치주치료 후 지각과민 감소를 위한 다이오드 레이저와 Ms coat 처치 직후 임상적 효과 비교 (The comparison clinical study effect of Diode Laser & Ms coat usage for dentinal hypersensitivity following Periodontal therapy)

  • The purpose of this study was to evaluate the effect of diode laser & desensitizing agents to overcome hypersensitizing root surfaces problem after periodontal treatment. 20 patients(60 teeth) presented were volunteered in this study. Diode laser & MS coat was respectively applied on hypersensitizing root surfaces after periodontal treatments. Following application they were evaluated immediately. The results were as follows: 1. The frequency and degree of root surface hypersensitivity levels were measured by the sequence of tactile and air stream. 2. Reduction of root surfaces hypersensitivity by tactile were for Diode laser and Ms coat application respectively $3.0294{\pm}2.0224$ and $3.2692{\pm}1.6139$. 3. Reduction of root surfaces hypersensitivity by air stream were for Diode laser and Ms coat apapplication respectively $3.0294{\pm}2.0224$ and $2.2692{\pm}1.6139$. 4, It could be said that Diode Laser and Ms coat application were significantly effective in reducing dentin hypersensitivity as far as concerned effect, Ms coat applicatio showed more effective than Diode laser. In conclusion, both methods were significantly effective in reducing dentinal hypersensitivity. Therefore, it was recommended that Diode laser and desensitizing agents could be used routinely for patients dentinal hypersensitivity following periodontal treatment.

Bio-hybrid dental implants prepared using stem cells with β-TCP-coated titanium and zirconia

  • Purpose: This study investigated periodontal ligament (PDL) restoration in osseointegrated implants using stem cells. Methods: Commercial pure titanium and zirconium oxide (zirconia) were coated with beta-tricalcium phosphate (β-TCP) using a long-pulse Nd:YAG laser (1,064 nm). Isolated bone marrow mesenchymal cells (BMMSCs) from rabbit tibia and femur, isolated PDL stem cells (PDLSCs) from the lower right incisor, and co-cultured BMMSCs and PDLSCs were tested for periostin markers using an immunofluorescent assay. Implants with 3D-engineered tissue were implanted into the lower right central incisors after extraction from rabbits. Forty implants (Ti or zirconia) were subdivided according to the duration of implantation (healing period: 45 or 90 days). Each subgroup (20 implants) was subdivided into 4 groups (without cells, PDLSC sheets, BMMSC sheets, and co-culture cell sheets). All groups underwent histological testing involving haematoxylin and eosin staining and immunohistochemistry, stereoscopic analysis to measure the PDL width, and field emission scanning electron microscopy (FESEM). The natural lower central incisors were used as controls. Results: The BMMSCs co-cultured with PDLSCs generated a well-formed PDL tissue that exhibited positive periostin expression. Histological analysis showed that the implantation of coated (Ti and zirconia) dental implants without a cell sheet resulted in a well-osseointegrated implant at both healing intervals, which was confirmed with FESEM analysis and negative periostin expression. The mesenchymal tissue structured from PDLSCs only or co-cultured (BMMSCs and PDLSCs) could form a natural periodontal tissue with no significant difference between Ti and zirconia implants, consequently forming a biohybrid dental implant. Green fluorescence for periostin was clearly detected around the biohybrid implants after 45 and 90 days. FESEM showed the invasion of PDL-like fibres perpendicular to the cementum of the bio-hybrid implants. Conclusions: β-TCP-coated (Ti and zirconia) implants generated periodontal tissue and formed biohybrid implants when mesenchymal-tissue-layered cell sheets were isolated from PDLSCs alone or co-cultured BMMSCs and PDLSCs.

Clinical evaluation of root coverage in Miller class III/RT2 labial gingival recession treated with interdisciplinary periodontal-orthodontic therapy: a randomized controlled clinical trial

  • Purpose: The aim of current study was to evaluate percentage root coverage (RC%) in isolated Miller class III/RT2 labial gingival recession (GR) associated with malaligned mandibular anteriors, using interdisciplinary periodontal-orthodontic treatment as compared to mucogingival surgery alone. Methods: Thirty-six systemically healthy patients having isolated Miller class III/RT2 GR with respect to malaligned mandibular anteriors, were randomly divided into test group: mucogingival surgery using subepithelial connective tissue graft followed by orthodontic treatment and control group: mucogingival surgery alone. Primary clinical parameters included (RC%), recession depth, keratinized tissue width, mid-labial clinical attachment level, interdental clinical attachment level (iCAL), periodontal phenotype (PP), gingival thickness (GT), root coverage esthetics score (RES) and hypersensitivity. Total duration of follow up was 12 months. Results: Mean RC% was significantly more achieved in test group (66.67%±40.82%) in comparison to control group (39.93%±31.41%) at the end of study (P=0.049). Further, complete root coverage was attained in 5/8 cases of test group versus 1/2 cases of control group after 3/12 months respectively. RES and hypersensitivity, showed statistically significant improvement after complete follow up period in both the groups. An ideal RES score of 10 was achieved in 4/7 cases of test group while in 1/2 cases of control group after 3/12 months respectively. Correlation analysis revealed significant negative correlation between RC% and iCAL. Correlation of RC% with GT and PP was non-significant. Conclusions: Interdisciplinary periodontal-orthodontic approach may be more beneficial in terms of achieving improved RC%, esthetic and resolution of hypersensitivity in the management of Miller class III/RT2 GR in malaligned mandibular anteriors.

발치 창에 삽입한 임시 수복물의 치은형태 보존 효과 (Effect of immediate provisional restoration on the preservation of gingival contour)

  • One of the central components of periodontal therapy is the improvement of esthetics. The presence and appearance of interdental papillae plays an important role of periodontal esthetics. The aim of the present study was to investigate how immediate provisional restoration preserve the shape of interdental papilla around the extraction socket and the width of bucco-lingual of gingiva. Another aim was to investigate the change in the interdental papilla and the amount of vertical bone fill of a extraction socket in relation to the interdental alveolar bone levels adjacent the alveolar socket. A total of 19 patients (11 male, 8 female, mean age of 50.57${\pm}$8.16), who visited the Department of Periodontology, Pusan National University and had more than one anterior tooth scheduled to be extracted due to an advanced periodontal disease were included in the present study. After initial periodontal therapy, the extracted teeth were reshaped of the root and placed into the socket followed by splinting with adjacent teeth with self-curing resin. The width of hucco-lingual of gingiva and interdental papilla height were measured at baseline, 1, 3, 6, 9 and 12 month and the periapical radiographic examination were taken at baseline, 6 and 12month following the extraction. The amount of vertical bone fill in the extraction socket were calculated. At 12 months following the extraction, the changes in mesial and distal interdental papilla and the width of bucco-lingual showed -1.06${\pm}$0.48mm, -0.844${\pm}$0.50mm, -1.50${\pm}$0.96m, relatively. The positional change in the interproximal papillae was significantly associated with the interdental bone level adjacent to the extraction socket(p=0.028). The higher the interproximal bone level adjacent the extraction socket, the greater the amount of bone fill in the extraction socket(p<0.001). In conclusion, it was thought that immediate provisional restoration could minimize the loss of the width of bucco-lingual and interproximal papillae around the extraction socket. In addition, the higher the interproximal bone level adjacent the extraction socket, the greater the amount of bone fill and the smaller the reduction of papillary height around the extraction socket.

Adjuvant therapy with 1% alendronate gel for experimental periodontitis treatment in rats

  • Purpose: The aim of this study was to evaluate the effects of locally delivered 1% alendronate (ALN) gel used as an adjunct to non-invasive periodontal therapy. Methods: Ligature-induced periodontitis was performed in 96 rats. The ligature was tied in the cervical area of the mandibular left first molar. The animals were randomly divided into 4 groups: 1) NT, no treatment; 2) SRP, scaling and root planning; 3) SRP/PLA, SRP followed by filling the periodontal pocket with placebo gel (PLA); and 4) SRP/ALN, SRP followed by filling the periodontal pockets with 1% ALN gel. Histomorphometric (percentage of bone in the furcation region [PBF]) and immunohistochemical (receptor activator of nuclear factor-κB ligand, osteoprotegerin, and tartrate-resistant acid phosphatase) analyses were performed. Data were statistically analyzed, with the threshold of statistical significance set at P≤0.05. Results: The SRP, SRP/PLA, and SRP/ALN groups presented a higher PBF than the NT group (P≤0.01) at 7, 15, and 30 days. The SRP/ALN group presented a higher PBF than the SRP/PLA group in all experimental periods, as well as a higher PBF than the SRP group at 15 and 30 days. No differences were observed in the immunohistochemical analyses (P>0.05 for all). Conclusions: Locally delivered 1% ALN gel used as an adjunct to SRP enhanced bone regeneration in the furcation region in a rat model of experimental periodontitis.

2000년대 치주처치의 전략 (Periodontal Management strategies for the future in Korea)

  • 과학기술이 발전함에 따라 치주과학 영역에서도 1960년 이후 1980년대에 걸쳐 괄목할 만한 학문적 발전이 이루어졌으며 1990년에는 이에 의거한 치주질환의 예방 및 조절방법이 개발되어 사용되고 있다. 미래의 치주처치전략은 대상 지역의 특성과 보건환경에 따라 달라지므로 여기에서는 한국내 치주치료요구도와 치주치료 담당 일반치과의와 치주전문의의 수, 처치 내용에 대해 점검하고 현재의 치주처치 추세를 검토한 후 향후의 치료전략에 대해 언급하고자 한다. 1980년도 역학조사에 의하면 전체 인구의 82%가 치주질환(치은염+치주염), 35세 성인 인구의 30-40%가량이 치주염에 이환되어 있고 15세 청소년 인구중 0.1%가 유년형치주염에 이환되고 있다. 평균 수명이 증가하면서 65세 이상의 노년층도 전인구의 6%(성인 50%), 2000년도에는 6.9%(성인 52%)로 증가하고 치아보유율도 증가하므로 치주질환 치료요구도는 상승하리라 전망된다. 현재 한국내 치주전공 과정을 거친 치과의사는 약 780인 정도이다, 1996년 류의 연구에 의하면 이들도 대부분 일반치과의(개원의, 공증보건의 포함)로서 진료하고 있으며 의료보험하에서의 치주치료의 비중은 매우 낮다(4.66%). 포괄적 치주적 검사도 보편적이지 않고 대학병원급의 극히 일부에서만 이행되고 있는 실정이다. 향후의 치주질환이 처치전략에는 의료보험 운용시 치주적 배려와 간편한 치주검사과정을 통한 조기진단 및 조기치료, 진행기질환 및 난치성질환자의 치주전문으로의 의뢰가 포함될 것이며 각 환자에게는 개별적이며 임상시험 결과에 근거하는 evidence-based approach에 의한 치료선정 과정이 활용되리라 전망한다.

  • The purpose of this was to evaluate the in vitro effects of different application modes of tetracycline HCI on dentin root surfaces. The study group was comprised of 30 teeth with advanced periodontal disease extracted before the start of periodontal therapy. The diseased root surface was vigorously root planed to expose dentin. The teeth were sectioned and cotton pellets. soaked in a saturated solution of tetracycline HCI, were either "placed" or "burnished" on the prepared root surface for 3, 5 min. respectively. Control surfaces were similarly treated with root planing only. Following the various treatments, teeth were fixed. dehydrated, critical point dried, and coated for scanning electron microscopic(SEM) evaluation. The surfaces of tetracycline HCI treated specimens considerably differed from specimens treated with root planing only. Results demonstrated that root-planed, no-tetracycline HCI specimens had amorphous, irregular surface which corresponded to a smear layer. Tetracycline HCI specimens exhibited collagen fibers and dentinal tubules exposed by the removal of the smear layer. Although some differences were seen in surface structures among some specimens. "burnished" application causes more extensive change than "placed" application, and application time of the tetracycline HCl did not seem to have any major impact on the surface morphology.

우골 유도 합성골이 사람 태아 골모세포의 골 광물화 과정에 미치는 영향 (Effects of a xenographic bovine bone on the bone mineralization in human fetal osteoblasts)

  • The ultimate goal of periodontal therapy is to promote the regeneration of lost periodontal tissue, there have been many attempts to develop a method to achieve this goal, hut none of them was completely successful. The purpose of this study is to evaluate the effects of Bio-Oss(R) on alkaline Phosphatase (ALP) activity in human fetal osteoblasts (hFOB1). The results of this study were as follows, in ALP Activity, 100 ${\mu}g/ml$ Bio-Oss(R) treated group showed significantly increased value than negative control group, but positive group($10^{-7}$ M dexamethasone treated group) showed the highest ALP activity at 3 day. In mineralization assay, numerous mineralized nodules were identified as darkly stained spots in 100${\mu}g/ml$ Bio-Oss(R) treated group than two control groups, whereas a small number of mineralized nodules were showed in the positive control. ALP may relate to the initial phase of bone nodule formation. On the basis of these results, this study showed Bio-Oss(R) is capable of accelerating new bone formation through hFOBl differentiation in vitro.

치근면 처치시 결합조직 부착에 관한 초기효과의 조직병리학적 연구 (THE EARLY EFFECT OF DIFFERENT ROOT TREATMENT MODALITIES ON CONNECTIVE TISSUE ATTACHMENT IN RATS)

  • An essential fact in the regeneration of new periodontal tissue after periodontal therapy is the reattachment of collagen fibers to the tooth. Two phenomena play a fundamental role in preventing new connective tissue attachment to the exposed root surface ; 1) The apical migration of the junctional epithelium 2) The contamination of cementum by toxic substances, especially endotoxins. Authors have used rat submucosal implantation of root sections to study the connective tissue healing to periodontally diseaed root, previously planed and demineralized with citric acid and tetracycline- HCl. The results were obtained as follows. 1. The connective tissue attachment was increased in tetracycline, citric acid, non disease, scaling and root planing order and inflammatory reaction was seen in the rat teeth, no treatment group. 2. Collagen fiber attachment at the dentin surface was more increased than cementum surface 3. In 2 week of citric acid and tetracycline-HCl specimens, osteoid was seen near the fibrotic band. 4. In the MT view, collagen fiber formation was increased with time and the numerous collagen fiber and connective tissue was more densly attached to the tooth surfaces in the tetracycline-HCl group than the citric acid group.

  • The Chlorhexidine(CHX) has been a widely used adjunt in periodontal therapy due to its bactericidal effect. In spite of the effects of CHX exhibits cytotoxic to human cells and delays granulation tissue formation. Therefore, understanding the effects of CHX on fibroblast attachment and cell growth will provide the rationale for its use during healing phase of periodontal surgery. This study was undertaken to examine the effects of standardized CHX-pretreated dentin slices and direct CHX exposure on human gingival fibroblasts. The results were as follow : 1. In experiment 1, there was a significant reduction in the number of fibroblast attachment in 0.12, 1%-pretreated groups relative to the control, 0.05%-pretreated groups(P<0.05). 2. In experiment 1, the control, 0.05%-pretreated groups showed considerable attachment and typical fibroblastic morphology, but 0.12, 1%-pretreated groups showed irregular, round-up (unattached) fibroblastic morphology. 3. In experiment 2, it appeared that all experimental groups exhibits significant inhibition of cell growth when compared with the control group.
