Object : Dyspnea is a one of the common symptom in pulmonary disease. Jungchunghwadam-tang was used to treat chronic pulmonary disease patients with dyspnea. Thus in this study we evaluate the effect of Jungchunghwadam-tang on dyspnea. Methods : In this study, ten chronic pulmonary disease patients were treated with Jungchunghwadam-tang. Peak expiratory flow were obtained by peak flow meter. Result : After the treatment, peak expiratory flow was increased significantly compared with before treatment. Percentage of predict peak expiratory flow was also increased significantly compared with before treatment. Conclusion : The result of this study demonstrate that Jungchunghwadam-tang taken for dyspnea on chronic pulmonary disease are effective. Further investigation in well designed follow up study is needed.
It is important to find the effective position for cough and sputum clearance in respiratory physical therapy. The purpose of this study was to compare the changes in peak expiratory flow (PEF), forced expiratory volume in 1 second ($FEV_1$), and peak cough flow (PCF) related to functional level and measurement position in patients with Duchenne muscular dystrophy. Twenty one subjects were classified into three functional levels, and measurements was undertaken in three different measurement positions (upright sitting, $45^{\circ}$ reclining and supine). Vitalograph PEF/FEV DIARY was used to measure PEF and $FEV_1$, and Ferraris Pocket Peak was used to measure PCF. Mixed two-way analysis of variance and Bonferroni post-hoc test were used for statistical analysis. The results of the study were as follows: 1) Significant main effects for measurement position were found. 2) PEF was the highest in upright sitting, followed by $45^{\circ}$ reclining, and supine in order. 3) $FEV_1$ in upright sitting and $45^{\circ}$ reclining were significantly greater compared with that in supine. 4) PCF in upright sitting and $45^{\circ}$ reclining were significantly greater compared with that in supine. 5) No significant main effects for functional level were found in PEF, $FEV_1$, and PCF. 6) No significant functional level by measurement position interactions were found in PEF, $FEV_1$, and PCF. Therefore, it is concluded that upright sitting and $45^{\circ}$ degree reclining positions are recommended for effective cough and sputum clearance.
Impaired respiratory function is common in patients with stroke. The purpose of this study were to investigate the effectiveness of exercises and to assess forced vital capacity and peak cough flow after completion of neck stabilizing and respiratory reeducation exercises (combining diaphragmatic breathing and pursed-lip breathing exercises). The 45 participants were randomly assigned to an experimental group 1 ($n_1=15$), experimental group 2 ($n_2=15$), and a control group ($n_3=15$). All subjects performed conservative physical therapy for 30 minutes. Experimental group 1 undertook the neck stabilizing exercise and the respiratory reeducation exercise. Experimental group 2 undertook the respiratory reeducation exercise. Additional exercise did not exceed 30 minutes, five times a week for six weeks. The subjects were assessed for deep neck flexor thickness and breathing function (forced vital capacity, forced expiratory volume at one second, forced expiratory volume at one second/forced vital capacity, peak expiratory flow, and manual assisted peak cough flow) at pre-post value. The results of this study were as follows. Experimental group 1 showed a significant increase only in deep neck flexor thickness change rate (p<.05). All groups showed significant increases in forced vital capacity, forced expiratory volume at one second, and peak expiratory flow in pre-post measurement (p<.05). Experimental groups 1 and 2 showed an increase in manual assisted peak cough flow in pre-post measurement (p<.05). There was no significant difference between experimental group 1 and experimental group 2, but experimental group 1 improved more than experimental group 2 in respiratory function as a whole. In conclusion, these findings suggest that the neck stabilizing exercise in combination with the respiratory reeducation exercise can improve forced vital capacity and peak cough flow in patients with stroke.
The effects of pharmacist's intervention for asthma patients using self-monitoring of peak expiratory flow rate in medication teaching model was evaluated for 3 months in improving clinical outcomes including emergency visits, hospitalizations, antibiotics use, symptoms and sleep disturbance. Twenty seven patients were enrolled in study and twenty three patients completed the follow-up schedules. The selected patients were given the pre-designed instruction for medication including appropriate use of medication, metered-dose inhaler(MDI) technique, identifying and controling asthma triggers and recognizing early signs of deterioration. There were significant improvements in clinical outcomes, in terms of emergency visits, hopitalizations, antibiotics use, symptoms and sleep disturbance. There were also significant improvements in the MDI use, environmental control, and medication knowledges. There was a progressive increase in peak expiratory flow rate during the three-month intervention. In conclusion, pharmacist's intervention using self-monitoring of peak expiratory flow rate has a significant impact on improving clinical outcomes in asthma patients.

Background: For the diagnosis or evaluation of airway obstruction in bronchial asthma and chronic obstructive lung disorders, various parameters derived from the forced expiratory volume curve and maximal expiratory flow volume curve have been used. Recently the peak expiratory flow(PEF) measured by the peak flow meter is widely used because of its simplicity and convenience. But there were still no data of the predicted normal values measured by the peak flow meter in Korea. This study was to obtain the predicted normal value of PEF and to know the accuracy of this value to predict $FEV_1$. Method: The measurements of PEF by the MiniWright peak flow meter and several parameters derived from the forced expiratory volume and maximal expiratory flow volume curves by the Microspiro HI 501(Chest Co.) were done in 129 men and 125 women without previous history of the respiratory diseases. The predicted normal values of parameters according to the age and the height were obtained, and the regression equation of $FEV_1$ by PEF was calculated. Results: The predicted normal values of PEF(L/min) were -2.45$\times$Age(year) +1.36 $\times$ Height(cm)+427 in men, and -0.96 $\times$ Age (year) + 2.01 $\times$ Height (cm) + 129 in women. FEFmax derived from the maximal expiratory flow volume curve was less than by 125 L/min in men and 118 L/min in women respectively compared to PEF. $FEV_1$(ml) predicted by PEF was 5.98 $\times$ PEF(L/min) + 303 in men, and 4.61 $\times$ PEF(L/min) + 291 in women respectively. Conclusion : The predicted normal value of PEF measured by the peak flow meter was calculated and it could be used as a standard value of PEF while taking care of patients with airway obstruction. $FEV_1$, the gold standard of ventilatory function, could be predicted by PEF to a certain extent.

Asthma is one of the important respiratory diseases requiring home self care usually performed by commercialized peak expiratory flow meter (PEFM). However, this simple device can measure only single parameter, PEF, due to its purely mechanical principle, significantly limiting desease management quality. The present study introduced a new expiratory flow measurement technique by miniatured air expansion chamber easily installed within PEFM. Continuous pressure signal obtained from the chamber demonstrated an accurate quadratic relationship with flow. The volume measurement error was $<{\pm}1%$ well within the American Thoracic Society (ATS) criteria of 3%. Important spirometric parameters of FVC, PEF, and FEF25-75% were all accurately estimated with correlation coefficients > 0.95. The present technique obtains continuous expiratory air flow signal, making possible and convenient to perform spirometric test at home. Electronic interface capability would be also useful for remote asthma management.
This meta-analysis was performed to evaluate the difference of the therapeutic effi cacy and adverse effects of leukotriene receptor antagonist and theophylline added to inhaled corticosteroids in adult asthma. Databases were searched for studies published through Nov, 2010. Randomized-controlled trials containing inhaled corticosteroids plus leukotriene receptor antagonist and inhaled corticosteroids plus sustained-release theophylline for asthma therapy were selected. For each report, data were extracted to the outcomes analyzed: mean change in morning peak expiratory flow, mean change in evening peak expiratory flow, mean change in morning forced expiratory volume in 1 sec, mean change in daily short bete2-agonist use, asthma exacerbation and adverse effects. Four assessable trials including 182 asthmatic patients were identified. Inhaled corticosteroids plus leukotriene receptor antagonist was superior to inhaled corticosteroids plus theophylline therapy in improving morning peak expiratory flow in asthmatics (mean difference 19.08 [95% confidence interval 13.37-23.79] l/min, p<0.001) and morning forced expiratory volume in 1 sec in asthmatics (mean difference 0.09 [95% confidence interval 0.03-0.14] liter, p=0.001). In evening peak expiratory flow, daily short bete2-agonist use, asthma exacerbation and adverse effects, there was no significant difference between these two therapies (All p>0.05). Our meta-analysis showed that the combination of inhaled corticosteroids plus leukotriene receptor antagonist resulted in more improvement in both peak expiratory flow and forced expiratory volume in 1 sec in the morning than inhaled corticosteroids plus sustained-release theophylline in adult asthmatics. Further trials are necessary to evaluate the dominant effects of the former combination.

Purpose : Obesity is associated with disturbances of ventilatory functions in adults. But few studies have evaluated the pulmonary complications of obesity in the pediatric population. The purpose of this study is to clarify the effects of obesity on pulmonary function and body composition in obese children. Methods : Forty seven obese children whose ages ranged from nine to twelve years were evaluated for their body composition(intracellular fluid, extracellular fluid, protein mass, mineral mass, soft lean mass, fat mass, percent body fat, fat distribution) by bioelectrical impedance analysis. Hemoglobin, serum glucose, aspartate aminotransferase(AST), alanine aminotransferase(ALT), total cholesterol and triglycerides were measured. Pulmonary function test was performed by spirometer. Results : Intracellular fluid, protein mass, fat mass, percent body fat and fat distribution were significantly higher in severely obese children with an obesity index of more than 150 percent compared with those with an index of less than 150 percent. Peak expiratory flow rate(PEFR) was significantly lower in severely obese children with obesity index of more than 150 percent compared with those with less than 150 percent($241.7{\pm}14.6L/sec$ vs $276.8{\pm}64.3L/sec$). PEFR, forced expiratory flow 25 percent($FEF_{25}$), mid expiratory flow rate(MEFR), forced expiratory flow 50 percent($FEF_{50}$), forced expiratory volume in 1st second($FEV_1$) and forced vital capacity(FVC) were decreased in 37.0 percent, 14.8 percent, 14.8 percent, 11.1 percent, 3.7 percent and 3.7 percent of obese children, respectively. Conclusion : PEFR was significantly decreased in obese children. Pulmonary function test must be performed in severely obese children and more extended study is needed in other age groups.

Purpose : The purpose of this study was to analysis on diaphragm thickness and lung function of stroke patients by walking ability. Methods : We recruited thirty-five adults after stroke(20 male, 15 female) for our study. The subjects were divided into two groups; independent walking group(11 male, 9 female) and non-independent walking group(9 male, 6 female). Assessment of diaphragm thickness was performed using ultrasound in B-mode with a 7.5 MHz linea probe. During the experiment, the subject was seated in the chair. All subjects performed maximal expiratory flow maneuvers using a spirometer in order to determine the forced expiratory volume in 1 second ($FEV_1$), forced vital capacity(FVC), peak expiratory flow(PEF) and $FEV_1$/FVC. Chest expansion was measured with a tape-measure placed circumferentially around the chest wall at the xiphoid process. The collected data analyzed by independent t-test. Results : The diaphragm thickness were significant differences between the independent walking and nonindependent walking group. Values of forced vital capacity, forced expiratory volume at one second, peak expiratory flow in pulmonary function tests were significant differences between the independent walking and non-independent walking group. However, chest expansion were not significant differences in both of the group. Conclusion : This study showed that walking ability of stroke patients have influenced on diaphragm thickness and pulmonary function.

PURPOSE: This study examined effects of the untact trunk stabilization exercise program on muscle thickness, trunk strength, maximal expiratory flow, and static balance. METHODS: The subjects were 20 normal adults divided into 10 in the contact exercise group and 10 in the untact exercise group. The trunk stabilization exercise program was conducted for four weeks. The muscle thickness was measured using ultrasound. The maximal expiratory flow was measured using Personal Best Full Range Peak Flow Meter. The static balance was measured through Bio-rescue; and the trunk muscle strength was measured by bending the upper body forward and measuring the time for maintaining the posture. RESULTS: Both contact and untact exercise groups showed significant differences in muscle thickness, muscle strength, maximal expiratory flow, and static balance (p < .05). A significant difference in muscle thickness on ultrasound was observed between the contact and untact exercise groups (p < .05). CONCLUSION: Activation of the transverse abdominal muscle requires accurate instructions of the contact exercise, but despite environmental constraints, the untact exercise program is as effective as the contact exercise for improving muscle strength, maximum expiratory flow, and static balance.
