 환자에 대한 적극적 영양관리의 효과 (Effect of Active Nutrition Care on Underweight Elderly Patients Receiving Long-term Enteral Tube Feeding)

  • Objectives: This study was performed to investigate the effect of active nutrition care on feeding and nutritional status of elderly patients receiving long-term enteral tube feeding. Methods: Subjects included 77 elderly patients who had received enteral nutrition more than one week before admission. Nutrition care was provided to patients supplied less calories than required. Feeding intolerance was examined and managed every day and formula was adjusted to meet nutritional requirement during the first 3 months after admission. Patients were classified into under or over 80% of percent ideal body weight (PIBW) and medical records were used to compare changes in weight,, biochemical indices, and nutritional status during the study. Results: Weight, BMI, triglyceride and total cholesterol in blood, hemoglobin, and hematocrit levels were significantly lower in patients under 80% of the PIBW than in those over 80% of the PIBW at admission. The percentage of supply to required calories was also lower in patients under 80% of the PIBW. After 1 month of nutritional care, supplied volume of formula was significantly increased in patients under 80% of the PIBW. Weight, BMI, and PIBW were increased and there were no differences between groups after 6 months. In addition, the concentrations of triglyceride and total cholesterol in blood, hemoglobin, and hematocrit tended to increase in patients under 80% of the PIBW, leading to no difference between groups after 3 months. Conclusions: Personalized active nutrition care is effective to increase weight and improve feeding and nutritional status in underweight elderly patients receiving long-term enteral nutrition.
