Development of Hazardous Materials Management Standard for Decoction Type of Personalized Herbal Medicine

  • Objectives: This study was conducted to development of hazardous materials management standards for the decoction type of personalized herbal medicines (PHMs). Methods: This study was conducted in two stages. We searched documents about criteria to use words such as 'Herb', 'Herbal medicine', and 'Botanical Drug' and summarized the results. We organized the committee consisted of seven experts, and held two meetings to reach an agreement on hazardous management standards of the decoction type of PHMs. Results: The seven documents were presented in the literature review and six items related to hazardous management standards of decoction were identified. The second expert meeting brought that a total of six items, including heavy metal, pesticide residues, sulfur dioxide, benzopyrene, mycotoxin, and micro-organism limits, were selected for safety management of decoction type of PHMs. Also, the criteria and test methods for each standard were suggested for monitoring the decoction type of PHMs. Conclusion: The study suggested hazardous material management standards and criteria for the decoction types of PHMs. In the future, it would be necessary to conduct a pilot test to ensure the validity and credibility of the safety management standard and criteria. Furthermore, the government level safety management system should be introduced to verify the safety of decoction medicines.

Potential application of herbal medicine treatment based on pattern identification for canine cognitive dysfunctional syndrome: a comparative analysis of Korea medicine therapy for patients with dementia (반려견 인지기능장애증후군에 대한 한의 진단 및 한약치료 적용 가능성 고찰: 치매환자 국내한의치료기술과 비교 분석)

  • Canine cognitive dysfunction syndrome (CDS) is a neurodegenerative disease that causes cognitive and behavioral disorders and reduces the quality of life in dogs and their guardians. This study reviewed the complementary and alternative medicine (CAM) for CDS and compared the diagnosis and therapy of CAM between CDS in canines and dementia in humans. The evaluation tools for the diagnosis of CDS and dementia were similar in the neurological and neuropsychiatric examinations, daily life activity, cognitive tests, and neuroimaging, but the evaluation for dementia was further subdivided. In CAM, pattern identification is a diagnostic method for accurate, personalized treatment, such as herbal medicine. For herbal medicine treatment of cognitive impairment in canines and humans, a similar pattern identification classified as deficiency (Qi, blood, and Yin) and Excess (phlegm, Qi stagnation, and blood stasis) is being used. However, the veterinary clinical basis for verifying the efficacy and safety of CAM therapies for CDS is limited. Therefore, based on CAM evidence in dementia, it is necessary to establish CDS-targeted CAM diagnostic methods and therapeutic techniques considering the anatomical, physiological, and pathological characteristics of dogs.

Systematic Review on Herbal Treatment for Obesity in Adults (성인비만의 한약치료 임상연구에 대한 체계적 고찰)

  • Objectives This systematic review is to support the effectiveness of herbal medicine for the treatment of simple obesity in adults. Methods A total of 8 electronic databases were searched in English, Chinese and Korean. Standard methodologies for the systematic review were used to search, analyze and synthesize data based on NECA's guideline for undertaking systematic review and meta-analyses for intervention. Results After screening 1781 potential articles, we identified 13 randomized controlled trials (RCTs). We conducted a meta-analysis for the study using Bangpungtongsung-san and Taeeumjowi-tang to evaluate the effectiveness of herbal medicines on weight loss compared to placebo control. Both herbs showed positive effect in weight, body mass index and waist circumference. In an analysis comparing herbal effects considering constitutions showed that proper treatment can lower adverse events. Conclusions Our results provide evidence that herbal medicine can be an effective treatment for the obesity compare to placebo or to conventional medicine without serious adverse events. Further study is needed to evaluate effectiveness of herbal medicines considering personalized treatment and its safety.

An Investigation into the State of Doctors' Linkage and Oriental Medical Treatment at All Postpartum Care Center in Seoul (서울시 소재 산후조리원의 의료인 연계 및 한방의료 현황 조사)

  • Objectives: The purpose of this study is to investigate all postpartum care center in Seoul about its state of doctors' linkage and oriental medical treatment. Methods: From May 1st 2011 to July 10th 2011, survey was done using a selfmade questionnaire by phone or visit. There were 12 categories including the state of doctors' linkage in postpartum care centers, treatment form of oriental medical doctors, utilization of herbal medicine and so on. The data collected was analyzed using Excel 2007 FOR WINDOWS. Results: 1. The doctors' linkage with postpartum centers included pediatricians(96.1%), oriental medicine doctors(61.2%), and gynecologists(34.0%). Of those postpartum centers, 66% had both pediatrician and oriental medical doctor which were the vast majority. 2. Out of 70 postpartum centers providing herbal medicine, 9 of them were supplied of herbal medicine by herb sellers(not doctor). Where as, at 63 oriental medical doctors-affiliated centers, 85.7% of them had oriental medical doctors' examinations. 3. Out of 70 postpartum centers providing herbal medicine, personalized prescription amounted to 35(50.0%), same prescriptions amounted to 34(48.6%), and 1(1.4%) refused to answer. 4. Out of all the patients that received explanation about the effects of herbal medicine, 62.9% had them explained by oriental medical doctors. Conclusions: These findings suggests that oriental medical doctors are actively involved with postpartum care centers but problems surfaced regarding individualized prescription insufficiency and the lack of administration. In the near future, if the expertise expand in the areas of pediatrics as well as obstetrics and gynecology, oriental medicine will have a more solid role in postpartum care part.

An Observational Multi-Center Study Protocol for Distribution of Pattern Identification and Clinical Index in Parkinson's Disease (파킨슨병 변증 유형 및 지표 분포에 대한 전향적 다기관 관찰연구 프로토콜)

  • Objectives: This study investigated the pattern identification (PI) and clinical index of Parkinson's disease (PD) for personalized diagnosis and treatment. Methods: This prospective observational multi-center study recruited 100 patients diagnosed with PD from two Korean medicine hospitals. To cluster new subtypes of PD, items on a PI questionnaire (heat and cold, deficiency and excess, visceral PI) were evaluated along with pulse and tongue analysis. Gait analysis was performed and blood and feces molecular signature changes were assessed to explore biomarkers for new subtypes. In addition, unified PD rating scale II and III scores and the European quality of life 5-dimension questionnaire were assessed. Results: The clinical index obtained in this study analyzed the frequency statistics and hierarchical clustering analysis to classify new subtypes based on PI. Moreover, the biomarkers and current status of herbal medicine treatment were analyzed using the new subtypes. The results provide comprehensive data to investigate new subtypes and subtype-based biomarkers for the personalized diagnosis and treatment of PD patients. Ethical approval was obtained from the medical ethics committees of the two Korean medicine hospitals. All amendments to the research protocol were submitted and approved. Conclusions: An objective and standardized diagnostic tool is needed for the personalized treatment of PD by traditional Korean medicine. Therefore, we developed a clinical index as the basis for the PI clinical evaluation of PD.   • Objective : This study is achieved to know the way that we can use metabolomics for the scientific research of Korean medicine. Results : We elucidate that metabolomics can be contributed to four ways for objectification of Korean medicine 1. Diagnosis of BeunJungLonChi(辯證論治). 2. Application toward Personalized medicine. 3. Quantification of Herbal medicine. 4. Evaluation of efficacy/safety for Korean medicine formulation Conclusion : Metabolomics opens up a new opportunity to reinvestigate Korean medicine from holistic perspective. Furthermore, Metabolomics is expected to provide effective method in research of acupuncture and meridian.

Factors associated with Utilization and Satisfaction of Traditional Korean Medicine in Korea: A National survey

  • Objective : To assess the utilization patternsand satisfaction factors of Traditional Korean Medicine(TKM) in Korea. Method : The study design of this paper is from a nationally representative random e-mails and interview surveys. In this survey, we asked 2000 Korean adults between the ages of 20 and 69, whether they have used TKM in the past three years and if so, what were their experiences and how satisfied they were with the care they have received. Results : Sixty percent of respondents(n=1239) answered that they had used TKM in the past three years. The odds of using TKM were higher in female(OR 1.48; 95% CI 1.24-1.78) and academic backgrounds of university graduate or beyond (OR 1.27; 95% CI 1.01-1.60), and lower in unmarried person (OR 0.68; 95% CI 0.55-0.84) in the multivariable analysis. The most frequent purpose for seeking TKM and receiving treatment was to 'treat illness'.In addition, the use herbal medicine among types of TKM is statistically significant in sexual groupings.748(60.4%) among TKM users responded that they had a satisfying experience during TKM treatment. In multivariable logistic model, the odds of satisfaction from using TKM were higher in employed groups(OR 1.34; 95% CI 1.02-1.77), the reason for using of personalized medicine into TKM(OR 1.50; 95% CI 1.00-2.26). Conclusion : Korean traditional medicine doctors need to develop and provide the personalized medicine for the group of high utilization rate and satisfaction, and make strategies to disseminate TKM utilization for the group of lower utilization.

  • Objectives : This paper serves to explore current trends of systems biology in Traditional Chinese Medicine (TCM) and examine how it may influence the Traditional Korean medicine. Methods : Literature review method was collectively used to classify Introduction to systems biology, diagnosis and syndrome classification of systems biology in TCM perspective, physiotherapy including acupuncture, herbs and formula functions, TCM systems biology, and directions of academic development. Results : The term 'Systems biology' is coined as a combination of systems science and biology. It is a field of study that tries to understand living organism by establishing a theory based on an ideal model that analyzes and predicts the desired output with understanding of interrelationships and dynamics between variables. Systems biology has an integrated and multi-dimensional nature that observes the interaction among the elements constructing the network. The current state of systems biology in TCM is categorized into 4 parts: diagnosis and syndrome, physical therapy, herbs and formulas and academic development of TCM systems biology and its technology. Diagnosis and syndrome field is focusing on developing TCM into personalized medicine by clarifying Kidney yin deficiency patterns and metabolic differences among five patterns of diabetes and analyzing plasma metabolism and biomarkers of coronary heart disease patients. In the field of physical therapy such as acupuncture and moxibustion, researchers discovered the effect of stimulating acupoint ST40 on gene expression and the effects of acupuncture on treating functional dyspepsia and acute ischemic stroke. Herbs and formulas were analyzed with TCM network pharmacology. The therapeutic mechanisms of Si Wu Tang and its series formulas are explained by identifying potential active substances, targets and mechanism of action, including metabolic pathways of amino acid and fatty acid. For the academic development of TCM systems biology and its technology, it is necessary to integrate massive database, integrate pharmacokinetics and pharmacodynamics, as well as systems biology. It is also essential to establish a platform to maximize herbal treatment through accumulation of research data and diseases-specific, or drug-specific network combined with clinical experiences, and identify functions and roles of molecules in herbs and conduct animal-based studies within TCM frame. So far, few literature reviews exist for systems biology in traditional Korean medicine and they merely re-examine known efficacies of simple substances, herbs and formulas. For the future, it is necessary to identify specific mechanisms of working agents and targets to maximize the effects of traditional medicine modalities. Conclusions : Systems biology is widely accepted and studied in TCM and already advanced into a field known as 'TCM systems biology', which calls for the study of incorporating TCM and systems biology. It is time for traditional Korean medicine to acknowledge the importance of systems biology and present scientific basis of traditional medicine and establish the principles of diagnosis, prevention and treatment of diseases. By doing so, traditional Korean medicine would be innovated and further developed into a personalized medicine.

Effect of Nokyongdaebo-tang on chronic anemia in Taeumin wiwansuhan-pyohan disease (胃脘受寒表寒病) patient: a case report

  • Objectives: The present study reports a case of chronic anemia patient who was significantly improved after taking Nokyongdaebo-tang (NYDBT) Methods: A 48-year-old male patient diagnosed with iron deficiency anemia (IDA) visited the Daejeon Korean medicine Hospital of Daejeon University on June 2018. The patient was treated with Korean medicine for 23 months including acupuncture, herbal medicine and Sasang constitutional medicine prescription. Improvement of anemia was assessed by changes of Hemoglobin value. Laboratory analysis was used to evaluate the safety of treatment. Results: Chronic anemia has significantly improved since taking NYDBT on 9 days (2019.12.09-2019.12.17). The Hb value has been maintained in the normal range since it first entered the normal range on January 18, 2020. The patient had become transfusion-free and this condition persisted for 6 months after stopping the transfusion. Conclusion: This case demonstrates the therapeutic potential of Sasang constitutional medicine for chronic anemia as a personalized medicine.

Exploring the feasibility of developing an education tool for pattern identification using a large language model: focusing on the case of a simulated patient with fatigue symptom and dual deficiency of the heart-spleen pattern (거대언어모델을 활용한 변증 교육도구 개발 가능성 탐색: 피로주증의 심비양허형 모의환자에 대한 사례구축을 중심으로)

  • Objective : This study aims to assess the potential of utilizing large language models in pattern identification education by developing a simulated patient with fatigue and dual deficiency of the heart-spleen pattern. Methods : A simulated patient dataset was constructed using the clinical practice examination module provided by the National Institute for Korean Medicine Development. The dataset was divided into patient characteristics, sample questions, and responses, and utilized to design the system, assistant, and user prompts, respectively. A web-based interface was developed using the Django framework and WebSocket. Results : We developed a simulated fatigue patient representing dual deficiency of the heart-spleen pattern through prompt engineering. To make practical tools, we further implemented web-based interfaces for the examinee's and evaluator's roles. The interface for examinees allows one to examine the simulated patient and provides access to a personalized number for future access. In addition, the interface for evaluators included a page that provided an overview of each examinees' chat history and evaluation criteria in real-time. Conclusion : This study is the first development of an educational tool integrated with a large language model for pattern identification education, which is expected to be widely applied to Korean medicine education.
