A Study on the Factors Influencing Health Care Expenditure of Elderly Households : Focused on the Elderly Single and Elderly Couple Households (노인가계의 보건의료비 지출에 영향을 미치는 요인의 분석 : 노인독신가게와 노인부부가게를 중심으로)

  • The purpose of this study was to examine the health care expenditure of elderly single and elderly couple households whose head is more than 60 years old. The data analyzed for the study were Korean Retirement and Income Study(KRelS) conducted in 2007 by National Pension Corporation. The major finding of this study were as follows: First, the amount of health care expenditure of elderly single households was lower than that of elderly couple households. However, ratio of health care expenditure to total consumption expenditure of elderly single households was larger than that of elderly couple households. Second, the ratio of health care expenditure in consumption expenditure in this study was larger than the ratios in past analyse showed in the previous studies. Third, common factor affecting on health care expenditure of elderly single and elderly couple households was the existence of the family member with chronic disease or handicap. The health care expenditure of elderly singer households was influenced by income, gender and the ownership of national health insurance. The influence of income for elderly singer households seemed to be greater than for elderly couple households. The variables which affected health care expenditure of elderly couple households were age and housing tenure status. The amount and ratio health care expenditure were increased as the age increases. These results show that the health care expenditures for each groups varied according to socio-demographic variables and health-related behavior variables. It is suggested that there should be a discriminative health care policy for each elderly single and elderly couple households. In addition, the health care policy for the elderly households of which member has a chronic disease is certainly necessary. Especially a health care plan for the elderly single households with lower income is in need. For the elderly couple households, the priority group of health care policy would be the high age group.

The Effects of Safety Income System on Recipient's Economic Independence and Life Satisfaction under the Social Security System (사회보장제도의 안심소득제가 수혜자의 경제적 자립과 생활만족에 미치는 영향)

  • This study is empirically intended to look into the effects of safety income system on recipient's economic independence and life satisfaction under the social security system. To achieve this, a survey was carried out to 650 adult householders benefiting from the social security system. The SPSS 23 statistical program was used to apply measurement variables related to the safety income system. The results of this study are summarized as follows. The social insurance and the social assistance of safety income system were adopted, having a significant effect on the economic independence. The effect of economic independence on life satisfaction was rejected, while the effect of social assistance on life satisfaction was adopted, showing a difference in the effect of each factor. The findings imply that social insurance and social assistance with support for safe income can be a positive help for economic independence, and unemployment benefit, national pension, long-term care subsidy and health insurance can help to achieve real economic independence, adopting only social assistance with limitation to life satisfaction. Consequently, the safety income system is very useful in helping recipients to achieve their economic independence through its introduction, but there is some distance to meet their life satisfaction.

Longevity Bond Pricing by a Cohort-based Stochastic Mortality (코호트 사망률을 이용한 장수채권 가격산출)

  • We propose an extension of the Lee and Jho (2015) mean reverting the two factor mortality model by incorporating a period-specific cohort effect. We found that the consideration of cohort effect improves the mortality fit of Korea male data above age 65. Parameters are estimated by the weighted least squares method and Metropolis algorithm. We also emphasize that the cohort effect is necessary to choose the base survival index to calculate longevity bond issue price. A key contribution of the article is the proposal and development of a method to calculate the longevity bond price to hedge the longevity risk exposed to Korea National Pension Services.

A Study on the Necessity of Home Visit Rehabilitation Therapy by Rehabilitation Professionals Working at Social Welfare Facilities (사회복지시설에 종사하는 일부 재활전문가들의 방문재활에 대한 필요성 조사)

  • Purpose: The purpose of this study was to analyze the necessity of home visit rehabilitation therapy by rehabilitation professionals working at social welfare facilities. Methods: The subjects of this study were 227 rehabilitation professionals (social worker, nurse, physical therapist, occupational therapist, speech-language therapist, special education teacher, and staff of institutions for the disabled who were working at community rehabilitation centers. The data were collected over 65 days (2008. 09. 10~11. 14). The results were analysed statistically by the Chi-square test. Results: The results are as follows: Long-term care service was established in Korea by the Ministry of Health in July, 2008. But there are limits to their being able to provide various rehabilitation services for chronic patients and old people. According to a recent survey, almost all rehabilitation professionals (N=227) stated that home visit rehabilitation therapy is necessary. Conclusion: In Korea, the long-term care service has a nursing service and a service supporting physical activities of daily living, but not physical therapy. So, home visit rehabilitation services should include physical therapy, rehabilitation exercise, and pain management.

Influential Factors of Continuous Residence in Profit Assistant Living Elderly Facilities -Focused on Literature Review of the Cases of the United States- (의존형 유료노인시설 거주의 지속성과 관련 요인 -미국 사례의 고찰을 중심으로-)

  • This study has been performed to review critical issues affecting continuous residence in profit(self-paid) assistant living facilities for the elderly in the United States. There are three factors influencing continuous residence of frail elderly in the facilities as long as they want without involuntary discharge : Financial ability, behavioral aspects, and facility management situation. Regarding financial ability, income, purchase of insurance policy, cashable assets, willingfulness to pay, and health status were found to be crucial determinants of continuous residence. Overall, 40-50% of those aged 65+ are financially able to stay in the assistant facilities as long as they want, without involuntary discharge. As for behavioral aspects, extroversions, congeniality, culture & life style, conscientiousness, preference for person, and physiological habits were found to be crucial determinants of continuous residence. As for the management situation, it is essential that facilities reach nature level of self-governing rights of the older residents and good local images, and conflicts among the residents and managing parties be resolved on the agreeable standards of inspectors. finally in this study, it is proposed that in Korea, national pension authority offer a liability package for assistant residence, for the case of individual bankruptcy.

  • Since our country is rapidly progressing from an aging society to an advanced age society, it already puts in effect basic old-age pension system, hurries training human resources like care-giver, and has ahead offering the aged long-term care insurance. But our country still needs to develop a systematic curriculum in order to train professionals that can fundamentally deal with an aging society problems. In this paper, therefore, we describe the development of a curriculum for training silver-care professionals improving qualities of old person life in order to suit to an aged society and prepare for an advanced age society. It includes a network model of a silver-care curriculum based on subject classifications according to fields and levels.

The Study on New Poverty and Change of Poverty Policy in Korea (한국의 신빈곤현상과 탈빈곤정책에 관한 연구: 근로빈곤층(the working poor)의 실태를 중심으로)

  • The object of the study is to examine the change of social-economic structure and poverty-shape to escape poverty. In Korea, the working poor have been increased by flexibility and division of labor market since the economic crisis in 1997, and are faced with hard conditions due to the vulnerable welfare system. Especially the workers who engage in irregular jobs were increased by restructure of labor market. Besides they are in unstable employment terms such as low payment, low-skill and exclusion from welfare-benefit. Many small independent businessmen are also in danger of poverty for enterprises trend to move abroad by globalization. Poverty policy in our country was focused on the absolute poor class that has relation with old age, unemployment, disable, disease etc, so they were the object of welfare policy. The poverties, however, are increasing rapidly after the economic crisis, and they work so hard but are still poor, that is, participation in labor market doesn't become an element to escape poverty. Thus the emergence of new poverties whose core consists of the working poor becomes to need new poverty policy. The study is to survey change of their economic conditions, their welfare conditions, their experiences and responses of social dangers after the economic crisis, then to explore the policy to escape poverty. As the result of the study, it shows that the working poor experienced many kinds of social dangers like unemployment, decrease of income etc. In their welfare conditions as their responses to the social dangers, the benefit of social insurance, enterprise welfare like legal retirement pay and paid leave and private welfare such as private pension and insurance are low. The working poor are faced with social dangers, moreover, they don't have skill or education for adapting themselves to information society. The study says that it needs variable policies for the working poor to escape poverty, and suggests payment & tax policies as stable income policy, occupational discipline and skill-education for promoting the quality of employment, moreover, social insurance as expansion of social welfare policy and housing & education policies whose objects are the working poor.

Influencing Factors of Health Status of Status according to Income Class and Socioeconomic Class Recognition by Employment Type (고용형태별 소득계층과 사회경제적 계층인식에 따른 건강상태 영향 요인)

  • This study examined the factors influencing the health status according to class and socioeconomic class recognition by the employment type. To take advantage of the 18 original sources of the Korea Labor panel materials carried out in the South Korea Labor Institute, 5,158 adults over 20 years old were included in the final analysis. The research results revealed that the incomes of regular workers and non-regular workers between the hierarchy and socioeconomic hierarchy recognition showed a statistically significant difference between the cage; it was consistent between the hierarchy in only the "heavy" category. Regular workers of society, and regardless of non-regular workers, were analyzed to be relatively low compared to the actual income. Regression analysis showed that regular jobs had higher socioeconomic hierarchy recognition. Non-regular workers had a lower income bracket and lower socioeconomic hierarchy recognition. In particular, in the case of non-regular workers, the pension was not subscribed and they had a poorer state of health. Therefore, the pension insurance payment for non-regular workers needs to compensate for the lost income during non-employment periods. In addition, the government should improve public relations through education, management fields, and cooperation with labor.

A Phenomenological Study on the Meaning of Economic Life of Marriage Immigrant Women (결혼이주여성의 경제생활 의미에 관한 현상학적 연구)

  • The purpose of this study is to listen to vivid story on economic life of marriage immigrant women using phenomenological study out of qualitative study methods, and to analyze the meaning of dynamicity of experiences through in-depth interviews. The research question is "What is the meaning of economic life that marriage immigrant women experience?" From the research, 67 meaningful statements were abstracted and 15 core meanings were organized. The 15 core meanings were categorized as 5 theme categories such as 'Tough Life', 'Unstable Income such as Children Education Expense and Insurance Premium', 'Search for Changes in Life Style for Adaptation', 'Pursuit of Economic Stability through Employment', 'Expectation of Supports and Return to Married Woman's Parents' Home.' The researcher made structural description through first person speaker for the application of hermeneutical writing. In other words, the meaning of economic life of marriage immigrant women in Korea is 'difficult coping process to family-oriented culture pursuing changes in life style to adapt themselves to difficult reality.' Various undertones of practice were proposed through those statements such as policy to expand opportunities to receive an old-age pension by applying 'Joint Scheme for Couples' (Virtual Name) to People's pension for stable economic life of marriage immigrant women in old age.

Pricing an Outside Barrier Equity-Indexed Annuity with Flexible Monitoring Period (배리어 옵션이 내재된 지수연동형 보험상품의 가격결정)

  • Equity-indexed annuities(EIAs) provide their customers with the greater of either the return linked to the underlying index or the minimum guaranteed return. Insurance companies have developed EIAs to attract customers reluctant to buy traditional fixed annuities because of low returns and also reluctant to buy mutual funds for fear of the high volatility in the stock market. This paper proposes a new type of EIA embedded with an outside barrier option with flexible monitoring period in order to increase its participation rate. It also derives an explicit pricing formula for this proposed product, and discusses numerical examples to show relationships among participation rate, barrier level, index volatility and correlation.
