Perforation of an Idiopathic Small Bowel Ulceration after Blunt Abdominal Trauma in a Child (소아에서 복부둔상 후 발견된 특발성 소장 궤양의 천공 1예)

  • Idiopathic small bowel ulceration distal to the duodenum is rare. Less than 5 % of the reported cases were in children. In the majority of the patients, a single ulcer of unknown cause is found in the jejunum or ileum. The diagnosis is difficult and usually made at the time of surgical exploration for complications, such as perforation, hemorrhage or obstruction. We treated a pediatric patient with perforation of an idiopathic ileal ulceration. The child was an 11-year-old boy who sustained blunt abdominal trauma. The involved ileal segment was resected. Pathologic findings were compatible with idiopathic small bowel ulceration. The clinical and pathological aspects of idiopathic ulcerations are discussed, and the literature reviewed.

  • Noroviruses have been recognized as the leading cause of epidemic and sporadic gastroenteritis since the advent of molecular diagnostic technique. They have been documented in 5-31% of pediatric patients hospitalized with gastroenteritis. Although norovirus gastroenteritis is typically mild and self-limited, it causes severe, but sometimes fatal, conditions in the vulnerable population such as immunocompromised patients, young children, and the elderly. Bowel perforation due to norovirus infection is rare. We report a case of small bowel perforation with norovirus gastroenteritis in the infant with Down syndrome during the hospitalization with pneumonia. Severe dehydration may cause bowel ischemia and could have triggered bowel perforation in this case. Physicians should be alert to the potential surgical complications followed by severe acute diarrhea, especially in high risk groups.

Surgical Management of Esophageal Perforation due to Fish Bone: A Report of Four Cases (생선뼈'에 의한 식도천공의 외과적 치료: 4례 보고)

  • This is a report on a total of four cases of esophageal perforation due to fish bone in the Department of Thoracic Surgery, Hanyang University Hospital. The perforated portions of esophagus were upper third of esophagus, that is, cervical esophalgus principally. The complications after esophageal perforation were acute mediastinitis with mediastinal emphysema in 2 cases, acute mediastinitis with both pyothorax in one case and cervical subcutaneous abscess alone in one case. Collar mediastinostomy was required to control disturbance of cardiopulmonary function as emergency procedure. Gastrostomy was of worthy for the various purposes, that` is, for feeding, absolute rest of the esophagus, and for prevention against continuous infection from esophageal leakage. After the gastrostomy. 3 cases were healed by spontaneous closure of esophageal perforation between one to four weeks. One case expired from severe septic shock due to acute diffuse mediastinitis and both pyothorax.

  • Root canal perforations are defined as the communication between the pulp cavity, the periodontal tissue and alveolar bone. The occurrence of perforations during endodontic treatment is reported to range from 2.3%~12%, which is not a complication rarely happens. Perforations have iatrogenic or pathological etiologies that involve caries or resorption. It leads to inflammation and the destruction of periodontal fibers and alveolar bone, followed by periodontal defects. Mineral trioxide aggregate (MTA) is currently the most indicated material for repair of root perforation, because of its favorable biocompatibility and sealing ability. Using magnification with dental operating microscope enhance the accessibility and visibility to manage the root perforation. It is important to diagnose and repair perforations immediately if possible.

  • The paper is reporting some comparisons between experimental and numerical results in terms of failure mode, failure time and ballistic properties of mild steel sheet. Several projectile shapes have been considered to take into account the stress triaxiality effect on the failure mode during impact, penetration and perforation. The initial and residual velocities as well as the failure time have been measured during the tests to estimate more physical quantities. It has to be noticed that the failure time was defined using a High Speed Camera (HSC). Thanks to it, the impact forces (average and maximum level), were analyzed using numerical simulations together with an analytical description coupled to experimental observations. The key point of the model is the consideration of a shape function to define the pulse loading during perforation.

Successful Surgical Treatment of Feline Acute Bullous Keratopathy and Corneal Perforation with a Conjunctival Hood Graft

  • We report the successful treatment of feline acute bullous keratopathy (FABK) and corneal perforation with a conjunctival hood graft. Two cats with FABK and corneal perforation had severe corneal edema with large central bullae. Their corneas were thinned and perforated. Conjunctival hood grafts were performed. All corneas recovered after 3-4 weeks. The grafts were then removed from the cornea using superficial keratectomy and no ocular irritation was observed. One month later, the corneas had recovered, although there was very mild fibrosis. The cats had normal vision without any corneal problems at this time. Conjunctival hood graft was thus useful for the treatment of severe FABK and allowed the cats to maintain vision.

  • Spontaneous perforation of CBD in infant is a rare but fatal disease. We report a case of bile leakage from common bile duct in 11 months old girl with progressive abdominal distension and vomiting, preoperatively diagnosed by hepatobiliary scan with 99mTc-DISIDA, which was confirmed by surgery, Operative cholangiogram showed a small perforation at the confluence of cystic duct and common bile duct with mild fusiform dilatation, and no definite abnormality in confluence of the common bile duct and pancreatic duct. Simple drainage of the free peritoneal bilous fluid and T-tube drainage were performed without any evidence of the complication. Patient was inevitable for 6 months OPD follow-up examination.

  • Pyriform sinus perforation is a rare complication of endotracheal intubation. It most commonly occurs at the hands of the less experienced physician in emergency situations. It can occur after traumatic intubation and is potentially lethal. The site most commonly perforated is the pharynx, posterior to the cricopharyngeal muscle; the second most common site is the pyriform sinus. We report a case of pyriform sinus perforation after endotracheal intubation, which was successfully treated with primary closure.

  • Anatomical characters of secondary xylem in the trunk of Magnoliaceae, Winteraceae and Schizandraceae, including six genera and eleven species grown naturally in Korea, were studied to elucidate the relationship between genera or families. It is considered that among these families specialization in the perforation plate, the angel of end wall to the vessel axis, and diameter of vessel element, is in the order of Winteracae, Magnoliaceae, and then Schizandraceae. In Winteraceae, vessel elements have wholly scalariform perforation plates with very numerous bars. Among Magnoliaceae the peroration plates are scalariform with very numerous bars in Michelia, with few bars in Liriodendron, and with few bars or simple in Magnolia. In Schizandraceae, Schizandra shows scalariform perforation plates with few bars or simple perforation plates, and Kadsura shows almost simple ones.

  • Spontaneous perforation of the bile duct without any traumatic or iatrogenic injury is extremely rare. We report a case of abscess formation related to spontaneous perforation of the common bile duct by a gallstone, mimicked a cholecochal cyst.
