The present condition of Korean children born small for gestational age (국내 부당경량아의 현황)

  • Depending on the definition used, between 3% and 10% of live neonates are small for gestational age (SGA). The definition of SGA requires the following: (1) accurate knowledge of gestational age; (2) accurate measurements at birth of weight, length, and head circumference; (3) a cutoff, which has been variably set at the 10th percentile, 3rd percentile, or at less than 2 standard deviation from the mean, and (4) race and ethnicity-specific growth curve. Consensus statements are needed on the management of growth hormone therapy in SGA children, as well as treatment and long-term health outcomes such as impaired cognitive function, increased risk of adult cardiovascular disease, and type 2 diabetes.

  • Automobile airbag deployment process has been studied with MADYMO software. Based on the FMVSS208 and USNCAP(United States New Car Assessment Program) regulations, four parameters were chosen for the design improvement with reference to the 5th percentile female passenger dummy: time to fire, vent hole size, tether length and tank test pressure of inflator. Sensitivity analyses based on orthogonal arrays show that enhanced protection of small females can be achieved with improved USNCAP rating within the boundary of FMVSS 208.

Two Sequential Wilcoxon Tests for Scale Alternatives

  • Two truncated sequential tests are developed for the two-sample scale problem based on the usual Wilcoxon rank-sum statistic for two different dispersion indices - absolute median deviations, when the medians of the two populations X and Y are equal or known and sums of squared mean deviations, when the medians are either unknown or unequal. The first test is briefly called SWAMD test and the second SWSMD test. For the SWAMD test, the percentile points for both the one-sided and two-sided alternatives, (equation omitted) have been found by Wiener approximation and their values computed for a range of values of a and N; analytical expression for the power function has been derived through Wiener process and its performance studied for various sequential designs for exponential distribution. This test has been illustrated by a numerical example. All the results of the SWAMD test, being directly applicable to the SWSMD test, are not dealt with separately Both the tests are compared and their suitable applications indicated.

Bootstrap Confidence Intervals of Precision-to-Tolerance Ratio (PTR의 붓스트랩 신뢰구간)

  • ANOVA is widely used for measurement system analysis. It assumes that the measurement error is normally distributed, which may not be seen in certain industrial cases. In this study, the exact and bootstrap confidence intervals for precision-to-tolerance ratio (PTR) are obtained for the cases where the measurement errors are normally and non-normally distributed and the reproducibility variation can be ignored. Lognormal and gamma distributions are considered for non-normal measurement errors. It is assumed that the quality characteristics have the same distributions of the measurement errors. Three different bootstrap methods of SB (Standard Bootstrap), PB (Percentile Bootstrap), and BCPB (Biased-Corrected Percentile Bootstrap) are used to obtain bootstrap confidence intervals for PTR. Based on a coverage proportion of PTR, a comparative study of exact and bootstrap methods is performed. Simulation results show that, for non-normal measurement error cases, the bootstrap methods of SB and BCPB are superior to the exact one.

Comparative Study on Evaluating Low-Flow in Ungauged Watershed (미계측 유역에서 저수량 산정 방법 비교 연구)

  • In this study, the methodologies for evaluating the low-flow at the ungauged watershed are reviewed and assessed. The ungauged watershed can be classified into different situations such as the partially recorded watershed and the completely ungauged watershed. The extension method and the percentile method are used to evaluated the low-flow at the partially recorded watershed. The drainage-area ratio method and the regional regression method are used at the completely ungauged watershed. These four methods are applied and validated based on the hydrological and geometric data acquired from unit watersheds in Han River basin for TMDLs. In case of partially recorded watershed, the values of low-flow evaluated by the extension method are in better agreement with measured flow-rate rather than those by the percentile method. In case of completely ungauged watershed, the drainage-area method is broadly used to estimate the low-flow. It must be paid attention to consider the treated sewage discharge produced at watersheds when applying the method.

Clinical & Nutritional Assessments of Long-term Survivors of Biliary Atresia (수술후 10년 이상 장기 생존 담도폐쇄증 환장에서의 영양상태 및 임상적 특성에 관한 분석)

  • To assess the clinical and nutritional status of long-term survivors of biliary atresia, history taking, medical record review, physical examination (height, weight, MAC, TSF), blood tests (LFT, prothrombin time, platelet count, prealbumin, calcium) and liver needle biopsy were performed in 12 patients in whom Kasai procedure were performed more than 10 years ago at Department of Pediatric Surgery in Seoul National University Hospital. None were below the 5th percentile in height and weight. TSF was above the 75th percentile in all patients and showed good subcutaneous fat deposition. MAC was above the 5th percentile in all patients. Serum prealbumin level was abnormal in 2 patients with abnormal liver function and revealed visceral protein malnutrition. Serum calcium level was decreased below normal range in 4 patients with abnormal liver function. One patient had mild ascites. Five patients had abnormal liver function and 7 patients showed clinical manifestation of portal hypertension. Liver needle biopsy was performed in 5 patients and no cirrhotic change was observed. Although some patients who have survived for more than 10 years after Kasai procedure developed protein malnutrition and vitamin deficiencies, growth and development and nutritional status were generally satisfactory. Five patients(42%) showed normal liver function and no portal hypertension. In conclusion, Kasai procedure is satisfactory as a primary treatment in biliary atresia but significant portion of long-term survivors had abnormal liver function and portal hypertension. Continuous and careful follow-up is necessary to determine when liver transplantation may be indicated.

Musculoskeletal and Functional Status of Adults with Cerebral Palsy (뇌성마비 성인의 근골격계와 기능 실태)

  • The purpose of this study was to report the physical status of adult with cerebral palsy who live in local community areas including Seoul, Busan, Daegu in Republic of Korea at 2004. 202-individuals with cerebral palsy(140 male, 62 female; mean age 31.9 years; range 19 to 64 years) were surveyed. Measures included a 29-item self-reported health status measure focusing on musculoskeletal status and functional performances. Adult cerebral palsy required continuous management for musculoskeletal and ambulatory function through therapeutic approach. This survey is restricted to population who had cared in welfare center for cerebral palsy in local community, so it needs to investigate home-residual adults with cerebral palsy and the population in rural community. The results were as follows: 1. Participants reported that they had a limitation of a daily of life activity due to musculoskeletal pain($43\%$), there was significant differences between a limitation of a daily of life activity and the decreased ambulatory function(p<.05). 2. Thirty six point six percentile of participants had the decreased ADL function, it was significant related with the impaired body location(p<.05) 3. Thirty nine point one percentile of participants who can independently gait had the decreased ambulatory function, it was significant differences with the usage of ambulatory assistive devices(p<.05). 4. There was significant differences ambulatory function both the body region with impairment and a grade of disability(p<.05). 5. Sixteen point three percentile of participants could not ambulate any more, there was significant differences between a age and a point of time for non-ambulation(p<.05). 6. There was significant differences between ambulatory function and physical activity(p<.05).

Wanting Extremely Low BMI May be Associated with Higher Depression and Undesirable Dietary Habits in High School Girls Who were Not Overweight (연령별 체질량지수 3백분위수 미만을 희망하는 여고생의 식습관과 심리적 상태)

  • The widespread pursuit for the thin physique may have detrimental impact on the wellbeing of the young generation, especially females. This study aimed to determine the effect of wanting very low body weight on dietary habits and psychological factors of female adolescents. Participants were 619 girls from 5 high schools in Kyeonggi, Korea. Information on dietary behaviors, psychological factors as well as current heights and weights and the desired weight for current height were obtained by questionnaire. Of total subjects, 38.1%, 35.5%, and 26.5% desired weights corresponding to normal (10~85 percentile), low (3~10 percentile) and very low BMI (< 3 percentile), respectively. The subjects who wanted to be very low weight had the average BMI of 18.57 kg/m2, which was significantly lower than $21.21kg/m^2$ of those who wanted to be normal weight (p < 0.001). The subjects who desired very low weight had significantly higher scores for depression symptoms (p < 0.05), while there were no differences in obsession to lose weight and obesity stress. Moreover, more subjects in this group had undesirable dietary habits such as eating fast foods more than weekly (p < 0.05). These findings suggest that the desire for extreme thinness may lead female adolescents to have not only unreasonably similar obesity stress and obsession to lose weight but also higher depression symptoms, along with undesirable dietary habits. The findings suggest the potential harm from excessive weight concerns of female adolescents; thus efforts to teach this group about healthy weights are urgently needed.

Evaluation of the Bacteriological Safety for the Shellfish Growing Area in Jaranman.Satyangdo Area, Korea (자란만.사량도 패류생산해역의 위생학적 안전성 평가)

  • Bacteriological examination of a 12,700 ha area within Jaranman Saryangdo area, located in Jaran bay and Saryangdo island, was conducted with 35 fixed (designated areas and adjacent areas) seawater sampling stations between January 2006 and December 2008. According to results, the geometric mean range and the estimated 90th percentile range of fecal coliform counts in sea water samples collected in the designated area were <1.8-2.4 and <1.8-8.6 MPN/100mL, respectively. The estimated 90th percentile range of fecal coliform counts in sea water samples collected from 6 sampling stations in the adjacent areas were 6.1-34.6 MPN/100mL. Based on these results, bacteriological water quality of the designated areas in the Jaranman. Saryangdo area meet the NSSP (National Shellfish Sanitation Program) guidelines for approved area. The bacteriological sea water quality in Jaranman.Saryangdo area has been shown to be favorable at all investigated sampling stations except for July and August which coincided with heavy rainfall. In fact, the bacteriological water quality was not affected if rainfall was less than 30 mm rainfall. However, the degree of bacteriological contamination increased rapidly and the water quality exceeded approved NSSP guidelines set for rainfall above 60 mm.
