ANALGESIC EFFICACY OF CAPSAICIN IN PERIODONTAL THERAPY (Capsaicin이 치주 치료시 통증에 미치는 영향)

  • The purpose of present study is to assess the effects of capsaicin topically applicated to the chronic periodontal pain suffering area. In the First study, twenty patients with chronic pain caused by mild periodontal disease were selected, and periodontal pack containing capsaicin(PPC) was attached to these patients gingiva around pain suffering area. Then the presence of discomfort had been recorded every ten minutes for the first 1 hour. After 1 hour again, It had been recorded according to the presence of pack and to the existence of pain. In the second study, twenty moderate periodontitis patients were selected. After subgingival curettage of two quadrant area, non-euginol periodontal pack or PPC were attached to the curetted gingival margins of them (Non-euginol pack bearing area and capsaicin pack bearing area is supposed to control group and test group respectively.), and the degrees of pain with time had been recorded eight times with 1 hour interval (at that day) or recorded once in a day (from the next day to the next appointment day). The results are as follows : 1. PPC has caused discomfort accompanied by burning sensation to the mild periodontitis patients with chronic pain. 2. PPC has given little effects to improve the pain after subgingival curettage of moderate periodontitis patients.

THE EFFECTS OF CHLORHEXIDINE USING PATTERN WITH PERIODONTAL PACK ON CLINICAL AND MICROBIOLOGIC PARAMETER AFTER PERIODONTAL SURGERY (치주포대에 클로르헥시딘 이용형태가 치주수술후 임상과 미생물학적 변수에 미치는 영향)

  • A study was conducted to compare the effects of chlorhexidine mouthrinse and chlorhexidine acetate powder with periodontal pack during 4 weeks following periodontal surgery. Twelve patients were treated with Modified Widman flap procedures and devided into three groups of four patients each; control group, chlorhexidine mouthrinse group, chlorhexidine acetate powder group. Each group wasn't significant difference in clinical and microbiologic parameters at preoperation. Examination regarding plaque index, gingival index, pocket depth, change of gingival color postsurgically, pain index were performed by each methods and observed bacteria in gingiva by phase contrast microscope. Evaluations were made at 7, 14 and 28 days postsurgically. The results were as follows: l. In comparison of results revealed no significant differences in Plaque Index, Gingival Index, pocket depth and change of gingival color postsurgically. 2. The pocket depth of three experimental groups were significant reduced at 4 weeks. 3. Chlorhexidine mouthrinse group and chlorhexidine acetate powder group with periodontal pack were significant differences in pain index at 3, 4 days postsurgically. 4. Cocci and Non-motile rods were increased at 1, 2 weeks. But, motile rod and spirochete were increased at 4 weeks.

  • Glanzmann's thrombasthenia is a Qualitative platelet disorder characterized by a deficiency in the platelet membrane glycoproteins IIb/IIIa. It belongs to a group of hereditary platelet disorders typified by normal platelet numbers and a prolonged bleeding time. The severity of bleeding does not correlate with the severity of the platelet glycoprotein IIb/IIIa a abnormality. The present case report describes the periodontal treatment of a patient with Glanzmann's thrombasthenia. A 30-year-old female with a history of Glanzmann's thrombasthenia was referred for gingival bleeding on tooth brushing and discomforts in #38 area. The periodontal finding revealed a diagnosis of localized slight adult periodontitis. Root planing and extraction of #38 was performed under 12 pack of platelets transfusion and digital compression was done for hemostasis. The gingival bleeding ceased within a day in maxilla and 2 days later in mandible. 42 pack of platelets was administered for 3 days of post-treatment and for iron-deficiency anemia 3 pack of RBCs was transfused 2 days later. 1 week later the inflammation in gingiva disappeared and gingival stippling appeared. The clinical result we got was good and in such a medically compromised patient it is an ability to maintain a proper oral hygiene that is essential both for oral and systemic health.

THE EFFECTS OF CHLORHEXIDINE AND LISTERINE MOUTHRINSE WITHOUT PERIODONTAL DRESSING AFTER PERIODONTAL SURGERY (치주수술후 치주포대 비부착시 클로르헥시딘과 리스트린 구강 양치의 효과)

  • The purpose of this study was to compare the effects of Chlorhexidine, Listerine and hypertonic saline mouthrinse without periodontal pack during 2 weeks following periodontal surgery. Twelve patients were treated with modified Widman flap procedures. Each fourth patients assigned to chlorhexidine mouthrinse group, Listerine mouthrinse group and control group. Each group wasn't significant difference in clinical and microbiologic parameters at preoperation. Examination regarding plaque index, gingival index, pocket depth, change of gingival color postsurgically, pain index and contrast phase microscope were performed. Evaluations were made at the first, the second and the fourth weeks postsurgically. The results were as follows: 1. Chlorhexidine mouthrinse group and Listerine mouthrinse group were significantly different in plaque index at the first, the second and the fourth weeks 2. The pocket depth of three groups were significantly reduced at the fourth weeks. 3. Chlorhexidine mouthrinse group and Listerine mouthrinse group were significantly different in change of gingival color at the first week. 4. Cocci and non-motile rods were increased at the first and the second weeks. But, motile rods and spirochete were increased at the fourth weeks.

CYTOTOXIC EFFECTS OF SOLUBLE EXTRACTS FROM RERIODONTAL DRESSINGS ON HUMAN GINGIVAL FIBROBLASTS (치은 섬유아세포에 대한 치주포대 추출물의 세포동성에 관한 연구)

  • It is well known that the application of dressings after periodontal surgery have benefits to provide the comforts to patient and to promote the healing process with action of bleeding control and temporary stabilization for the operated mobile teeth. But until recently the relationship between periodontal dressings and cells which are composed of periodontium has not been clear. The purpose of this study was to evaluate the cytotoxic effect of soluble extracts from the four different kinds of periodontal dressings, two of them were eugenol type (K.H.pack, Wondrpak) and the others were non-eugenol type (Coe-pak, Periocare), on the human gingival fibroblasts in vitro. Human gingival fibroblasts were primarily cultured from gingiva around third molar during the extraction for preventive purposes. Extracts solution were prepared with culture medium by means of imersing the consistent size of periodontal dressing made from plastic mold. Cell were inoculated into the 24 well plate with $3\;{\times}\;10^4\;cells/well$ of medium at $37\;^{\circ}C$, 100% of humidity, 5% of $CO_2$, incubator for 24 hours. After discard of the supernatant of medium, those cells were cultured with original, 1/2, 1/5, 1/10 diluted soluble extract for 24, 48 and 72 hours, and counted the number of cells using the hemocytometer at each designed time and concentration. Also, the cytotoxic effect of soluble extract was measured by Wataha's MTT assay method. In briefly, cells were inoculated and cultured into 96 well culture plate with $2\;{\times}\;10^4\;cells/well$ for 24 hours. Soluble extracts were applied to cultured cells and incubated for 48 hours at same condition. $50\;{\mu}l$ of MTT solution and DMSO were added into each well for the detection of absorbance with ELISA reader. The measured data were calculated by value of colorimetric assay for survival rate. The results were as follows ; In the case of eugenol type of dressing, original, 1/2 and 1/5 diluted extracts of K.H.pack showed very low survival rate. And original extract of Wondrpak showed strong cytotoxic effect and 1/2 diluted extract showed moderate cytotoxic effect. In the case of Non-eugenol type of dressings, only original extract of Coe-pak revealed strong cytotoxic effect and Periocare had little cytotoxic effect. It is concluded that eugenol type of dressings showed more cytotoxic effect than non-eugenol types. This study suggest that use of non-eugenol dressings after periodontal surgery is recommended.

Free gingival graft in combination with apically positioned flap for establishment of keratinized gingiva around the implants: Report of two cases (근단변위판막술과 함께 유리치은이식술을 사용하여 임플란트 주변 각화치은을 증대시킨 2건의 증례 보고)

  • Narrow zone of attached gingiva and shallow vestibule around the implants might contribute to difficulty of cleasing, periimplant mucositis caused by incomplete cleansing and further peri-implantitis. The aim of this case report is to present modification of soft tissue biotype around the implants by free gingival grafts according to timing of surgical intervention and shape of free gingiva. A 44 year-old male patient had a missing area on lower right second molar area with 1 to 2 mm of narrow attached gingiva zone and wanted to be treated by implant placement. In radiographic analysis, there was enough alveolar bone to install an implant, free gingiva from hard palate was grafted following implant placement using double layer flap. The width of attached gingival was increased to 4 to 5mm and well maintained during 5 months of follow up. A 69 year-old female patient also had a missing area on lower right first and second molar area with 1 to 2 mm narrow attached gingiva. Since she had systematically angina pectoris and dental phobia, minimal invasive free gingival graft after implants placement was planned. After 2 months of implant surgery, free gingival graft surgery was performed with healing abutments connection. The grafted gingiva was composed of two strip shaped free gingiva, and they were immobilized by periodontal pack. The width of attached gingival was increased to 4 to 5mm and well maintained during 10 months of follow up. With prosthesis delivery, the patients recovered ideal periodontal environment around implants and masticatory function. In conclusion, periodontal health and masticatory function could be achieved through implant placement and free gingival graft.

Surgical Treatment of Phenytoin Induced Gingival Hyperplasia : A Report of Case (Phenytoin으로 인한 치은비대증의 치험례)

  • Enlargement of the gingiva caused by phenitoin, an anticonvulsant used in the treatment of epilepsy, occurs in some of the patients receiving the drug. Its incidence varies from 3 to 62 percent, with the greater frequencies in younger patients. The hyperplasia is usually generalized throughout the mouth, but is more severe tendency in the maxillary and mandibular anterior regions, 18 year old male patient was admitted to our Department of Dentistry with the complaint of generalized painless gingival swelling. After the consult of the N.M. and laboratory study, the gingivectomy and gingivoplasty was performed. The periodontal pack and tin foil was applied on the attached gingiva to protect a surgical site and bleeding control. We obtained a good result of improved esthetics and function.

A Study on Oral Health Status of Industrial Workers Gumi City (구미시 일부 근로자의 구강건강실태 조사연구)

  • The number of workers in the study were 831 in their academic backgrounds are 53.6% workers who graduated from high schools 80.7% of the subjects replied that their economic state belonged to the middle class. 55.7% brushed their teeth at least three times a day and 58.7% of them brushed their teeth after they had dinner. In questions about living habits, 68.4% were found to drink alcohol, among whom 55.7% had more than one bottle of So-ju. As well, 57.7% of the subjects smoked, among whom 34.8% smoked half the pack or one whole pack of cigarettes a day and 30.0% had smoked more than 10 years. The result of oral examination showed that 25.9% had more than 1 decayed tooth, 14.8% had their teeth missing, 42.2% suffered periodontal diseases, and 11.7% had problems with wisdom teeth.
