Purpose: Dentigerous cyst is the most common intra-osseous lesion of the jaw. Dentigerous cysts can cause delays in eruption of the affected permanent tooth in mixed dentition. It has been suggested that the affected permanent tooth could be erupted spontaneously after the dentigerous cyst was enucleated. But in some cases, orthodontic treatment or autotransplantation technique is known to be required. This study reviews previously performed prognoses of affected permanent teeth, which will lead to a more efficient treatment plan. Patients and Methods: With 28 patients who have undergone cyst enucleation and 10 patients who have undergone autotransplantation, the prognosis of permanent teeth was observed. Results: After cyst enucleation, spontaneous eruption of a permanent tooth was observed in 56.3% patients, orthodontic treatment was performed in 25% patients. The success rate of autotransplantation was 60.0%. Discussion: The first choice for treatment of dentigerous cyst in mixed dentition is to guide spontaneous eruption of permanent teeth. For cases without enough eruption space, the orthodontic treatment should be considered. Autotransplantation should be considered when the spontaneous eruption is not expected. It should be considered that the rate of successful autotransplantation is decreased on maxilla anterior area.

A material of 48 patients with 60 avulsed and replanted permanent teeth were followed retrospectively in the period of 1996. 1 to 1998. 12 (mean observation period=1year 7months). The age of the patients at the time of replantation ranged from 9 to 63 years (mean=24 years). Clinical records of patients were reviewed to obtain valid data concerning the extent of injury and treatment provided. Pulpal and periodontal healing states were examined with periapical x-rays and clinical examination procedures (i.e. percussion test and mobility test) at their recall visit. Root ankylosis was found in fifty-two teeth(87%) and root resorption in twenty-four(40%). Only two of the replanted teeth(3%) showed partial regeneration of the periodontal ligament. Six teeth(10%) resulted in tooth loss, but the remaining fifty-four were clinically well functioning. Most of teeth have mild marginal bone loss accompanied by gingival retraction without pathological periodontal pockets. The incidence of root resorption was much higher in younger age group. However, it was not affected by the interval between avulsion and replantation, the condition of supporting tissues, the degree of root formation and the type of splinting, indicating that multiple factors involved in determining the prognosis of replanted teeth. Based on these findings, avulsed teeth in unfavorable conditions (i.e. long extra-alveolar periods, etc.) should be preserved if possible.

Mineral trioxide aggregate (MTA) has recently been used as a pulpotomy medicament for primary molars. The aim of this study was to evaluate and compare the proliferation and differentiation potential of dental pulp stromal cells of permanent teeth and deciduous teeth cultured on MTA-coated surface. Human dental pulp stromal cells were obtained from human permanent premolars and deciduous teeth and cultured on MTA-coated culture plates. The cells were subjected to proliferation assay and cell cycle analysis. Their differentiation potential was evaluated by analysing changes in the mRNA expressions of runt-related transcriptional factor 2 (Runx2) and alkaline phosphatase (ALP). Morphological changes of cells in direct contact with MTA were observed using scanning electron microscopy (SEM). The proliferation rates, distribution of cell cycles and mRNA expression patterns of Runx2 and ALP were similar in both types of pulpal cells. SEM observations revealed that both types changed into more dendrite-like cells. On the surface of MTA, human dental pulp stromal cells from deciduous and permanent teeth were able to both proliferate and differentiate into cells that induce mineralization. MTA is suitable as a biocompatible pulpotomy medicament for primary teeth.

The purpose of this study was to investigate the effects of Er:YAG laser on cutting of efficacy of enamel and dentin in primary and permanent teeth. We used the enamel and dentin specimens of human teeth which contain the physiologic saline and maintain the pulpal pressure in dentinal tubules. Each specimen was exposed to Er:YAG laser with non-contact mode under different treatment condition of irradiation energy, pulse repetition rate. We investigated the cutting efficacy of Er:YAG laser by Scanning Intensity Microscopy, and obtained following results. 1. Cutting volume of enamel and dentin in primary and permanent teeth were increased by increasing the irradiation energy, pulse repetition rate. 2. Cutting volume of primary teeth was larger than that of permanent teeth. 3. Cutting volume of dentin was larger than that of enamel in primary and permanent teeth. From these results, Er:YAG laser would be more effective in cutting dentin than enamel, and in cutting primary teeth than permanent teeth for clinical application.
Purpose : The purpose of the present study was to retrospectively evaluate the clinical findings and treatment results for impacted permanent teeth associated with odontomas. Materials and Methods : We retrospectively investigated 73 odontomas in 72 patients who visited Kyungpook National University Dental Hospital from April 2004 through November 2011. The study was performed using medical records, panoramic radiographs, and pathological reports. Data gathered included age, gender, location, chief complaints, effects on dentition, and treatment of odontoma and the impacted tooth associated with odontoma. Results : Most compound odontomas (46.7%) were found in the second decade and complex odontomas were not related to age. Odontomas showed no gender predilection. Fifty-five cases (75.3%) of odontomas were detected on routine dental radiographs. Sixty percent of compound odontomas occurred in the canine area and 57.1% of complex odontomas in the molar areas. Impaction of permanent teeth (61.6%) was the most common complication on the adjacent teeth. Most odontomas (84.9%) were removed surgically and impacted permanent teeth were managed by surgical removal (53.2%), orthodontic treatment (25.5%), or surgical repositioning (6.4%). There was a statistically significant relation between age and preservation of the impacted permanent teeth associated with odontomas (p<0.01). Conclusion : Early detection and treatment of odontomas increase the possibility of preservation of the impacted tooth. Therefore, it would be suggested that periodic panoramic examination during the first and second decade of life might be beneficial for the early detection and better prognosis of odontomas.

In case of luxation injuries, loss of tooth vitality is common. And in case of trauma in the immature permanent teeth, precise diagnosis of pulp necrosis is very difficult. That is because limitation in distinguishing between normal dental papilla in immature permanent teeth, transient apical breakdown(TAB), which is part of normal healing process, and apical radiolucency in pulp necrosis. Especially in non-vital immature permanent tooth, the treatment is complex and requires long time. This clinical case report shows that severely infected immature teeth with periradicular periodontitis can undergo healing and apexogenesis or maturogenesis with no definative treatment or after conservative treatment. In the cases reported, we emphasize the considerable power of regeneration of the tooth, probably due to its large number of undifferentiated mesenchymal cells in the dental papilla, pulp tissue, periodontal ligament tissues. Thus, when endodontic treatment in immature permanent teeth, over instrumentation is not recommend for preserve the apical vital stem cells.

The purpose of this study was to compare the microleakage pattern of glass ionomer with resin restoration in microleakage pattern of primary and permanent teeth. Microscopical observation of interface between tooth structure and restoration was also performed. 80 and 8 sound molar teeth were used for the microleakage test and SEM study respectively. Data were analyzed statistically using ANOVA test and/or t-Test. The results of the present study were as follows: 1. According to the result of microleakage pattern between primary and permanent tooth, primary tooth generally showed more micro leakage than permanent tooth in all groups (p<.05). 2. In the resin-filled groups, occlusal margin was shown to have more microleakage than gingival margin(p<.05). Whereas the glass ionomer-filled groups showed no statistically significant differences between them(p>.05). 3. No statistically significant differences in microleakage could be found between two different resin groups(p>.05), while Fuji II LC group showed less microleakage than Ketac-Fil group(p>.05). 4. The various type hybrid layer was evident under SEM in resin-filled groups both in primary and permanent teeth with generally thicker layer in primary group. Among glass-ionomer group, Fuji II LC group showed more intact adhesion to tooth surface than Ketac-Fil group

Treatment options for impacted permanent molars include orthodontic traction, surgical repositioning, transplantation, and extraction of the impacted teeth. Orthodontic traction is recommended because it is the most conservative method. However, it has limitations, such as loss of tooth anchorage. In an effort to overcome these limitations, skeletal anchorage devices tailored for orthodontic use were developed. In this case report, 3 patients were diagnosed with impacted permanent molars. The impacted teeth of these patients were surgically exposed, the orthodontic devices were attached, and the skeletal anchorage devices were implanted for the successful traction of the impacted teeth.

One or two mesiodens are mostly common to the clinician. However, three mesiodens is rarely found and can cause complications such as orthodontic problems or interruptions of the adjacent teeth with more than 1 or 2 mesiodens. Many factors cause irregularities in the permanent dentition. Mesiodens, especially in the anterior maxilla, can disturb the eruption of adjacent permanent teeth and cause diastemas, ectopic eruptions, root resorption, or the formation of dentigerous cysts. The early diagnosis of mesiodens is important for preventing such complications, and the timing of intervention should be based on their location and number. Periodic checkups and improved diagnostic devices make it easy to find mesiodens and associated complications. In this case, 3 supernumerary teeth in the maxillary anterior region were affecting the eruption of the adjacent permanent incisors. To minimize complications and preserve the deciduous teeth, the three supernumerary teeth were extracted in 2 steps. Since cone-beam computed tomography was not available, a brace wire was used to measure the depth of the mesiodens.
Recently the micropig has been developed as human disease model. The dental and orofacial region of micropig is similar to that of humans, so it has been used for testing implant materials and techniques. The purpose of this study is on dental image at each age using radiography and computed tomography. Total twenty-two male micropigs, two or three animals of each 1, 3, 5, 7, 9, 12, 18 and 24 months old, were given radiographic examinations. After general anesthesia, extra- and intra-oral radiographic technique and computed tomographic scans were performed to assess the dental characterization of micropigs. The total deciduous dental formula comprised 28 teeth and was depicted as Di 3/3, Dc 1/1, Dp 3/3. The total permanent dental formula comprised 44 teeth and was depicted as I 3/3, C 1/1, P 4/4, M 3/3. Hypodontia of the first premolars was common in the micropig. The permanent teeth erupted from 3 to 24 month after birth. The sequence of eruption of the permanent teeth was M1, P1, I3, C, M2, I1 + P3 + P4, P2, I2, M3. Dental imaging enables visualization of the unerupted teeth and gives more information about the development of the teeth. The growth pattern of the teeth obtained through radiographic and computed tomographic examination provides basic data in the micropig as animal model for dental research.
